A Case of Mucosal Bridge of The Vocal Fold (성대교 1례)

  • The mucosal bridge of the vocal fold is an arch of mucosa, formed by some portion of the vocal fold mucosa which is detached in various extension and length. The etiology is uncertain but some is related to vocal sulcus. Because it affects the voice with variable impact, it must be differentiated from functional voice disorder. We report a case of a mucosal bridge of vocal fold with a vocal polyp treated by microlaryngeal surgery and voice therapy.

Nonlinear FEM analysis of Cable-stayed PSC Bridges Considering Time-dependent Behavior (시간 의존적 거동을 고려한 PSC 사장교의 비선형 유한요소해석)

  • In this paper the nonlinear analysis that include time-dependent characteristics of materials and geometric nonlinearity of elements for the cable-stayed PSC bridges is presented. Analysis models for finite element method were developed based on the flexibility based fiber beam-column model originally proposed by Spacone et al.(1996). The developed analysis model implemented in general purpose object-oriented finite element analysis program named HFC(Cho 2009). The performance of proposed analysis models is evaluated by comparing with the former results of the design data. The deflection of time dependent analysis is larger than time ignored analysis on construction sequences, and the bridge is destructed at a smaller deflection than the time ignored analysis on failure behavior.

  • 소형 PEMFC(Proton Exchange Membrane Fuel Cell)는 전기를 만들기 위해서 고순도의 수소를 필요로 한다. 각각의 마이크로 성형된 금속박판(스테인레스 스틸, 알루미늄)을 진공 브레이징법으로 접합하여 수소공급용 소형 개질기를 제작하였다. 마이크로 채널의 내부는 졸-겔법(스테인레스 스틸)과 양극산화법(알루미늄)으로 촉매를 지지하기 위한 다공성 $Al_2O_3$ 층을 형성시켰다. 스테인레스 스틸 박판은 에칭과 브레이징에 유리하였으나, 표면산화층 코팅을 균일하게 하여 안정적인 촉매반응을 유도하기 위한 균일한 표면 산화층 형성이 힘들었다. 반면 알루미늄 박판은 표면 산화층 형성이 상대적으로 용이했으며, 촉매를 상하지 않는 낮은 온도에서의 적층이 가능했다.

Analysis of benthic macroinvertebrates community stability and similarity in the Giran stream (길안천 저서성대형무척추동물의 군집안정성 및 유사도 분석)

  • This study was conducted to investigate the community stability and similarity of benthic macroinvertebrates in the Giran stream between August and September 2018, and compare results to those reported by Lee (2004). As relates to the total number of species in each taxon in 2018, 45 species were additionally discovered compared to the 2003 study; the number of EPT taxa increased by 14 species and OCH taxa increased by 18 species. The diversity and richness indexes increased while the dominance index tended to decrease. According to analysis of functional feeding groups, 11 more Gathering-collector species were found, making it the highest functional feeding group with 24 species. According to analysis of functional habitat groups, 15 more clinger species were found than in the past, making it the highest functional habitat group with 41 species. A community stability comparison showed that species belonging to 'Stability Group I' had the highest stability rate at 57.1% in 2003 and 61.5% in 2018. According to the biological water quality assessment, in 2018, the average water quality level at each survey site was 'Ia' and 'Very Good' in terms of environmental conditions. As a result of the similarity analysis between the survey points for the species that appeared, two large groups of similarities were classified (similarity group 1: 2003 sites, similarity group 2: 2018 sites).

Usefulness and Surgical Strategies of Pulmonary Artery Banding in Functional Univentricular Heart (단심실에서 폐동맥 교약술의 유용성 및 수술전략)

  • Pulmonary artery banding (PAB) in the functional univentricular heart (UVH) is a palliative procedure for staging toward the Fontan procedure; however, it is known to be a risk factor. Material and method: The records of all 37 patients with functional UVHs who underwent surgical palliation using PAB between September 1989 and August 1999 were reviewed retrospectively. We investigated the aortic arch obstruction, the development and progression of subaortic stenosis after PAB, and risk factor of mortality according to surgical method. Result: In 37 neonates and infants with single ventricular physiology, aortic arch obstruction was combined in 7. There were 6 early deaths (16.2%) after PAB and 3 late deaths (8.1%) after Fontan operation. The actuarial overall survival including early mortality at 3 and 5 years were 80.7$\pm$6.6%, 72.2$\pm$8.2% respectively. Among 31 patients who survived PAB, 27 patients (87.1 %) could become candidates for Fontan operation; 22 patients (71.0%) completed Fontan operation with 3 deaths and 5 were waiting bidirectional cavopulmonary shunt(BCPS) or Fontan operation (follow-up mean 4.5 year, minimal 2 year). Subaortic stenosis developed in 8 patients after PAB (8/29, 27.6%); 3 cases in the patients without arch anomaly (3/22, 13.6%) and 5 in those with arch anomaly (5/7, 71.4%). The subaortic stenosis was managed with Damus-Kaye-Stansel procedure (DKS) in 6 patients without operative mortality and conal septum resection in 2 without long-term survivor. Analysis of risk factors established that aortic arch obstruction was strongly associated with subaortic stenosis (p<0.001). The only risk factor of late mortality was Fontan procedure without staged palliation by BCPS (p=0.001). Conclusion: PAB is effective as an initial palliative step in functional UVH. And the high risk group of patients with aortic obstruction can undergo effective short-term PAB as an initial palliative step, with subsequent DKS for subaortic stenosis. This strategy, initial PAB and careful surveillance, and early relief of subaortic stenosis can maintain acceptable anatomy and hemodynamics for later Fontan procedures.

Prevalence of enterovirulent Escherichia coli from diarrhea of cattles in Jeonbuk, Korea (전북지역 소 설사유래 병원성대장균 감염실태 조사)

  • Enterovirulent Escherichia coli are among the most important causes of diarrhea in cattles. Between January and December, 2017, a total of 150 stool specimens from cattles were investigated for enterohaemorrhagic E. coli (EHEC), enteropathogenic E. coli (EPEC), enteroaggregative E. coli (EAEC), enterotoxigenic E. coli (ETEC) and enteroinvasive E. coli (EIEC) using real-time PCR. 131 E. coli were isolated from feces. The most frequently isolated pathotype in feces was EHEC (37 isolates). EPEC, ETEC and EAEC were detected in feces with 14, 7 and 3 respectively. EIEC was not detected. Antimicrobial resistance test was performed by agar disc diffusion method with 14 antimicrobials. Enterovirulent E. coli isolates showed the highest antimicrobial resistance to ampicillin 61.3%, followed by tetracycline 54.5% and streptomycin 45.5%. They had low resistance to amikacin 11.4%. Of 44 isolates, 37 (84.1%) were resistant to more than 2 antimicrobials. futher study a highest antimicrobial susceptibility to trimethoprim/sulfamethoxazole 50.0% and florofenicol 47.7%.

Effect of Scutellariae Radix extraction (SRE) on oxidant-induced cell injury in human glimona cells (황금 추출물이 사람의 glioma 세포에서 oxidant에 의한 세포손상에 미치는 효과)

  • 신경교세포에서 황금추출물이 반응성 산소기에 의한 세포 사망을 방지할 수 있는지를 확인하기 위하여 사람의 glioama 세포주인 A172 세포를 사용하여 $H_2O_2$의 독성작용에 대한 영향을 조사하였다. 세포 사망 정도는 tryptan blue exclusion과 MTT reduction assay로 평가하였다. $H_2O_2$는 세포 사망을 유도하였으며 또한 세포내 ATP 함량을 감소시켰으며, 이러한 효과는 황금 추출물에 의해 방지되었으며 그 효과는 농도 의존적으로 나타났다. $H_2O_2$에 의한 세포 사망은 잘 알려진 flavonoid인 quercetin과 철착염제인 phenanthroline에 의해 방지되었으나, 항산화제인 DPPD나 Trolox에 의해서는 영향을 받지 않았다. $H_2O_2$는 poly (ADP-ribose) polymerase를 활성화시켰으며, 이러한 효과는 황금, quercetin 및 phenanthroline에 의해 억제되었다. 황금 추출물은 유기산화제인 t-buthyhydroperoxide 및 중금속인 수은에 의한 세포 사망을 방지하였다. 이러한 실험 결과는 황금 추출물이 $H_2O_2$에 의한 세포 사망을 방지하며 그 효과는 황금의 flavonoid 성분이 철과 결합하여 $H_2O_2$로부터 hydroxy radical의 생성을 억제함으로써 나타나는 것으로 추측된다.

  • Mucosal bridges of vocal fold have been described as a parallel band to vocal folds with the presence of sulcus vocalis. However, the type of mucosal bridge crossing each vocal fold has not been well documented in the literature. Herein, we present two cases of mucosal bridge found in the vocal fold connecting the mid-portions of true vocal folds. Two patients who had no history of laryngeal trauma, surgery or oro-tracheal intubation visited our clinic due to voice change. Laryngoscopic examination revealed that they had a mid-portion mucosal band without any other mucosal lesions. Two patients underwent laryngomicrosurgery with pulsed dye laser. After the surgery, they showed significant improvement of voice quality.

Lateral pressure is due to. water pressure and cargo. These load components are nat always applied simultaneously, but mare than one can normally exist and interact. Hence, far mare rational and safe design of ship structures, it is af crucial importance to. better understand the interaction relationship af the buckling and ultimate strength far ship plating under combined loads. Actual ship plates are subjected to relatively small water pressure except far the impact load due to. slamming and panting etc. The present paper describes an accurate and fast procedure for analyzing the elastic-plastic large deflection behavior up to. the ultimate limit state of ship plates under combined loads. In this paper, the ultimate strength characteristics of plates under axial compressive loads and lateral pressure loads are investigated through ANSYS elastic-plastic large deflection finite element analysis with varying lateral pressure load level.
