A Case of Phthiriasis Palpebrarum in the Eyelids of a Child (소아 안검에서 관찰된 사면발이증 1례)

  • A two year old male child complained of foreign body sensations bilaterally, and an itching sensation at both upper and lower eyelids for 2 months of duration. He was diagnosed with Phthiriasis palpebrarum after observation of adult louse and nits on slit-lamp examination. The treatment was accomplished by mechanical removal of the lices and nits by forceps and scissors, and then irrigation with 20% fluorescein was done under general anesthesia. Antibiotic eyedrops and ointment were applied to both eyes. The morphologic evaluation was performed with light microscopy and scanning electron microscopy. The follow-up interval was every weeks for one month and no louse or nit was found under slit-lamp examination. So we concluded that the Phthiriasis palpebrarum was eradiated completely. We report a case of Phthriasis palpebrarum had been review of the literatures.

Endophthalmitis after Removal of an Intralenticular Foreign Body in Place without Symptoms for 20 Years (20년 동안 증상없이 잔존한 수정체 내 이물제거 후 발생한 안내염)

  • Purpose: We report a case of postoperative endophthalmitis in the cataract patient, associated with removal of an intralenticular foreign body that had remained in place without symptoms for 20 years. Case summary: A 45-year-old male visited our outpatient clinic complaining of gradual visual loss in his right eye over the past 3 months. In slit-lamp examinations, anterior capsular opacification, nuclear sclerosis, and posterior subcapsular opacity were observed in the right eye. Twenty years before, a tiny metallic projectile had hit his right eye, but slit-lamp examination at the time of injury did not reveal any intraocular foreign body. We decided to undergo cataract surgery. During phacoemulsification, a metallic foreign body was found in the lens and safely removed; then an intraocular lens was implanted. As hypopyon was evident 3 days later, we injected intravitreal antibiotics and applied fortified antibiotic eye drops to the right eye. The anterior chamber inflammation improved and the best-corrected visual acuity recovered to 1.0. Conclusions: Surgeon should be aware of that endophthalmitis could develop after cataract surgery with removal of an intralenticular foreign body that had been in place for 20 years. But did not trigger inflammation or cause any symptoms as the cataract progressed.

A Study on Prescription of RGP Contact Lens in Korean Optometrist (안경사의 RGP콘택트렌즈 처방 실태조사)

  • There has been an increase in rigid gas permeable contact lens prescription by the optometrists in Korea since 1995. Therefore, the aim of our study was to find out which equipment is needed for adequate RGP contact lens prescription and the level of the clinical knowledge that they have and the level of education for the patient, to seize about the extended lens education programmes for the Korean optometrist. Also, we tried to figure out the problem that they had in the RGP contact lens prescription, to develope the educational programs for students and to improve legal uncertainties, through this research. The result came out with the following through questionnaires of 400 male and female opticians who is in clinical field. The distributions of respondents are 270 male (67.5%), 130 female (32.5%), and 356 optometry major (89%). We found out the optometry-majored people had faster clinical lens prescriptions (p=0.000), 72.5% of opticians prescribe the RGP-Lenses, and 27.5% do not because of laking in experience (P=0.000). They responded that they need slit-lamp, button lamp, fluorescein dry paper, and etc,for prescription or fitting test. In regular-check-up (Follow-up), the result came out very low by 83% of respondents who are not getting it regularly. And 83.5% (334 people) of them wanted to have those Extended Educational Programs and also the Clinical Education Programs.

Studies on Corrected RGP Lens Fitting and Use of Care System (올바른 RGP렌즈의 처방과 관리용품의 사용에 대한 결과 연구)

  • We performed refraction, keratometry, slit lamp biomicroscopy. We selected 58 current spherical RGP lens wearers for this three-month study. All patients exhibits at least 0.75D of corneal astigmatism measured with the keratometer, and 37 patients had corneal astigmatism of 1.50D or greater. At least follow-up visit, we measured Snellen acuity with lenses, and performed overrefraction, overkeratometry and slit lamp biomicroscopy. We charted lens position, movement and surface quality. During the three month, biomicroscopy revealed no corneal edema and neovascularization on any patients. Fluorescein staining were 52 patients case of grade 0.5 patients case of grade 1, and 1 patient case of grade 2. In evaluating post-fit residual cylinder, on overrefraction as a percentage of refractive cylinder. By the initial visit, one-week visit, one-month visit, and two-month visit are 41%, 34%, 29%, respectively. In this data, we knew no change after one month. The average overrefraction for these eyes in absolute diopters is 0.26D(initial visit), 0.22D(one-week visit, 0.17D(one-month visit), and 0.16D(two-month visit). The use of a regimen containing a dedicated daily cleaner was more effective in maintaining patient comfort and lens cleanliness than was the use of a regimen containing only a multipurpose solution.

A Case of Unilateral Focal Pigmented Paravenous Retinochoroidal Atrophy (단안에 국소적으로 발현된 색소정맥옆망막맥락막 위축 1예)

  • Purpose: We report a case of unilateral, focal, pigmented paravenous retinochoroidal atrophy (PPRCA). Case summary: A 46-year-old female visited our clinic in complaint of a vague problem with her right eye identified during a general medical examination. The visual acuity (without correction) of both eyes was 1.0. Slit-lamp examination of both eyes revealed no specific signs. Fundus examination of the right eye revealed focal, bony-spicule-shaped retinochoroidal atrophy with pigmentation along the course of the superior retinal vein. A fundus autofluorescence examination revealed principally hypofluorescence with some hyperfluorescence at the margin of the atrophic retinochoroidal lesion. Optical coherence tomography revealed mixed clumping and atrophy of the retinal pigment epithelium (RPE) layer and thinning of the choriocapillaris layer. Fluorescence angiography revealed a window defect and blockage at the site of the lesion (the fluorescent material did not enter the lesion). The site of the window defect was in correlation with the atrophic RPE region. The site of the blockage at lesion also matched with the site of the regional pigment clumping. No definite leakage was observed. Conclusions: To the best of our knowledge, this is the first case of unilateral focal PPRCA reported from Korea.

Two Cases of Corneal Edema Due to Vaporized Amine (기화된 아민에 의한 각막부종 2예)

  • Purpose: We report two cases of corneal edema in patients who presented with bilateral blurry vision due to vaporized amines while working in a polyurethane processing plant. Case summary: A 28-year-old male presented with bilateral blurred vision. His work involved solidifying polyurethane liquid and he often found himself exposed to polyurethane heat and gas. The central corneal thickness (CCT) was also increased in both eyes, measuring $698{\mu}m$ (right) and $672{\mu}m$ (left). After prescribing 0.5% moxifloxacin and, 1% fluorometholone eye drops for 3 days in both eyes, the UCVA recovered to 20/40 (right) and 20/20 (left). The CCT decreased to $644{\mu}m$ (right) and $651{\mu}m$ (left), and the microbullae improved significantly in the left eye. The second patient was a 34-year-old female who presented with bilateral decreased visual acuity while at work. She worked in a factory that produced car seat filling. Her UCVA was 20/25 (right) and 20/20 (left). The CCT by specular microscopy was $537{\mu}m$ (right) and $541{\mu}m$ (left). On slit lamp examination, both eyes demonstrated bilateral central subepithelial edema. The patient did not attend any follow-up outpatient appointments after the initial presentation. Conclusions: Exposure to vaporized amines such as polyurethane may causereversible corneal toxicityeven without direct contact. The Efficacy and Effect of Reverse Geometry Contact Lens on Cornea (역기하학 렌즈의 유효성과 각막에 미치는영향)

  • Object of this research is to estimate the effect of myopia correction and safety on reverse geometry contact lens fitting in school children. This research include 53(106eyes) schoolchildren among 7 to 18 years who has low to moderate myopia(-1.00D~-5.00D) and prescribed reverse geometry contact lens for purpose on orthokeratology between January to July 2004 and had 3months full follow up examination. They were tested for slit lamp examinations, BUT(Break up time), direct ophthalmoscopy, retinoscopy, uncorrected visual acuity, best corrected visual acuity, autorefraction, autokeratometry and corneal topography in each examination(1day, 1week, 2weeks, 1, 2, and 3months) of before-and-after lens wearing to find out the effect of myopic correction and side effect. The results came out as follow. The average of uncorrected visual acuity was $0.0938{\pm}0.378$ before lens wear and $0.3136{\pm}0.283$ after 1day lens wear, and there was fast improvement after 1week($0.7925{\pm}0.301$) and little improvement after 2weeks period but still they shows better uncorrected visual acuity(p<0.01). The result of this study, the reverse geometry lens is very useful to correct refractive error and control the progression of myopia temporally among low to moderate myopic patient. The side effects were relatively rare but further study should be necessary with long term lens wear effect on eye health. For the lens prescription, the clinical fitting process had higher rate of success with consideration of eccentricity and corneal topography.

Usefulness of Rotation for Toric Soft Lenses Using Objective Refraction (타각적 굴절검사를 이용한 토릭 소프트 렌즈 회전 평가의 유용성)

  • Purpose: The clinical usefulness of rotation evaluation using objective refraction in toric soft lenses fitting was investigated. Methods: Toric soft lenses were fitted for 32 subjects (64 eyes; mean age of 24.69 ${\pm}$ 1.65 years) with astigmatism and both eyes of each subject were fitted with toric soft lenses. Objective refraction-based lenses rotation was evaluated from refraction and over-refraction data by indirect calculating technique. These calculated data were compared with the measured data from slit lamp with direct measuring technique. Results: Orientation of toric soft lenses around zero position (within ${\pm}$ 5$^{\circ}$ vertical line) was investigated. The orientations to the direction of nose of measured and calculated values were 69.78% and 63.64%, respectively, which showed similar values between two techniques. Agreement frequency between measured and calculated values in the magnitude of lenses rotation 54.69% and 82.82% for 10$^{\circ}$ and 20$^{\circ}$ of vertical line, respectively. The 95% limits of agreement between calculation and measurement were from -10.08$^{\circ}$ to 12.65$^{\circ}$, and mean difference was 1.29$^{\circ}$ within ${\pm}$ 10$^{\circ}$. The result showed there was no significant difference (p = 0.1984) and high correlation (r = 0.56, p = 0.0004) between two techniques. But the 95% limits of agreement was widen in ${\pm}$ 20$^{\circ}$ of vertical line. The magnitude of lens rotation between two methods was 9.66 ${\pm}$ 6.16$^{\circ}$, 16.17 ${\pm}$ 12.38$^{\circ}$ and 10.58 ${\pm}$ 12.02$^{\circ}$ for normal, loose and tight fitted conditions. Conclusions: From the results with smaller difference between two techniques, it was found that higher availability of subjective over-refraction data can be used as a supplementary tool for subjective refraction. An application using objective refraction with direct measuring could be provide high success in prescription on toric soft lenses.
