The Relationship between Advanced Airway Management and Self-Efficacy by Level 1 Emergency Medical Technicians(EMT) in 119 Emergency Medical Service (119구급대 1급 응급구조사의 전문기도유지술과 자기효능감과의 관계)

  • This study was to identify the frequency and related factors of advanced airway management for patients with cardiac arrest by the Level 1 emergency medical technicians in the 119 Emergency medical service. 95 level-1 medical technicians belonging to Gwangju Fire Service Headquarter were surveyed with structured questionnaire composed of general and job-related characteristics, self-efficacy, barrier factors to performing the advanced airway for patients with cardiac arrest. From January 1, 2010 to June 30, 2011, data obtained from the Gwangju Fire Service Headquarter by performance of advanced airway. Self-efficacy toward the necessity of advanced airway revealed positive correlation with self-confidence. The barrier factor to advanced airway performance had negative correlation with the frequency of performance. The most important factor of advanced airway performance was necessity and self-efficacy. In conclusion, it is necessary to develop the field-based practice education program and to improve self-efficacy.

Analysis of Disaster Safety Situation Classification Algorithm Based on Natural Language Processing Using 119 Calls Data (119 신고 데이터를 이용한 자연어처리 기반 재난안전 상황 분류 알고리즘 분석)

  • Due to the development of artificial intelligence, it is used as a disaster response support system in the field of disaster. Disasters can occur anywhere, anytime. In the event of a disaster, there are four types of reports: fire, rescue, emergency, and other call. Disaster response according to the 119 call also responds differently depending on the type and situation. In this paper, 1280 data set of 119 calls were tested with 3 classes of SVM, NB, k-NN, DT, SGD, and RF situation classification algorithms using a training data set. Classification performance showed the highest performance of 92% and minimum of 77%. In the future, it is necessary to secure an effective data set by disaster in various fields to study disaster response.

Muscuoloskeletal Disorders of Korean Fire Fighters: Applicants for Public Worker's Compensation from 2011 to 2013 (소방공무원의 근골격계 질환: 2011년~2013년 공상신청 분석)

  • The aims of this study were to analyze the Korean fire fighters' applications for the public worker's compensation between 2011 and 2013 and to suggest a feasible solution for prevention of work-related musculoskeletal disorders. The Korean Ministry of Public Safety and Security provided the complete list of fire fighters' application from January 2011 to December 2013. 510 applicants with chief complaint of musculoskeletal disorders were screened out of all 1596 applicants. Male applicants were 92%. Fire fighters in their early 40's applied most frequently, followed by those in early 30's. Musculoskeletal disorders of fire fighters were reported most frequently in May and June. Regionally, Seoul had most applicants and Jeonbuk showed the highest prevalence. Low back was the most frequently reported body part followed by the knee. Ligament was insured most frequently followed by vertebral disc and bone. The fire fighter's injury occurred while fire extinguish (38.3%), emergency medical service (37.7%), rescue (12.4%), and other duties. Result of this study suggests further investigation on the large variation in regional prevalence and on the injury mechanism of musculoskeletal disorders especially during the fire extinguish and emergency medical service.

The effects of out of hospital ACLS simulation training on the paramedic's duty ability (구급대원의 전문심장소생술 시뮬레이션훈련이 직무수행융합능력에 미치는 영향)

  • The purpose of this study is to analyze the effects of the simulation-based professional cardiac resuscitation training on the performance of professional cardiac resuscitation performed by paramedics in the pre-hospital stage and to provide basic data for effective cardiac resuscitation. This study is an experimental study of the design before and after the control of non-equality. The subjects of this study were 16 newly recruited paramedics from K firefighting school. The simulation training program and evaluation papers used as the evaluation tool were reviewed and commented by 6 ACLS simulation experts (2 emergency medical doctor, 2 emergency medical professors, 2 KALS instructors)Respectively. The training consisted of 30 minutes of theory and 150 minutes of practical training. The lecturer first demonstrated for 5 minutes, and after training by individual debriefing after individual training, individual and team education was conducted The evaluation scale was given a 5 - point Likert scale. The SPSS 22.0 program for Windows was used. The general characteristics of the subjects were analyzed for frequency, the examination of homogeneity between the experimental group and the control group wasfulfilled by t test, and the difference test between the groups of the two groups was performed using the paired t-test. The homogeneity test was able to confirm the homogeneity between experimental group and control group. In the evaluation of six ACLS techniques, it was proven that the experimental group that received the simulation training had better performance in all aspects than the non - training control group. The following are the technical items to be performed. 1. Electrocardiogram 2. Specialized instrument 3. Treatment of fluid 4. Leadership and teamwork 5. Medical guidance 6. Evaluation during transfer. It was proved that paramedics who received simulation training were improved on their job performance ability than general lecture and training group. Therefore, if simulation training and education are applied to a student in the synthetic course or an emergency resident who is engaged in clinical practice, he / she will be able to perform his / her duties more proficiently. It is expected that emergency services provided to patients with cardiac arrest will be improved.

  • This study was conducted to find out the relationship between knowledge and attitudes about COVID-19 and infection control performance among college students in paramedicine. The subjects of the study were 154 university students attending a 3-4 year paramedicine located in Gwangju Metropolitan City, Jeollabuk-do Province, and the collected data were frequency, percentage, sum, average and It was analyzed by standard deviation, t-test, ANOVA(Scheffe), Pearson's Correlation Coefficient, and Multiple Linear Regression. The order of attitude and knowledge about COVID-19 affected the infection control performance of college students in the paramedicine. Therefore, it is necessary to develop and apply regular or non-regular curriculum and multidisciplinary curriculum that can improve knowledge and attitudes about COVID-19 in order to improve the infection control performance of paramedic students.

Trend Analysis for the Beehive Removal Dispatch of the 119 Rescue Teams in Busan (부산지역 119구조대의 벌집 제거 출동 경향 분석)

  • Purpose: The aim of this study is to analyze the trend for the dispatch of the 119 rescue teams to remove the beehive according to the distribution of temperature and time in Busan metropolitan city for 5 years from 2015. Method: From January 2015 to December 2019, 11 fire stations in Busan were dispatched and the source data of rescue and emergency activities were collected. The number of beehive removal dispatches was determined by the Busan Metropolitan Fire Station's jurisdiction over the past five years, and the temperature meteorological factors and honeycomb removal dispatches were analyzed in frequency and percentage. Result: The frequency of dispatch began to increase at an monthly average temperature of more than 20℃ and was higher at 23℃ to 29℃ than other temperature range. The highest frequency of dispatch was 7,900 cases in 2017. In particular, we found that the start timing of the honeycomb removal is getting faster as the year goes by. Gijang-gun had the largest frequency of dispatch, and Haeundae-gu, Geumjeong-gu, and Nam-gu were found to have a higher that. Conclusion: We found that the start timing of the honeycomb removal is getting faster as the year goes by and temperature changes. The results of this study are considered to be useful in future studies of wasps in urban areas.

An Analysis of Call Demands of Two Squads In Kyonggi Provincial fire and Disaster Headquarters (경기도 소방재난본부에 소속된 두 구급대의 출동수요 분석)

  • The purposes of this research which was conducted by surveying lost/added unit hours reports, unit hour demand analysis worksheets from prehospital care reports of two squads in Kyonggi Provincial Fire and Disaster Headquarters for 20 weeks (January 1, 2002 - May 20, 2002) are to get Unit Hour Utilizations. Call Demands such as Unit Hour Demand, Simple Average Demand, High Average Demand, Peak Average Demand, the High Actual Demand. The conclusions from this analysis were summarized as follows: (1) By revealing Unit Hour Produced 3223.9, Call Volume 964, Unit Hour Utilization 0.299 at the Squad A and Unit Hour Produced 3328.4, Call Volume 901, Unit Hour Utilization 0.271 at the Squad B induced Korean Squads to chance identification, definition, direction of Unit Hour Utilization. (2) By revealing Simple Average Demand 7.4 on Monday Tuesday, High Average Demand 9.6 on Tuesday Friday. Peak Average Demand 11.5 on Tuesday, the High Actual Demand 12 on Tuesday Wednesday at the Squad A and Simple Average Demand 6.8 on Sunday, High Average Demand 10.4 on Monday, Peak Average Demand 11.5 on Monday, the High Actual Demand 13 on Monday at the Squad B enabled Korean Squads to utilize System Status Management. (3) The Maximum Calls per Unit Hour were 115 for 23:00~23:59, the Minimum Calls per Unit Hour were 46 for 05:00~05:49 in two squads. The Maximum Calls per Unit Hour were 7.4 on Tuesday Saturday, the Minimum Calls per Unit Hour were 6.1 on Thursday at the Squad A. The Maximum Calls per Unit Hour were 7.3 on Monday Saturday, the Minimum Calls per Unit Hour were 5.6 on Thursday at the Squad B. (4) Analyzing demand for EMTs in the optimum emergency medical service of Korea, we have been able to utilize this Unit Hour Utilization in company with the established estimation methods such as international comparisons or the number of ambulances for scientific reasonable estimation. (5) These Call Demands which were limited to the demand time in this study will make us expect some following studies including demand time, demand time, demand map for Strategic Deployment.

  • 1990년 이후 공산체계인 구소련과 동구권이 몰락하고 세계적으로 탈냉전의 변화와 우리의 민주정부 수립 후 햇빛정책 등에도 불구하고 북한은 적화통일을 포기하지 않고 작년 11월에 연평도에 포격을 가하여 민간인과, 군이 2명이 사망하는 사건이 발생 하였다. 또한 북한의 공격은 과거의 포탄공격뿐만 아니라 미사일이나 화생방과 같은 생화학물질 탑재 가능한 공격이 예상된다. 이에 대비 정부에서는 고정시설의 화생방 방호는 일단유사시에 국민의 생명과 재산을 보호하는데 필수요소로서 집단보호개념에 도입하고 있으며, 현재 각 중요 대피시설 및 지자체의 기존 대피시설을 개인방호, 탐지, 제독, 운용절차를 포함한 실태파악 및 재래식 탄을 포함한 통합적인 조치를 마련하고 있다. 작년 2010년 11월 연평도에서의 북한 포격이후 우리는 서해5도나 접경지역 주민과 그리고 근접거리의 서울시민들이 안전하게 포격으로부터 피난 할 수 있는 민방위대피소에 대한 문제점이 대두되고 대책방안을 요구하고 있다. 소방방재청 기준에 따르면 직격(直擊) 핵폭탄을 제외한 화생방, 재래식 무기 공격에 견딜 수 있는 시설은 '1등급 대피시설'인데 서울에는 한 곳도 없다. 고층건물의 지하 2층 이하나 지하철, 터널 같은 2등급 대피시설도 폭격은 견딜 수 있지만 화생방 공격에는 취약하다. 지하상가 등 건축물의 지하층, 지하차도나 보도는 그보다 더 취약한 3등급이며 단독 주택의 지하층처럼 방호 효과가 떨어지는 곳이 4등급이다. 2등급으로 분류된 대피소의 경우 분류만 되어 있을 뿐 장기간 대피시 시민들이 사용 할 수 있는 거주공간이 아닌 임시 대피로서의 역할만 하고 있다. 본 연구에서는 민방위 기본법에 따라 지하에 $3.3m^2$ 당 4명 이상이 대피할 수 있는 규모의 공간을 갖추기만 하면 대피시설로 볼 수 있는 현 문제점과 1등급에서 4등급으로 나뉘어져있는 민방위 대피소와 대피소가 있지만 피난공간으로서의 역할과 구조적으로 안전한가에 대한 국내외 실태 분석을 통하여 분석한 결과 우리는 서해5도 대피시설의 반 이상이 무용지물이고 접경지 또한 피난시에 피난장으로서의 대피소가 난방이나, 구급품과 식구 시설 등이 구비가 되어 있지 않고 대피시설로 지정한곳이 간판이나 안내시설이 없다. 외국의 경우 스위스는 연방정부의 관리책임으로 전쟁이나 핵 확산에 따른 화생방 업무 전담팀을 두고 있으며, 방독면은 개인 방호물자로서 전국민 100% 보유하고, 각가정이나 건축, 시설물의 경우 화생방 표준 대피시설 설치관련 규정 마련 시행하고 있다. 대피시설은 화생방 방호가 가능한 지휘부용 대피시설과 일반 주민대피용 시설을 구분하여 설치운영하고 있으며, 전국에 650만개를 설치하고 있다. 결론은 대피시설이 북한 공경시 피난시설로의 활용방안을 모색 등급구분에 따른 현실적용 수정과 기존 대피소 보강과 재선별이 필요하고, 신축시 설계 표준설계안 그리고 기존의 대피소에 대한 보수, 보강방법 가이드라인과 장기간 거주에 대한 설비구축 등과 국민 대홍보등을 제안하였다.

A Study on the Special Needs of the Hearing-Impaired Person for Disaster Response (청각장애인 재난대응 욕구에 관한 연구)

  • This study evaluated the actual status of special needs of the hearing-impaired person for disaster response. The analysis revealed a significant level of unmet needs in disaster response for hearing-impaired person. The 5 special needs in disaster response include: 1) communication needs, which involve securing the means to make an emergency rescue request and communicating information during the rescue process; 2) transportation needs, which indicate the effective evacuation capacity and the level of training; 3) medical needs, which address the degree of preparedness for physical and mental emergency measures and the delivery of health information for rescue and first aid process; 4) maintaining functional independence needs, which refer to the level of self-preparedness to minimize damage in disaster situations, and; 5) supervision needs, which correspond to a personalized support system provided to disaster-vulnerable groups.
