It is important to get clinical ultrasonic images of good quality for accurate diagnosis. In this study, it observed the change of ultrasonic images against setting frequency, dynamic range(DR) and type of probes on ultrasonic scanner. In the experiment it evaluated image of LCS (Low Contrast Sensitivity) targets(-15, -6, -3, +3, +6, +15 dB) of a standard ultrasonic test phantoms(539,551, ATS, USA) similar to solid and cystic lesions. Its imaged from convex (C3-7IM) and linear probe (L5-12IM) on SA-9900 (Medison Ltd, Korea) scanner. The images obtained altering the setting parameters which are frequency(gen, pen, res, harmonic) mode and DR($40{\sim}100\;dB$). The quality of images evaluated compare with the nominal LCS value of target and measured LCS value. The results show that there was no significant changing of quality images altering DR 40, 60, 80, 100 dB against frequency in Convex probe but the image being the highest in LCS target at DR 60 dB, harmonic of frequency mode in the -15 dB target close to cystic lesion. In Linear probe, DR 40 dB, harmonic mode at -15 dB LCS target close to nominal value. It discussed necessity of evaluation about ROC(Receiver Operating Characteristic) from the psychological viewpoint and limit of evaluation from quantified images.

Supernumerary teeth are dental abnormalies in the permanent dentition and the primary dentition. The etiology is unclear, but it may occur due to dichotomy of the tooth bud or hyperactivity of dental lamina. They occur more in the permanent dentition than in the primary dentition, with the most common site being the premaxillary area. Supernumerary teeth can be classified by morphology and position. Supplemental tooth refers to normal shape tooth. The treatment of supernumerary teeth depends on its shape, position, effect on dentition, and child's physiological condition. In this case, supernumerary primary tooth in the maxillary molar area was revealed by radiographical and clinical examination, but it was difficult to determine which is the supernumerary tooth. The tooth on the mesial side was extracted to induce the formation of adequate space and to prevent excessive space loss, and the result was favorable.

This study aims to examine Life Managers for Elderly People Living Alone (LMEPLAs) in Seoul regarding their traumatic experience and the status of their posttraumatic stress disorder and also how they are coping with it in order to find out ways to improve it. As a study method, we investigated LMEPLAs in Seoul through a self-administered survey regarding whether they had faced any traumatic experience, types of their traumatic experience, diagnosis on posttraumatic stress, and the status of their coping with traumatic experience. According to the study results, 186 respondents (37.57%) have been found to indicate either partial or complete posttraumatic stress symptoms, but the status of their coping with it is very poor. The followings are the results of our discussion. First, it is needed to find out life managers suffering from posttraumatic stress disorder and build up a system to manage them consistently. Second, it is necessary to vitalize education about traumatic experience and posttraumatic stress management. Third, it is urgently needed to build up a system to support life managers who have faced any traumatic experience or been diagnosed to have posttraumatic stress disorder. Fourth, it is demanded to cultivate and arrange experts equipped with specialized knowledge and technique. Fifth, it is needed for them to build a network with medical institutes to receive a prompt diagnosis and specialized treatment.
Object : This study aimed to identify the factor structure of Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile (K-CBCL 1.5-5 DP) in infants and toddlers with mental disorders and verify differences in K-CBCL 1.5-5 DP among the diagnosis groups. Methods : The participants were 265 mothers of infants and toddlers with mental disorders who completed K-CBCL 1.5-5 DP. The data was analyzed using AMOS 25.0 and SPSS 25.0. Results : First, the bifactor model was the most suitable for the factor structure of the K-CBCL 1.5-5 DP. Second, there were significant differences among the diagnosis groups, such as communication disorders, pervasive developmental disorders, emotional disorders, and developmental delays. It was confirmed that the pervasive developmental disorder and emotional disorder groups showed significantly higher dysregulation compared with the communication disorder group. Conclusion : This study confirmed that infants and toddlers had dysregulation problems. Using the bifactor model, the multidimensional nature of the K-CBCL 1.5-5 DP was assessed. It was also meaningful that dysregulation could contribute to onset and deepening of symptoms of pervasive developmental disorders and emotional disorders in infancy.

Objectives and Methods:This study investigated clinical characteristics, treatment modality, outcome of 57 children and adolescent inpatients(male 53, female 4) who were diagnosed as pervasive developmental disorder(PDD) by DSM-Ⅳ criteria recent five years. Results:1) The mean age at admission was $96{\pm}28.2$ months, and the mean age at which they first visited treatment facility was $52{\pm}26.6$ months. The mean hospitalization period was $43.7{\pm}31.3$ days. 2) Diagnosis:Twenty-seven(47.4%) of subjects met DSM-Ⅳ criteria for PDD NOS. Fifteen (26.3%) met for autistic disorder, nine(15.8%) met for Asperger's syndrome, and two(3.5%) met for childhood disintegrative disorder. 3) Comorbid diagnosis:The most common comorbid dignosis was attention deficit hyperactivity disorder(23.8%). 4) IQ test:IQ test for twenty-eight subjects was possible. The Average of the subjects was $70{\pm}27.5$. Fifteen(53.6%) of the subjects were approximate or under 70. 5) Neurology Abnormality:EEG findings of eleven(21.2%) subjects were abnormal, brain CT or MRI findings of eight subjects(21.6%) were abnormal. 6) Family Hx:Depressive disorder were found in Eight mothers(14%). Familial loading was found in twenty families(35.1%), and familial loading of PDD was found in three(5.3%). Conclusion:The most important thing for the management of PDD is early detection and early treatment. To do so, multidisciplinary team approach should be emphasized.
The aim of this study was to investigate the impact of depression, somatization, and jaw disability on graded chronic pain of TMD using Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Ninety-three patients (17 men and 76 women, mean$\pm$SD age of 30.1$\pm$12.5 years) diagnosed with TMD based on RDC/TMD axis I criteria were administered RDC/TMD axis II history questionnaire. The relationships between depression, somatization, jaw disability, and each parameters of graded chronic pain (e.g. pain intensity, pain days, disability score, disability days, graded chronic pain scale) were analyzed by multiple regression analysis. The obtained results were as follows: 1. Among 93 TMD patients, the prevalence of low disability group of graded chronic pain scale was 42.0% and high disability group of graded chronic pain scale was 51.5%. 2. Depression did not show any significant influences on pain intensity, pain days, disability score, disability days, and graded chronic pain scale. 3. Somatization showed a significant effect on pain intensity (p<0.01), disability days (p<0.01), and graded chronic pain scale (p<0.01) except for both pain days and disability score. 4. Jaw disability also showed a significant effect on pain intensity (P<0.001), disability days (p<0.01), and graded chronic pain scale (P<0.001) except for both pain days and disability score. Somatization and jaw disability may closely relate to the pain intensity and degree of disability that TMD patients perceive. Therefore, comprehensive understanding of psychological profile and improvement of functional limitation of jaw movements in the patient should be considered to obtain an excellent outcome of chronic TMD management.

Objectives:As increasing number of new antidepressants have been being introduced in clinical practice, pharmacological understanding has been broadened. These changes mandate new information and theories to be incorporated into the treatment process of children with depressive disorders. In light of newly coming knowledge, this review intended to recapitulate the characteristics of new antidepressants and to consider the pivotal issues to develope guidelines for the treatment of depression in childhood and adolescence. Methods:Searching the Pub-Med online database for the articles with the key words of 'new', 'antidepressants' and 'children' ninety-seven headings of review articles were obtained. The author selected the articles of pertinent subjects in terms of either treatment guideline or psychopharmacology of new antidepressants. When required, articles about the clinical effectiveness of individual antidepressants were separatedly searched. In addition, the safety information of new antidepressants was acquired by browsing the official sites of the United States Food and Drugs Administration and Department of Health and Human Services. Results:1) For the clinical course, treatment phase, and treatment outcome, the reviews or treatment guidelines adopted the information from adult treatment guidelines. 2) Systematic and critical reviews unambiguously concluded that selective serotonin reuptake inhibitors(SSRIs) excelled tricyclic antidepressants( TCAs) for both efficacy and side effect profiles, and were recommend for the first-line choice for the treatment of children with depressive disorders. 3) New antidepressants generally lacked treatment experiences and randomized controlled clinical trials. 4) SSRIs and other new antidepressants, when used together, might result in pharmacokinetic and/or pharmacodynamic drug-to-drug interaction. 5) The difference of the clinical effectiveness of antidepressants between children and adults should be addressed from developmental aspects, which required further evidence. Conclusion:Treatment guidelines for the pharmacological treatment of childhood and adolescence depression could be constructed on the basis of clinical trial findings and practical experiences. Treatment guidelines are to best serve as the frame of reference for a clinician to make reasonable decisions for a particular therapeutic situation. In order to fulfill this role, guidelines should be updated as soon as new research data become available.

Sasang Constitutional Medicine has the organized and unique ideas of the theory and the practice. So it is widly used in korea. But It has a problem to be popular. That's the objectification of Sasang constitution. There are so many studies to classify Sasang constitution analytic and objectively. I try to classify the studies to identify Sasang constitution objectively and check the problems. 1. The morphologic studies of Sasang Constitution are dividied into the figure and the face-speech through the morphologics and the phonetics. We need to standardize and make a simple method through computer software. 2. The study of the mind is used by the questionnaire about the telent, the character, and the greed each constitution. Recently QSCC II computer software is made and is used at many clinics in Korea. We need to recheck children and Taeyangin. 3. The symptom and the disease are subjective. So the study of them is usually used by some diagnostic equipments. But these have to be revaluated on the view of Oriental Medicine and Sasang Constitutional Medicine and try to study the objectification of Sasang constitution. And then we must try to objectify Sasang constitutional disease. 4. Disease and symptoms have to be studied much more about the mechanism and the relation. The study of medicine was done the way of the homeopathy using EAV. But that had some problems. So it should depend on the new science.
Purpose: It was well known that cerebral blood perfusion is normal or diffusely decreased in the majority of patients with Parkinson's disease (PD). Actually we interpreted brain perfusion SPECT images of PD patients in the clinical situation, we observed various cerebral perfusion patterns in patients with PD. So we performed brain perfusion SPECT to know the brain perfusion patterns of PD patients and the difference of perfusion patterns according to the sex and the age. Also we classified PD patients into small groups based on the brain perfusion pattern. Methods and Materials: Two hundred nineteen patients (M: 70, F: 149, mean age: $62.9{\pm}6.9$ y/o) who were diagnosed as PD without dementia clinically and 55 patients (M:15, F:40, mean age: $61.4{\pm}9.2$ y/o) as normal controls who had no past illness history were performed $^{99m}Tc$-HMP AO brain perfusion SPECT and neuropsychological test. Results: At first, we compared all patients with PD and normal controls. Brain perfusion in left inferior frontal gyrus, left insula, left transverse temporal gyrus, left inferior parietal lobule, left superior parietal lobule, right precuneus, right caudate tail were lower in patients with PD than normal controls. Secondly, we compared male and female patients with PD and normal controls, respectively. Brain perfusion SPECT showed more decreased cerebral perfusion in left hemisphere than right side in both male and female patients compared to normal controls. And there was larger hypoperfusion area in female patients compared with male. Thirdly, we classified patients with PD and normal controls into 4 groups according to the age and compared brain perfusion respectively. In patient below fifties, brain perfusion in both occipitoparietal and left temporal lobe were lower in PD group. As the patients with PD grew older, hypoperfusion area were shown in both frontal, temporal and limbic lobes. Fourthly, We were able to divide patients into small groups based on cerebral perfusion pattern. There was normal cerebral blood perfusion in 32 (14.7%) of 219 patients with PD, decreased perfusion on the frontal lobe in 45 patients (20.6%), the temporal lobe in 39 patients (17.4%), the parietal lobe in 39 patients (17.9%), the occipital lobe in 40 patients (18.3%), diffuse area in 14 patients (6.4%) and unclassified in 10 patients (4.6%). Fifthly, we compared the results of the neuropsychological test and cerebral perfusion pattern. There was no correlation between two tests except visuospatial function. Conclusion: Various perfusion state were found in patients with PD according to the age and sex. Also we were able to classify perfusion state into several groups and compare the neuropsychological test with cerebral perfusion.
Objective : Postpartum depression(PPD) was known to be caused by many factors including various psychosocial risk factors. This study was performed to identify the psychosocial risk factors for ppd, preliminarily in Korea. Methods : A group of 119 postpartum women, each of whom was at 6 to 8 weeks after delivery was identified at the time when they visited to the child health clinic or postnatal check-up clinic. The risk factors were surveyed by the self-reported questionnaire. The items of questionnaire were consisted of known risk factors in other studies and other possible stress-related factors. PPD was assessed by the Edinburgh Postnatal Depression Scale(EPDS) and the degree of postpartum depression was determined by its score. Results : 16 women(13.45%) in the high risk group were diagnosed as PPD among the 119 women. Risk factors including past experience of depressive symptoms and low level of marital satisfaction were founded more frequently in women in the high risk group than in the low risk group. The score of EPDS was significantly high in the group who experienced depressive symptoms in the past, anxiety or depression during pregnancy, stressful life event during the period of recent pregnancy and postpartum, and who had low level of marital satisfaction. There was a positive correlation between age and the score of EPDS. However, the postpartum depressive symptoms were not influenced by the level of education, job, retirement due to pregnancy and delivery, wanted or unwanted pregnancy, delivery method, feeding method, the hospitalization of infant, expected and real gender of infant. Conclusion : These results suggest that PPD is quite frequent at postpartum period. Various risk factors contribute to the development of PPD. If clinicians pay attention to the risk factors of PPD and give appropriate psychiatric intervention to the mothers during pregnancy and postpartum, it will be easy for the clinicians to recognize and treat PPD in the early stage.
