Development of Impedance Cardiograph and its Application (임피던스 심장기록기의 개발과 응용)

  • The thermodiluton is a standard method to measure cardiac output in clinical medicine. However it has many disadvantages such as expensive instrument and measurement, limited number of measurement, pain, safety problem, and side-effect due to insertion of catheter into heart. Electrical Impedance Cardiography has no such disadvantages and that it can continuously monitor stroke volume, contractility of cardiac muscle, and systolic time interval (STI) as well as cardiac output. While this impedance technique has been widely used and vigorously studied adroad, it is not introduced yet in Korea. Thus an Impedance Cardiograph has been developed in order to introduce this new technique. Its accuracy also has been verified by simultaneous measurement of cardiac output with the thermodilution technique. Finally changes of cardiac function during exercise were also measured.

Modeling and Estimation of Cardiac Conduction System using Hidden Markov Model (HMM을 이용한 심장 전도 시스템의 모델화와 추정)

  • To diagnose cardiac arrhythmia owing to reentry mechanism, cardiac conduction system was modeled by modified Hidden Markov modeled by evaluated. First, simulation of transient conduction states and output waves were made with initially assumed parametric values of cardiac muscle repolariztion time, conduction velocity and its automaticity. The output was a series of onset time and the name of the wave. Parameters determined the rate of beating, lengths of wave intervals, rate of abnormal beats, and the like. Several parameter sets were found to simulate normal sinus rhythm, supraventricular /ventricular tachycardia, atrial /vetricular extrasystole, etc. Then, utilizing the estimation theorems of Hidden Markov Model, the best conduction path was estimated given the previous output. With this modified estimation method, close matching between the simulated conduction path and the estimated one was confirmed.

Effect of Rope-skipping Exercise on the Enhancement of Cardiopulmonary Function (줄넘기 운동 훈련이 심폐기능 항진에 미치는 효과)

  • In an attempt to observe the effects obtained by the regular physical training, nine soldiers performed regularly the rope-skipping for nine weeks. All subjects were healthy and did not experience any special military training Programs. During the course of the training, their cardiopulmonary functions were measured in the resting and the Post-exercise recovery periods, and the values were compared with ones of the pre-trained. The test exercises loaded to the subjects were rope-skipping and step-rising & falling. The results obtained were as follows: 1) By the training, heart rates decreased very significantly in the resting and post-exercise recovery periods. And the effects began to bring out at the early stage, about the 7th day. 2) As the duration of the training increased, the systolic blood pressures decreased meaningfully in the resting and recovery periods. 3) Only in the early recovery phase after the exercise of the rope·skipping, the respiration rates decreased significantly by the training. 4) The lighter the intensity of the test exercise loaded was, the more prominent the effect of the physical training on the cardiopulmonary functions was. The above results suggest that the 9 week training of the rope-skipping would bring about the enhancement of the cardiopulmonary functions.

  • 체성-교감 반사 유발시 동맥혈압의 조절에 관여한다고 생각되는 상부 복외측 연수 내의 심혈관계 세포를 대상으로 흥분발사의 반응특성을 측정, 분석하였다. 심장 박동주기와 시간적으로 동기되는 흥분발사를 보이는 심혈관계 세포를 찾은 후 체성-교감 반사의 승압반응과 감압반응을 유발시키며 각각에 대해 동일한 세포의 흥분발사의 변화를 측정한 결과 5가지 유형의 반응특성을 얻을 수 있었다. 이들 간에 구성되어 있는 회로망 연결을 설명하고자 2 가지 종류의 특성 세포와 적절한 자극 전달 경로 및 세포간 회로망 연결을 도입하여 본 연구의 실험결과를 모두 설명할 수 있는 가장 간단한 죄소 세포회로망 모델을 구성하였다. 최소 모델의 고찰 결과 고유한 감압경로의 존재가능성이 도출되었다.

  • 본 논문에서는 심장 박동 변이 (HRV: heart rate variability) 센서와 표준 기반 사물인터넷 (IoT: Internet of Things) 플랫폼을 활용한 스트레스 측정 시스템을 구현하고 그 활용 방법을 제안한다. HRV를 측정하기 위해 맥파 (PPG)센서와 오픈소스 하드웨어를 이용해 센싱 시스템을 구축하고 표준 사물인터넷 플랫폼을 이용하여 서버 시스템을 구축하였다. 추후 분석을 통해 유추한 스트레스 정도에 따라 플랫폼에 연결된 다양한 기기들을 이용하여 스트레스를 낮추기 위한 개인별 스트레스 관리 솔루션을 구현할 수 있음을 알 수 있다.

Acceleratory Action of Ginseng Glycosides on the Recovery Heart Rate after Strenuous Exercise in Men and Women (격심한 운동후 심장박동수에 미치는 인삼 총배당체의 회복 촉진작용)

  • In 24 men and 12 women recovery of heart rate after strenuous exercise was observed before and after administration of total ginseng glycosides. In men 100 mg of ginseng glycosides were given twice with a 3 hour interval on the day of treadmill test and in women two more administrations of ginseng glycosides were added on the day before the exercise test. The action of ginseng glycosides was interpreted as an adaptogen, i.e., to enhance the recovery to the physiological equilibrium from the displace4 state. Recovery of heart rate to the pre-exercise state was observed for 20 minutes after strenuous exercise of 2 minutes duration on a treadmill (8 km/hr, 15% grade). In men the recovery of heart rate was enhanced$(P<.2{\sim}.4)$ after ginseng glycosides administration. In women the acceleration of recovery heart rate was highly significant $(P<.01{\sim}.05)$ after ginseng glycosides administration. In men and women recovery heart rate at 3 minute of recovery period after ginseng glycosides equaled to the value at 6 minute of recovery period before ginseng administration.

  • Cases of applying game in the medical area are increasing thesedays. This game has been developed to prevent disease in self-nervous system through the control of respiration after monitoring the mental stress and physical stress from the heart rate variability (HRV) of the game user. The game is a kind of a stress relaxation program (SRP) designed not to avoid boring or rejecting mind of users by traditional simple training of continuous repetition, but to increase interaction which is a unique feature of games.

Heterotopic Heart Transplantation in the Rat Model (쥐모델에서의 이소성의 심장이식)

  • In 1964, Abbott and Colleagues published the world's first heterotopic heart transplantation technique in the rat. Their method established circulation by end-to-end anastomoses of the graft's aorta and pulmonary artery to the recipient's abdominal aorta and Inferior Vena Cava(IVC), respectively. In 1966, Tomita et al altered Abbott's technique by employing end-to-side rather than end-to-end anastomoses, thus eliminating the hind leg paralysis that sometimes resulted from Abbott's technique. In order to prevent postsuture hemorrhage (since 7-0 silk suture was the finest available at that time), Tomita's aortic anastomosis was done with double up-and-down continuous suture technique. A single layer continuous anstomosis effected the pulmonary artery-IVC anastomosis. The availability of Nylon monofilament suture made it possible for Ono and Lindsey to use a single layer suture technique for the aortic end-to-side anastomosis in their modified rat heart transplantation. We observed survival time between control group and Immunosuppression(Cyclosporine administration, 10mg/Kg${\times}$4 times postoperatively) group after heterotopic heart transplantation in the rat model. The cyclosporine adminstration group survived longer than the control group, thus we concluded that cyclosporine was based on Immunosuppressive drugs.

Surgical Complications in Heart Transplant Recipients - A Single Center Experience - (심장이식후에 발생한 외과적 합병증 - 단일 센터 경험 -)

  • Background: As the patients who undergo heart transplantation have achieved better survival in recent years, growing number of recipients are at a risk for experiencing surgical complications in addition to rejection and infection. In this paper, we report on our experience with the surgical complications that occurred in heart transplant recipients. Material and Method: From April 1994 to September 2003, 37 heart transplantations were performed at our center by a single surgeon. The indications for transplantation were dilated cardiomyopathy, ischemic cardiomyopathy, valvular cardiomyopathy and familial hypertrophic cardiomyopathy. Result: Twenty postoperative complications required surgeries in 15 patients (41%). The types of operations required were; redo-sternotomy for bleeding (5), pericardiostomy for effusion (4), implantation of a permanent pacemaker (1), right lower lobe lobectomy for aspergilloma (1), removal of urinary stone (1), cholecystectomy for gall bladder stone (1), drainage of a perianal abscess (1), paranasal sinus drainage (1), total hip replacement (1), partial gingivectomy due to gingival hypertrophy (1), urethrostomy (1), herniated intervertebral disc operation (1) and total hysterectomy for myoma uteri (1). The locations of the complications were mediastinal in 10 (27%) cases and extramediastihalin 10 (27%) cases. Conclusion: The relatively high incidence of extrathoracic complications associated with heart transplantation emphasizes the importance of a multidisciplinary approach to the improve long-term survival when managing those complex patients.
