A cephalometric evaluation of anterior j hook headgear traction to the maxilla (Anterior j hook headgear를 이용한 상악골 성장억제에 관한 연구)

  • This study was aimed to observe the effect of Anterior J hook headgear on the craniofacial structures in mixed dentition with Class II malocclusion. The laterial cephalograms of 20 children treated by Anterior J hook headgear were traced, digitized and statistically analyzed. The results were as follows : 1. Forward growth of maxilla was inhibited. 2. Rotational effect of maxilla was not observed. 3. There was distal movement of maxillary dentition. 4. Maxillarly_dentoalveolar growth changes were more effective in anterior portion than posterior portion. 5. Mandible maintained a normal growth and mandibular plane angle was maintained during treatment period. 6. The ratio of anterior facial height to posterior facial height was almostly not changed.

Management of Maxillary Impacted Canines (매복 상악 견치의 처치)

  • The canine tooth is important both functionally and aesthetically, being positioned between the anterior and posterior teeth. The upper canine has the longest eruption path, forming in the deepest part of the maxillary bone and often experiencing eruption disorders, leading to significant aesthetic and functional issues. Early detection and management of canine impaction are crucial in pediatric dentistry, which focuses on tooth growth and eruption. The prevalence of maxillary canine impaction ranges from 1.1% to 3.0%. In Western populations, palatal impaction is twice as common as labial impaction, while some Korean studies report more labial impaction. Maxillary canine impaction occurs more frequently in women and is associated with various factors such as structural obstacles, pathological conditions in surrounding tissues, developmental abnormalities, and genetic factors. Labial displaced canines are linked to narrow maxillary intercanine width, total dental crowding, and skeletal Class III malocclusion. Maxillary palatal displaced canine impaction is more prevalent in patients with familial agenesis of lateral incisors or conical supernumerary lateral incisors. Understanding these factors aids in early diagnosis and appropriate intervention for canine tooth impaction, ensuring optimal oral health and aesthetics.

  • 저자는 한국인 정상교합에 있어서 상하악 전치의 위치가 ANB각과 어떠한 상관관계가 있는지 그리고 전치의 위치가 돌출도와 경사도와 상관관계가 있는 지를 조사 연구하기 위하여 $20\~24$세 연령의 남자 54명, 여자 56명 합계 110명의 한국인 정상교합의 측모두부 X-선사진을 사용 분석했다. 이의 통계분석은 Computer 과정을 거쳤으며 다음과 같은 결과를 얻었다. 1. 상하악 전치 경사는 ANB각과 밀접한 상관관계가 있고, 상악전치의 위치는 ANB각에 (-)상관관계 하악전치의 위치는 ANB각에 (+) 상관관계가 있으며 이는Steiner 분석법의 acceptable compromise에서 보여준 것과 유사한 경향을 나타냈다. 2. 상악전치 경사는 상악돌출도와 (-) 상관관계가 있다. 3. 1-NA각은 상악경사도와 상호독립적이다. 4. 1-NA각은 하악돌출도와 하악경사도와 의의있는 상관관계를 나타냈다. 5. 1-NA거리와 상악경사도와의 상관관계는 여성군에서만 의의있는 것으로 나타났다. 6. 하악전치치축경사는 하악돌출도와 하악경사도와는 남성군에서만 상관관계가 있고 여성군에서는 의의가 없는 것으로 나타났다.

  • Acute osteomyelitis of the maxilla with orbital cellulitis is occasionally seen secondary to a buccal infection, dental infection or necrotic process of the maxillary antrum. A case of acute osteomyelitis of the maxilla with orbital cellulitis in 5 years old boy has recently been experienced and cured by immediate incision and drainage combined with adequate antibiotics therapy. This paper was attempted to report the case with a brief review of reference.

DENTAL CARIES PATTERNS IN THE PRIMARY DENTITION: A CLUSTER ANALYSIS AND A MULTIDIMENSIONAL SCALING ANALYSIS (군집분석과 다차원척도법으로 본 유치열의 우식패턴)

  • The purpose of this study was to identify patterns of caries in the dentition of preschool children using cluster analysis and multidimensional scaling. 815 preschool children aged 36-71 months old were examined for caries experience. The first four clusters in the cluster analysis were (1) molars, (2) upper incisors, (3) lower 1st molars, and (4) upper 1st molars in teeth, (1) occlusal surfaces of lower molars and upper 2nd molars, (2) mesial surfaces of upper central incisors, (3) occlusal surfaces of lower 1st molars, and (4) separation between occlusal surfaces of upper and lower 2nd molars in tooth surfaces, and (1) proximal surfaces of upper anterior teeth, (2) occlusal surfaces of lower molars, (3) buccolingual surfaces of upper anterior teeth and (4) buccolingual, proximal, and occlusal surfaces of upper molars and buccolingual and proximal surfaces of lower molars in tooth surfaces groups. In the multidimensional scaling analysis, teeth could be divided into lower 1st molars, upper 1st molars, upper central incisors, upper lateral incisors, lower 2nd molars, upper 2nd molars, and the rest(canines, lower incisors), tooth surfaces could be divided into occlusal surfaces of lower molars, mesial surfaces of upper central incisors, occlusal surfaces of upper molars, and the rest, and tooth surface groups could be divided into proximal surfaces of upper anterior teeth, buccolingual surfaces of upper anteiror teeth, occlusal surfaces of lower molars, and the rest in the order of the distance from others. In the cluster analysis and the multidimensional scaling analysis, caries patterns were different according to the age.

A STUDY ON THE SKELETAL CHANGES IN MAXILLARY PROTRACTION OF THE SKELETAL GLASS III MALOCCLUSION PATIENTS (골격성 III급 부정교합 환자에서 상악골 전방 견인시 일어나는 골격 변화 양상에 관한 연구)

  • A Skeletal Class III malocclusion may be the result of a large mandible, a small maxilla or combination of the two. Protraction devices for the maxilla are used to promote the growth of a deficient maxilla by applying extraoral force to actively growing patients. This study has been performed to determine whether there are significant differences in skeletal and dental changes between FH/Pal 1 and FH/Pal 2 group, SNA 1 and SNA 2 group, SNB 1 and SNB 2 group, and LFH 1 and LFH 2 group after RME and facial mask therapy. The results of this study can be summarized and concluded as follows ; 1. In all patients after maxillary protraction, the maxilla and maxillary dentitions moved forward, and the mandible rotated backward and downward. In most of them, palatal plane is tends to have an upward inclination. 2. The FH/Pal group 1, having an upward inclination of the palatal plane as a result of Facial mast showed statistically significant maxillary forward movement compared to the FH/Pal group 2. 3. The SNA group 1 showed significantly less mandibular backward movement and there was a tendency for the palatal plane to upward inclination compared to SNA group 2. 4. The SNB group 1 showed significantly less maxillary forward movement, but the vertical dimension, especially the lower facial height increased by mandibular downward rotation compared to SNB group 2. 5. LFH group 1, which had large saddle angle and posterior positioned mandible in the pre-treatment stage, showed maxillary protraction effect without significant increase in lower facial height compared to LFH group 2.

A STUDY ON PROFILE CHANGE OF SKELETAL CLASS III MALOCCLUSION PATIENTS AFTER WEARING PROTRACTION HEAD GEAR (골격성 제III급 부정교합 환자에서 상악골 전방견인장치 사용후 측모 변화에 대한 연구)

  • Cause of skeletal Class III malocclusion in growing patients can be classified into maxillary deficiency, mandibular overgrowth, and combination of the two. Use of Protraction Head Gear(P.H.G.) has been recommended for treatment of growing Class III malocclusion patients, for it results in forward & downward movement of maxilla and backward & downward rotation of mandible. Numerous animal experiments were performed and clinical study data have been reported ; nevertheless, studies on soft tissue profile change and comparison of treatment effects among the patients who had undergone treatment are considered to be somewhat insufficient. The author selected 93 patients, who had been diagnosed as skeletal Class III malocclusion with maxillary deficiency and then treated with P.H.G. ; the sample group was divided according to sex, treatment beginning age, palatal suture opening(intraoral appliance), and facial growth pattern. For each group, changing patterns of hard and soft tissue profile observed, and comparision with 20 normal group(Angle's Class I) patients of statistical significance in amount of growth and treatment of hard and soft tissue was done. The following results were obtained. 1. Skeletal, dental, and soft tissue measurements indicated that more growth changes was induced in the sample group that used P.H.G. compared to the growth amount of normal group. 2. No statistical significance was observed in the amounts of maxillary forward movement and mandibular backward & downward rotation depending on treatment beginning age in both sex group. 3. R.P.E. showed more significant maxillary forward movement and less protrusion of upper incisor than La-Li. 4. There was no statistical significance in the amount of maxillary forward movement depending on facial growth pattern. On the other hand, measurements indicating mandibular downward & backward rotation indicated greater change in counterclockwise growth pattern group than the clockwise. 5. Changes in upper and lower lip thicknesses showed a close relationship with positional changes in underlying bone tissue and upper and lower teeth, and upper lip height and nasolabial angle increased and mentolabial angle decreased.

Spatial changes of the maxillofacial complex following maxillary protraction of human dry skull (건조 두개골에서 상악의 전방 견인후 상악 안면 복합체의 공간 변화에 관한 연구)

  • The purpose of this investigation was to study the spatial changes of the maxillofacial complex following maxillary protraction transmitted to the center of resistance of a dry juvenile human skull by a modified maxillary protraction appliance. Four dry juvenile human skulls (without mandible) with well aligned upper deciduous dentition and early mixed dentition were used as experimental samples. A modified protraction headgear was fabricated from a Delare's facemask, and following an alginate impression, an orthodontic resin maxillary splint was made for each dry skull. Protraction force level was maintained at approximately 1000gm per side for 6 hours. Cephalometric radiographs were taken pre- and post- protraction, and nine reference markers with 1.5 mm length of $.017\times.025$ TMA wire were placed on the right side of the skull for an accurate superimposition of serial cephalometric radiographs. The present investigation demonstrated that vertical changes associated with an anterior displacement of the maxillary complex was observed, and the most prominent effect of protraction headgear was a counterclockwise rotation of the maxilla, that is, a forward and downward tipping around the palatomaxillary region.

The effects of maxillary protraction appliance (MPA) depending on vertical facial patterns (수직적 안모 형태에 따른 상악골 전방 견인 장치의 효과 비교)

  • Preadolescent children with deficient maxillae are suitable candidates for the maxillary protraction appliance(MPA). The theoretical effect of the MPA is protraction or anterior displacement of the maxilla. However, it is known that complex effects such as anterior displacement of the maxillary teeth, downward and backward rotation of the mandible, linguoversion of the mandibular anterior incisors, are known to play a role in improving the Cl III malocclusion. There have been much studies with regard to maxillary protraction, but the different effects of MPAs depending on the vertical facial pattern are not known precisely. This study was based on 67 patients (31 males, 36 females) aged from 6 years 6 months to 13 years 3months, who visited the Dept. of Orthodontics at Yonsei Univ., Dental Hospital and diagnosed as skeletal Class III with maxillary deficiency. They were divided into 3 groups (low, average, high angle groups) depending on genial angle and the SNMP (Go-Gn) angle, respectively. Pretreatment and post-treatment lateral cephalograms were used to compare the effects of MPA and the following conclusions were obtained: 1) A significantly large amount of backward movement of the B point was observed in patients with a low SNMP angle. Those with a high SNMP angle had significant forward movement at A point. 2) The patients with low genial angle had the least forward movement at the A point, and those with a high angle had more forward movement. 3) In comparing the arcTan of the A point, the high angle group showed more horizontal movement while the low angle group showed more vertical movement. 4) There was no significance between the treatment duration of the SNMP and the Genial angle groups.
