The Comparison of the Accuracy in Preparing Small Vancomycin Dosage : Single vs. Double Dilution Method (소용량 Vancomycin 주사액 조제방법의 차이에 의한 정확성 비교: 단일희석방법과 이중희석방법)

  • This study was to find a more accurate method fur measuring small vancomycin dosages which are commonly used in neonates by comparing single and double dilution method. For single dilution method, 500 mg of vancomycin powder was accurately measured and reconstituted with 5 ml of distilled water to make a concentration of 100 mg/ml. Volumes of 0.05, 0.1, 0.15, 0.2, 0.3, 0.4, and 0.5 ml, which equal the target dosages of 5, 10, 15, 20, 30, 40, and 50 mg, were measured using syringes made by Shina and each sample was further diluted with 2 ml of $5\%$ dextrose. The solution of 100 mg/ml concentration was further diluted with $5\%$ dextrose to make a concentration of 20 mg/ml. Volumes of 0.25, 0.5, 0.75, 1.0, 1.5, 2.0, and 2.5 ml, which correspond to 5, 10, 15, 20, 30, 40, and 50 mg, were sampled by the same Shina's syringe as in single dilution method and then each sample was further diluted to make a total volume of 10 ml. Each sample was analyzed by HPLC. The measured dosages of each sample in both single and double dilution methods were lower than the target dosages; however, e values in double dilution method were higher than those in single dilution method for seven target dosages. Percent target dosages in single dilution method were 65 to $90\%$, while in double dilution method 91 to $94\%$. Statistically significant difference between two groups was shown in 5, 10, 15, 20, and 40 mg dosages (p<0.05). In conclusion, when preparing small vancomycin dosages lower an 20 mg $(volume{\leq}0.2\;ml)$, using Shina's syringes, the double dilution method has a closer value to the target dosage than single dilution method.

Assessment of Basal Energy Expenditure in Normal Healthy Volunteers and Patients Receiving TPN by Bioelectrical Impedance Analysis (Bioelectrical Impedance Analysis를 통한 건강한 성인과 TPN환자의 기초대사량 산출)

  • Adequate nutrition is important in maintaining optimal health. Malnutrition can expose individual to increased risks of morbidity and mortality. The purposes of this study were to determine the basal energy expenditure (BEE) of Korean healthy subjects and TPN patients using Bioelectrical Impedance Analysis (BIA) method and to compare these values with those predicted by Harris-Benedict equation (H-B). BEE values measured by BIA were compared with predicted BEE values by the H-B formula in 59 clinically stable TPN patients and 65 healthy volunteers. In healthy volunteers and TPN patients, statistically significant differences were not shown between the BEE values measured by BIA (1392.5 Kcal and 1325.9 Kcal) and those predicted by H-B formula (1384.1 Kcal and 1270.1 Kcal). In male volunteers, statistically significant differences were not shown between BEE values measured by BIA (1670.7 Kcal) and the H-B formula (1550.9 Kcal), but in female volunteers, statistically significant differences were shown between BEE values measured by BIA (1194.8 Kcal) and the H-B formula (1265.6 Kcal). In male TPN patients, statistically significant differences were shown between BEE values measured by BIA (1453.5 Kcal) and the H-B formula (1335.9 Kcal), but in female TPN patients, statistically significant differences were not shown between BEE values measured by BIA (1126.4 Kcal) and the H-B formula (1167.2 Kcal). In normal healthy volunteers, $90.8\%$ of BEE values measured by BIA and in TPN patients $89.8\%$ of BEE values measured by BIA were within $15\%$ of BEE values predicted by the H-B formula in non-obese subjects. In conclusion, BEE values predicted by H-B formula or measured by BIA can be applied to non-obese Koreans. However, these values should be confirmed with Indirect calorimetry for Koreans.

Drug Prescribing Patterns for the Treatment of Asthma and Chronic Obstructive Pulmonary Disease in Korea (우리나라에서의 천식 및 만성폐쇄성폐질환 치료제 처방 양상)

  • Purpose: The aim of this study was to investigate drug prescription patterns for the treatment of asthma and chronic obstructive pulmonary disease (COPD) patients in Korea. Methods: Ambulatory adult patients who were diagnosed and received treatment for the asthma (ICD-10 code J45) or COPD (ICD-10 code J44) from January 2009 to September 2011 in two independent secondary hospitals in Korea were enrolled in this study. Prescribed drug lists were generated based on the evidence-based guidelines and prescribed drug dosage forms were identified from the patient medical records and computerized drug prescription databases of the study centers. Results: Total numbers of asthma and COPD patient enrolled in this study were 2,432 and 2,615, respectively. Individual prescription-based accumulated numbers of patient were 12,021 for asthma and 16,584 for COPD. The most commonly prescribed three drugs were oral predisolone, oral formoterol and oral montelukast for asthma and oral formoterol, oral doxofylline and inhaled tiotropium for COPD. Frequencies of oral drugs were 83.4% and 63.3% while inhalers were 16.4% and 30.2%, for asthma and COPD, respectively. Conclusion: The oral treatment was prescribed more in asthma and COPD patients than inhalers. To enhance the compliance of evidence-based guidelines for these chronic airway diseases, more realistic and specific strategies to increase the use of inhalers recommended as primary treatment options for asthma and COPD would be required.

Bioequivalence Evaluation of Commercially Available Choline Magnesium Trisalicylate Tablets in Healthy Volunteers (건강한 지원자를 대상으로 한 시판 Choline Magnesium Trisalicylate 정제의 생물학적 동등성 평가)

  • The bioequivalence of two commercial choline magnesium trisalicylate (CMT) tablets was evaluated in 10 normal male subjects (age 21-27 yr, mean 23 yr) following single oral administrations of two products. Test product was $Trimax^{\circledR}$ tablet (Hyundai Pharm. Ind. Co., Ltd., Korea) and reference product was $Trilisate^{\circledR}$ tablet (Purdue Frederick, U.S.A.). Both products contained 500 mg salicylate. In the study, ten volunteers were administered one tablet of $Trimax^{\circledR}$ or $Trilisate^{\circledR}$ with randomized two period cross-over study. The pharmacokinetic parameters of two products were statistically compared using Student's t-test and ANOVA. When Student's t-test was applied, mean area under the curves (AUC) of $Trilisate^{\circledR}$ and $Trimax^{\circledR}$ were $388.88{\pm}74.99\; {\mu}g{\cdot}hr/ml$ and $390.63{\pm}63.02\;{\mu}g{\cdot}hr/ml$ hrlm!, respectively, which were not significantly different (p>0.05). The mean peak concentrations $(C_{max})$ and mean times to peak $(T_{max})$ of $Trilisate^{\circledR}$ and $Trimax^{\circledR}$ were $71.1{\pm}12.2$ and $72.9{\pm}10.7\;{\mu}g/ml$, and $72{\pm}33$ and $57{\pm}36min$, respectively, which were not significantly different (p>0.05). The mean terminal phase half-lives $(t_{l/2ter})$ of the two products were $2.57{\pm}0.47$ and $2.43{\pm}0.40$ hr, and also they were not significantly different (p>0.05). When ANOVA was applied, the parameters of the two products were not also significantly different each other. Based on the above results, it has been concluded that the bioavailability of $Trimax^{\circledR}$ tablet was not significantly different from that of $Trilisate^{\circledR}$ tablet.

  • The purpose of this study is to assess the usefulness of thermography in patients with musculoskeletal disease. Thermography is noninvasive, easy to reading, objective and physiologic instrument by measuring and imaging infrared energy emitted from skin surface. Thermography can show the skin temperature changes in various conditions of the body with musculoskeletal disease. This literature review was done for the usefulness of thermography in diagnosing musculoskeletal disease. In conclusion, thermography was adapted in radiculopathy, MPS, peripheral neuropathy, RSD, Raynaud's phenomen, TMJ dysfunction, etc. It was useful as a secondary diagnostic method in those diseases, also possible as a primary diagnostic method in RSD, Raynaud's phenomen. And, it might be reliable tool for estimating disease procedure and consequence after treatment. But, the objectivity of the reading and the development of the operating method are required for further adaptation in musculoskeletal disease.

Effect of Electro-acupuncture on Regional Fat by Measuring Thigh Circumference (저주파 전침자극이 허벅지 둘레에 미치는 영향에 대한 연구)

  • Objectives: Although electro-acupuncture is being utilized to reduce weight in Korea, its usefulness in obesity management has not been fully evaluated yet. The objective of this study is to evaluate the effect of Electro-acupuncture on thigh circumference. Methods: 12 healthy volunteers participated in this study. We measured the circumference of both thighs, and treated the thicker one with transcutaneous electro-acupuncture at 8 acupoints, 10 times in total. The electrical parameters were : 1st step frequency set was at 15Hz and the treatment proceeded for 50 minutes, 2nd step at 50Hz for 10 minutes and at 3rd step the acupunctures were removed and electrodes were attached on the lower limb at 166Hz for 15 minutes. In order to evaluate the effect of the treatment, total body weight, percentage of body fat and thigh circumference were measured 4 times. Before the treatment, after 5th and 10th treatment and 1 month after completion of the whole treating procedure. Statistical significance(P<0.05) was determined using the Wilcoxon signed rank test. We did not control diet, exercise and lifestyle of the volunteers. Results: No significant change was shown in body weight after 10 times of treatment. Yet the thigh circumference of effect group significantly decreased after 5th treatment and 10th treatment. The circumference of control group that was not treated by electro-acupuncture significantly decreased too. At first, difference between the two thighs in circumference was obvious, but no difference was left after 10 times of treatments on the thicker thigh. Percentage of body fat was significantly decreased. 1 month after having completed the treatment, there was no significant difference of body weight, thigh circumferences and percentage of body fat. Conclusion: These results suggest that electro-acupuncture effectively regulates regional fat, body fat percentage and body shape.

Correlation between Short Stature and Obese Degree - Intended for the Case of 236 Patient in the Name of Short Stature (저신장과 비만도의 상관성 분석 - 저신장을 주소로 내원한 환아 236명을 대상으로)

  • Objectives The purpose of this study was to investigate correlation between short stature and obese degree. And we also wanted to know the characteristics of patients visited clinic in the name of 'short stature'. Methods Height, body weight, BMI, fat mass, lean body mass, percent body fat were taken intended for 236 patients in the name of short stature. To all the patient questionnaire for growth clinic was drawn up, and ultrasound scan was taken through calcaneous of the right foot. The patients were classified to three groups - short, average, tall group - according to relative position of mid-parental height. It was analysed that the differences between groups in obese degree. Also was investigated correlation between position of short stature and obese degree, and between obese degree and bony maturity. Results & Conclusion 1. The average ages of patients in the name of 'short stature' were $12.69{\pm}3.93$ years old in boys, $10.66{\pm}3.67$ years old in girls. And it seemed to be just before second rapid maturing period. 2. The average BMI were $20.58{\pm}4.07kg/m^2$ in boys, $18.65{\pm}2.85kg/m^2$ in girls, and average percent body fat were $21.99{\pm}7.35%$ in boys, $26.01{\pm}6.35%$ in girls. 3. The numbers of obese children were 34(31.2%) in boys, 19(14.9%) in girls on the basis of BMI. And the numbers were 39(35.8%) in boys, 53(41.7%) in girls on the basis of percent body fat. There was a big difference in case of girls. 4. The numbers of AG(average group) were 48(44.0%) in boys, 60(47.2%) in girls, the numbers of SG(short group) were 35(32.1%) in boys, 31(24.4%) in girls, and numbers of TG(tall group) were 26(23.9%) in boys, 35(27.6%) in girls. 5. There were no significant differences among the groups in BMI, lean body mass, fat mass, percent body fat. Only significant difference in DI(disease index). 6. There were no significant correlation between PH(percent height) and BMI, leanbody mass, fat mass, percent body fat. Only significant correlation in DI(disease index). 7. There were no significant differences between DA(difference between bone age and chronorogical age) and BMI, leanbody mass, percent body fat. Only significant correlation in fat mass.

  • The composite fiber adsorbents containing amidoxime group were prepared and separation properties of uranium ion from seawater were investigated. The amount of uranium adsorption was increased with an increase in adsorption time. When the mole ratio of monomer and comonomer, such as acrylonitrile (AN), tetraethyleneglycol dimethacrylate(TEGMA), and divinylbenzene (DVB), were 1 :0. 1 :0.003, this resin showed the maximum adsorption ability for uranium at a level of pH 8. The amount of uranium adsorption was also increased linearly to one hour with an increase in the content of adsorbent which was added in the composite fiber adsorbents(CFA). The maximum adsorption for uranium of CF A showed at $25^{\circ}C$. Hence, the adsorption ability of CF A for calcium and magnecium ions were increased gradually by the recycling of adsorption and disorption, the adsorption content of their on were 0.3, 0.9mmole/g-adsorbents, respectly. It also showed that the adsorption contents of Ca and \1g ions were much lower than them of uranium. The desorption of uranium on the CF A was carried out , bout 100% within 30min, and the desorption rate of various CF A were equalled.

The Evaluation of Therapeutic Control with Warfarin in Patients with Mechanical Heart Valve Prostheses (인공심장판막 환자를 대상으로 한 Warfarin 치료의 적정성 평가)

  • The goal of oral anticoagulation therapy with warfarin is to maintain INR values within the therapeutic range in order to prevent complications such as bleeding and thrombosis. The purposes of this study were to investigate the current level of anticoagulation control using INR values, to investigate the incidences of thromboembolism and bleeding complications, and to compare the effect of low intensity INR regimen with therapeutic range recommended by ACCP (American College of Chest Physician). Two hundred three patients with mechanical heart valve replacement done at Yonsei University Cardiovascular Center between January 1994 and December 1996 were selected and reviewed retrospectively. The target INR ranges of $2.5\sim3.5$ (ACCP standard) and low intensity INR of $2.0\sim3.5$ were used for evaluation. According to ACCP standard, $51.2\%$ of patients and $31.1\%$ of INR values were within the therapeutic range when average INR and cumulative INR were used, respectively. Applying low intensity INR values of $2.0\sim3.5$, the therapeutic control was achieved in $57.4\%\;and\;90.1\%$, using average INR and total INR, respectively. The incidences of major and minor bleedings were $0.5\%\;and\;26.6\%$, respectively. The incidence of thromboembolism was $0.5\%$. There was no significant difference in terms of complication incidences between INR $2.0\sim2.5\;and\;INR\;2.5\sim3.5$ groups. However, INR values at the time of bleeding were generally high. In conclusion, the evaluation of patients with mechanical heart valve replacement showed low level of therapeutic control with warfarin therapy. This is partially explained by the fact that the physicians at Yonsei University Cardiovascular Center were using lower intensity INR values as a goal than recommended INR. Also, in the near future, systematic anticoagulation service should be implemented at various hospitals in Korea so that patients on anticoagulant therapy can be more closely monitored to be within the recommended INR by ACCP.

Development of Agenda for Pharmaceutical Care Service Research and Development using the Analytical Hierarchy Process (약료서비스 R&D 고도화를 위한 우선순위 기반 전략과제 설정)

  • Background: Although the importance of pharmaceutical care service has been growing to meet the needs of customers with the improvement of clinical and humanistic outcomes, there was not a systematic strategy to promote research in Korea. The aim of this study was to suggest the core agendas for pharmaceutical care services research and development (R&D) considering priorities. Methods: Based on desk researches, we developed R&D agendas for the needs of improving pharmaceutical care services in the area of institution, community, and public health. To determine the priority of agendas in developing pharmaceutical care service, analytic hierarchy process (AHP) analysis was performed by the 14 experts. Criteria and subcriteria were assessed for significance by pairwise comparisons. Then, agendas were evaluated for importance according to each subcriteria, and rank ordered considering the weight calculated by multiplying the importance scores of the criteria and the subcriteria. Results: We derived 25 agendas including 13 for institutional pharmaceutical care service, 8 for community pharmaceutical care service, and 4 for public-health related pharmaceutical care service. AHP model was constructed based on 4 criteria and 8 subcriteria by a hierarchical structure. From the AHP survey, the 'Development of pharmaceutical care service for metabolic and chronic disease' agenda accounted for the highest priority. Conclusion: We have developed the R&D agendas of the pharmaceutical care service which should be promoted. The results should be utilized by the government to nationally support the development of the standards and relevant regulations related to pharmaceutical care services in Korea.
