Heterogeneous Responds to Demand and Supply Oil Price Shocks: Evidence from Korea (수요와 공급 요인의 유가쇼크에 대한 한국 경제의 상이한 반응)

  • The article studies macroeconomic effects of the oil shock for Korea, which is a representative emerging economy of Asia and a small open economy. This article analyzed the macroeconomic effects of oil shocks in terms of demand and supply. In the case of Korea, oil price shocks different responds depending on factors of shock. Oil supply shock have led to a decline in industrial activity and interest rate, and oil specific demand shock have shown the greatest increase in interest rate relative to other oil price shocks. In addition, oil demand shock driven by economic activity showed that the comsumer price and the exchange rate are the largest compared to the oil shock caused by other factors. Therefore, policy makers will need to identify the source of the oil shock.

  • A군 연쇄구균에 의한 독성 쇼크 증후군은 기저질환이 없는 젊은 사람에서 쇼크와 다장기 부전증을 일으키는 신종 질환이다. 이 질환은 진행이 매우 빠르고 치명적이기 때문에 신속한 진단과 항균제 투여, 괴사조직의 수술적 제거가 필요하고 수액 주입 혹은 심근 강화제 등으로 쇼크를 적극적으로 치료를 해야 한다. 11세 여자 환아가 쇼크 목 부위 연조직 괴사, 급성 호흡부전, 신부전 및 패혈증으로 사망하여, 연쇄구균성 독성 쇼크 증후군(streptococcal toxic shock syndrome)으로 진단 받았기에 보고하는 바이다.

Shock Thyroid in a Patient with Septic Shock: A Case Report and Literature Review (패혈성 쇼크 환자에서 동반된 쇼크 갑상샘: 증례 보고 및 문헌 고찰)

  • Shock thyroid is a rare manifestation of the CT hypotension complex and can be diagnosed when thyroid and perithyroidal edemas are observed on CT during the onset of shock. Shock thyroid can be a useful CT sign for decompensated shock. This condition is reversible and recovers rapidly with adequate treatment. We present the case of an 84-year-old female with septic shock, exhibiting CT features consistent with a shock thyroid. We also reviewed the clinical and radiological findings reported in the literature. The present case emphasizes that shock thyroid can be an early indicator of impending hemodynamic instability and has important prognostic and therapeutic implications.

  • Inlet hammershock is the critical loads condition for designing the inlet duct structure of a fighter. The sudden flow reduction in engine compressor causes inlet hammershock with high pressure. The traditional method was used to combine extreme conditions (maximum speed, sea level altitude, and cold day) to analyze this compression wave inlet hammershock pressure. However, after the 90s there have been papers that presented the probabilistic approach for the inlet hammershock to achieve the appropriate design pressure. This study shows how to analyze the inlet hammershock pressure by making practical use of the Republic of Korea Air Force real flight usage data under probabilistic approach and then analyze approximately 30% decreased inlet hammershock pressure compared with the traditional valve.

  • Vasodilatory shock has been implicated in life-threatening complications after cardiac surgery. This syndrome may result from the vasopressin deficiency following cardiopulmonary bypass (CPB), which do not respond to fluids or usual intravenous inotropes. We used arginine-vasopressin in adults with vasodilatory shock coming on cardiopulmonary bypass. Therefore, we report these cases with a review of articles.

  • 이 논문에서는 백서를 이용한 출혈성 쇼크 실험에서 수집되는 혈압 데이터 분석을 위한 프로그램을 개발하였다. 일반적인 혈압 데이터는 기존 프로그램을 이용하여 분석이 가능하지만 출혈성 쇼크 실험 중에 획득되는 혈압 데이터의 경우 일반적인 경우와 다른 모양의 파형이 나타나는 경우가 있으며 파형의 크기 변동이 크다. 이런 문제들로 인해 기존 프로그램으로 분석하는데 어려움이 있어 출혈성 쇼크 실험에 적합한 프로그램을 개발하였다. 본 논문에서 개발한 프로그램을 이용하여 혈압 파형을 분석한 결과 기존 프로그램에서 잘못 분석된 이완기 혈압을 정상적으로 분석 할 수 있었다. 또한 전체 구간 데이터를 분석하면서, 혈압 파형 크기의 급격한 변화로 인해 잘못된 수축기와 이완기 혈압이 출력되던 문제점을 해결 할 수 있었다. 이렇게 분석된 데이터를 이용하여 추후에 다양한 방법의 혈압분석이 가능할 것으로 사료된다.

  • 본 논문에서는 쇼크(shock) 그래프 기반의 뼈대 특징을 이용하여 모양 정보를 분류하기 위해 그래프 편집 거리(edit cost) 기반의 k-means 군집화 알고리즘을 적용하는 방법을 제안한다. 본 논문에서 제안된 방법에서는 먼저 질의 영상과 대상 데이터베이스 영상으로부터 뼈대 기반의 쇼크 그래프를 추출한 후 종점(end points)과 분기점(branch points)을 가중치를 이용하여 적응적으로 선택한다. 그런 다음, 두 영상 사이의 편집 거리를 구하여 이를 k-means 군집화 알고리즘의 거리 척도로 적용함으로써 대용량의 영상을 보다 효과적으로 분류한다. 성능을 평가하기 위해서 제안된 알고리즘을 MPEG-7 데이터베이스에 적용하였으며, 그 결과 제안된 영상 분류 방법이 기존의 영상 분류 방법에 비해서 보다 효과적으로 모양 기반의 영상을 분류하였음을 확인하였다.

Arginine Vasopressin Therapy of Vasodilatory Shock after Cardiac Surgery (심장 수술 후 혈관 확장성 쇼크가 발생한 환자에서 바소프레신 투여 요법)

  • Background: Vasodilatory shock has been implicated in life-threatening complications after open heart surgery, where the systemic inflammatory reaction is attributed to the cardiopulmonary bypass(CPB). The secretion of arginine vasopressin(AVP) has been found to be defective in a variety of vasodilatory shock states and administration of AVP markedly improves vasomotor tone and blood pressure. So we reviewed our experience of AVP therapy in patients with vasodilatory shock following heart surgery using CPB. Material and Method: From January 2004 to July 2006, we reviewed the records of patients who received AVP therapy for vasodilatory shock following heart surgery using CPB. Vasodilatory shock was defined as a mean arterial pressure lower(MAP) than 70 mmHg, a cardiac index greater than 2.5 $L/min/m^2$, peripheral vascular resistance lower than 800 $dyn/s/cm^5$, and vasopressor requirements. The hemodynamic responses of patients who received AVP therapy for vasodilatory shock after cardiac surgery were analyzed retrospectively. Result: One hundred ninety nine open cardiac surgery patients were consecutively included in this study. Twenty two patients(11.1%) met criteria for vasodilatory shock. Despite the administration of high dose catecholamine vasopressor, all patients were hypotensive with a mean arterial pressure less than 70 mmHg. AVP therapy increased MAP from $53.3{\pm}7.4\;to\;82.0{\pm}12.0$ mmHg at 1 hour (p<0.001) and decreased other vasopressor requirements from $25{\pm}7\;to\;18{\pm}6$ at 1 hour(p<0.001) and individually maintained it for 12 hours. Conclusion: Our date suggest that AVP may be a safe and an effective vasopressor in patients with vasodilatory shock. In patients exhibiting vasodilatory shock after heart surgery, replacement of AVP increases blood pressure and reduces catecholamine vasopressor requirements.

Phospholipase A2 Contributes to Hemorrhage-induced Acute Lung Injury Through Neutrophilic Respiratory Burst (출혈성 쇼크에 의한 급성 폐손상에서 Phospholipase A2의 활성화에 의한 산화성스트레스의 역할)

  • Background : The present study was carried out in association with neutrophilic respiratory burst in the lung in order to clarify the pathogenesis of acute respiratory distress syndrome(ARDS) following acute severe hemorrhage. Because oxidative stress has been suggested as one of the principal factors causing tissue injury, the role of free radicals from neutrophils was assessed in acute hemorrhage-induced lung injury. Method : In Sprague-Dawley rats, hemorrhagic shock was induced by withdrawing blood(20 ml/kg of B.W) for 5 min and the hypotensive state was sustained for 60 min. To determine the mechanism and role of oxidative stress associated with phospholipase A2(PLA2) by neutrophils, the level of lung leakage, pulmonary myeloperoxidase(MPO), and the pulmonary PLA2 were measured. In addition, the production of free radicals was assessed in isolated neutrophils by cytochemical electron microscopy in the lung. Results : In hypotensive shock-induced acute lung injury, the pulmonary MPO, the level of lung leakage and the production of free radicals were higher. The inhibition of PLA2 with mepacrine decreased the pulmonary MPO, level of lung leakage and the production of free radicals from neutrophils. Conclusion : A. neutrophilic respiratory burst is responsible for the oxidative stress causing acute lung injury followed by acute, severe hemorrhage. PLA2 activation is the principal cause of this oxidative stress.

Immunotoxicity Study of Separated Antigen from Helicobacter pylori. (Helicobacter pylori로부터 유래된 항원의 항원성에 관한 연구)

  • The anaphylaxis shock reaction on the whole cells of H. pylori exhibited a symptom of slight illness for the first and second medication of causing antigen at an antigen concentration of WC (H) $60\;{\mu}g/100\;{\mu}l$ for WC (H) and no anaphylaxis shock symptom was observed at an antigen concentration of $20\;{\mu}g/100\;{\mu}l$ for WC (L). In the case of anaphylaxis shock reaction on the crude urease, no symptom was observed at an antigen concentration of $20\;{\mu}g/100\;{\mu}l$ for both urease (L) and urease (H). In the heterologous passive cutaneous anaphylaxis (PCA) test using a guinea pig-rat, no positive reaction was detected in all the medication groups of WC (H), WC (L), urease (H) and urease (L). In the skin sensitization test, it was observed that the best antigen concentration not causing skin disorder at each of $80\;{\mu}g/100\;{\mu}l$, $40\;{\mu}g/100\;{\mu}l$, $20\;{\mu}g/100\;{\mu}l$, and $20\;{\mu}g/100\;{\mu}l$ was $40\;{\mu}g/100\;{\mu}l$.
