The Comparison of Pulmonary Function in Jeju female divers (제주 해녀의 폐기능 비교)

  • In this study, we measured lung function and lung diffusing capacity of Jeju women divers who were gathering activities in the sea waters of the low temperature. The results Compared with ordinary women of the same age group were as follows: First, FVC and FVC% of Jeju women divers were significantly higher than the general population. Second, $FEV_1/FVC$ had no significant difference the difference between the general population and Jeju women divers. Third, $FEF_{25{\sim}75%}$ in both groups was in the normal level, especially Jeju women divers groups showed significantly higher FEF results than normal women. Fourth, DLCO is no significant difference between the Jeju divers and the general female group. Despite the advanced age of Jeju women divers These results indicated that adaptation and development of state institutions while performing long-term lung apnea immersion activities.   • A variety of mechanism may generate pain resulting from injury to the central and peripheral nervous system. None of these mechanism is disease-specific, and several different pain mechanism may be simultaneously present in anyone patient, independent of diagnosis. Diagnosis of neuropathic pain is often easily made from information gathered on neurologic examination and from patient history. Although treatment of neuropathic pain may be difficult, optimum treatment can be achieved if the neurologist has a complete understanding of therapeutic options, the mainstay of which is pharmacotherapy. Selection of an appropriate rharmacologic agent is by trial and error since individual responses to different agents, doses, and serum levels are highly variable. An adequate trial for each agent tried is key to pharmacologic treatment of neuropathic pain. Tricyclic antidepressants are first-line agents, although other drugs, including anticonvulsants, local anesthetic antiarrhythmics, clonidine, opiates, and certain topical agents, also offer pain relief in some patient populations. The novel antidepressants venlafaxine and nefazodone are potentially useful new drugs that are better tolerated than tricyclic antidepressants. Also Gabapentine seems an interesting and promising drug for the treatment of neuropathic pain.

A convergence study on the relationships of alcohol consumption patterns to mental distress and self-rated health (성인 음주자의 음주 패턴에 따른 정신건강과 주관적 건강상태에 대한 융합적 연구)

  • The purpose of this study is to assess the relationships of alcohol consumption patterns to mental distress and self-rated health. Data of 19,856 adult drinkers over 19 years of age and older from the Korea National Health and Nutrition Examination Survey were evaluated by multivariate logistic regression analysis. Risks of experiencing perceived high stress, depressive symptoms and suicidal thoughts were higher in alcohol use disorder group in male, whereas the risks were higher in both alcohol use disorder and hazardous drinking groups in female, reflecting higher susceptibility to mental distress. In both male and female, former drinkers presented poor self-rated health compared with low-risk drinkers, although no difference was found in the prevalence of mental distress. Specialized mental health management measures for female drinkers are required, and former drinkers are suggested to be assessed separately from never or low-risk drinkers for their physical and mental health status.

The Use of Internal Obturator Transposition for Perineal Herniorrhaphy in Three Dogs (개에서 내폐쇄근 변위술을 이용한 회음탈장 정복 3예)

  • Three dogs presented to the Veterinary Medical Teaching Hospital of the University of Konkuk and Hangang Animal Hospital with a history of perineal swelling. No tenesmus, stranguria, or any clinical signs other than the swelling was observed by the owners in three dogs. On physical examination, the swelling was observed unilaterally in two dogs and bilaterally in a dog. Digital palpation to the swelling confirmed reducible perineal herniation in two dogs and irreducible perineal herniation in a dog. Plain radiographs revealed that no pelvic or abdominal contents other than the fat were displaced into subcutaneous perineal region in three dogs. Internal obturator transposition herniorrhaphy was performed for correction of perineal herniation in three dogs. Contralateral herniation involving fat was noted after surgery in a dog. The follow-up information was based on physical examination by veterinarians or telephone interview with owners. The owners reported that there was no evidence of complications related to surgery such as sciatic nerve injury, rectal prolapse, wound dehiscence, or perineal hernia recurrence in all dogs.

  • The purpose of this study is not only to define and cause of sarcopenia and frailty due to aging, but also to explore prevention and delay through regular exercise and right nutrition. Sarcopenia and frailty are known to cause frailty, cachexia, osteoporosis, metabolic syndrome and early death due to decreased muscle mass and muscle function caused by aging. The two disease were related to nervous system damage and lack of protein synthesis within the muscles due to the increase in the amount of oxidative stress, and inadequate nutrition and lack of physical activity. It also emphasizes the role and importance of right nutrition and physical activity (such as aerobic exercise, resistance movement, etc) to prevent and improve such syndromes. Conclusions, it is in proposing the supply of high-quality protein, including regular exercise, as the most beneficial way to prevent and overcome aging-related sarcopenia and frailty.

  • Transdermal and topical drug delivery with minimal tissue damage has been an area of vigorous research for years. Our research team has initiated the development of an effective method for delivering drug particles across the skin (transdermal) for systemic circulation, and to localized (topical) areas. The device consists of a laser ablation based micro-particle acceleration system that can be integrated with endoscopic surgical techniques. We have successfully delivered 3μm size cobalt particles into gelatin models that represent soft tissue with remarkable penetration depth.

  • Unilateral neglect is associated with stroke, a neurological disorder caused by cerebrovascular injury, and is a symptom of not recognizing or responding to the opposite stimulus of the cerebral hemisphere, mainly in the right cerebral hemisphere injury. In this paper, we implemented contents using Virtual Reality based on 10 items and contents of Korean Catherine Bergego Scale(K-CBS), which is used as a behavioral evaluation scale for unilateral neglect. Implemented contents can evaluate body disregard, motility disregard, visual and spatial disregard, auditory attention, etc., which are symptoms of unilateral neglected patients, and can confirmed progression of disease quantitatively by measuring time and rotation angle of head during visual recognition. This method is expected to be useful for rehabilitation training using convenient unilateral neglect diagnosis and its applied contents.

Relations between Somatic Symptoms, Depression, Anxiety, and Cognitive Function in Patients with Mild Traumatic Brain Injury (경증 외상성 뇌손상 환자에서 신체적 증상, 우울, 불안과 인지기능의 관계)

  • Objectives : This study was aimed at evaluating the relationship between somatic symptoms, depression, anxiety and cognitive function in the patients with Mild Traumatic Brain Injury(MTBI). Methods : Thirty seven patients with MTBI were selected from those patients who had visited the Department of Neuropsychiatry of Wonkwang University Hospital from 2003 to 2007. To assess and quantify the somatic symptoms, depression and anxiety, Personality Assessment Inventory(PAI) was used. Assessment of cognitive function was carried out by using Korean Wechsler Adult Intelligence Scale(K-WAIS), Rey-Kim Memory Test, and Kims Executive Function Test. The effects of somatic symptoms, depression, and anxiety on the cognitive function were evaluated by Pearson correlation test. Results : Somatic symptoms, depression, and anxiety, all showed inverse correlation to cognitive function. Specifically, 1) an increase in somatic symptoms was associated with a decrease in attention, verbal short term memory, verbal recall and recognition, and visual memory. 2) An increase in anxiety was associated with a decrease in verbal recall and recognition. 3) An increase in depression was associated with a decrease in cognitive function that requires high attention and verbal memory. Conclusion : The patients with MTBI displayed diverse symptoms ranging from cognitive impairment to somatic symptoms, depression, and anxiety. Somatic and emotional symptoms were correlated with cognitive function(especially executive function). Importantly, this study raises the possibility of treating the cognitive impairment associated with MTBI by treating somatic symptoms, depression, and anxiety.

Factors Affecting the Upper Limb Function in Stroke Patients (뇌졸증 환자의 상지기능에 영향을 미치는 요인)

  • This study was carried out to identify the factors which correlated with upper limb function after stroke and to analyze the effect of related factors on upper limb function. The 100 stroke patients(MMSE-K>24) were participated. The upper limb function according to gender, hand dominance, stroke type, affected location, site of paralysis, speech disorder showed no significant difference, and show significant difference according to shoulder subluxation. The upper limb muscle strength(Manual Muscle Testing), proprioception, muscle tone(Modified Ashworth Scale), grip strength(Dynamometer), paint (Visual Analog Scale) showed significant correlations with upper limb function. These predictors explained 77.6% of the upper limb function and the most significant affecting factor of upper limb function was upper limb muscle strength. In conclusion, the upper limb muscle strengthening will be effective strategy to improving the upper limb function and considering the proprioception, muscle tone, grip strength, pain, subluxation will be helpful to develop the strategies.

  • There are many traumatic foot and ankle problems in orthopaedic fields. Though it is not life-threatening problems, a delay in accurate diagnosis and treatments can danger limb function and therefore correct diagnosis can prevent long-term complications. Achilles tendon rupture is relatively common injury for active sports people. Ultrasonography is cost-effective, irradiation -free, effective for evaluation of soft tissues and dynamic analysis. It has been growing importance in Achilles tendon rupture. Ultrasonography is a diagnostic tool in Achilles tendon rupture. Physical examination and patient history is needed to diagnose Achilles tendon without image, but it is missed up by 20% in private clinic. Discontinuity of normal fibrillar architecture seen on an ultrasonographic image is diagnostic for Achilles tendon rupture, and can be accentuated by the performance of dorsi-flexion and plantar flexion, while observing in real time. And ultrasonography is a reliable method for serial observation after surgical treatment or conservative treatment.
