  • Tooth movement facilitated by corticotomy and distraction osteogenesis, new paradigm in orthodontics, was discussed. Intrusive tooth movement of anterior or posterior teeth was thought to be difficult or impossible. In this study, a part of cortical bone, which was a sort of resistance to tooth movement in alveolar bone, was removed. On the other hand, active bone deposition was made possible in the tension side. That was the main concept of tooth movement facilitated by corticotomy and distraction osteogenesis. Teeth moved at such a speedy tate as we could not imagine in conventional tooth movement, which lead to the reduction of total treatment Period. And intrusive movement was Possible without a side effect, lot example, root resorption or the periodontal breakdown. Those were the superior aspects to the conventional orthodontics.

  • This study was undertaken to investigate the effect of vitamin C deficiency on the orthodontic tooth movement and bony remodeling processes. Thirty six male guinea pigs were divided on the basis of the given amount of vitamin C (normal group: 5mg/day, deficient group: 0.2mg/day) and 75gm of force was applied to the maxillary incisors. Experimental animals were sacrificed at day 0. day 1 day 3, day 5. day 7 and day 14 after force application and the amount of tooth movement was measured and tissues were studied histologically. The results showed that the amount of collagen fiber in the periodontal ligament and alveolar bone of the deficient group was less than that of the normal group. In the stretched side. the osteoblastic activity and alveolar bone formation of the normal group increased in a time dependent manner during experimental periods, but the deficient group showed less activity and formation. The amount of tooth movement in the deficiency group was more than in the normal group at day 0. day 1, day 3, day 5, and day 7. According to the above results, a deficiency of vitamin C resulted in a defect of collagen synthesis of the periodontium and inhibition of bone formation and stimulation of bone resorption with rapid tooth movement in early periods of force application.

The effects of pregnancy on alveolar bone turnover during experimental tooth movement in rats (백서에서 실험적 치아이동시 임신이 치조골 교체(Turnover)에 미치는 영향에 대한 연구)

  • The purpose of this study was to observe the effects of pregnancy on the experimental tooth movement and alveolar bone turnover process of Sprague-Dawley female rat. Sixty rats were divided into pregnant-tooth movement group(P-Tm), normal-tooth movement group(N-Tm) and normal group(N). Maxillary first molar appliances were inserted bilaterally and activated to 40grams. To measure the amount of tooth movement, x-ray was taken 2 times after appliance insertion and before sacrifice. Animals were sacrificed at 1,3,7,14 days(N=5). Just after sacrifice, alveolar bones were collected and frozen immediately for biochemical analysis. Tooth movement was assessed cephalometrically and tartrate-resistant acid(TRAP) and alkaline phosphatase (ALP) activities were measured in extracts of paradental alveolar bone. The results were as follows: 1. The amount of tooth movement in P-Tm group was greater than that of N-Tm group(p<0.01). 2. Alveolar bone ALP of normal tooth movement group was not significantly different from the control, TRAP was significantly different from the control(p<0.01). In normal tooth movement group, alveolar bone ALP was increased gradually and peak(day 7) fell off significantly at day 14(p<0.05). The Peak of alveolar bone TRAP(day 7) fell off slightly, sustained day 14(p<0.01). 3. Alveolar bone ALP and TRAP of pregnant tooth movement group were not significantly different from that of normal tooth movement group. In pregnant tooth movement group, alveolar bone ALP was increased at day 3(p<0.01) and fell off significantly at day 7-14, alveolar bone TRAP were increased at day 3 and sustained day 14. 4. The peak of alveolar bone phosphatases in pregnant tooth movement group(day3) preceded the peak in normal tooth movement group(day7) (p<0.01). According to the above results, we suggested that bone resorption activity was increased in alveolar bone of pregnant rat, and the degree of tooth movement in pregnancy may be greater than that of normal group because of high bone turnover of alveolar bone in pregnant rat.

In this study, a section of cortical bone, which may act as resistance to tooth movement in alveolar bone, was removed. Active bone deposition was also made possible in the tension side This forms the main concept of tooth movement facilitated by corticotomy and distraction osteogenesis. Molars in two cases were uprighted at such a speedy rate as we could not imagine in conventional tooth movement. which lead to reduction of the total treatment period. And molar uprighting was possible without side effects. for example, supraversion oif the tooth or anterior anchorage loss. These were the superior aspects to conventional orthodontics.

  • Self-ligating brackets have a permanently installed movable component to entrap the archwire which reduces much of the friction created by conventional ligation. Therefore, it allows for rapid tooth movement and shorter overall treatment time. Due to these advantages, interest in self-ligating brackets is increasing. In this report, several practical considerations for using self-ligating brackets are given to appreciate and maximize their benefits. It is thought that self-ligating brackets make shorter and more efficient treatment possible.

  • This study was undertaken to investigate the relation between orthodontic force magnitude and the amount of tooth movement. And more light force application for reducing root resorption Twenty-four rats were divided into three experimental groups(A, B, C) based on force magnitude and application method. Springs of 50g force were applied to A group, springs of 100g force were applied to B group and springs of 25g force were applied to C group initially, and after 4 days springs of C group were changed to springs of 50g force. Two kinds of $sentally^{(R)}$(GAC U.S.A.) closed coil spring, 50g and 100g, were used. And we made 25g springs by heat treatment process of 50g springs. Each spring was inserted between the maxillary central incisor and the maxillary left first molar. Amounts of tooth movement were measured everyday by digital caliper($Digimatic^{(R)}$, Mitutoyo, Japan) under inhalation anesthesia for 15 days. After 15 days, all rats were sacrificed and histological samples were obtained with Hematoxyline-Eosin stain and Masson's trichrome stain. Following conclusion were made; 1. Group B showed the mean cumulative tooth movement of $2.19{\pm}0.41mm$ at 15th day, which was greatest among three groups, followed by group C($2.06{\pm}0.10mm$), group A($1.90{\pm}0.49mm$) respectively. however, there was no statistically difference among three groups. 2. All groups showed general tooth movement pattern and A, B, C group finished lag phase at 9th, 8th, 7th day, but there was no statistical significance. 3. Group A,B,C showed root resorption and especially group B showed the most severe root resorption and group C showed milder root resorption than other groups. According to the above results, large initial force with the development of a flirty widespread hyalinized zone may cause severe root resorption, so initial force should be applied lightly to reduce hyalinized area and eventually root resorption and then increased force will induce efficient tooth movement.

The Effect of Splinting Methods on the Rearrangement of Periodontal Fibers after Tooth Movement in Adult Dogs (치아이동 후 고정방법이 성견 치주인대 섬유의 재배열에 미치는 영향)

  • The purpose of this study was to evaluate the effect of splinting methods on the rearrangement of periodontal fibers after experimental tooth movement. Orthodontic force was applied by placing closed coil spring between upper third incisor and canine in seven dogs, weighing 20 kg or more. After 3 weeks of force application, 0.0215 inch multistrand wire and polyethylene ribbon were bonded to each side, as a flexible and rigid splinting respectively in 6 experimental animals. The remaining one served as a control. Each two animals were sacrificed at 4, 8 and 12 weeks after splinting respectively and prepared histologically for hematoxylin-eosin and Masson's trichrome staining. The results of this study were obtained as follows: 1. After tooth movement, periodontal space was narrowed and periodontal titers were thick on pressure side while elongated fibers were observed on tension side. 2. After 4 weeks of retention, the rearrangement of periodontal fibers was observed in the flexible splinting group, but not in the rigid splinting group. 3. After 8 weeks of retention, the rearrangement of periodontal titers was observed in both groups, but the difference could not be detected between two groups. 4. During the retention period, the rearrangement of periodontal fibers was faster in tension side than in pressure side. These results show that the rearrangement of periodontal fibers is also obtained by rigid splinting after tooth movement. It is suggested that the rigid splinting by polyethylene ribbon can be used as a way of postorthodontic retention.

  • The prolonged neglect of the posterior teeth missing area may cause mesial drift, extrusion, unexpected movement of the adjacent teeth and alveolar bone loss with occlusion collapse. Therefore it is recommended to treat that area by the prosthesis as soon as possible after tooth missing. However, if orthodontic treatment is applied to move the remained teeth, it can create improved biomechanical dentoalveolar environment. The use of the third molars in teeth missing area provides advantages as optimizing of prosthesis size. However, crown shape, location, soundness of the third molar and possible of eruption failure should be considered. In this case report, two patients closed a second teeth missing site and reduced the size of the first and second teeth missing area for an implant by protraction of impacted third molars. This case reports the considerations for closing or reducing the posterior teeth space with protracting the third molars by comparing two patients.

  • 수술과 교정의 혼합술식으로 안면부의 기형과 부수적으로 발생하는 부정교합을 치료하는 경향이 점차적으로 증가하고 있다. 수술-교정 술식은 과거 20년동안, 특히 1970년대에 무척 빠른 속도로 발전하였다. 수술-교정 술식이 발달함에 따라 이전에는 수술이나 교정치료 단독으로는 치료하기가 어려웠던 case들의 치료가 가능하게 되었다. 교정의사들은 성장이 완료된 성인의 심한 skeletal dysplasia를 치아이동에 의해서만 치료하는 데에는 매우 제한이 많다는 것을 인식하게 되었으며, 반면에 구강외과 의사들은 수술전후의 교정치료에 의해 더욱 좋은 수술 결과를 얻을 수 있는 동시에 좋은 functional occlusion을 이룰수 있다는 사실을 인식하였다. 이러한 수술-교정 술식은 교정의사들과 구강외과의사들에게 새로운 면들을 요구하고 있다. 좋은 치료결과를 얻기 위해서는 교정의사와 구강외과의사간의 긴밀한 협조관계가 필수적이다. 이러한 협조관계를 유지하기 위하여는 서로 상대 전문분야의 용어, 진단원리, 치료기법 등에 대한 이해가 선행되어야 한다. 이 논문에서는 진단, 치료계획, 수술전 교정치료, 수술술식, 수술후 교정치료, 증례보고의 순서로 기술하려 한다.
