  • Foreign body in the air way occurs most frequently in children and can produce different symptoms and signs, which may be cofused with other disease. Recently we experienced 3 cases of air way obstruction due to foreign body, and removed under general anesthesia with foreign body forceps and brief review of literature was made.

  • Foreign body in the esophagus is not uncommon in the otolaryngological field and esophageal perforation followed by mediastinitis is one of the most serious complications. Authors had experienced such case developed in 69 year old female patient. This woman swallowed a piece of sharp glass accidentally. Severe pain and swelling around the neck developed after ingestion. Marked subcutaneous emphysema was noted on first examination. By esophagoscopy, longitudinal laceration at right lateral wall of the cervical esophagus was noted and a lot of food debries were removed through this perforation, but foreign body could not be found. On third hospital day, patient complained chest pain and dyspnea. Mediastinal widening was noted on chest P-A. Tracheostomy was performed on next day and neck swelling decreased much. In spite of massive antibiotics, mediastinal abscess was developed and external drainage was performed on 15th hospital day. She was discharged on 38th hospital day with marked improvement and recovered completely on follow-up study. Still, we do not know where the foreign body is located.

A Review of Endoscopic Removal Methods in 127 Cases of the Esophageal Foreign Bodies (소아 식도 이물의 내시경적 적출방법 변화에 대한 고찰)

  • Purpose : The aim of this study was to evaluate the latest tendency of esophageal foreign body's extraction and to obtain a consensus from recent trends of indications and techniques of flexible endoscopy of esophageal FB in children. Methods : We retrospectively reviewed medical records of 127 cases with foreign bodies in esophagus at Dept. of Pediatrics and Otorhinolaryngology, Gyeongsang National University Hospital (GNUH) from Jun, 1987 to July, 2001. They were divided into two groups by the kinds of endoscopy : flexible endoscope(66 cases) or rigid endoscope(61 cases). Rigid endoscopy was performed under general anesthesia at Dept. of Otorhinolaryngology but flexible endoscopy was performed without general anesthesia or sedative drugs(midazolam or diazepam). Results : An annual number of cases of two groups were similar from 1991 to 1998. But from 1999, flexible endoscopy was performed actively. Asymptomatic cases were frequently observed in flexible endoscopy(28 cases/66 cases) but swallowing difficulties were frequently observed in the rigid endoscopy group(25 cases/61 cases). Other symptoms were vomiting, irritability, chest discomfort and abdominal pain. The total number of cases with underlying disease(esophageal stenosis, cerebral palsy) was 8. The total number of cases with complications (erosion, ulcer, bleeding, perforation) was 11. The above cases were not correlated between the two groups. In 55 cases(83.3%) of the flexible endoscopic group and 53 cases(86.8%) of the rigid endoscopic group, foreign bodies in the esophagus were removed within 24 hours. Conclusion : We could not find any benefit in rigid endoscopic technique. Flexible endoscopic FB removal can be performed safely and effectively in children by an experienced endoscopist.

  • Frequently, we have been noted foreign bodies in air way, which can produce different signs and symptoms needed for differential diagnosis with other diseases. Occasionally, it can be followed by some pulmonary complications and removal difficulty. Therefore it can be serious and fatal in few cases, especially in children. Three cases were evaluated with clinical and roentgenographic features of air way obstruction due to foreign bodies and discussed with previous many literatures.

A Case of Esophageal Foreign Body Complicated by Pneumothorax (기흉을 초래한 식도이물)

  • Foreign body in the esophagus is not uncommom in the otolaryngological field, but esophageal perforation followed by pneumothrax due to esophageal foreign body is very rare. Authors recently experienced such a case developed in 1 year old male baby. This baby had been treated at local clinic for 2 weeks prior to admission under the impression of U.R.I.. Thereafter foreign body in the esophagus (fine wire pin) with left pneumothorax was detected by chest X-ray and the body was transfered to our hospital. Closed chest tube was inserted on left and under the general anesthesia, foreign body was removed by esophagoscopy. He was discharged on 16th postoperative day uneventfully.

  • 위식도 역류 질환의 전형적인 증상에 반하여 인후두에서 나타나는 애성, 만성 기침, 헛기침, 인후두 이물감등의 비전형적인 증상에 대해 최근 관심이 높아지고 있다. 특히 애성이 주된 증상으로 나타나는 경우 일부에서 후두 후방에 점막의 비후 소견이나 육아종을 보이기도 하지만 많은 경우 후두내 기질적 병변을 찾기 어려울 때가 많으므로 의식도 역류 질환과 연관된 애성으로 진단하기 위해서는 자세한 병력 파악 및 이학적 검진, 그리고 위식도 역류 진단을 위한 검사 둥이 요구된다. 저자들은 인후두 이물감을 주소로 내원하여 위식도 역류 검사상 위식도 역류 질환으로 판명되었던 환자 중 애성이 동반된 환자 20례에 대하여 치료전 음성 검사를 시행하였으며 일반적인 치료 후 8주째 추적 검사를 시행하여 그 결과를 문헌 고찰과 함께 보고하고자 한다.   • We have recently removed three interesting foreign bodies uneventfully. Case 1: In 20 years old boy, the bullet which penetrated his trachea and esophagus through, lodged on second thoracic vertebra. Case 2: In 1 5/12 year old boy, 52 cm metallic neck lace was hung on the bifurcation of bronchus. Case 3: In 17 years old boy, the pin was stuck in the wall of his esophagus.

  • Foreign bodies in the esophagus is a common problem encountered in the ENT field. Small toys were the most common type of esophageal foreign body found in Korea until the government started making coins. Although 10 won coins are most commonly found, there is a tendency for an increased percentage of 100 won coins parallel with the economic development of the country. Recently, the authors experienced a very intersting case in which a 7 year-old child had a 5 won coin lodged horizontally at a level of the third esophageal constriction portion for 4 years, which eventually resulted in esophageal dilatation. A review of the literature of this subject is included.

  • Foreign bodies in the airway are very important and not uncommon conditions in the field of E.N.T., and practically there are many reports of them. Occasionally, it can be followed by serious complications and technical difficulty to remove. Therefore, foreign body in the airway is serious and fatal in some cases. Recently, the authors experienced 8 interesting cases of foreign bodies in the airway and a brief review of literature was made.
