Evaluation of Renal Uptake Rate in 99mTc-DMSA Scan on Pediatrics (소아 99mTc-DMSA 검사에서 신장 섭취율의 평가)

  • The aims of this study were to evaluate the difference of renal uptake rate in $^{99m}Tc-DMSA$ scan on pediatrics by including the bladder. Phantom and Clinical studies were performed. In the phantom study, we put $^{99m}TcO_4{^-}$ (300uCi, 11 MBq) in 3cups filled with distilled water at the rate 1:1:0, 1:1:0.5, 1:1:1, 1:1:2 and were placed Lt kidney, Rt kidney and bladder position on the table. To acquire the image, we used Symbia-E gamma camera from Siemens with preset count method(400,000 counts). In quantitative analysis, the counts of drawing ROIs on the phantom were analyzed. In clinical studies, we analyzed the 20 pediatrics who were examined by $^{99m}Tc-DMSA$ scan. At first, the images were acquired with both kidney and bladder. Secondly we acquired images after shielding the bladder. And the data were compared using a pared t-test by SPSS(ver.22.0). As a result of renal phantom's experiment, we compared with average of uptake rate(%), 1:1:0 was Lt 43.32%, Rt 45.97%, 1:1:0.5 was Lt 35.79%, Rt 36.89%, 1:1:1 was Lt 29.68%, Rt 31.45% and 1:1:2 was Lt 22.89%, Rt 24.32%. There was no correlation between the zoom and uptake rate. The results of patient were that excluded bladder was $29.83{\pm}8.81%$(Lt), $24.29{\pm}6.66%$(Rt) and included bladder was $26.65{\pm}8.03%$(Lt, $21.78{\pm}6.24%$(Rt). This is deemed statistically significant (p<0.05). Renal uptake rate was undervalued because the counts of bladder were included in the total counts.

SUPERNUMERARY PRIMARY TOOTH IN THE PRIMARY LATERAL INCISOR REGION (상악 유측절치 부위의 유치 과잉치에 관한 증례 보고)

  • Most supernumerary primary teeth erupt in the primary lateral incisor region and are reported to occur in 0.03 to 1.9% in the primary dentition. Most parents fail to recognize them because of their normal shape, eruption and exfoliation. Several theories have been proposed for the etiology of hyperdontia and the dental lamina hyperactivity theory is most widely accepted. When a supernumerary primary tooth develops, it is reported that there also exists a supplemental or rudimentary form of a succedaneous supernumerary tooth. Supernumerary primary teeth do not need specific treatment as 75% of them erupt and exfoliate normally Only periodic radiographic exams are necessary to find out whether there is any problem with the eruption of the succedaneous tooth. In the following two cases, a supplemental supernumerary primary tooth with a loss of the anterior developmental space was observed at the maxillary lateral incisor region. Radiographic examination revealed an impacted succedaneous supernumerary tooth at the palatal side of the supernumerary primary tooth.

TRAUMATIC ROOT FRACTURE IN YOUNG PERMANENT TEETH : A CASE REPORT (미완성된 근첨을 가진 영구치 치근 파절의 치유에 관한 증례)

  • A 7-year-old male was refered to Department of Pediatric Dentistry, Wonkwang Dental Hospital for treatment of a traumatic injury to the teeth of the maxillary anterior region of the mouth. His right central incisor presented subluxation and root fracture, the left central incisor had suffered intrusive luxation and root fracture. The initial treatment involved reposition and fixation of the teeth with 0.5mm stainless steel wire and composite resin. The patient was submitted for clinical and radiographic fallow-up. After 4 years, radiographically the right central incisor seemed to be healed by hard tissue union and showed to be indistinct fracture line, intact lamina dura. The left central incisor radiographically was healed by interposition of bone and connective tissue and showed to be distinct horizontal fracture line separating the fragments, and pulp canal obliteration. In clinical examination, the teeth showed a normal response to elective pulp test, percussion and mobility test. Pulp survival after injuries appears to be dependent upon the type of luxation injury, age of patient, stage of root development and degree of dislocation. In this case, the two teeth with incomplete root formation were suffered different type of injury by trauma and has showed different healing aspect.

Causative Organisms of Community Acquired Urinary Tract Infection and Their Antibiotic Susceptibility at a Secondary hospital in Korea (2차 병원에서의 지역 사회 획득 요로 감염의 원인균 및 항생제 감수성 분석)

  • Purpose : We investigated the causative organism and its antibiotic susceptibility of community acquired urinary tract infection (UTI) in children at a secondary hospital to test the adequacy of the current guidelines. Methods : Children diagnosed with UTI at the Department of Pediatrics, Kwandong University MyMyongji Hospital by pyuria and bacterial growth of greater than $1.0{\times}10^5CFU/mL$ on clean catch midstream urine from January 2005 to December 2008 were studied retrospectively. The epidemiologic data, causative organism, and the antibiotic susceptibility were analyzed. Results : Sixty two children were diagnosed with sixty four cases of UTI's. Two bacteria were isolated in one case and thus data on 65 urine cultures were analyzed. The male:female ratio was 1.6:1 and 78.1% were less than 12 months of age. Escherichia coli was the predominant cause consisting of 53 cases (82.8%) of the cases. K. pneumoniae (5), Enterobacter (4), Enterococcus (1), $\beta$-streptococcus (1), Diphtheroides (1) were isolated. The antibiotic resistance of E. coli were as follows; ampicillin 69.8%, cefotaxime 1.9%, gentamicin 15.1%, amikacin 0.0%, levofloxacin 1.9%, and trimethoprim/sulfamethoxazole 26.4 %. Only one case of the E. coli was extended spectrum $\beta$-lactamase (ESBL) positive. Conclusion : Compared to prior reports from other tertiary hospitals in Korea, E. coli was the predominant cause in childhood UTI and the rate of ESBL positivity was low. The antibiotic resistance was also different compared to prior reports. We conclude that a difference in the cause and antibiotic resistance of childhood UTI exists between centers and this should be taken into consideration when prescribing antibiotics for childhood UTIs.

  • Pilomatricoma is a benign soft tissue tumor of hair follicle origin. They occur most commonly in the head and neck region and are usually found in girls during the first two decades of life. Although malignant transformation has been described, it is exceedingly rare. The clinical presentation is typically that of an asymptomatic, superfical, solitary, firm mass that is often accompanied by a reddish-blue discoloration of the overlying skin. Histopathologically pilomatricoma are seen as epithelial islands embedded in a cellular stroma. The epithelial component consist of two main cell types : basaloid cells and ghost cells. Intracellular and stromal calcification is reported in many cases. Diagnosis is usually suspected based on palpation of a superficial, rock-hard mass and confirmed by histopathologic examination. Surgical excision is both curative and the treatment of choice. Recurrence is rare. In this case, the radiographs showed a dense calcification, measuring about 1cm diameter. So simple excision was peformed intraorally, and resultantly the lesion was proved to be a pilomatricoma by histological examination.

The natural history and prognostic factors of Graves' disease in Korean children and adolescents (한국 소아 및 청소년 Graves병의 자연 경과 및 예후 인자)

  • Purpose : Graves' disease is the most common cause of hyperthyroidism in children and adolescents. In this study, we investigated the natural course and the prognostic factors of Graves' disease in Korean children and adolescents. Methods : One-hundred thirteen (88 girls and 25 boys) patients were included in this study. A retrospective analysis was made of all patients who were diagnosed with Graves' disease. The following parameters were recorded and analyzed: patient's sex, age at diagnosis, duration of disease, laboratory findings, symptoms and signs, and family history of autoimmune thyroid disease. Results : All patients were initially treated with antithyroid drugs, either methimazole (93.8%) or propylthiouracil (6.2%). Antithyroid drugs had been discontinued in 75 (66.4%) of 113 patients. Of these 75 patients, 23 (20.4%) relapsed after $25.5{\pm}33.7$ months. Thirteen (11.5%) of 23 patients, who experienced the first relapse, showed a second remission. However, 2 (1.8%) of 13 patients relapsed again. Euthyroid state could not be achieved by antithyroid drugs in 1 patient, and radioactive iodine therapy was performed. The older the patient at diagnosis, the greater the likelihood of remission ($P$=0.034). Conclusion : Age at diagnosis seems to be a prognostic factor in Korean children and adolescents with Graves' disease, and should be taken into account in treatment plan determination.

Research of Protocols for Optimization of Exposure Dose in Abdominopelvic CT - (복부-골반 CT검사 시 피폭선량 최적화에 관한 프로토콜 연구)

  • This study measured the exposure dose during abdominal-pelvic CT exam which occupies 70% of CT exam and tried to propose a protocol for optimized exposure dose in abdomen and pelvis without affecting the imagery interpretation. The study scanned abdomen-pelvis using the current clinical scan method, the 120 kVp, auto exposure control(AEC), as 1 phase. As for the newly proposed 2 phase scan method, the study divided into 1 phase abdomen exam and 2 phase pelvis exam and each conducted tube voltage 120 kVp, AEC for abdomen exam, and fixed tube current method in 120 kVp, 100, 150, 200, 250, 300, 350, 400 mA for pelvis exam. The exposure dose value was compared using $CTDI_{VOL}$, DLP value measured during scan, and average value of CT attenuation coefficient, noise, SNR from each scan image were obtained to evaluate the image. As for the result, scanning of 2 phase showed significant difference compared to 1 phase. In $CTDI_{VOL}$ value, the 2 phase showed 26% decrease in abdomen, 1.8~59.5% decrease in pelvis for 100~250 mA, 12.7%~30% increase in pelvis for 300~400 mA. Also, DLP value showed 53% decrease in abdomen and 41~81% decrease in pelvis when scanned by 2 phase compared to 1 phase, but it was not statistically significant. As for the SNR, when scanning 2 phase close to heart, scanning 1 phase close to pelvis, scanning and scanning 1 phase at upper and lower abdomen, it was higher when scanning 2 phase for 200~250 mA. Also, the CT number and noise was overall similar, but the noise was high close to pelvis. However, when scanning 2 phase for 250 mA close to pelvis, the noise value came out similar to 1 phase, and did not show statistically significant difference. It seems when separating pelvis to scan in 250 mA rather than 400 mA in 1 phase as before, it is expected to have reduced effect of exposure dose without difference in the quality of image. Thus, for patients who often get abdominal-pelvic CT exam, fertile women or children, this study proposes 2 phase exam for smaller exposure dose with same image quality.

Eight cases of incidentally diagnosed as subclinical rickets (우연히 발견된 무증상 구루병 8예)

  • Purpose : Vitamin D plays a key role in bone mineralization of the skeleton and vitamin D deficiency can lead to rickets. It is well known that vitamin D deficiency is common in breast fed infants. Of these patients, clinically, some have no signs of rickets, but laboratory and radiographic findings are diagnostic for vitamin D deficiency rickets (subclinical vitamin D deficiency rickets). The purpose of this study is to clarify current causes and ways to prevent this disease. Methods : We reviewed the clinical and laboratory characteristics of children who were incidentally diagnosed as subclinical rickets during treatment of other disease such as pneumonia, gastroenteritis, urinary tract infection at Eulji Hospital, Seoul, Korea from March, 2003 to July 2007. Results : Eight patients (six boys and two girls) were diagnosed with subclinical vitamin D deficiency rickets. The mean age of the patients was $12.6{\pm}5.8months$, and they were diagnosed from January to July. The associated diseases were pneumonia, urinary tract infection, acute gastroenteritis, and iron deficiency anemia. All patients were breast-fed. Two showed growth failure. The mean serum alkaline phosphatase was $1995.8{\pm}739.5IU/L$, the mean calcium count was $9.5{\pm}0.6mg/dL$, and the mean phosphorus content was $3.6{\pm}1.5mg/dL$. The mean intact parathyroid hormone was $214.8{\pm}155.9pg/mL$ (reference range, 9-65), the mean 1,25-dihydroxyvitamin D was $82.4{\pm}49.3pg/mL$ (reference range, 2070), and the mean 25-hydroxyvitamin D was $29.6{\pm}10.6ng/mL$ (reference range, 1030). A radiographic examination showed cupping, fraying, and flaring of metaphyses in all patients. Six patients were administered calcitriol (400 IU/day) for three months. A consequent radiographic and laboratory examination showed improvement. The first two patients were initially diagnosed with metaphyseal dysplasia, without the detection of vitamin D deficiency and they spontaneously improved without vitamin D supplements. However, two years later, they showed mild scoliosis and metaphyseal dysplasia, respectively. Conclusion : Breast-feeding without supplementation involves high risk of vitamin D deficiency. Some infants may also develop rickets; therefore, such groups should be considered for vitamin D supplementation.

EARLY DETECTION OF INITIAL DENTAL CARIES USING A $DIFOTI^{TM}$ (Digital Imaging Fiber-Optic Trans-illumination을 이용한 초기 법랑질 우식병소의 조기 진단)

  • Over the past 20 years, great strides have been made in research regarding the mechanisms involved in the progression of carious lesions, but new equipment and research tools need to be developed to continue these advancements in caries research. Various methods have been applied to reduce the incidence of carious lesions, which have led to a significant decrease in the number of occlusal caries, but a concurrent increase in the proportion of proximal carious lesions. New diagnostic equipment has been developed to detect early stage carious lesions, and these have demonstrated excellent laboratory results and show promise in clinical applications. The research presented here examines the efficacy of the newly developed $DIFOTI^{TM}$ system in detecting proximal carious lesions compared to traditional intraoral exam and bitewing radiography, possible problems or deficiencies of using the system in clinic, possible improvements that can be made to the system, and the efficacy of detecting early, reversible carious lesions that can be remineralized by preventative fluoride applications. The subject pool consisted of 23 grammer school age patients just prior to entering the mixed dentition phase. Each patient was given a thorough oral examination, radiographic examination consisting of bitewing radiographs of the posterior teeth, and $DIFOTI^{TM}$ examination of the anterior and posterior teeth. Each examination was carried out two times by two examiners, and the data were statistically analyzed. The results are as follows: 1. The mean alpha value of reliability test of the visual oral examination was as follows; occlusal surface was 0.8470. mesial surface was 0.6430, distal surface was 0.5727. lingual surface was 0.2807 and distal surface was 0.2339. When the examination was limited to posterior teeth, the mean alpha value was as follows; occlusal surface was 0.8577, distal surface was 0.8211, lingual surface was 0.7728, buccal surface was 0.7152 and mesial surface was 0.6782. 2. The alpha value of reliability test of the radiographic analysis of carious lesions of the occlusal, mesial, and distal surfaces was 0.8500. 3. The alpha value of reliability test of the $DIFOTI^{TM}$ diagnostic analysis of carious lesions of the occlusal, buccal, lingual, mesial, and distal surfaces was determined to be 0.7917. 4. The $DIFOTI^{TM}$ diagnostic system was found to be the most accurate means of detecting occlusal, buccal, and lingual surface carious lesions (p<0.05), while mesial and distal proximal carious lesions were most accurately assessed using bitewing radiography (p<0.05).

Hepatitis Complicated with Mycoplasma pneumoniae Infection (Mycoplasma pneumoniae 폐렴에 합병된 간염)

  • Purpose: Mycoplasma pneumoniae infection is relatively common in childhood. Its extrapulmonary manifestations have been reported so much, but hepatitis associated with it has not been studied yet until now in Korea. Methods: We performed the prospective study of 19 cases that had AST and ALT>50 IU/L respectively without evidence of hepatitis A, B, C, cytomegalovirus, and Ebstein-Barr virus infections amongst 143 patients with M. pneumoniae pneumonia who were admitted to the Department of Pediatrics, Seoul Red Cross Hospital from Jan. 1999 to Dec. 2000. Results: 1) Hepatitis occurred in 13.3% of Mycoplasma pneumoniae infection, especially in fall and winter times. The average age was 4.86 years and male to female ratio was 2.2:1. 2) Vomiting was developed in 21.1%, diarrhea in 36.8%, and hepatomegaly in 21.1%, respectively. And leukocytosis was noted in 21.1%, eosinophilia in 15.4%, anemia in 10.5%, and thrombocytosis in 5.3%, respectively. The average level of C-reactive protein was $6.34{\pm}4.82$ mg/dl. 3) There was no hyperbilirubinemia, but hypoalbuminemia was detected in 42.1%. The average serum levels of AST and ALT were $214.05{\pm}183.22$ IU/L and $284.16{\pm}286.84$ IU/L, respectively. 4) Chest radiology showed lobar or lobular consolidation in 73.7%, bronchial infiltration in 26.3%, and pleural effusion in 31.6%. 5) The average length of hospitalization was 7.9 days, and the average length of normalization of serum transaminases was 8.8 days. All patients recovered within 2 weeks completely. Conclusion: The prognosis of M. pneumoniae hepatitis is good. However, its incidence is not so low that liver function should be considerately checked in case of M. pneumoniae infection.
