산욕기 질병관리(유열)

  • 목장 경영의 성공과 실패에는 여러 가지 요인이 작용하지만 연중 얼마나 적은 소가 도태되는가 하는 것 또한 아주 중요하게 생각해 보아야 할 사항이다. 낙농에 있어 거의 대부분의 질병이 산욕기에 발병하는데 산욕기(産褥期)란 분만으로 인한 상처가 완전히 낫고, 자궁이 평상시 상태가 되며 신체의 각기관이 임신 전의 상태로 회복되기까지의 기간으로 대개 분만 후 6~8주간을 말한다. 산욕기에 주로 발생할 수 있는 다양한 질병(산욕기 부전마비, 난산, 급성유방염, 제4위 전위증 등)들이 조기에 발견되어 적절히 치료되지 못하고 만성화되면서 전해질불균형, 에너지와 단백질의 섭취부족에 의한 영양장애로 허약과 지방간 및 케토시스 등으로 진행되어 결국 도태됨으로서 목장 경영에 있어 막대한 영향을 끼치는 것이다. 이러한 산욕기 질병의 주 원인은 건유기 사양관리에 있다. 건유기간 동안 체점수(BCS)의 변화를 최대한 줄이면서 분만전의 스트레스를 얼마만큼 줄일 수 있도록 관리하는가에 달려있다. 다시말해 분만 전 마지막 한달간에 건유에서 분만으로의 이행과정을 얼마나 매끄럽게 연결 지어주는가에 따라 산욕기 질병의 발생률이 결정되는 것이다. 본 고에서는 건유기 사양관리 소홀로 발생되는 산욕기 부전마비에 관해서 자세히 설명해 보고자 한다.

  • 국내 젖소 우군 사육규모의 다두화, 고능력우의 증가에 따라 발정발견의 실패, 미약발정와 건유기 및 이행기에 영양관리, 건강관리, 생리적 또는 환경적인 스트레스의 증가에 따라 번식률이 저하되고 있는 실정에 있다. 또한 우군 규모가 대규모화로 젖소에서 발생하는 질병의 대부분이 분만을 중심으로 100일간에 집중되어 발생하고 있어, 우군 질병의 제어가 중요하다. 또한 분만 전 후의 산욕기에 대사성 질병의 증가에 따라 번식장애를 초래하는 경우가 많으므로 이에 대한 예방관리가 중요하다 하겠다. 따라서 소에서 산욕기의 생리 및 질병예방프로그램, 소 임신 진단요령과 번식률 제고를 위한 발정과 배란의 control의 처치에 응용되고 있는 최근의 프로그램에 관한 응용의 실제를 소개하고자 한다.

Health Care Experiences of Vietnamese Marriage Immigrant Women during Pregnancy, Childbirth, and Postpartum Period in Korea (베트남 결혼이주여성의 임신, 분만, 산욕기 건강관리 경험)

  • Purpose: This study was conducted in order to identify and describe the experiences of health care during pregnancy, birthing, and postpartum period for Vietnamese marriage immigrants. Methods: The participants were 15 Vietnamese married immigrant women who became pregnant and gave birth within the last five years. Data were collected by in-depth interview with Vietnamese women. Data were analyzed using Colaizzi's method of phenomenology. Results: Six theme clusters were extracted as follows: 'being left with no other option in loneliness and longing filled in a limited life', 'continued trials and errors amidst frustration and fear', 'silently following orders despite the quality of medical services that change with each medical care provider', 'compromise by selecting amidst confusion between the Korean way and the way at home', 'depending on family, who is the communication channel, but becoming disappointed', and 'finding the reason for existence and struggling by herself to become a mother amidst doubled confusion'. Conclusion: A program for effective empowerment of Vietnamese immigrant women should be developed. In addition multicultural family centered programs should be developed with emphasis on acceptance of women's culture, respect for her culture, and supports. Medical staffs and nurses should also improve culturally sensitive competence in order to provide care for immigrant women.

Development and Validation of a Postpartum Care Mobile Application for First-time Mothers (초산모를 위한 산후관리 모바일 앱 개발 및 효과 검증)

  • Purpose: The aims of the study were to develop mobile application for postpartum care of first-time mothers and to validate it's effect. Methods: Using a nonequivalent control group pretest-posttest design, 52 first-time mothers were recruited (26 each in experimental and control) and the experimental group used the mobile application for 6 weeks after delivery. Postpartum self-care knowledge and confidence, infant care knowledge and confidence, and postpartum depressive mood (Edinburgh Postnatal Depression Scale) were measured before discharge from the hospital and 6 weeks later. Results: Women who have used the postpartum care mobile application reported higher levels of postpartum self-care knowledge (p=.030) and confidence (p=.023) infant care knowledge (p=.001) and confidence (p=.004), while scores of postpartum depression (p=.021) were lower than those in the control group. Conclusion: The postpartum-care mobile application developed in this research may be effective in reinforcing knowledge and confidence for postpartum self-care and infant care and in reducing postpartum depressive mood.

A Study on the Effects of Father Class upon Father's Level of Knowledge and Confidence about Postpartal Woman and Newborn Care (산후 아버지 교실이 산욕기관리와 신생아 돌보기의 지식정도와 자신감에 미치는 영향에 관한 연구)

  • The purpose of this study is to find out how the Father Class, which is being practiced at a hospital located in Seoul, influences on fathers' level of knowledge and confidence of postpartal mother and newborn care. Research design was quasi-experimental study using non equivalent experiment and control group. The data, questionaires, were collected from Sep. 13, 2001 to Oct. 31, 2001 through e-mail, telephone and mail, after discharge from S hospital located in Seoul, where the experimental group was educated at the Father Class and control group didn't take part in the class. The objects were the fathers who have the first baby through normal delivery or C-sec delivery including experimental group 48 persons and control group 52 persons. Four instruments were the revised ones of Lee Mi Kyoung's(1990) and Kwak Yon Hee's tool based on the resources of direct interviews of fathers and one expert, professor. The reliability of four instruments were Cronbach's $\alpha$ scores .85 -93. Data were analyzed by SPSS 10.0 program using $x^2$-test, and t-test and, One- way ANOVA, and Pearson correlation, etc. The results of this study are as follows; 1.The two groups were confirmed as a homogeneous group by showing no significant difference statistically at the level of 5%. 2.The results of proving the hypothesis are followings: 1)The first hypothesis, "the fathers who participated in the Father Class will have the higher level of knowledge about postpartal mother care than the fathers who didn't" was accepted by significant difference statistically (p=0.000). 2)The second hypothesis, "the fathers who took part in the class will have the higher confidence in postpartal mother care than the fathers who didn't", was supported by significant difference statistically (p=0.000). 3) The third hypothesis, "the fathers who attended the class will have the better knowledge about taking care of newborn than the others", was accepted by significant difference statistically (p=0.000). 4)The fourth hypothesis, "the fathers who participated in the class will have the higher confidence in newborn care than others," was supported by significant difference statistically (p=0.000). 3.The further analysis showed whether they participated in Prenatal Birth Preparing Class influenced on the father's level of knowledge and confidence in postpartal mother and newborn care. In conclusion, the Father Class is the effective nursing intervention strategy that can help new fathers attain father and husband roles by enhancing the level of confidence in and knowledge of the postpartal mother and newborn care.

Effects of a Reinforcement Program for Postpartum Care Behavioral Skills of Couples with Their First Baby (산욕기 첫 출산 부부를 위한 산후관리 행동기술 강화 프로그램 효과)

  • Purpose: This study aimed to investigate the effects of a reinforcement program for behavioral skills in postpartum care for couples with their first baby. Methods: The study used a non-equivalent control group and pretest-posttest design. It was conducted from January 14 to April 10, 2016 at a postpartum care center in D city. It analyzed 43 couples (22 in the experimental group and 21 in the control group.) For data analysis, descriptive statistics, test of homogeneity in pretest, independent t-tests, and repeated measures ANOVA were used. Results: For maternal fulfillment of postpartum care and postpartum fatigue, there was no significant difference in the interaction between group and time. In terms of parent-newborns attachment, the interaction between group and time showed a significant difference for mothers (F=13.63, p=.001) and fathers (F=6.51, p=.001). In marital intimacy, the interaction between group and time showed a significant difference for mothers (F=14.40, p<.001) and fathers (F=9.46, p=.004). In parenting stress, the interaction between group and time showed a significant difference for mothers (F=31.8, p<.001) and fathers (F=11.69, p=.001). A significant difference was found for the mothers' postpartum sleeping hours (F=0.14 p=.004). Conclusion: This program for behavioral skills in postpartum care, which is based on the information-motivation-behavioral skills model, improves postpartum care, parent-newborn attachment, marital intimacy, parenting stress, and maternal postpartum sleeping, by reinforcing behavioral skills required for postpartum care.

Analysis of Research Trends on Postpartum-women Healthcare : A Scoping Review (스코핑 고찰 방법을 통한 산욕기 산모 건강관리에 대한 연구동향 분석)

  • Objectives: The purpose of this scoping review is to describe the research trends of postpartum healthcare for postpartum women, as well as to identify further direction for research in the area of postpartum healthcare. Methods: This study was conducted sequentially according to the Scoping Review Method referred to as 'frameworks of Arskey and O'Malley'. We searched for articles published as of May 15th, 2020 in seven Korean databases in order to obtain the data. A key term search strategy was employed, including terms such as "Postpartum period care", "Postpartum health", "Postpartum care", and "Postpartum status" to identify relevant studies. Results: Eighty-three articles were selected from a total of 259. The research has been undertaken steadily since 1997, particularly in the fields of Nursing and Korean medicine. In terms of study design, survey studies were the majority (48.2%), followed by interventions and experimental papers (32.5%). However, qualitative research areas are lacking. The research topics were largely divided into two categories: postpartum care characteristics and postpartum care interventions. The characteristics of postpartum care were highly connected with care for mental health. Among the postpartum care interventions, the most common employed was Korean medicine intervention. Conclusion: This study confirms that management of mental health during the postpartum period is necessary. In addition, it is necessary to identify the experiences of postpartum women and the effectiveness of a postpartum care intervention program through the utilization of qualitative research methods. In particular, such research is needed in the field of Korean medicine.

Advanced Aged Women's Needs for Pregnancy and Childbirth Care (고령임산부의 임신과 출산 건강관리 요구)

  • Purpose: Advanced aged pregnancy may be related with health problems so that more aggressive health care is necessary for these women. This study aimed to provide the basic data for developing nursing intervention programs to enhance the health of pregnant women and their new-born babies and by identifying the advanced aged women's need for pregnancy and childbirth. Methods: It is the cross-sectional descriptive study to identify the advanced aged women's need on pregnancy and childbirth. Subjects were pregnant women 35 years or older and postpartum women. Total number of subjects was 95. Measurement tool is self-reporting survey that consisted of 67 items with four-point Likert scale, which was completed during October to November 2014. Results: Average score was 3.44 out of maximum 4 on the care need on pregnancy and childbirth. Average scores according to category were as follows: baby rearing and parental role, 3.55; preconception care, 3.49; delivery care, 3.47; postpartum care 3.42; and prenatal pregnancy, 3.39. The degree of needs on pregnancy and childbirth was different according to delivery experience (t=-2.49, p=.014). Conclusion: Prenatal and postpartum nursing interventions were completed regardless of pregnant women's age until now; however, new nursing intervention programs are necessary to prevent the risk of advanced aged pregnancy, to provide the preconception care, and to increase the infant care and family support.

A Preliminary Study with the Objective of Developing a Community based Postpartum Women's Follow-up Program (지역사회 중심의 산욕기 산모 추후관리 프로그램 개발을 위한 기초조사)

  • The purpose of this study was to discern the level of satisfaction of postpartum care in hospitals. the level of importance of traditional postpartum care and the need for home health care for postpartum women. The study's design was to obtain data needed to develop a community-based postpartum care program and to improve the quality of nursing for postpartum care recipients. Data for the study was collected from 1 March to 31 June 2000 from 116 postpartum care recipients treated in Chung-Buk, Korea. Data analysis consisted of frequency, percentage, t-test, and ANOVA. using SPSS win program. The results the of analysis were as follows: 1) The level of satisfaction of postpartum care in the hospital (mean 1.31) was very low. The most laudable items of nursing care by category were: moderate satisfaction with 'perineal wound care' (2.04) and 'afterpain observation'(2.09). The overall level of satisfaction of postpartum care in the hospital. however, was very low. 2) Among other items of postpartum care in the hospital, 'postpartum exercise' $(25.9\%)$. 'assessment and support for postpartum depression' $(25.9\%)$, 'operation wound care for women who underwent Caesarean sections' $(24.5\%)$. and 'contraception and family planning' $(20.4\%)$ showed a need for home health care for postpartum care recipients above $20\%$. 3) The level of importance of traditional postpartum care (Sanhujori) was relatively high (mean 2.72). The importance of Sanhujori by category was as followed: 'the fourth principle: protecting the body from harmful strains' was the highest (2.88), 'the third principle: eating well', 'the first principle: invigorating the body by augmenting heat and avoiding cold', 'the second principle: resting without working', 'the fifth principle: keeping clean' and 'the sixth principle: handling with the whole heart' showed 2.85, 2.80, 2.70, 2.51 and 2.46 respectively. 4) The need for home health care with Sanhujori was very low. 5) The relationship between demographic factors and the level of satisfaction with postpartum care in the hospital was as follows: the satisfaction levels were significantly different among' delivery frequency' and 'health status' alteration after delivery'. 6) The relationship between demographic factors and the importance of the Sanhujori category was as follows: There were no significant differences in the level of importance of the first and the forth principle of Sanhujori. The level of importance of the second principle of Sanhujori was significantly different among 'income' and 'family type'. The level of importance of the third principle of Sanhujori was significantly different among 'having a boy'. The level of importance of the fifth principle of Sanhujori was significantly different among 'income' and 'feeding type'. The level of importance of the sixth principle of Sanhujori was significantly different among 'education background' and 'feeding type'. In conclusion. the findings of this study illustrate the nursing needs of postpartum care recipients. It provides a challenge to caregivers in the healthcare industry to develop a continuous postpartum care program and integrative postpartum care system that embodies the oriental and western paradigm for the promotion of women's health.

Clinical Study on Prenatal care, and Dietary Intakes for Pregnant Women and new Mothers (임산부의 산전관리와 산욕기 영양실태에 관한 연구)

  • This study was projected to provide basic data on prenatal care for future direction in maternity and child care, and also to investigate the diet of women during pregnancy and the period directly afterwards in order to offer to mothers appropriate advice for the improvement of nutritional standards. A clinical study on prenatal care was based on 1054 delivery cases. A nutritional survey was performed on 174 mothers admitted to the department of obstetrics at St. Mary's Hospital during the period of March, 1975 to February, 1976. The results obtained are summarized as follows; I. Clinical study on prenatal care 1) The age distribution showed 59.4% of the mothers were between the ages of 25 to 29 years old. 2) The gestational period was highest between the 37th and 40th gestational weeks. 33.7% of the mothers were primigravidae and 31.8% of them primiparae. 3) 41.3% of the mothers had not received prenatal care or had only received it once before. 4) Induced deliveries were 61.8% and spontantaneous deliveries 38.2%. 61.9% of the mothers had received prenatal care, while those without prenatal care accounted for 61.6% of the total induced deliveries. 5) Low birth weights were 7.7% and 5.0% of the mothers had received prenatal care, while 11.5% had no prenatal care. 6) There were 1.13% of still births, 0.32% of the mothers had prenatal care and the remainder did not have prenatal care. 7) Of those receiving prenatal care, 2.1% showed in the $0{\sim}3$ Apgar score group, 6.3% in the $4{\sim}6$ Apgar score group, and 91.6% in the $7{\sim}10$ Apgar score group. Among the non-prenatally cared for group 5.0% of the newborns were in the $0{\sim}3$ Apgar score group, 9.7% were in $4{\sim}6$ Apgar score group and 85.3% were in the $7{\sim}10$ Apgar score group. 8) Obstetrical complications were developed in 11.86% of the pregnant women when they were hospitalized. Among the group receiving the prenatal care 8.1% of the mothers had obstetrical complications. In the group without prenatal care 17.16% of the mothers had obstetrical complications. The most common obstetrical complication was malpresentation. 9) The first prenatal care was received between the 37th and 40th gestationl weeks. II. Food intake during pregnancy The following are the results from the questionnaires of the mothers concerning diets during pregnancy; 1) Main meals and snacks In 32.2% of the cases, their main meals during the diet amounted to more than was usually eaten at other times. In 67.8% of the cases, their main meals during the diet were the same as that usually eaten. In 22.4% of the cases, snacks during the diet amounted to more than usually eaten at other times. In 77.6% of the cases, snacks during the diet were the same as usually eaten. 2) Itemized list The mothers made a special effort to include certain items in their diets, the following is a breakdown of those items; a. egg, meat, fish 33.3% b. fruit, vegetables 32.2%. c. milk, fruit juice 18.4% d. cake, bread 2.9% e. nothing special 13.2% 3) Milk 44.8% of the mothers had at least one cup of milk everyday. 33.4% of the mothers had at least one cup of milk on occasion. 15.5% of the mothers did not have any milk. 4) Vitamins 39.7% of the mothers had vitamins everyday. 24.7% of the mothers had vitamins occasionally. 35.6% of the mothers did not have any vitamins. 5) Anemic symptoms 9.2% of the mothers very often had anemic symptoms during pregnancy. 39.1% of the mothers often had anemic symptoms during pregnancy. 51.7% of the mothers did not have anemic symptoms at all. 6) Taboos on food 23% of the mothers recognized 'taboos' on food during pregnancy 27% of the mothers displayed on uncertainty about the 'taboos' on food during pregnancy 50% of the mothers displayed indifference toward the taboos. III. Nutritional survey on the new mothers diet. 1) The diets for new mothers can be divided into four categories, such as general diet, low sodium diet, soft diet and liquid diet. 2) Cooked rice and seaweed soup were the main foods for the new mothers as has been the traditional diet for Korean mothers. 3) The average diet contained 1,783g. And the average consumption of the basic food groups per capita per day was 1,265g for cereals and grains, 456g for meats and legumes, 58g for fruits and vegetables, 0g for milk and fish and 4g for fats and oils. 4) In addition to the 1,783g of food in the main diet there was also 142.8g of food taken as snacks. 5) The average daily consumption of calories and nutrients was 2,697 Kcal and 123.4g for proteins, 44.9g for fats, 718.2mg for calcium, 14mg for iron, 2,101.4 I.U. for vitamin A, 0.43mg for thiamine, 1.02mg for riboflavin, 15.88mg for niacin, 5.26mg for ascorbic acid. When these figures are compared with the recommended allowances for new mothers in Korea, the calories and nutrients taken in were satisfactory. But the intake of minerals and vitamins was below the recommended allowance.
