Recent Advance in Pathogenesis and Treatment of Precocious Precocity (사춘기 조숙증의 기전 및 치료의 최신 지견)

  • Precocious puberty is defined as the appearance of secondary sex characteristics before age 8 years in girls (or menarche before age 9 years) and before 9 years in boys. The age of onset of puberty is progressing to younger age. The pubertal activation of gonadotrophin releasing hormone(GnRH) release requires coordinated changes in excitatory or inhibitory amino acids, growth factors, and a group of transcriptional regulators. A number of factors affecting precocious puberty were explored, including the role of genetic factors, nutrition(body fat) and exposure to endocrine disrupting chemicals. In addition to the psychosocial disturbances associated with precocious puberty, the premature pubertal growth spurt and the accelerated bone maturation result in reduced adult height. Precocious puberty may be gonadotrophin-dependent premature activation of the GnRH pulse generator or gonadotrophin-independent. In rapidly progressing central precocious puberty, GnRH agonists appear to increase final height. However more data on the effect and long-term safety of GnRHa are needed.

  • Pulmonary hypoplasia is an entity of pulmonary agenesis. Pulmonary agenesis is a rare congenital anomaly, usually diagnosed soon after birth. It is commonly associated with other anomalies, mainly of the cardiovascular systems. Although it may hasten the death of a child, sometimes it is compatible with normal growth. We report a right lateral thoracotomy approach for mitral valve repair in a young woman with pulmonary hypoplasia, in whom preoperative computed tomography showed severe right side shifting of the mediastinum and total collapse of the right lung.

  • Vitamin D plays a major role in bone metabolism, and its deficiency has an impact on fracture risk and healing. Low vitamin D levels are a cause of poor bone mineralization and have been associated with a significantly higher risk of physeal injury in children. This paper presents a case of a 13-year-old boy with a vitamin D deficiency, who sustained multiple sequential epiphyseal injuries at various areas. This report suggests that vitamin D deficiency is not only a significant cause of the clinical disease itself, but also an important factor affecting the successful recovery of injuries.

  • The form and function of the craniofacial structure critically depend on genetic information. With recent advances in the molecular technology, genes that are important for normal growth and morphogenesis of the craniofacial skeleton are being rapidly uncovered, shaping up modem craniofacial biology. One of them is fibroblast growth factor receptor 2 (FGFR2). Specific point mutations in the. FGFR2 gene have been linked to Apert syndrome, which is characterized by premature closure of cranial sutures and craniofacial anomalies as well as limb deformities. To study pathogenic mechanisms underlying craniosynostosis phenotype of Apert syndrome, we used a transgenic approach; an FGFR2 minigene construct containing an Apert mutation (a point mutation that substitute proline at the position 253 to arginine; P253R) was introduced into fertilized mouse germ cells by DNA microinjection. The injected cells were then allowed to develop into transgenic mice. We used a bone-specific promoter (a DNA fragment from the type I collagen gene) to confine the expression of mutant FGFR2 gene to the bone tissue, and asked whether expression of mutant FGFR2 in bone is sufficient to cause the craniosynostosis phenotype in mice. Initial characterization of these mice shows prematurely closed cranial sutures with facial deformities expected from Apert patients. We also demonstrate that the transgene produces mutant FGFR2 protein with increased functional activities. Having this useful mouse model, we now can ask questions regarding the role of FGFR2 in normal and abnormal development of cranial bones and sutures.

Studies on INtestinal TRematodes in Korea XVII. Development and Egg Laying Capacity of Echinostoma hortense in Albino Rats ans Human Experimental Infection (한국의 장흡충에 관한 연구 XVII. 호르텐스극구흡충의 흰쥐내 발육, 충란산출양상 및 인체실험감염)

  • The worm development and egg laying pattern of Echinostoma hortense (Trematoda; Echinosto-matidae) were studied in albino rats and the brief clinical course was observed in human volunteers. A total of 21 rats were infected with 20~69 metacercariae each and two humans were with 7 and 27 metacercariae, which were collected from the loaches. For recovery of worms, the rats were sacrificed at irregular intervals from the 6th to 150th day after infection and the human volunteers were treated with praziquantel and purged with magnesium salt on the 26~27th day. The stools of the rats and humans were examined for the eggs. The results were as follows: 1. The worm recovery rate from the rats was not affected by the increase of infection time but varied individually; 9.1~50.0% (31.1 % in average). From humans, 14.3% and 37.0% (32.4% in average) of challenged were recovered. 2. In the rats, it was revealed that the worms rapidly grew for the first 14 days to become 7.59mm in average length and 1.17mm in average width but the growth became much slower thereafter until the 150th day; 7.95mm in length on the 21 th day, 9.04mm on the 28th day, 10.21mm on the 49th day and 12. 62mm on the 150th day. During the early stage of infection, the growth of genital organs (male or female) was expressed as sigmoid curves whereas non-genital organs (such as suckers) was simply as straight lines. 3. The prepatent period of this fluke was 10~12 days in the rats and 16~17 days in men. After the start of oviposition, the egg production by the worms remarkably increased, reached maximum on the 32~33th day, followed by decrease thereafter. The maximum value of E.P.G./worm was 390. 4. The major subjective symptoms in human volunteers were abdominal pain and diarrhea during the early stage of infection. The results show that human is as susceptible as the rats to E. hortense infection and the amount of egg production in the rats is greatly affected by the age of worms.

A LONGITUDINAL STUDY ON THE INTERRELATIONSHIP BETWEEN THE GROWTH CHANGE OF THE MANDIBLE AND DEVELOPMENTAL AGE OF GROWING GIRLS (성장기 소녀에서 하악골 성장 변화와 발육 연령의 상호 관계에 대한 누년적 연구)

  • This study was undertaken to investigate the interrelationship between the growth change of mandible and the developmental age of growing girls in the longitudinal data from 7 years to 15 years. The data were obtained from maturation stage of cervical vertebrae through the serial lateral cephalogram, height increment records which was taken at one year interval, menarche date through interview. On the basis of findings of this study, the following conclusions were obtained. 1. In general the growth of mandible (Ar-Pog, Ar-Go, Go-Pog) showed two peaks of growth velocity. 2. The maximum growth of mandible (Ar-Pog, Ar-Go, Go-Pog) almost appeared between stage 3 and stage 4 in maturation of cervical vertebrae. 3. It showed the tendency that the maximum increment of height appeared faster than that of mandible (Ar-Pog, Ar-Go, Pog-Go). 4. The maximum growth of height almost appeared between stage 2 and stage 3 in maturation of cervical vertebrae. 5. It showed the tendency that the menarche appeared later than the maximum growth of mandible (Ar-Pog, Ar-Go, Pog-Go). 6. The menarche almost appeared at stage 4, stage 5 in maturation of cervical vertebrae.

DISTRIBUTION OF CGRP-IMMUNOREACTIVE NERVE FIBERS IN THE RAT SQUAMOSOMANDIBULAR JOINT WITH POSTNATAL DEVELOPMENT (성장발육에 따른 흰쥐의 악관절 신경분포의 변화)

  • The purpose of this study was to investigate the distribution of CGRP immunoreactive nerve fibers in rat squamosomandibular joint with postnatal development. Squamosomandibular joint with adjacent tissue of 8 groups of rats(1, 5, 10, 15, 20, 25, 35-postnatal day and adult groups) were removed on bloc and processed for immunohistochemistry and were subjected to light microscopic examination. The results obtained were as follows; 1. The anterior portion of the articular disk was most densely innervated, followed by the posterior, lateral, and medial portions in each group. 2. Increase of CGRP immunoreactive nerve fibers was evident in 10, 15, 20 postnatal day groups. 3. Almost no CGRP immunoreactive nerve fibers were observed in articular surface of disk proper, mandibular fossa and condyle head of each group. These result suggest that CGRP immunoreactive nerve fibers increased in synovial membrane and peripheral portion of articular disk during 10-20 postnatal day may play a important role in squamosomandibular joint function after weaning period.

  • In case of endodontic treatment and extensive restoration of severe dental caries, trauma, and developmental defect, esthetic restoration of primary incisors and permanent anterior teeth for children and adolescents period is delicate matter for pediatric dentists. Existing restorative methods for anterior teeth have retentive and esthetic limitations for badly damaged teeth, especially for the adolescent anterior teeth. Therefore, the preparative stage for setting the permanent prosthesis as well as the retention and esthetics have to be considered. In this case, esthetic restoration for badly destroyed anterior teeth was tried with fiber-reinforced post and the result was satisfactory.

  • For the purpose of finding out the differences between urban and rural primary school children aged from 7 to 12 in their physipue, 1146 (578 boys and 567 girls) urban children and 1378 (693 boys and 635 girls) rural children were obserbed by the author during 2 months period from September, 1968 to October, 1968. Following were the findings obtained from this study. 1. Physique In general, urban children showed superiority than rural children if their physique, There could be seen significant differences in their height, weight and chest-girth specially in the age group of 11 and 12. 2. Physique indices In the physique indices based on the height, the value of urban children showed superiority than rural children in relative weight but in relative chest-girth it was the other way around and there were no significant differences between these two groups in relative sitting height. The Kaup index showed higher value in urban children and the Vervaeck index in the rural children and there were no significant differences between these two groups in the Rohrer's index. 3. Comparing these figures with the Korean Standard Value, urban children showed superiority and rural children showed inferiority. Our children were inferior than the Japanese children in their physiques.

  • Changes in total lipid content. total cholesterol content and fatty acid composition of human milk were investigated longitudinaly from 2-5 days to 12 weeks postpartum Milk samples were collected from 19 Korean lactating women at 2-5 days and at 1. 2. 4. 6. and 12 weeks postpartum. The obtained results were as follows : On average the daily energy intake protein intake and fat intake of Korean lactating women was 1812 kcal 72.5g and 29.8g respectively. The composition ratio of energy consis-ting of protein fat and carbohydrate was 16:15:69 The total lipid content increased from 1.39g/이 at 2-5 days to 2.86g/dl at 12 weeks ; while the total cholesterol concentration (mg/g) decreased significantly with time following postpartum. The total unsaturated fatty acids content was higher in colostrum than in mature milk. and the total saturated fatty acids were higher in mature milk. The average DHA content was 0.55% and the P/S ratio of human milk lipids was 0.37.
