Changes in Multiple Sleep Latency Test Results according to Different Criteria of Sleep Onset (수면시작 기준의 차이에 의한 수면잠복기반복검사결과의 변화)

  • Objectives: The multiple sleep latency test (MSLT) is commonly used as a valid objective measure of sleepiness. The procedure of MSLT is well standardized but the sleep onset criterion is somewhat variable. One epoch of stage 1 sleep is the most commonly used criterion, and the criterion of three epochs of stage 1 sleep is also used. The purpose of this study was to compare the two criteria used to determine sleep onset. Methods: We retrospectively analyzed 60 consecutive MSLT that were performed according to a standaridized protocol. We scored each test using the two different criteria for sleep onset and then statistically analyed the results. Results: Using the different criteria, 20 patients among 60 showed changes in mean sleep latency (33.3%). The extent of change ranged from 1.3% to 38.5% (mean 15.9%). Non-narcoleptic patients showed a significantly higher incidence of change than other sleep disorder patients. Conclusion: Changes in mean sleep latency occurred according to the different criteria of sleep onset. But the difference arising from different criteria was statistically not significant in patients with moderate to severe sleepiness. Considering that 1 epoch criterion for sleep onset is more sensitive in detecting clinically significant sleepiness, the authors suggest that the 1 epoch criterion is more reliable than the 3 epochs criterion.

Spectral Analysis of Hidden EEG Arousal Activity in Periodic Leg Movements in Sleep without Microarousal (미세각성이 없는 수면중 주기성 사지운동증 뇌파의 스펙트럼 분석)

  • Objectives: Periodic leg movements in sleep (PLMS) might be subdivided based upon whether or not they are associated with visible EEG microarousals (MA). MA is considered to be responsible for nonrestorative sleep and daytime fatigue. The American Sleep Disorders Association's (ASDA) scoring rules for MA based on visual analysis of the EEG changes suggest that MA should last more than 3 seconds. However, it has been suggested that visual analysis may not detect some changes in EEG activity. This study is aimed at measuring changes in EEG spectra during PLMS without MA in order to better understand the arousing response of PLMS. Methods: Ten drug-free patients (three men and seven women) diagnosed with PLMS by polysomnography were studied. Spectral analysis of the EEG was performed in each patient on 30 episodes of PLMS without MA, chosen randomly across the night in stage 2 non-REM sleep. We applied stricter criteria for MA compared to ASDA, by defining it as a return to alpha and theta frequency lasting at least 1 second. Results: The mean PLMS index was $16.7{\pm}10.0$. The mean PLMS duration was $1.3{\pm}0.7$ seconds. Comparison of 4-second EEG activity both before and after the onset of PLMS without MA using independent t-test showed that the movements were associated with significant increase of relative activity in the delta band (p=0.000) and significant decrease of activity in the alpha (p=0.01) and sigma (p=0.000) bands. No significant decrease in the theta (p=0.05), beta (p=0.129), or gamma (p=0.062) bands was found. Conclusion: PLMS without MA was found to be associated with EEG change characterized by increase in the delta frequency band. This finding seems to be compatible with the hypothesis of an integrative hierarchy of arousal responses of Sforza's. Considering that the subjects had lower PLMS index and shorter PLMS duration than those of the previous study, it is suggested that an even less severe form of PLMS without MA could induce neurophysiologic change, which may potentially be of clinical significance.

  • 경인 아라뱃길이 2011년 10월 개장을 앞두고 있다. 경인 아라뱃길이 시작되는 경인항 서해갑문 입출거시 예선사용기준에 대한 연구는 선박운항의 경제성 측면에서 매우 중요한 의미를 지닌다고 할 수 있다. 통상 선박의 크기(톤수)에 따른 예선의 소요마력 및 사용 척수는 대상선박의 크기, 모양, 접근수로, 부두시설의 위치 및 종류, 기타 환경조건 등의 영향을 받으며, 선체에 작용하는 수면상부의 풍압력과 수면하부의 유압력을 고려하여 규정하고 있다. 따라서 본 연구에서는 국내외 예선 규정 및 대상선박에 대한 예선 조종시뮬레이션을 통하여 경인항 서해갑문 입출거시 사용되는 예선의 기준을 설정하였다.

Automatic Detection of Stage 1 Sleep Utilizing Simultaneous Analyses of EEG Spectrum and Slow Eye Movement (느린 안구 운동(SEM)과 뇌파의 스펙트럼 동시 분석을 이용한 1단계 수면탐지)

  • Objectives: Stage 1 sleep provides important information regarding interpretation of nocturnal polysomnography, particularly sleep onset. It is a short transition period from wakeful consciousness to sleep. The lack of prominent sleep events characterizing stage 1 sleep is a major obstacle in automatic sleep stage scoring. In this study, utilization of simultaneous EEG and EOG processing and analyses to detect stage 1 sleep automatically were attempted. Methods: Relative powers of the alpha waves and the theta waves were calculated from spectral estimation. A relative power of alpha waves less than 50% or relative power of theta waves more than 23% was regarded as stage 1 sleep. SEM(slow eye movement) was defined as the duration of both-eye movement ranging from 1.5 to 4 seconds, and was also regarded as stage 1 sleep. If one of these three criteria was met, the epoch was regarded as stage 1 sleep. Results were compared to the manual rating results done by two polysomnography experts. Results: A total of 169 epochs were analyzed. The agreement rate for stage 1 sleep between automatic detection and manual scoring was 79.3% and Cohen’s Kappa was 0.586 (p<0.01). A significant portion (32%) of automatically detected stage 1 sleep included SEM. Conclusion: Generally, digitally-scored sleep staging shows accuracy up to 70%. Considering potential difficulty in stage 1 sleep scoring, accuracy of 79.3% in this study seems to be strong enough. Simultaneous analysis of EOG differentiates this study from previous ones which mainly depended on EEG analysis. The issue of close relationship between SEM and stage 1 sleep raised by Kinnari remains a valid one in this study.

A Systematic Study of the Intervention Effect of Social Stories in Children with Sleep Disorders (수면장애 아동을 위한 사회적 이야기 중재 효과: 체계적 고찰)

  • Objective : This study sought to systematically examine the intervention effect of social stories when applied in relation to children with sleep disorders. Methods : Studies available in the SCOPUS, ScienceDirect, PsycArticles, and PubMed databases that were published from 2001 to 2022 were searched. The keywords used for the search were as follows: ("social story" OR "social stories") AND ("sleep" OR "sleep disorders" OR "sleep wake disorder bedtimes" OR "sleep initiation and maintenance disorders" OR "sleep wake disorder" OR "sleep arousal disorders"). Based on the selection criteria, six experimental studies were selected and analyzed. Results : The selected studies were two randomized controlled trials, three individual trials, and one case study. The subjects were mostly children diagnosed with autism spectrum disorder who were school-aged or adolescent. The intervention types were often complex interventions, including social stories and other interventions, while the durations of the interventions varied from one day to more than 40 days. The interventions had a positive effect on the subjects' sleep quality, with night wakings, sleep onset delay, and sleep anxiety all being improved. As standardized assessment tools to evaluate the effectiveness of social stories, the Child Sleep Habits Questionnaire and the Child Behavior Checklist were used in two papers each, and were the most commonly used. As non-standardized assessment tools, each of the four papers used turbulence and sleep diaries as assessment tools. Conclusion : The effect of social story mediation can be divided into sleep quality and sleep-related behavior. In terms of sleep quality, studies showing improvements in night wakings, sleep onset delay, and sleep anxiety accounted for a large proportion of the sample. The detailed effect area of sleep quality showed a significant improvement after the interventions in most studies, and in all six studies analyzed in the present study, the continuation of the effect after the intervention was confirmed via follow-up tests. Thus, the findings of this study are expected to be helpful when applying social stories in children with sleep disorders in clinical practice due to presenting the intervention effects, outcome evaluation tools, and intervention periods in children with sleep disorders in prior investigations involving social stories.

  • 충적하천의 하상과 제방은 흐름에 의해 쉽게 침식되는 사립자들로 구성되어 있다. 흐름이 더욱 강해지면 커다란 입자들도 움직이게 되고 더욱 커지면 결국 모든 입자들이 움직인다. 이렇게 흐름에 의해 하상과 제방을 구성하는 사립자들이 움직이기 시작하는 상태를 한계운동이라 하며, 이때의 흐름조건을 한계조건이라 한다. 한계조건은 이동상 수리학의 시작으로, 이때부터 흐름은 물과 유사의 혼합이라는 이상류가 되어 순수한 물의 흐름보다 더 복잡하게 된다. 개수로는 관수로와 달리 반드시 자유수면이 있으며, 따라서 물과 공기와의 마찰은 상대적으로 매우 작으므로 개수로의 전단응력 분포는 관수로와 달리 근본적으로 비대칭이다. 따라서 전단응력은 수로 바닥이나 측벽에서만 작용하게 된다. 한계조건은 이러한 추상적인 의미에 앞서 바닥이 침식되지 않는 하도나 수로의 설계 등에 기본적인 자료가 된다. 개수로에서 경계면의 전단응력을 힘으로 표시하는 것을 통상 소류력이라 하며, 개수로 경계면에서 전단응력의 분포도; 이른바 단면의 평균 전단응력의 개념을 도입하여 해석하고 설계기준으로 제시되고 있다. 본 연구에서는 자연하천과 유사한 조건의 건설기술연구원 하천실험센터의 급경사수로에서 연구를 진행하였으며, 기존 연구를 바탕으로 제작된 소류력 측정장치를 이용하였다. 하천의 설계나 평가에 적용되는 평균 소류력 개념은 복잡한 난류흐름에서 평가지표로써 대표하기 힘들기 때문에 유사 하천환경의 바닥에서 발생하는 소류력을 직접 측정하고자 연구를 진행하였고, 연구결과를 기존의 소류력 산정방법과 비교하였다. 본 연구에 사용된 장치는 실규모 실험을 위해 제작되었으며, 실규모 적용성 검토를 위해 실험실에서 충분한 검증실험을 거친 후 실규모 실험에 적용하였다.

Algorithms for the Accuracy Improvement of the ADCP Discharge Based on the Correction of Velocity and Bathymetry Measurement (ADCP 유속 및 수심자료 보정을 통한 유량 관측 정확도 향상 알고리즘 개발)

  • ADCP는 하천 단면에서 매우 짧은 시간에 유속과 수심을 정밀하게 측정할 수 있어 재래식의 부자나 프라이스 유속계를 활용한 방식에 비해 유량 관측 정확도가 매우 높고 유체의 흐름장 분석 등 부가적인 정보를 제공하여 최근 국내외에서 매우 활발하게 이용되기 시작하였다. 또한 고정식 수위계나 유속계를 활용한 실시간 유량관측 시 요구되는 유량-수위관계곡선식 검보정에도 활용되고 있다. 하지만 ADCP는 난류나 하천 부유물, 낮은 수심 등으로 음향신호의 교란으로 인해 발생하는 관측 오차로 인해 유속이나 수심을 과다 혹은 과소 산정하여 유량 관측 정확도를 현저히 낮추는 경우가 종종 있어 왔다. 그리고 미세한 셀 단위의 유속 및 수심관측 자료와 측정되지 않은 수면, 바닥, 하안 부근의 영역을 고려한 ADCP 유량 관측 알고리즘의 복잡성으로 인해 일부 관측오차의 수정을 통한 유량 보정이 매우 까다로운 실정이다. 본 연구에서는 ADCP 제작사 별 유량 산정 알고리즘을 파악하여 유속 및 수심 자료의 보정을 통해 유량을 재계산할 수 있는 알고리즘을 계발하였다. 또한 ADCP의 에러속도를 기준으로 통계적인 방법을 통해 과다 혹은 과소 산정된 유속을 필터링하고 수심을 보정하는 알고리즘을 개발하여 원 관측값의 정확도를 높였고 보정된 관측값을 유량 산정에 반영시켜 유량 관측 정확도를 향상시키고자 하였다. 본 알고리즘은 국내외에 다양한 현장조건에서 관측된 ADCP 자료를 바탕으로 적용되어 그 효용성을 입증하였다.

A study on the appropriate size of rainwater utilization facilities in the urban areas (도시지역에서의 빗물이용 적정 규모 분석)

  • 기후변화로 인한 전지구 미래는 호우 및 가뭄 등 자연재해의 발생이 증가할 것으로 전망되고 있으며, 불투수율이 높은 도시의 경우 극한강우에 따른 유출량 증가할 것으로 전망된다. 환경부에서는 물순환 선도도시에서 유출량, 불투수면, 비점오염 등을 저감하기 위해 LID(Low Impact Development)를 도입하고 있다. 본 연구에서는 도심지 내 LID 기법 중 인공습지를 이용하여 용량변화에 따른 빗물 가용 수량을 정량적으로 분석하고자 하였다. 연구대상지의 면적은 444,081.5m2이며, 유출계수는 토지이용별 면적 및 토지이용별 기초 유출계수를 이용하여 평균유출계수 0.72를 적용하였다. 인공습지의 총 용량은 2,244.8m3이며, 강우 시 월류웨어로 유입되는 지표수를 집수매거로 빗물을 취수하는 방식으로 구축될 예정이다. 대상지의 계획취수량은 150m3/day이며, 70m3/day를 빗물로 취수하는 경우와 150m3/day를 빗물로 취수하는 경우 2가지 시나리오를 대상으로 최종 취수가능여부를 분석하였다. 연간 빗물 취수 가능한 인공습지 용량을 분석하고자 부산관측소 강우자료를 이용하였으며, 강수량이 많은 6월을 시작으로 취수 가능량을 산정하기 위해서 18.06.05 ~ 19.12.31(약 1년 6개월) 강우자료를 이용하였다. 또한, 장기분석을 위해 부산관측소의 2011년 ~ 2020년 자료를 활용하였으며, 총 강수일수는 979일로 총 강수량은 16,139.8 mm로 나타났다. 연간 빗물을 항시 취수하기 위해서는 70m3/day를 빗물로 취수하는 경우 2,357.0m3의 인공습지 용량이 요구되었으며, 150m3/day를 빗물로 취수하는 경우 5,567.8m3의 인공습지 용량이 요구되었다. 또한, 10년 강우에 의한 빗물 취수 특성 분석은 70m3/day를 빗물로 취수하는 경우만 고려하였다. 분석 결과 강우 시작일을 기준으로 58일 동안 빗물 취수가 불가능한 것으로 나타났으며, 2012년도 1일, 2017년도 32일, 2018년도 8일, 2019년도 13일 동안 취수가 불가능한 것으로 나타났다. 70 m3/day의 빗물을 취수하기 위해 인공습지는 4,356.5m3의 용량이 필요한 것으로 나타났다.

The Characteristics of a Life Cycle of a Theme Park - A Case Study of Everland in Yongin, Korea - (테마파크 에버랜드의 수명주기 특성)

  • The purpose of this study is to analyze the characteristics of a life cycle of a theme park through looking at Everland in Yongin, Korea. The life cycle of Everland is divided into four stages. In "initial development" stage(1976 - the mid-80's), Everland become inclined the characteristics of an amusement park by constructing the amusement facilities in the Flower Center(botanical garden). In "growth" stage(the mid-80's - the early 90's), Everland had full grown to the amusement park and inclined to a theme park. In "maturity" stage(the early 90's - the mid-90's), many visitors were not motivated to re-visit Everland because the facilities and events did not change often. Therefore, Everland makes plans for multiple theme park. In "rejuvenation" stage(after the mid-90's), Everland has practiced the plans for multiple theme park and has become the family and accommodating-oriented theme resort park. Since 1976 when Everland was opened, it has evolved from an amusement park to a theme park by creating various and continuous innovations in facilities and events.
