Full mouth rehabilitation of fully edentulous patient using implant hybrid prosthesis (완전무치악 환자에서 임플란트 하이브리드보철물을 이용한 전악 수복 증례)

  • Loss of teeth may not only imply impaired oral function and loss of alveolar bone but is also often accompanied by reduced self-confidence. This results in a larger problem with the fully edentulous patient. The patient introduced in this study showed multiple missing teeth and mobility of remaining teeth and wanted to have fixed dental prosthesis using implants. Remaining teeth were extracted because of periodontally bad prognosis. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using implant hybrid prosthesis in fully edentulous patient.

Influencing factors on anxiety and depression before and after dental prosthetic treatment in the patients (치아상실 환자들의 보철치료 전과 후의 불안 및 우울에 영향을 미치는 요인)

  • Objectives: The purpose of the study was to investigate the influencing factors on anxiety and depression before and after prosthetic treatment in the patients. Methods: A self-reported questionnaire was completed by 248 patients with tooth loss and over 20 years old. The questionnaire was carried out before and after oral examination and dental prosthetic treatment from July to December, 2013. The study instruments included Beck's anxiety inventory and self-rating depression scale by Zung. The questionnaire was adapted and modified from Kwon's anxiety inventory and Lee's depression scale. Results: While the anxiety level increased from 69.4% to 78.2% in the normal people, the depression level changed from 53.2% to 64.1% in the normal people. The gender variable had a positive effect on anxiety and depression while educational level had a negative effect on them. The dental fear was closely related to anxiety and depression before and after the prosthetic treatment. The entire body health perception showed a negative effect on anxiety and depression. The distrust towards the dentist had an influence on the anxiety and depression in the prosthetic patients. Conclusions: The influencing factors on anxiety and depression in the prosthetic patients included gender, dental fear, entire body perception, and distrust towards the dentist.

  • Patients who lost posterior teeth due to periodontitis or dental caries have collapsed vertical dimension, unstable occlusion and change of the mandibular position. In particular, patients in orthognathic surgery, clinician should re-establish the pre-operative stable position of mandibular condyle in articular fossa and favorable vertical dimension for high post-operative stability of mandible. Therefore, interdisciplinary approach and co-operation, including prosthetics, orthodontics, oral and maxillofacial surgeon, from diagnosis and treatment plan is important to get a good outcome. This case report was patients who had collapsed occlusal plane due to severe dental caries on maxillary molars with skeletal Class III malocclusion. Before orthognathic surgery, resetting of maxillary occlusal plane with temporary removable denture was performed. Then successful multidisciplinary approach was done and lead to acceptable clinical outcome.

Flapless implant surgery on atrophied alveolar ridge in a patient with growth disorder (성장 장애를 가진 부분 무치악 환자에서 위축된 골에 무피판 임플란트 수술 증례)

  • The growth disorder influences craniofacial development and early loss of permanent teeth. This case reports the importance of computerized tomography (CT) and surgical guide to identify horizontal bone loss, adjacent teeth and to guide drills when placing implants in a short stature patient. The patient has idiopathic short stature and the 3rd grade of intellectual disability. To recover posterior mandibular teeth, implant treatment was planned. CT images showed that the adjacent teeth were located markedly to the buccal side. A CT-based surgical guide was fabricated and implants was placed using flapless surgery. Bone dehiscence and fenestration may happen when the surgical guide was fabricated just based on adjacent clinical crowns. Thus, it is essential to analysis bone and teeth and to make surgical guide through CT, especially in atrophied bone on grow disorder patients. Furthermore, systematic researches are recommended to elucidate the relationship between growth disorder and tooth malposition.

Three-dimensional finite element analysis on intrusion of upper anterior teeth by three-piece base arch appliance according to alveolar bone loss (치조골 상실에 따른 three-piece base arch appliance를 이용한 상악전치부 intrusion에 대한 3차원 유한요소법적 연구)

  • At intrusion of upper anterior teeth in patient with periodontal defect, the use of three-piece base arch appliance for pure intrusion is required. To investigate the change of the center of resistance and of the distal traction force according to alveolar bone height at intrusion of upper anterior teeth using this appliance, three-dimensional finite element models of upper six anterior teeth, periodontal ligament and alveolar bone were constructed. At intrusion of upper anterior teeth by three-piece base arch appliance, the following conclusions were drawn to the locations of the center of resistance according to the number of teeth, the change of distal traction force for pure intrusion and the correlation to the change of vertical, horizontal location of the center of resistance according to alveolar bone loss. 1. When the axial inclination and alveolar bone height were normal, the anteroposterior locations of center of resistance of upper anterior teeth according to the number of teeth contained were as follows : 1) In 2 anterior teeth group, the center of located in the mesial 1/3 area of lateral incisor bracket. 2) In 4 anterior teeth group. the center of resistance was located in the distal 2/3 of the distance between the bracket of lateral incisor and canine. 3) In 6 anterior teeth group, the center of resistance was located in the central area of first premolar bracket .4) As the number of teeth contained in anterior teeth group increased, the center of resistance shifted to the distal side. 2. When the alveolar bone height was normal, the anteroposterior position of the point of application of the intrusive force was the same position or a bit forward position of the center of resistance at application of distal traction force for pure intrusion. 3. When intrusion force and the point of application of the intrusive force were fixed, the changes of distal traction force for pure intrusion according to alveolar bon loss were as follows :1) Regardless of the alveolar bone loss, the distal traction force of 2, 4 anterior teeth groups were lower than that of 6 anterior teeth group. 2) As the alveolar bone loss increased, the distal traction forces of each teeth group were increased. 4. The correlations of the vertical, horizontal locations of the center of resistance according to maxillary anterior teeth groups and the alveolar bone height were as follows : 1) In 2 anterior teeth group, the horizontal position displacement to the vortical position displacement of the center of resistance according to the alveolar bone loss was the largest. As the number of teeth increased, the horizontal position displacement to the vertical position displacement of the center of resistance according to the alveolar bone loss showed a tendency to decrease. 2) As the alveolar bone loss increased, the horizontal position displacement to the vertical position displacement of the center of resistance regardless of the number of teeth was increased.

Migrant Multi-Cultural Family Women's Life Quality Related to Oral Health: Survey in Dae-Gu (다문화가족 이주여성의 구강건강관련 삶의 질: 대구지역 조사)

  • This study conducted oral examinations and individual interviews on migrant multi-cultural family women in Daegu and measured their socio-demographic characters, oral health conditions and OHIP-14 in an aim to investigate the relevance between the oral health of migrant multi-cultural family women living in some big cities and their quality of life. Based on data finally collected from 189 women, the t-test, ANOVA and binary logistic regression analysis were conducted and the conclusions are as follows: The average number of decayed teeth was 2.23, loss teeth was 1.48, and treated teeth was 5.58. Women from the Philippines had more number of loss teeth than those from other countries, and women from China relatively had a small number of filled permanent teeth. The quality of life related to oral health was found to be poor in proportion to the number of loss teeth. A comparison of life quality related to oral health depending on loss teeth showed that life quality related to oral health was lowest in the areas of mental discomfort, physical ability decrease, mental ability decrease, social ability decrease and social disadvantage. Life quality related to oral health was found to be low in proportion to the number of permanent teeth with decay experience and poor monthly household income, which shows that the number of permanent teeth with decay experience and monthly income are mostly related to life quality related to oral health. As migrant multi-cultural family women's life quality related to oral health is low in proportion to the number of loss teeth and decayed teeth, it needs to develop a program to improve their oral healthrelated life quality and conduct follow-up research to verify its effect.

Affecting Factors Related to Oral Health Conditions of Workers (근로자의 상실치아 실태와 관련요인)

  • This study was conducted to investigate the health behavior and the oral health status of workers engaged in a manufacturing business, and to analyze the related factors that affect their number if teeth loss. References for the study involved the materials of 526 individuals whose occupational categories are of a manufacturing business, amongst the survey participants in seven city or gun (county) area including Gyeongsan-si, Gyeongju-si, Mungyeong-si, Chilgok-gun, Yecheon-gun and Sangju-gun for the Community Health Survey (CHS) conducted in 2008. The CHS 2008 was conducted for three months from September to November 2008, and the survey details utilized in the study included the number of loss of teeth and teeth-brushing frequency, which was a part of CHS questionnaires and also additionally included for the study. The analysis with respect to the number of teeth loss on sociodemographic factors, was found to be significantly related to the age, educational status, average monthly income and occupation (p<.01). In the $x^2$-test in terms of the oral health status and the number of teeth loss, it was found that the teeth-brushing after lunch (p<.05) and an experience in an oral examination were statistically significant factors (p<.01). According to the multiple-regression analysis with the number of teeth loss being a dependent variable, the age, educational status, marital status and subjective oral health status were the statistically significant (p<.01). In conclusion, it is important to provide education on the teeth-brushing subjected to workers engaged in a manufacturing business with many dental caries and low educational status, and to recommend a regular scaling as well as to establish policy for creating conditions upon an oral health check-up and a tooth scaling and thus allow the maintenance of an oral health.

Full mouth Rehabilitation in a Patient with Occlusal Collapse with Vertical Dimension Increase (교합 붕괴 환자에서 수직 고경을 증가한 보철 수복 : 증례 보고)

  • In a case of multiple posterior teeth loss, antagonistic teeth extrude to the edentulous space and compensatory occlusion on the remained anterior teeth leads to occlusal trauma. Extrusion of antagonistic teeth breaks down occlusion plane and loss of posterior support bring about severe wear of remained teeth. In this situation, it is needed to restore remained teeth and edentulous space by increasing vertical dimension to obtain prosthodontic rehabilitation space and to correct occlusion plane. In this case report, the patient had a masticatory problem with loss of posterior teeth support and an esthetic problem of shortened anterior teeth. Before the tooth preparation for the prosthodontic restoration, the patient used removable device for 2 months to increase vertical dimension reversibly. After that, he got provisional fixed restoration with irreversible tooth reduction and used it for 3 months. It had spent 5 month to evaluate the adaptation state on final restoration with incresed vertical dimension. The increasing amount was 3 mm, which was relatively in less degree and masticatory system adapted to the increased vertical dimension without any pathologic changes. Final restoration was made to have equal-intensity contacts on all teeth in a verifiable centric relations and immediate disclusion of all posterior contacts the moment the mandible moves in any direction from centric relation. In addition, metal occlusion surface on posterior teeth was applied to prevent excessive muscle activation, occlusal trauma and the porcelain fracture.

TREATMENT OF CLASS I CROWDING WITH EXTRACTION OF THE SECOND PERMANENT MOLAR (제2대구치 발치를 이용한 Class I crowding의 치험례)

  • Since Non-Extraction treatment in some orthodontic case shows unstable result and unfavorable profile, extraction is selected as inevitable treatment option for the harmonious profile, facial skeleton, and the stable dentition on both arches. For the achievement of proper goal, premolars, molars, sometimes incisors or canines are selected to be extracted. The first Premolar is usually extracted for relieving the crowded dentition with which the molar relations are class I to gain stable dentition and proper profile, but often results in the depression of profile or the loss of vortical dimension. On the contrary, the extracton of the second molar helps maintaining the fullness of profile and the vertical dimension, prevents additional space closural procedures which often make the procedures complicated, relieves both anterior and posterior crowding, and substitutes the extraction fossae for newly erupting 3rd molars. From the point of recurrence, the second molar extraction procedure, therefore, is more beneficial. This cases showed the good results of second molar extraction procedures in the patients with class I crowding.

Comparison of Radiographic Measurement and Clinical Parameters of Periodontal Condition on Premolars and Molars (소구치 및 대구치의 치주임상지수와 방사선학적 측정과의 상관관계에 대한 연구)

  • 치주상태의 진단은 일반적으로 치주탐침과 같은 임상적 검사와 보조적으로 방사선 검사를 이용한다. 방사선학적인 검사가 치주조직의 파괴정도와 골 흡수를 진단하는데 유용하지만 실제적인 골 흡수를 정확하게 나타나는데는 한계가 있다고 하였다. 그리고 임상적 측정과 방사선 사진을 비교연구하여 이들간의 관계를 알아보고자 하는 노력이 있었다. 방사선이 임상에서 참고사항으로는 중요하지만 절대적 신뢰를 주는데는 문제가 있는 것으로 생각되어왔다. 방사선학적 측정과 임상지수 측정간의 상관관계에대하여 계속적인 연구를 통해 규명하는 것이 중요하다고 생각되어 저자는 연세대학교 치과 대학병원 치주과를 내원한환자중 연령 및 성별에 관계없이 전신질환이 없는 환자 130명을 대상으로 소구치의 원.근심부, 대구치의 원.중.근심부에서 치주낭의 깊이와 (P.D), 부착상실 (A.L)을 측정하였다. 방사선 사진상의 원.근심부에서 백악 법랑질 경계부에서 치조골능 상부까지의 치조골 수준 (Rx.B.L)을 측정하였고 이개부 부위에서는 이개부의 천정으로부터 나타나는 골 흡수의 방사선 투과상을 측정하였다. 이를 기초로 임상지수와 방사선학적 측정간에 상관관계를 연구하여 다음과 같은 결론을 얻었다. 1. 원·근심부의 P.D와 Rx.B.L을 비교 했을 때 상관관계는 모든 치아에서 높게 나타났다(r=0.897$\sim$r=0.725, p<0.01). 2. 원·근심부의 A.L와 Rx.B.L을 비교 했을 때 상관관계는 모든 치아에서 높게 나타났다 (r=0.915$\sim$r=0.732, p<0.01 ). 3. 대구치 769개가 실험 대상인었다. 상악 대구치중(356개) 40%가 이개부 병소를 나타냈고 (146개), 하악 대구치중(413개) 41%가 이개부 병소를 나타냈다(170개). 4. 대구치의 이개부 병소의 중앙부에서 측정한 P.D와 Rx.B.L을 비교 하였을 때 상관관계는 상악 제1대구치를 제외한 모든 치아에서 높게 나타 났다(r=0.811$\sim$r=0.435, p<0.01). 5. 대구치의 이개부 병소의 중앙부에서 측정한 A.L와 Rx.B.L을 비교 하였을 때 상관관계는 상악 제1대구치를 제외한 모든 치아에서 높게 나타 났다(r=0.906$\sim$r=0.479, p<0.01). 이상의 결과를 종합하여 볼 때 임상지수와 방사선학적 측정을 비교한 바, 이들의 상관관계는 모든 치아에서 밀접하게 나타났다. 따라서 방사선학적 측정은 임상지수와 더불어 치주질환의 진단과 치료계획에 크게 기여할 수 있을 것으로 사료된다.
