Importance of subgingival contour of anterior mandibular region with narrow space (공간이 협소한 하악 전치부에서 치은하 형태(Subgingival contour)의 중요성)

  • A lack of space can be a key challenge when restoring lower anterior teeth using implants, especially for proper implant placement and the limitation on suitable implant selections. Moreover, depending on the shape of the prosthetic, the adjacent teeth and bone around the implant can also have a significant impact, while in certain cases, the implant can have a detrimental effect on the surrounding bone. Therefore, this case series highlights how variations in the subgingival contour can produce different results for the biology of the marginal bone around an implant and the interproximal bone of the adjacent teeth.

  • The mandibular 4 anterior incisor restoration has small teeth and a small space, so the choice of abutment is always a concern. The mandibular 4 anterior incisor extraction is caused by periodontal disease, interdental embrasure greatly open is advantageous of oral hygiene management. Try to make it small diameter with custom abutment, it is limited to the space for the screw. Rather than setting the post site of one body implant to a horizontal cross-section, it is advantageous to set it to a longitudinal cross-section, for interdental embrasure formation. When using an internal bone level implant, rather than using a two-piece abutment, using a one-piece abutment can more effectively secure space for interdental embrasure.

  • In case with severely atrophied mandibular ridges, it may be difficult to achieve adequate retention and stability of dentures through the normal functional impression process of complete dentures. In these patients, an approach using the neutral zone raising technique may be an effective alternative. The neutral zone is defined as the area where the pressure of the tongue towards the outside of the mouth and the pressure of the cheek and lips towards the inside are balanced during the normal oral function of the muscular-nervous system. Complete dentures made through neutral zone impression not only improve retention and stability, but also provide adequate tongue space in the posterior teeth area. Additionally, food residues are reduced in the area around artificial teeth, and aesthetics are improved through appropriate facial support.

  • The patient was a 69-year-old male who had decreased VDO and overbite of the anterior teeth due to severe teeth wear. Complete mouth rehabilitation was planned to restore esthetics and function. Due to the presence of a subgingival finish line, poor fit of the margin of the temporary crown, and insufficient oral hygiene management, gingival overgrowth and bleeding occurred in many teeth, and inaccurate impressions were taken. After dividing each arch and taking partial impressions, a transfer coping was made using pattern resin on each working model die, and after being placed on the abutment teeth, a master cast was made through pick-up impression taking to produce prosthesis. In this case, the final prosthesis produced on the second master cast were fitted in terms of proximal contact, marginal fit, vertical occlusal dimension, facial appearance, esthetics, and occlusion. After complete mouth rehabilitation, the patient was satisfied with function and esthetic appearance.

  • The aim of this study was to evaluate the biomechanical behavior and long-term safety of high performance polymer PEKK as an intraradicular dental post-core material through comparative finite element analysis (FEA) with other conventional post-core materials. A 3D FEA model of a maxillary central incisor was constructed. A cyclic loading force of 50 N was applied at an angle of $45^{\circ}$ to the longitudinal axis of the tooth at the palatal surface of the crown. For comparison with traditionally used post-core materials, three materials (gold, fiberglass, and PEKK) were simulated to determine their post-core properties. PEKK, with a lower elastic modulus than root dentin, showed comparably high failure resistance and a more favorable stress distribution than conventional post-core material. However, the PEKK post-core system showed a higher probability of debonding and crown failure under long-term cyclic loading than the metal or fiberglass post-core systems.

  • The CAD-CAM technique is a rapidly developing field in the dental field and is applied to various fields of prosthetic treatment. Among them, the manufacturing of dentures using the milling technique classified as subtractive manufacturing is one of the commercialized digital full denture manufacturing methods. At the same time, it is possible to more efficiently manufacture a metal framework for implant overdenture by selective laser sintering or melting technique classified as an additive manufacturing method. The purpose of this article is to describe the fabrication of CAD-CAM maxillary complete denture and mandibular implant overdenture as well as its features.

  • The restoration process for patients with collapsed occlusion or those requiring extensive rehabilitation often involves various trials and errors. Accurately transferring the patient's oral information to the articulator is key to producing esthetically pleasing and stable prostheses. This necessitates precise master model fabrication and accurate jaw relation records. For patients undergoing extensive implant-supported fixed prostheses, several techniques for recording the jaw relationship are available, including the use of record bases with wax rims, bite blocks with temporary abutments and pattern resin, or provisional prostheses. Among these, using provisional prostheses directly for mounting on the master model can help minimize errors during the jaw relation recording phase. The patient in this case was a 55-year-old female patient, who presented for implant-supported fixed prosthetic rehabilitation. She exhibited multiple tooth loss due to periodontal disease, necessitating comprehensive mouth rehabilitation involving extractions and implant placements. After evaluating esthetics and occlusion with provisional prostheses, the same provisional prostheses were used for mounting on the master model, which enabled the fabrication of customized implant abutments and zirconia prostheses. The final result was a successful restoration, both esthetically and functionally.

  • The perception of facial esthetics is critically important to orthodontists. A viewpoint to facial esthetics is influenced by various factors and dependent on the perception of observer. The purpose of this study was to examine the differences regarding esthetic viewpoints among orthodontists, to identify attractive profiles preferred to orthodontists and to present the characteristic aspects of attractive profiles upon the degree of facial convexity. 35 persons whose faces were judged as attractive one by S orthodontists were selected out of 133 young Korean women. Soft tissue profiles Identified as a good-profile group were measured and analyzed. And then according to the facial convexity, good-profile group was subdivided to convex (G-Sn-Pg$9^{\circ}$) and straight (G-Sn-Pg<$9^{\circ}$) groups for the purpose of this study. There were statistically no significant differences regarding esthetic viewpoints among S orthodontists(p<0.05), even if there exists prevailing concept that the standard for facial esthetics is substantially subjective. Analysis of esthetic factors and evaluation of esthetic perception for maxillary anteriors of dental students (치과대학생의 상악 전치부의 심미적 요소 분석 및 인식도 평가)

  • Purpose: This research is to investigate factors that affect dental aesthetics and analyze whether individual perception affects and difference of genders affects the esthetic factors. Materials and methods: Seventy dental students of Chonnam University aged from 25 to 35 years old without periodontal problems were included. Maxillary dental cast for participants were made, and standardized photo was taken with a digital camera. Maximum visual width and the position of gingival zenith of maxillary anterior teeth and maximum height of the maxillary central incisors were measured by a measurement program and the shape of central incisor was categorized. Questionnaire was conducted to evaluate esthetic perception. SPSS program was used to perform statistical analysis. Results: The average visual width ratio of right maxillary anteriors was 1.38:1:0.78 and 1.41:1:0.81 for the left which differ from the golden ratio. The width to height ratio for right and left central incisor was 0.84 and 0.83 respectively. The gingival zenith position was determined to be: mostly, central incisors and canines were located in the distal side, lateral incisors were located in the center. The visual width ratio of right maxillary anterior teeth, ratio of width to height of central incisor, gingival zenith position, crown morphology and amounts of gingival exposure upon smiling were not significantly different between genders, and facial patterns and these dental esthetic factors were not related to aesthetic perception. Conclusion: Even though participants had the visual width ratio of maxillary anteriors or ideal width to ideal height of central incisors that did not agree with the golden ratio, they had high satisfaction for dental esthetics. Esthetic perception depends more on subjective judgements of participants than objective indices.

  • The purpose of this study was to evaluate the shade of primary incisors. One hundred and nineteen children 394 maxillary anterior primary incisors were assessed. Digital shade analysing system ($ShadeScan^{TM}$, Cynovad, Canada) was used for measuring the shades This system enabled to improve the accuracy and objectivity The images of the teeth were taken with $ShadeScan^{TM}$ and shade maps of the teeth were obtained with $ShadeScan^{TM}$ software. The shades of three regions (cervical third middle third, incisal third) of the teeth were evaluated. The results were as follows : 1. In cervical third of primary incisors, D2 and D3 were the major shades and D2 was the most frequent shade. 2. In middle third of primary incisors, C1, D2, and B1 were the major shades and C1 was the most frequent shade. 3. In incisal third of primary incisors, A2 was the most frequent shade and D2 shade followed.
