Changes of Microembolic Signals after Heart Valve Surgery (심장 판막 수술 후 미세색전의 변화)

  • Background: The detection of circulating microemboli by transcranial Doppler ultrasonography (TCD) has the potential to select the patients with high risk for future symptomatic brain embolism. We prospectively evaluated the positive rate and the frequency of microembolic signals (MES) before and after the heart valve surgery (HVS). Material and Method: Fifty in-patients with heart valve disease were enrolled in this study. Patients with history of previous stroke or heart valve surgery were excluded. Two unilateral TCD monitoring sessions were peformed from middle cerebral artery for 1-hour, before and after HVS. Result: Mechanical Heart valves were implanted in 28 patients, tissue valves were implanted in 10 patients, and remaining 12 patients received mitral valve repair. Positive rate of MES was significantly increased after HVS (50%), compared to that of before HVS (8%, p=0.00). There was no relation between MES after HVS and intensity of anticoagulation, cardiac rhythm, patients' age, and history of hypertension. The positive rate of MES after implantation of mechanical heart valve (71.4%) was significantly higher than those after implantation of tissue valve or mitral valve plasty (p=0.002). Conclusion: Positive rate of MES was increased significantly after the implantation of HVS. The changes of MES in those with mechanical prosthesis may be related to the increased risk or embolism after Hvs.

  • 기계판막의 가장 빈번하고 치명적인 결함이 판막혈전이다. 판막혈정의 적절한 조기 치료를 위해서는 조기진단이 선행되어야만 한다. Cineradiography는 조기에 손쉽고, 빠르게 기계판막의 기능장애를 진단하는 방법으로 사료되며 저자들은 42세된 남자 환자에서 29mm Carbomedics 판막으로 승모판막 치환술을 시행한 1례와 58세된 여자 환자에서 승모판막 및 대동맥판막에 각각 25mm, 19mm Carbomedics 판막으로 치환술 후 각각 18개월, 20개월 후 기계판막 혈전이 발생하여 cineradiography로 손쉽게 진단하여 치료하였으므로 보고하는 바이다.

  • A new mechanical heart valve prosthesis has been designed appling systematic design methodology. The function of the heart valve was defined, and search for design variation has been carried out according to the functional structure, Optimal model among the various variations was determined in view of the design specificationn. Proto type valve was fabricated and test has been carried out using a mock circulation system. It has been observed that the pressure profile, cardiac output and behavior characteristics are generally satisfactory.

Valve Replacement in Children (소아심장판막치환술)

  • Background: Thirty children ranging from 3 to 15 years of age underwent cardiac valve replacement at Dongsan Medical Center from 1982 to 1997. Material and Method: There were 16 boys and 14 girls. The mean age was 12.1. The underlying pathological cause for valve replacement was congenital heart disease in 17 children and acquired heart disease in 13. The valve replaced was mitral in 15 children, aortic in 11, tricuspid in 3, and combined aortic and mitral in 1. Twenty-one mechanical and 10 tissue valves were placed: primary mechanical valve have been utilized since 1985. Eight of ten patients with tissue valves have had successful second valve replacements 4 to 11 years after the initial operation. Result: The operative mortality was 6.7%, but mortality was higher among patients less than 5 years of age and patients who had previous cardiac operations. Of the 28 operative survivors, 4 patients were lost to follow-up: the remaining patients were observed for a total of 2091 patient/months(mean 74.7 months, maximum 187 months). There was one late death from dilated cardiomyopathy after mitral valve replacement in 7 year-old patient with atrioventricular septal defect. After the operation, all patients with mechanical valves were placed on a strict anticoagulant regimen with Coumadin. The actuarial survival rate was 96% at the end of the follow-up. No instance of thromboembolism or major bleeding were observed in the survivors. Conclusion: These results indicate that valve replacement can be performed with low mortality in children, and with satisfactory long-term survival.

  • Edwards Duromedics Valve has been suspended from the market since May 1988 due to the frequent reports of leaflet escape. There were 2 case-reports of leaflet escapes in this country. We report a case of leaflet escape in an Edwards Duromedics bileaflet valve, in the mitral position.

Reoperation for Dysfunction of Cardiac Valve Prosthesis (인공 심장판막 기능부전 환자에서의 심장판막 재치환술)

  • From January 1988 to December 1995, 27 patients, 11 men and 16 women, underwent surgical intervention at our institution for prosthetic valve dysfunctions. The mean age was 43.5$\pm$12.2 years. Seventeen(63.0%) patients had the mitral valve replacement, 8(29.6%) the aortic valve, 1(3.7%) the aortic composite graft, and 1(3.7%) the tricuspid valve. Mean follow-up period was 49.5$\pm$30.9 months. In 12 bioprostheses, mean interval between the previous valve replacement and the reoperation was 104.9$\pm$34.9 months. The causes of redo surgery were structural deterioration of the prosthetic valve (12/12, 100%), paravalvular leak (2/12, 16.7%), and prosthetic valve endocarditis(1/12, 8.3%). In 15 mechanical prostheses, the mean interval was 55.2$\pm$43.7 months. The causes of redo surgery were pannus formation (8/15, 53.3%), paravalvular leak(4/15, 26.7%), and valve thrombosis(3/15, 20.0%). Posto- perative complications occurred in 7 patients (25.9%). There was no intraoperative death. But one patient, who received mechanical aortic valve replacement died on the 3rd postoperative day due to low cardiac output and multiorgan failure.

  • 임심 중 심폐바이패스를 사용하는 개심수술은 산모뿐 아니라 태아계에도 영향을 끼치기 때문에 중요하다. 임신 중 항응고제 투여을 부적절하게 하여 기계판막 기능부전이 초래된 임신 31주의 산모에서 심장 재수술에 앞서 제왈절개로 태아 출산 후 20시간 뒤 산모의 개시수술을 성공적으로 시행하였기에 보고하는 바이다.
