A Review of Postpartum Depression: Focused on Psychoneuroimmunological Interaction (산후 우울의 고찰: 정신신경면역계 상호작용을 중심으로)

  • Purpose: The purpose of this review was to describe a psychoneuroimmunology (PNI) framework for postpartum depression (PPD) and discuss its implications for nursing research and practice for postpartum women. Methods: This study explored the role of hypothalamic-pituitary-adrenal (HPA) axis and inflammation as possible mediators of risk factors for PPD through literature review. Results: From this PNI view, human bodies are designed to respond with the reciprocal interactions among the neuro-endocrine and immune system when they are faced with physical or psychological stressors. Chronic stress induces alterations in the function of HPA axis, and a chronic low-grade inflammatory response is associated with depression. The dysfunctions of cytokines and HPA axis have been observed during the postpartum period. Stress promotes glucocorticoid receptor resistance, which can promote inflammatory responses. This, in turn, can contribute to the pathophysiology of depression. This can especially affect populations at vulnerable time-points, such as women in the postpartum. Conclusion: From a PNI perspective, well-designed prospective research evaluating the role of stress and inflammation as an etiology of PPD and the effect of stress reduction is warranted to prevent PPD.

  • Objectives:Abnormalities in the frontal lobe have been consistently suggested in the pathophysiology of depression. The purpose of this review is to discuss the relationship between the frontal lobe and depression. Methods:Recent researches on the frontal lobe in depression were reviewed and abnormalities in this region were considered within the context of modern functional neuroanatomy. Results:This paper reviewed evidence strongly implicating the frontal lobe as a key brain structure in depression. Conclusion:Taken together, these abnormalities in the function of the frontal lobe implicate interconnected neural circuits in depression and offer suggestions for the themes of future research and treatment. Further research is needed to investigate the association between emotion and the brain in the paradigm of "affective neuroscience".

  • Auditory hallucinations are cardinal feature of psychosis. But the mechanism of hallucinated speech is unknown. The hypothesis that these hallucinations arise from pathologically altered brain monitoring system underlying speech perception is influential. With the help of rapidly developing neuroimaging study technologies, many researchers have been finding new organic deficits in the hallucinated schizophrenic patient's brain. In this article, we reviewed the general appearance of hallucination, a computer simulation model of hallucination and several neuroimaging study findings on hallucinating schizophrenic patients. In conclusion, we presented the presumptive mechanism of hallucination based on the anatomical dysfunction of schizophrenia.

Clinical Report of 3 Cases of Herbal Medicine Treatment Applied with Polysomnography for Breathing-Related Sleep Disorders (수면다원검사를 적용한 호흡관련 수면장애 한약 치료 3례 증례 보고)

  • Objectives: This case report intended to examine how herbal medicine treatment for breathingrelated sleep disorders applied with polysomnography affected the structure of sleep. Methods: From January to November 2019, we retrospectively reviewed the progress records and polysomnography reports of patients who visited Korean medical hospitals in Daejeon and Cheonan. Results: In case 1, the patient complained of daytime sleepiness and snoring and was diagnosed with obstructive sleep apnea disorder and treated with Ukgan-san-gami. There were significant changes in the polysomnographic parameters, such as sleep efficiency, waking after sleep onset, sleep latency, the total arousal index, and the apnea-hypopnea index after herbal treatment. In case 2, the patient complained of frequent waking after sleep onset, daytime sleepiness, and snoring, and was diagnosed with obstructive sleep apnea disorder and treated with Yeoldahanso-tang-gami. There were significant changes in the polysomnographic parameters, such as the total arousal index and the apnea-hypopnea index after herbal treatment. In case 3, the patient complained of daytime sleepiness and was diagnosed with obstructive sleep apnea disorder and treated with Bangpungtongsung-san. There was no significant change in the polysomnographic parameters after herbal treatment. Conclusions: It is meaningful to note that objectively identifying the outcome of treatment for breathing-related sleep disorders is necessary in the clinical practice of Korean medicine.

A Systematic Review and Meta-Analysis of Foot Bath Therapy for Insomnia Disorder (불면 장애에 대한 족욕 치료법의 체계적 문헌고찰과 메타분석 연구)

  • Objectives: The purpose of this study was to provide evidence for the clinical effects of foot bath therapy for insomnia disorder through a systematic review and meta-analysis. Methods: Studies were selected from seven domestic and international literature databases. Data on diagnostic tools, pattern identification, sample size, intervention methods, outcome measurements, results, and adverse events were extracted. The quality of the selected literature was assessed using Cochrane's risk of bias (RoB) assessment tool. Results: A total of 11 studies were included in this study. The primary diagnostic tool for insomnia dis- order was the criteria for the diagnosis and therapeutic effect of diseases and syndromes in traditional Chinese medicine (CDTDSTCM), along with the Chinese classification of mental disorders-3 (CCMD-3). Treatment effects were mainly evaluated by the effective rate and the Pittsburgh Sleep Quality Index (PSQI). The meta-analysis results showed statistically higher effectiveness rates for treating insomnia disorder in the experimental groups receiving herbal medicine foot bath therapy with sleeping or herbal medication compared to the control group. The PSQI was lower in the treatment group receiving herbal medicine foot bath therapy with sleeping medication. The herbal materials used in herbal medicine foot bath therapy mainly included Radix Angelicae Sinensis, Polygonum Multiflorum, Ziziphi Spinosae Semen, Rhizoma Coptidis, Radix Salviae Miltiorrhizae, and Cortex Albizziae. The quality of the included studies was generally poor. Conclusions: This study found that foot bath therapy had clinical efficacy for treating insomnia disorder. The research findings could provide foundational evidence for establishing the clinical basis of foot bath therapy in the treatment of insomnia.

  • The purpose of this review is to address the flow of current neuroscientific researches and to provide for the clinicians with therapeutic evidences for schizophrenia which can help them clinical decision making. Since the very beginning, a lot of scientific studies about schizophrenia have been undertaken. In this review, I describes the evidences focused on development of schizophrenia including neurobiological dysfunction, neurodevelopmental model, Kalirin, and Brain-Derived Neurotrophic Factor(BDNF) and neuroanatomic abnormalities based on neuroimaging studies. In conclusion, schizophrenia influencing on broad impairment of human function such as activities of daily life, occupations, and relationships has been studied underlying causes and treatments, but still remained uncertainty. However, there are plenty of useful evidences available for the clinicians to make a good therapeutic choice.

Expert Survey about Frequencies of Symptoms and Signs in Attention-Deficit/Hyperactivity Disorder Pattern Identification (전문가 설문을 통한 주의력결핍 과잉행동장애 변증의 주요 증상 빈도 연구)

  • Objectives The objectives of this study is to investigate the characteristics of Attention-deficit/hyperactivity disorder (ADHD) pattern identification based on the survey of frequencies of symptoms and signs according to the pediatricians and psychiatrists in Korean Medicine. Methods Eleven pediatricians and fourteen psychiatrists participated in this study. 38 symptoms and signs of 4 ADHD pattern identifications (Kidney yin deficiency and liver yang ascendant hyperactivity 腎虛肝亢, Dual deficiencies in the heart and spleen 心脾兩虛, Phlegm-fire harassing the heart 痰火擾心, Spleen weakness and liver energy preponderance 脾虛肝旺) were used to evaluate the frequencies of ADHD. The differences in frequencies of symptoms and signs amongst ADHD pattern identifications, and the correlations between them were analyzed. Results There were significant differences in the frequencies of symptoms and signs between each pattern identification. Dual Deficiencies in the Heart and Spleen 心脾兩虛 is negatively related with hyperactivity and impulsivity, and positively related with inattention. Phlegm-fire Harassing the Heart 痰火擾心 is positively related with hyperactivity and impulsivity, and negatively related with inattention. Conclusions The results of the characteristics of ADHD pattern identifications from the survey analysis could be used in the clinical practices of ADHD as well as to improve the ADHD pattern identification questionnaire.

  • ■ Objectives This case report is reported to show the effect of Ukgansan and Hwangnyeonhaedok-tang on a patient who complaining myoclonic seizure due to psychogenic non epileptic seizure, occurred after traumatic stress event. ■ Methods A female patients with myolconic seizure was treated with Ukgansan and Hwangnyeonhaedok-tang during 5 days. Then we assessed dyskinesia severity by AIMS(Abnormal Involuntary Movement Scale) every two days til discharge. ■ Results Seizure decreased two days after hospitalization, then disappeared third day. There was no recurrence till discharge. When she revisited outpatients clinic, there was no symptom. ■ Conclusion This case showed the effect of Ukgansan and Hwangnyeonhaedok-tang on psychogenic epileptic seizure.

A Study on the Emotional Characteristics of Adult with Fatigue Using the Core Seven Emotions Inventory-Short Form (피로를 호소하는 성인의 핵심칠정척도 단축형을 활용한 정서적 특성 연구)

  • Objectives: This study was conducted to investigate the emotional characteristics of adults who complained of fatigue using the Core Seven Emotion Inventory-Short Form (CSEI-s) followed by correlation analysis with the Chalder fatigue scale (CFQ) and Psychosocial Well-being Index-Short form (PWI-SF). Methods: In this study, the medical records of 45 participants who complained of fatigue and completed the CSEI-s, CFQ, and PWI-SF were evaluated. Records of a total of 45 adults were analyzed using SPSS (Statistical Package for the Social Sciences, Version 25.0). Frequency Analysis, Descriptive Analysis, Independent t-test, One Way ANOVA, and Correlation analysis were used for data analysis. Results: 1. According to the educational status of the participants, Thought (思) was identified to be significantly higher in below college graduate than in graduate school and above. 2. As the period of complaining of fatigue increased, a significant difference was observed in Depression (憂) and Fright (驚). 3. According to the degree of fatigue, there was a significant difference in Depression (憂) and Fright (驚), and especially Depression (憂) was higher in the middle fatigue group than in the low fatigue group. 4. PWI-SF, Anger (怒), Depression (憂), and Sorrow (悲) showed a positive correlation with CFQ scores. Conclusions: The results suggest that CSEI-s can be used to measure the psychological symptoms of a patient with fatigue.

Psychotherapy for Couples based one Short-Term Body and Mind Korean Medicine: A Case Report (단기 심신일여 부부치료를 통한 관계 개선 증례 보고)

  • Psychotherapy in Korean Medicine is characterized not only by management of mental issues, but also a holistic perspective of the mind and body, which includes physical treatment. In this case report, we describe the efficacy of Korean psychotherapy for couples with physical symptoms of heartache, emotional tension and marital relationships by addressing the challenges at the Mind and Body levels. For the physical treatment of the couple, the wife was treated with a Bunshimgi-Eum and the husband was administered a Cheonwangbosimdan, combined with a psychiatric interview based on Korean Medicine. It involves listening to the couple's story, YiJungBeongi therapy, understanding and learning about vases and defense mechanisms, self-understanding and understanding of the husband through self-understanding and expansion, and husband's understanding of the wife's position, self-interpretation and acceptance. Based on counseling, the couple's personal characteristics and expansion for self-growth, the progress and results of the couple's challenges and relationship improvement in a relatively short period of time are presented. In response, we hope that the evidence based on Korean Psychotherapy supporting the counseling for couples will continue to accumulate. We would like to report and share a few opinions.
