Clinical Study of Extra-anatomic Bypass (해부외 회로 조성술에 관한 임상적 고찰)

  • Background: Extra-anatomic bypass was performed in the patient who could not use anatomic bypass due to many causes. The purpose of this study is to evaluate the efficacy of extra-anatomic bypass surgery. Material and Method: We reviewed 31 patients who underwent extra-anatomic bypass surgery at Pusan national university hospital. We analysed the combined diseases, etiologic diseases, symptoms, patency rate and the factors affecting patency rate retrospectively. Result: There were 26 cases of femoro-femoral bypass and 5 cases of axillo-bifemoral bypass among 31 patients. The mean age was 70.23 years. Combined disease were hypertension, hyperlipidemia, and ischemic heart disease in order of frequency. The indications for surgery were disabled claudication, tissue necrosis, rest pain, and a cute ischemia. We analysed the ages, smoking history, hypertension, ischemic heart disease, severity of limb ischemia, and hyperlipidemia as factors affecting patency rate. We could not find any statistical differences between these factors. The primary graft patency rates were $73.65\%$ one year, $73.65\%$ two year, and $65.46\%$ three year respectively according to the Kaplan-Meier method. Conclusion: Nevertheless extra-anatomic bypass has a relatively low patency rate, it has good merits that is less dangerous, simple and easy re-do surgery compared to anatomic bypass. We think that extra-anatomic bypass is one of the good treatment modalities for the high risk vascular patients.

  • A Frequency synthesizer for mobile-DTV applications is implemented using $0.18{\mu}m$ CMOS process with 1.8V supply. PMOS transistors are chosen for VCO core to reduce phase noise. The measurement result of VCO frequency range is 800MHz-1.67GHz using switchable inductors, capacitors and varactors. We use varactor bias technique for the improvement of VCO gain linearity, and the number of varactor biasing are minimized as two. VCO gain deterioration is also improved by using the varactor switching technique. The VCO gain and interval of VCO gain are maintained as low and improved using the VCO frequency calibration block. The sigma-delta modulator for fractional divider is designed by the co-simualtion method for accuracy and efficiency improvement. The VCO, PFD, CP and LF are verified by Cadence Spectre, and the sigma-delta modulator is simulated using Matlab Simulink, ModelSim and HSPICE. The power consumption of the frequency synthesizer is 18mW, and the VCO has 52.1% tuning range according to the VCO maximum output frequency. The VCO phase noise is lower than -100dBc/Hz at 1MHz at 1MHz offset for 1GHz, 1.5GHz, and 2GHz output frequencies.

  • Speech is complex psycho-physiological mutual process which includes many orofacial organs. Especially, teeth play an especially important role as an articulation organ in conjunction with the lips and tongue. When teeth are missing, the position of tongue and mandible is adjusted to produce correct sound. If a patient's adaptive function is inadequate for modification of malocclusion, prosthodontic and orthodontic interventions are necessary. Missing teeth of young patients should be reconstructed bacause it can cause malocclusion, decreased masticatory efficiency, articulation disorders, esthetic problem, and temporomandibular disorder. Valplast$^{(R)}$(Valplast International Corp, New York, USA) flexible denture is useful for rehabilitation due to its excellent physical properties, esthetics, and small volume. This case is regardingan intellecturally disabled patient with anarticulation disorder caused by missing lower incisors due to trauma. The patient showed improvement in articulation by using flexible partial denture. The purpose of this report is to figure out the influence of missing lower anterior teeth on articulation.

Effect of Calcium Hydroxide on the Microhardness of Root Dentin of Primary Tooth (수산화칼슘 근관충전제의 적용이 유치 치근상아질의 미세경도에 미치는 영향)

  • Calcium hydroxide mixture medicaments can nearly be considered to be the ideal primary tooth filling material. However, long-term application of calcium hydroxide combinations as an intra canal medicament softens dentin. The aim of this study was to evaluate the effect of calcium hydroxide on the microhardness of root dentin of primary tooth. For the study, 60 extractedprimary incisors were divided into 3 groups (no medicament, calcium hydroxide/iodorform mixture, and calcium hydroxide/distilled water mixture). After the cleansing and shaping of canals, calcium hydroxide medicaments were applied and stored for different periods of time (1, 7, 30, 90 days). The root was horizontally sectioned into 2 mm thick specimens and the microhardness was measured using Vickers microhardness tester. The results were as follows : Root dentin microhardness of primary teeth decreased with long term exposure to calcium hydroxide medicaments according to the experimental period and showed statistically significance (p < 0.05). Root dentin microhardness of primary tooth filled with calcium hydroxide mixed with distilled water showed more decrease than filled with Vitapex and showed statistically significance (p < 0.05). Root dentin microhardness of a control group without exposure to calcium hydroxide decreased according to the experimental period and showed statistically significance (p < 0.05).

  • 1959년 한국원자력연구소가 창립됨과 동시에 &Health Physics&, 즉 보건물리라고 하는 명칭과 조직이 탄생되어, 방사선안전관리의 실무와 보건물리의 연구가 시작되었다. 최초 10년간은 선진제국의 보건물리분야의 연구와 기술을 추적하여 우리나라의 방사선안전관리 기술의 기초를 다지는 시기로서 개인방사선모니터링기술, 환경방사선(능) 모니터링기술 및 방사선방어용계측기기의 교정기술 개발에 중점을 두고 연구개발이 추진되었으며, TRIGA Mark-II 연구용원자로의 가동에 따라 원자로 생체차폐체의 건전성 검증에 관한 유익한 방사선량 측정자료도 얻게 되었다. 즉 이 기간은 방사선안전관리의 체제정비 및 기초기술 확립에 노력한 기간이었다. 1970년대는 원자력 연구개발에 대한 기본방향과 정책의 변경등으로 보건물리 연구조직은 방사선안전관리, 환경연구 그리고 방사화학분야로 분산되었으며, 그로인하여 연구개발활동은 거의 정체되어 겨우 방사선안전관리 실무만이 그 명맥을 유지하였다. 그 결과 우리나라 방사선안전관리 및 그와 관련된 연구개발의 기반이 흔들리게 되었으나, 그러한 환경하에서도 방사선량측정평가기술, 방사선차폐설계기술 및 원자로사고시 피폭선량평가기술의 선진화에 필요한 지식을 얻었으며, 방사선 안전관리에 유익한 실무경험도 축적하게 되었다. 1980년대는 통합된 원자력 연구개발체제의 구축으로 방사선작업종사자 및 일반공중의 피폭저감화 기술개발에 필요한 각종 최신기술을 도입하였고, 관리업무에 있어서도 측정의 정확도와 신뢰성향상 및 새로운 관리기술의 개발에 많은 노력을 한 결과, 유익한성과를 얻게된 기간이다. 특히, 이 기간은 방사선안전관리기술의 선진화를 위한 지식이 축적되어 90년대의 방사선안전관리기술자립화를 위한 전환기로서, 이와같이 축적된 기술은 원자력의 평화적 이용에 크게 기여할 것으로 기대된다.서 dithiothreitol를 투여한 군에서는 우라늄단독투여군에 비해 cretinine의 배설이 상당히 증가하였다(P<0.05). 6. 우라늄오염에 의한 신장의 소견에 있어 우라늄단독투여군은 근위곡세뇨관상피의 공포화 및 종창, microvilli와 brush border의 손실, 세뇨관 상피의 괴사가 관찰되었으며, 간장의 충혈, 중심성 괴사 및 모세관 확장증도 관찰되었다. 그리고 sodium bicarbonate와 생리적 식염수를 병행투여한 군과 우라늄을 투여하고 30분이 지나서 dithiothreitol를 투여한 군에서는 우라늄 단독투여군에 비해 높은 방호효과가 관찰되었으나 다른 실험군에서는 큰 효과가 없는 것으로 나타났다. 결론적으로 우라늄의 체내오염시에는 sodium bicarbonate와 생리적 식염수를 가능한 빨리 병행투여하거나 dithiothreitol을 체내오염후 30분이 지나서 투여하는 방법이 우라늄오염에 대한 제염에 매우 유효할 것으로 생각되며, 특히 우라늄에 의한 인체장해를 유의하게 경감시켜줄 것으로 사료되었다.내의 어떤 부위와도 관계가 되는 것으로 간주되는데 이것이 $(^3H)$ QNB가 $(^3H)$ NMS보다 높은 최대 결합능력 $(B_{max})$을 나타낼 이유이다.   • The CT number corresponds to electron density and its influence on dose calculation was studied. Five kinds of CT scanners were used to obtain Images of electron density calibration phantom (Gammex RMI 467), Then the differences between CT numbers for each scanners were ${\pm}2\%$ In homogeneous medium and $9.5\%$ in high density medium. In order to Investigate the influence of CT number to dose calculation, patients' thoracic CT images were analyzed. The maximum dose difference was $0.48\%$ for each organ. It acquired the phantom Images inserted high density material in the water phantom. Comparing the doses calculated with CT Images from each CT scanner, the maximum dose difference was $2.1\%$ in 20 cm in depth. The exact density to CT number conversion according to CT scanner is required to minimize the uncertainty of dose depends on CT number Especially the each hospital with various CT scanners has to discriminate CT numbers for each CT scanner. Moreover a periodic quality assurance is required for reproducibility of CT number.

The Measurement of Ho-166 Absorbed Dose for the Endovascular Irradiation with a Balloon Angio Catheter Using a GafChromic Film (GafChromic 필름을 이용한 Ho-166 의 혈관내 방사선조사를 위한 선량분포 측정)

  • The GafChromic film was used for the dosimetry of a balloon angio catheter filled with the radioisotope HO-166 for endovascular irradiation. The balloon angio catheter was 2 cm long and 3 mm in diameter when inflated. The isotope, Ho-166, was produced by the neutron bombardment using the research reactor in Korea Atomic Energy Research Insititute. Co-60 teletherapy beam was used for making H-D curve for the Gaf-Chromic film. The film dosimetry was measured with a videodensitometer. The radial dose distribution indicated that the absorbed dose dropped to about 20% of the surface dose at the 1 mm away from the balloon surface and at 5 mm position the dose decreased to below 1% of the surface dose. The result also shows that with the specific activity of Ho-l66, 250 mCi/ml it takes 230 seconds to deliver 1200 cGy to the region where is 1mm away from the balloon surface. The concentric isodose curves were also presented. The Ho-166 is an another alternative for endovascualr irradiation to prevent restenosis after PTCA (Percutaneous Trans Coronary Angioplasty)

The Current State of Lens Care in Contact Lens Wearer (콘택트렌즈 착용에 따른 사용자 실태조사)

  • The Hydroxyethyl methacrylate(HEMA) was developped in 1960's and later the use of contact lenses increased. At present more and more people use RGP contact lenses. Due to the wide range of optical corrections available and due to their high oxygen transmission Japanese and Korean people like them. Nevertheless one of nine CL users(beginners) stopped wearing the CL within the first year. We conducted a survey to evaluate the level of patient compliance in lens care and maintenance and to assess practitioner knowledge of contact lens information. Generally 6% of contact lens wearers drop out per year. The aim of this research was to find out what opticians can do to reduce the drop-out rate by getting information on handling, on general data of the patient and on optical prescription. Lens wearer were asked to complete a 10 question survey that focused on lens care-wear modality, lens replacement, storage, symptoms and case disinfection. There were total 180 participants in the study. Most of contact lens wearers have an inadequate understanding of contact lens care system. Therefore, it is important that contact lens specialists place more emphasis on practitioner education about general contact lens fields and reinforce patient education during the dispensing visit.

Diurnal fluctuations of vision in myopes (근시안의 일일 시럭변동에 관한 연구)

  • Subjective and objective visions were measured on young adults(mean 21 yrs, 126 eyes) who were free of any ocular diseases and laser surgery and none wore contact lenses. The aim of this study was to investigate the diurnal variation of vision through subjective and objective measurements. Subjective visual acuity were measured at 5 m three times a day, morning(8:00 AM-10:00 AM), noon(12:00 PM-2:00 PM) and afternoon(4:00 PM-6:00 PM). The instrument used for objective refraction right after visual acuity measurement was Nvision-K 5001(shin-nippon) which unique in being able to disregard subject's accommodation because of its unrestricted viewing conditions. Also, we measured that three times and then calculated the average values. The result showed that an average subjective visual acuity in the morning, noon, afternoon were 0.256(${\pm}0.263$), 0.266(${\pm}0.276$), 0.242(${\pm}0.249$) respectively. Average spherical equivalent power in objective refraction of right eyes showed -3.416 D(${\pm}2.907$), -3.359 D(${\pm}2.735$), -3.297 D(${\pm}2.709$) respectively and dioptric power was decreased from morning to afternoon. Vision changed throughout the day in both subjective and objective measurements nevertheless its variations were statistically insignificant(p<0.05). Therefore it does not seem to matter of time for either visual acuity test or refraction.

  • The beta-galactosidase of Aspergillus niger CAD 1 described in previous report was immobilized to cellulose triacetate. The optimal conditions for immobilization of the enzyme were obtained when 13%(v/v) of the soluble enzyme (349 unit/ml) was entrapped in fiber prepared from 10%(w/v) cellulose triacetate in methylene chloride. The optimal temperature and pH for the activity of the immobilized enzyme were $45^{\circ}C$ and 4.5, respectively, which stowed the same values as those of the soluble enzyme. It was found that Km of the immobilized enzyme for lacotse exhibited high value compared with that of the soluble enzyme. The immobilized enzyme showed good storage stability, reusability, and also hydrolysis of lactose when introduced into skim milk and acidic whey.
