PULSE OXIMETER AS A DIAGNOSTIC TEST OF PULP VITALITY (Pulse Oximeter를 이용한 치수생활력측정)

  • Traditionally, EPT and thermal tests were used as diagnostic methods for pulp vitality test. The thermal and electrical stimulation tests are the methods to determine the vitality of a tooth based on its neuronal response. These have certain limitations, one of them is the difficulty of approaching the correct result in case of treatment of children. The reason is management problem caused by the unpleasant stimulation. Also, the response from patients are not objective, and false positive or false negative could be happened. Recently, laser doppler flowmetry and pulse oximeter which evaluate vascular integrity are introduced in an effort of overcoming to limitation of traditional methods. The principle of pulse oximeter is to and out level of oxygen saturation by ratio of the two pulses between emitted light and detected light penetrating them to the termination of body, such as ears or fingers. From this point of view, it can be applied to a tooth to determine its vitality. The objective of this study lies mainly on varifying pulse oximeter as a method of determining tooth vitality and providing basic data of its clinical implementation. The result of the research showed that level of oxygen saturation in vital teeth was average of 96.3% and 0.0% in pulpless teeth. As a comprehensive result, pulse oximeter could be an useful diagnostic equipment in determining of tooth vitality.

Effect of pH and Oxygen Back-flushing on Hybrid Water Treatment of Tubular Ceramic MF and Photocatalyst Loaded Polyethersulfone Beads (관형 세라믹 정밀여과와 광촉매 첨가 PES 구를 이용한 혼성 수처리 공정에서 pH 및 산소 역세척의 영향)

  • The effects of pH and oxygen back-flushing were investigated in hybrid process of ceramic microfiltration and PES (polyethersulfone) beads loaded with titanium dioxide ($TiO_2$) photocatalyst for advanced drinking water treatment in viewpoints of membrane fouling resistance ($R_f$), permeate flux (J), and total permeate volume ($V_T$). As increasing pH, $R_f$ decreased and J increased. Finally the maximum $V_T$ could be acquired at pH 9. Treatment efficiencies of turbidity was almost same independent of pH. Treatment efficiency of dissolved organic matters (DOM) decreased as increasing pH. As results of comparing the oxygen and nitrogen back-flushing, $R_{f,180}$ at oxygen back-flushing was the lower than that at nitrogen back-flushing, and the dimensionless final permeate flux ($J_{180}/J_0$) by initial permeate flux ($J_0$) at oxygen back-flushing was maintained the higher than that at nitrogen back-flushing except 10 and 12 min of back-flushing period (FT). Treatment efficiency of turbidity at oxygen back-flushing was a little higher than that at nitrogen back-flushing. Treatment efficiency of the DOM at nitrogen back-flushing was the higher than that at oxygen back-flushing. Also, treatment efficiency of turbidity at saturated oxygen was similar with those of oxygen and nitrogen back-flushing, but the treatment efficiency of DOM was increased significantly because OH radical could be generated by reaction between saturated oxygen and photocatalyst.

  • Hepatopulmonary syndrome is a triad that includes: hepatic dysfunction, intrapulmonary vascular dilatation and abnormal arterial oxygenation. The incidence of intrapulmonary vascular dilatations, in adults with end-stage liver disease, has been reported to be 13% to 47%, however the incidence in children is unclear and the cases in Korean children have never been reported. The hepatopulmonary syndrome may occur as a result of chronic liver disease following nonalcoholic steatohepatitis in children with hypothalamic or pituitary dysfunction. We report a case of hepatopulmonary syndrome in a 13-year-old child who had rapidly progressive liver dysfunction secondary to panhypopituitarism after craniopharyngioma resection. Careful monitoring and treatment of endocrine abnormalities and metabolic status, as well as liver function, are required in all children undergoing pituitary tumor resection.

고온고압 수화학분위기가 원자로 압력용기강쟤 SA508 cl_3의 피로균열 성장속도에 대치는 영향

  • 고온고압 수화학 분위기를 모사한 조건에서 피로균열성장 거동에 관한 연구를 수행하였다. 고온고압분위기 모사를 위해 Load machaine, Autoclave, Water loop, 균열측정장치(Reversing DCPD) 그리고 전기화학변수 측정장치 등을 설치하였다. 우선 공기분위기 및 상온수화학 분위기에서의 시험을 통해 안정성을 확인하였으며, 안정한 조건에서 고온수화학분위기에서 실험을 수행하였다. 수화학 분위기에서 용존산소에 상관없이 취성파면이 관찰되었다. 용존산소의 양이 적을 경우(10ppb이하) 발견된 취성파면의 양은 연성파면에 비해 훨씬 적었으며, 용존산소가 높을 경우(8000ppb) 취성파면의 양이 많이 발견되었다. 산소포화된 고온수화학 분위기 피로시험결과는 피로균열성장이 주로 취성파면에 의해 이루어졌으며, 균열성장속도 또한 크게 증가하였다.

  • Tumor hypoxia caused by the unique characteristics of solid tumor sites such as lowered vascular density, irregular vasculature, longitudinal oxygen gradient, and unbalanced oxygen consumption has decreased therapeutic efficacy in several clinical trials such as radiation, chemotherapy, and surgery. Hence, tumor oxygenation studies at microvascular levels are important to provide better understanding of the complexity of microvasculature oxygen transport and exchange with tissue. However, polarographic microelectodes, was employed to measure $pO_2$ at the microvasculature level, but it is difficult to perform and does not provide significant spatial and temporal information of oxygen delivery. In this research, we introduce the hyperspectral imaging system able to provide a wide range of vascular characteristics by spatial maps on hemoglobin saturation information for better understanding of the relationship between blood oxygen delivery, hypervascularity, aberrant angiogenesis at microvasculature levels during tumor growth.

The Role of Jugular Venous Oxyhemoglobin Saturation Monitoring During Cardic Surgery (개심술 중 경정맥 산소포화도의 역할)

  • Postoperative brain damage is one of most serious complications of cardiopulmonary bypass (CPB). To prevent brain damage during CPB, adequate cerebral perfusion for cerebral oxygen demand should be maintained. This study monitored jugular venous oxyhemoglobin saturation ($SjO_2$), which reflects the overall balance of cerebral oxygen supply and demand, intermittently in 10 patients undergoing cardiac surgery. At the initiation of CPB, in spite of a significant decrease in mean arterial pressure, $SjO_2$ did not change, and it was stable during the hypothermic period of CPB. But a significan reduction in $SjO_2$ was observed during the rewarming period, and $SjO_2$ had an inverse linear correlation with esophageal temperature. Furthermore, the percent decrease of $SjO_2$ was related to rewarming speed. Therefore, therapeutic approaches for $SjO_2$ desaturation include slower rewarming, increasing cerebral blood flow, decreasing the cerebral metabolic rate for oxygen, increasing oxygen content, and increasing perfusion flow rate.

On an "Um~" Vocal Breathing to Relieve Stress When Wearing a Mask (마스크 착용시 스트레스 해소를 위한 "음~"발성호흡 연구)

  • In the post-epidemic era, COVID-19 has not yet been fully controlled. Wearing masks is still the main means of epidemic prevention, and the negative effects brought by masks continue to continue. Wearing a mask for a long time can cause two problems. The first problem is hypoxia, and the other is an increase in psychological stress. To reduce the negative impact of masks, this paper proposes a new breathing mode. It is the "Umm~" vocal breathing mode, which simultaneously solves the two problems of hypoxia and increased stress. This paper explores the reasons why new breathing patterns can relieve stress. Explains the relationship between HRV and stress index and uses SDNN as an indicator to detect stress index to confirm the effectiveness of this breathing pattern. Experimental results prove that the "Umm~" vocal breathing mode can not only relieve the stress induced by wearing a mask. And when not wearing a mask, it can also be used to relieve daily stress. This method that anyone can easily implement should be more popularized.

The Differences of Anthropometric and Polysomnographic Characteristics Between the Positional and Non-positional Obstructive Sleep Apnea Syndrome (체위 의존성 및 체위 비의존성 폐쇄성 수면 무호흡증후군의 신체계측인자 및 수면구조의 차이)

  • Backgrounds : Obstructive sleep apnea syndrome(OSA) can divided into two groups, positional(PP) and non-positional(NPP) obstructive sleep apnea syndrome, according to the body position while sleeping. In this study, we evaluated the differences of anthropometric data and polysomnographic recordings between the two types of sleep apnea syndrome. Materials : Fifty patients with OSA were divided two groups by Cartwright's criteria. The supine respiratory disturbance index (RDI) was at least two times higher than the lateral RDI in the PP group, and the supine RDI was less than twice the lateral RDI in the NPP group. This patients underwent standardized polysomnographic recordings. The anthropometric data and polysomnographic data were analyzed, statistically. Results : Of all 50 patients, 30% were found to be positional OSA. BMI was significantly higher in the PP group(p<0.05). Total sleep time was significantly longer in the PP group (350.6$\pm$28.2min, 333.3$\pm$46.0min, (p<0.05). Sleep efficiency was high in the PP group(89.6$\pm$6.4%, 85.6$\pm$9.9%, p<0.05). Deep sleep was significantly higher and light sleep was lower in the PP group than in the NPP group but no difference was observed in REM sleep between the two groups. Apnea index(AI) and RDI were significantly lower( 17.0$\pm$10.6, 28.5$\pm$13.3, p<0.05) and mean arterial oxygen saturation was higher in the PP group(92.7$\pm$1.8%. p<0.05) than in the NPP group. Conclusion : Body position during sleep has a profound effect on the frequency and severity of breathing abnormalities in OSA patients. A polysomnographic evaluation for suspected OSA patients must include monitoring of the body position. Breathing function in OSA patients can be improved by controlling their obesity and through postural therapy.

The Effects of $O_2$ Partial Prewwure on Soft Magnetic Properties of Fe-Hf-O Thin Films (Fe-Hf-O계 박막에서 산소 분압 변화가 박막특성에 미치는 영향)

  • The effect of $O_2$ partial pressure on microstructure and soft magnetic properties of as-deposited Fe-Hf-O thin film alloys, which are produced by rf magnetron sputtering method in $Ar+O_2$ mixed gas atmosphere, are investigated. Saturation magnetization ($4{\pi}M_s$) of Fe-Hf-O film were decreased with increasing $O_2$ partial pressure, the best soft magnetic properties exhibit at $O_2$ partial pressure of 10%. With further increase of $O_2$ partial pressure, soft magnetic properties decreased continuously. The $Fe_{82}Hf_{3.4}O_{14.6}$ film with $P_{O2}=10%$ exhibits good soft magnetic properties with $4{\pi}M_s=17.7kG$, $H_c=0.7Oe$ and ${\mu}_ {eff}$ (1~100 MHz)=2,500, respectively. The addition of O is effective in grain refinement. In case of $P_{O2}=15%$, it is observed that $Fe_3O_4$ compound is formed and high frequency soft magnetic properties are decrease. The electrical resistvity($\rho$) of Fe-Hf-O film is increased with increasing $O_2$ partial pressure. Electrical resistivity of $Fe_{82}Hf_{3.4}O_{14.6}$ film was 5 times higher than that of the film without oxygen. Thus, it is considered that the good magnetic properties of $Fe_{82}Hf_{3.4}O_{14.6}$ film results from decreasing the $\alpha$-Fe grain size by precipitates (Hf and O), high electrical resistivity.

Changes of Blood Gases, Plasma Catecholamine Concentrations and Hemodynamic Data in Anesthetized Dogs during Graded Hypoxia Induced by Nitrous Oxide (아산화질소에 의한 점진적 저산소가스 흡입이 혈중 가스치와 Catecholamine치 및 혈역학에 미치는 영향)

  • The sympathoadrenal system plays an important role in homeostasis in widely varing external environments. Conflicting findings, however, have been reported on its response to hypoxia. We investigated the effect of hypoxia on the sympathoadrenal system in dogs under halothane anesthesia by measuring levels of circulating catecholamines in response to graded hypoxia. Ten healthy mongreal dogs were mechanically ventilated with different hypoxic gas mixtures. Graded hypoxia and reoxygenation were induced by progressively decreasing the oxygen fraction in the inhalation gas mixture from 21%(control) to 15%, 10% and 5% at every 5 minutes, and then reoxygenated with 60% oxygen. Mean arterial pressure, central venous pressure and mean pulmonary arterial pressure were measured directly using pressure transducers. Cardiac output was measured by the thermodilutional method. For analysis of blood gas, saturation and content, arterial and mixed venous blood were sampled via the femoral and pulmonary artery at the end of each hypoxic condition. The concentration of plasma catecholamines was determined by radioenzymatic assay. According to the exposure of graded hypoxia, not only did arterial and mixed venous oxygen tension decreased markedly at 10% and 5% oxygen, but also arterial and mixed venous oxygen saturation decreased significantly. An increased trend of the oxygen extraction ratio was seen during graded hypoxia. Cardiac output, mean arterial pressure and systemic vascular resistance were unchanged or increased slightly. Pulmonary arterial pressure(PAP) and pulmonary vascular resistance(PVR) were increased by 55%, 76% in 10% oxygen and by 82%, 95% in 5% oxygen, respectively(p<0.01). The concentrations of plasma norepinephrine, epinephrine and dopamine increased by 75%, 29%, 24% in 15% oxygen and by 382%, 350%, 49% in 5% oxygen. These data suggest that the sympathetic nervous system was activated to maintain homeostasis by modifying blood flow distribution to improve oxygen delivery to tissues by hypoxia, but hemodynamic changes might be blunted by high concentration of nitrous oxide except PAP and PVR. It would be suggested that hemodynamic changes might not be sensitive index during hypoxia induced by high concentration of nitrous oxide exposure.
