Leiomyoma of the Bronchus -A Case Report- (기관지에 발생한 평활근종 - 수술치험 1례 보고 -)

  • Leiomyoma of the bronchus is a very rare, benign tumor of the lung. We herein report a case of endobronchial leiomyoma which was treated by sleeve bronchoplasty without resection of the lung and without any complications. A 35-year-old man was admitted to our hospital complaining of stridor. Bronchoscopy revealed a round, hypervascular and smooth tumor nearly obstructing the left main bronchus at 1.5 cm distal portion from the carina. Biopsy was not performed for fear of massive hemorrhage. A sleeve resection of left main bronchus including the tumor and end-to-end anastomosis were performed. The histologic diagnosis was leiomyoma. The early diagnosis and appropriate surgical treatment to preserve the pulmonary function are important points of consideration.

Biological Clock and Ultradian Metabolic Oscillation in Saccharomyces cerevisiae (Saccharomyces cerevisiae의 생물시계와 초단기 대사진동)

  • Biological clocks are the basis of temporal control of metabolism and behavior. These clocks are characterized by autonomous free-running oscillation and temperature compensation and are found in animals, plants, and microorganisms. To date, various biological clocks have been reported. These include clocks governing hibernation, sleep/wake, heartbeat, and courtship song. These clocks can be differentiated by the period of rhythms, for example, infradian rhythms (> 24-hr period), circadian rhythms (24-hr period), and ultradian rhythms (< 24-hr period). In yeast (Saccharomyces cerevisiae), at least five different autonomous oscillations have been reported; (1) glycolytic oscillations (T = 1~30 min), (2) cell cycle-dependent oscillations (T = 2~16 hr), (3) ultradian metabolic oscillations (T = 15~50 min), (4) yeast colony oscillations (T = a few hours), and (5) circadian oscillations (T = 24 hr). In this review, we discuss studies on oscillators, pacemakers, and synchronizers, in addition to the application of biological clocks, to demonstrate the nature of autonomous oscillations, especially ultradian metabolic oscillations of S. cerevisiae.

  • Although most of the patients with endobronchial tuberculosis have some degree of bronchial stenosis, more aggressive treatment is needed to restore the patency of the involved tracheobronchial tree for some patients not responding well to antituberculous chemotherapy combined with steroids. In our first case, we reseated stenotic trachea in a 42 years old women who showe overgrowing granulation tissue through the modified Gianturco steno wire which was previously inserted and anastomosed end to end. Another case was a 37 years old male with left main bronchial rcstenosis complicate, $\boxUl$ after inserting a Strecker stent and sleeve left upper lobectomy was performed.

A Study on the Neurotransmitters Acting on the Medullospinal Tract Cells Related to the Cardiovascular Activity (심맥관계 활동과 관련있는 연수 척수로 세포에 작용하는 신경흥분전달물질에 대한 연구)

  • The medullospinal tract cells are known to play an important role in the control of the cardiovascular activities. To clarify the modes of action of the neurotransmitters on these cells, glutamate, GABA(${\gamma}$-aminobutyric acid) and bicuculline were applicated iontophoretically into the rostral ventrolateral medulla in adult cats anesthetised with ${\alpha}$-chloralose. Followings are the results obtained : 1. The spontaneous activities of the cardiac-related neurons in rostral ventrolateral medulla (RVLM) were increased by the glutamate and decreased by the GABA. 2. Bicuculline, an antagonist of GABA, alone didn't increase the frequency of the action potentials, but could reverse the cellular response to the GABA, simultaneously applicated. 3. GABA seemed to decrease the peak as well as the basal discharge of the neurons in RVLM, but hardly changed their periodicities. 4. The cellular responses of RVLM evoked by the peripheral nerve stimulation could be inhibited by the iontophoretically released GABA. In conclusion, GABA seemed to act as an inhibitory neurotransmitter on the cardiac- related neurons in RVLM of the cats anesthetized with ${\alpha}$-chloralose. But the maintenance of the periodicities of these cells after the application of bicuculline suggested that the afferent activity of the baroreceptor didn't play a key role in the spontaneous activities of the RVLM neurons.

  • 본 논문에서는 심혈관 조영영상으로부터 좌심실의 형태 및 운동을 가시화하기 위해 기하학적 정보와 물리적 정보를 통합한 동적 가우시안 블럽(dynamic gaussian blob)모델을 개발하였다. 동적 가우시안 블럽 모델은 superellipsoid에 3차원 가우시안 형태함수를 갖는 단일 유한요소를 통합한 것으로, 좌심실에 작용되는 가상적인 힘에 의해 형태를 적하하고, 심박동 전 주기에서의 운동을 추적한다. 또한 벽 운동 정도에 따라 다른 색을 지정하여 볼 수 있도록 함으로써 벽 운동에 이상이 있는 부위를 육안으로 쉽게 판별할 수 있도록 하였다. 이와 같은 좌심실의 동적.색체 가시화는 형태가 변황되는 심장질환이나 허혈이나 심근경색증처럼 심실벽 운동에 이상이 있는 질환의 진단을 도울 수 있다.

  • Congenital bronchoesophageal fistula is a rare anomaly that may cause fatal complications if it goes unnoticed for many years. This anomaly may have various symptoms such as respiratory infections, coughing bouts when eating or drinking and even hemoptysis. Surgical resection is the treatment of choice and is definitive in almost cases. We report a case of type I congenital bronchoesophageal fistula misdiagnosed as chronic empyema thoracis with literature review.

  • Adenoid cystic carcinoma is a very slowly growing and directly invasive cancer. The treatment of choice is complete surgical resection but if major complications associated with remaining carcinoma occur, aggressive conservative treatment to prevent complication is able to gain long term survival even though remaining carcinoma metastases to other organs. We experienced a case of surgical treatment of uncontrollable fever that caused by multiple lung abscesses due to obstruction of left main bronchus with adenoid cystic carcinoma. The post operative course was uneventful for 4 months to now.

  • Benign papillary tumors commonly arise in the upper respiratory tract but are rare in the lower respiratory tract, especially in the bronchi. Solitary benign papillary tumors arising at the bronchi,covered with stratified squamous epithelium and associated with little or no inflammatory cell response,seem to be rarest of all types of benign bronchial tumors. Only six cases of such tumors had been reported in the medical literature over the past 74 years until 1968. The following is a case report of solitary benign papilloma arising at the opening of the left main bronchus which was treated with bronchoscopic removal at Chest Surgery Depariment, N.M.C. on Sept.

Circumferential Resection and End to End Anastomosis of Mediastinal Trachea for Long Tracheal Stenosis (주기관 긴 협착증 환자의 수술 치험)

  • Recently we have experienced one case of long tracheal stenosis which developed after pulmonary tuberculosis. The patient was 32 years old woman, 165cm in height. She complained severe dyspnea and headache. We could hear the inspiratory wheezing sound and stridor without stethoscope. Preoperative tracheogram and chest CT scan showed long tracheal stenosis from the posterior portion of clavicular head to the upper portion of carina and right main bronchus. Under the general anesthesia, the stenotic segment, about 7.5cm, was resected and end to and anastomosis was performed successfully through the right anterolateral thoracotomy and supraclavicular collar incision. Her postoperative course was uneventful and the patient has remained well till now.

Complete Transsection of Right Main Bronchus by Blunt Trauma -A Case Report- (둔상에 의한 우측 주기관지 완전 절단 치험 1례)

  • Main bronchial injury after blunt trauma is very rare in all bronchial injuries and the pathogenesis is variable and not well known in everycases. We report a case of complete transsection of right main bronchus by blunt trauma. This 24-year old patient was transferred from a local hospital with a chest tube. Because of the severe subcutaneous emphysema and tension pneumothorax, we inserted one more chest tube resulting no obvious interval change. With the impression of bronchial injury, we performed an exploratory thoracotomy. We couldn't proceed bronchoscopy in the operation room because of his unstable vital sign. After opening of the chest wall, we could identify completely transsected right main bronchus. We anastomosed the bronchus with 4~0 Vicryl interruptedly. After operation, the patient was recovered without any complication.
