A Study on Auti-extraction Methods of Heart Rate from ECG (ECG 심박수의 자동 추출법에 관한 연구)

  • The heart sends blood to the body with heart rate. When heart rate for men is from 60 to 80 per minute, he is generally normal. However, if heart rate is less than the normal heart rate, the symptom is called by bradycardia. Otherwise, the symptom is called by tachycardia. These symptoms make him even to death. Therefore, heartbeat rate has a very important role in a healthy life. In this study, we studied on auto-extracting methods of heart rates from ECG, and compared them with those measured by naked eyes. The first auto-extracting method employs the 2-order differential equations to extract heart rate. The second method uses the autocorrelation coefficients to detect heart rate. To verify its efficacy and validity in practical applications, these method has been applied to MIT/BIH database.

  • Behcet's disease is classically described as featuring recurrent aphthous ulcers in the mouth and genital organs and as having relapsing iritis. Now it is being recognized as a mul tisystem disorder that involves of the skin, gastrointestinal system, cardiovascular system, lung and the central nervous system as well as the joints, blood vessels and urologic systems. Large vessel diseases are unusual but aneurysm may occur in which the pulmonary circulation may give rise to the massive and often fatal hemoptysis. A 29 year-old man who complained of having dyspnea and hemoptysis during six months visited our hospital. He received right bilobectomy for a mass located in the right lower lobe. He underwent right bilobectomy. The final pathologic diagnosis was a pulmonary artery aneurysm which origina ted from the pulmonary artery.

Giant Venous Aneurysm after Brachiocephalic Arteriovenous Shunt for Hemodyalisis Access -A case report- (혈액투석을 위한 상완동맥-두정맥단락 수술 후 발생한 거대한 정맥의 동맥류 -1예 보고-)

  • We report a case of a 46-year-old man with end-stage renal failure who developed a giant aneurysm after a brachiocephalic arteriovenous shunt. The patient had complaints of pulsating pain and swelling of his left upper extremity The patient had abandoned use of the arteriovenous shunt and had a second arteriovenous shunt procedure over his right extremity. The giant venous aneurysm was removed just distal to his anastomosis. The patient's postoperative course was uneventful.

Psychophysiological Characteristics of Insomnia Patients Measured by Biofeedback System (바이오피드백을 이용하여 측정한 불면증 환자의 정신생리적 특징)

  • Background and Objectives: Insomnia is the most prevalent sleep disorder in the general population and is considered to be a disorder of hyperarousal. The aim of this study was to measure the psychophysiological responses in insomnia patients using a biofeedback system, and to compare them with results from normal healthy subjects. Materials and Methods: Eighty patients with primary insomnia (35 males and 45 females, average age $49.71{\pm}12.91years$) and 101 normal healthy controls (64 males and 37 females, average age $27.65{\pm}2.77$) participated in this study. Electromyography (EMG), heart rate (HR), skin conductance (SC), skin temperature (ST), and respiratory rate (RR) were recorded using a biofeedback system during 5 phases (baseline, stress 1, recovery 1, stress 2, recovery 2) of a stress reactivity test, and average values were calculated. Difference in values between the two groups in each corresponding phase was analyzed with independent t-test, and change in values across phases of the stress reactivity test was analyzed with paired t-test (all two-tailed, p<0.05). Results: Compared to normal controls, insomnia patients had higher EMG in all 5 phases (baseline : $7.72{\pm}3.88{\mu}V$ vs. $4.89{\pm}1.73{\mu}V$, t = -6.06, p<0.001 ; stress 1 : $10.29{\pm}5.16{\mu}V$ vs. $6.63{\pm}2.48{\mu}V$, t = -5.84, p<0.001 ; recovery 1 : $7.87{\pm}3.86{\mu}V$ vs. $5.17{\pm}2.17{\mu}V$, t = -5.61, p<0.001 ; stress 2 : $10.22{\pm}6.07{\mu}V$ vs. $6.98{\pm}2.98{\mu}V$, t = -4.37, p<0.001 ; recovery 2 : $7.88{\pm}4.25{\mu}V$ vs. $5.17{\pm}1.99{\mu}V$, t = -5.27, p<0.001). Change in heart rate across phases of the stress reactivity test were higher in normal controls than in insomnia patients (stress 1-baseline : $6.48{\pm}0.59$ vs. $3.77{\pm}0.59$, t = 3.22, p = 0.002 ; recovery 1- stress 1 : $-5.36{\pm}0.0.59$ vs. $-3.16{\pm}0.47$, t = 2.91, p = 0.004 ; stress 2-recovery 1 : $8.45{\pm}0.61$ vs. $4.03{\pm}0.47$, t = 5.72, p<0.001 ; recovery 2-stress 2 : $-8.56{\pm}0.65$ vs. $4.02{\pm}0.51$, t = -5.31, p<0.001). Conclusion: Psychophysiological profiles of insomnia patients in a stress reactivity test were different from those of normal healthy controls. These results suggest that the sympathetic nervous system is more highly activated in insomnia patients.

Role of Intra-aortic Balloon Pump in High Risk Patients undergoing Off-Pump Coronary artery bypass graft (심폐바이패스 없이 시행하는 관상동맥 우회술시 고위험군 환자에서 대동맥내 풍선 펌프의 유용성)

  • Background : This study aimed to evaluate the usefulness of preoperative placement of intraaortic balloon pump(IABP) in reducing operative risk and facilitating posterior vessel OPCAB in high risk patients with left main disease( 75% stenosis), intractable resting angina, postinfarction angina, or left ventricular dysfunction(ejection fraction 35%). Material and Method : One hundred eighty- nine consecutive patients who underwent multi-vessel OPCAB including posterior vessel revascularization were studied. The patients were divided into group I(n=74) that received preoperative or intraoperative IABP and group II(n=115) that did not receive IABP. In group I, there were 39 patients with left main disease, 40 patients with intractable resting angina, 14 patients with left ventricular dysfunction and 7 patients with postinfarction angina. Ten patients received intraoperative IABP support due to hemodynamic instability during OPCAB. Result : There was one operative mortality in group I and two mortalities in group II. The average number of distal anastomoses was not different between group I and group II(3.5$\pm$0.9 vs 3.4$\pm$0.9, p=ns). There were no significant differences in the number of posterior vessel anastomosis per patient between the two groups. There were no differences in ventilator support time, length of hospital stay, and morbidity between the two groups. There was one case of IABP-related complication in group I. Conclusion : IABP facilitates posterior vessel OPCAB in high risk patients, with comparable surgical results to low risk patients

  • Big data technologies are increasing the need for big data in many areas of the world. Recently, the health care industry has become increasingly aware of the importance of disease and health care services, as it has become increasingly immune to prevention and health care. To do this, we need a Big data system to collect and analyze the personal biometric data. In this paper, we design the biometric big data system using low cost wearable device. We collect basic biometric data, such as heart rate, step count and physical activity from Mi Band, and store the collected biometric data into MongoDB. Based on the results of this study, it is possible to build a big data system that can be used in actual medical environment by using Hadoop etc. and to use it in real medical service in connection with various wearable devices for medical information.

  • The thesis presents a system that continuously collects the human body's physiological vital information at rest with sensors and ICT information technology and predicts diabetes using the collected information. it shows the artificial neural network machine learning method and essential basic variable values. The study method analyzed the correlation between heart rate measurements of BCG and ECG sensors in 20 DM- and 15 DM+ subjects. Artificial Neural Network (ANN) machine learning program was used to predictability of diabetes. The input variables are time domain information of HRV, heart rate, heart rate variability, respiration rate, stroke volume, minimum blood pressure, highest blood pressure, age, and sex. ANN machine learning prediction accuracy is 99.53%. Thesis needs continuous research such as diabetic prediction model by BMI information, predicting cardiac dysfunction, and sleep disorder analysis model using ANN machine learning.

Evaluation of Whitening Efficacy of Natural Product Residue Using Zebrafish Embryos (제브라피쉬 배아를 이용한 천연부산물의 미백 효능평가)

  • Experiments on zebrafish embryo toxicity and whitening efficacy, which is an alternative experimental animal model, were conducted using coffee by-products. As a result of the embryo toxicity test treated with the coffee residue extract, the coagulation rate was 3, 3, and 5% at 24, 48, and 72 hpf and concentration of 125 ppm, respectively. The hatching rate of embryos was 73% at the highest concentration of 125 ppm. In the heart beat rate experiment of zebrafish larva, the heart beat rate after 72 hpf was confirmed to be 153 times/60 s' at a concentration of 125 ppm. The negative control group showed no significant change in heart rate compared to the control group at 148 times/60s', and showed low toxicity. In addition, as a result of evaluating the whitening effect in zebrafish, melanin formation was inhibited as the concentration of the coffee residue extract increased. The results of this study suggest the possibility that naturally derived by-product materials can be used as raw materials for cosmetics, and are expected to be used in the cosmetics industry as an example of research that increases the added value of natural product residue.

The Comparison of Clinical Outcomes of Off-pump versus On-pump Coronary Artery Bypass Grafting in Diabetes Patients (당뇨 환자에서 시행한 Off-pump CABG와 On-pump CABG의 비교 연구)

  • Avoiding cardiopulmonary bypass (CPB) in coronary artery bypass grafting (CAB G) has been known to reduce early mortality and morbidity. Diabetes Mellitus is a significant risk factor for adverse early and late outcomes after CABG. We compared the clinical results of off-pump CABG versus on-pump CABG in diabetes patients. Material and Method: 682 patients (424 off-pump CABG and 258 on-pump CABG) underwent isolated coronary artery bypass grafting between January 2001 and June 2003. Data were collected 242 patient who had diabetes. Among them, 154 patients underwent off-pump CABG and 90 patients underwent on-pump CABG. We analyzed the preoperative risk factors and postoperative results between 2 groups. Result: Two groups did not show statistical differences in age, sex, coronary and operative risk factors. Operative time was significantly shorter in off-pump CABG, however, number of grafts was fewer in off-pump CABG. Postoperative inotropic usage was lower in off-pump CABG. Postoperative CK-MB level was lower in off-pump CABG, and ICU stay and ventilation time was significantly shorter in off-pump CABG. However, there was no statistical difference between 2 groups in operative mortality, reoperation rate, perioperative myocardial infarction, wound infection, renal failure, neurological complications and hospital stay. Conclusion: Off-pump CABG group showed less myocardial damage and early recovery. We concluded that off-pump CABG is the more reasonable technique in diabetes patients although two techniques showed good results. The long-term follow up and prospective study may be warranted.
