Exploring the Thalamus of the Human Brain using Tractography Analysis at 3Tesla MRI (3 Tesla MRI에서 트랙토그래피 분석을 이용한 시상 탐색)

  • Thalamus is known to play an important role in the regulation of nerve function. Thalamus, located in the center of the brain, is involved in sleep, arousal, and emotional regulation, and has been reported to be associated with multiple sclerosis, essential tremors, and neurodegenerative diseases such as Parkinson's disease. In addition, it has been reported that iron deposits in the thalamus can cause depressive symptoms with age. Although there are discrepancies between studies, it can be deduced that the thalamus region has a clear effect on neurological disorders due to a strong relationship between the thalamus and neurological functions such as emotional control and processing. Through tractography analysis, the connectivity between the detailed areas of each subcortical region was investigated in the form of a matrix, showing strong connectivity and weak interhemispheric connectivity. In the 59> group, the WM connectivity of thalamus was found to be weaker than those of the two groups. Comparisons between the two groups showed that the young groups (10-39 and 40-59) had higher connection intensity than the 59> group and that statistically significant differences in 3 connection pathways were found in each hemisphere. A decrease in thalamus-related connection strength in aging has shown that it can affect emotional and neurological disorders such as anxiety and depression, and network measurements can help assess cognitive impairment across clinical conditions.

Factor Structure of the Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile in Infants and Toddlers With Mental Disorders and the Difference Among Mental Disorders (정신장애 영유아에 대한 K-CBCL 1.5-5 (Korean-Child Behavior Checklist 1.5-5) 조절곤란 프로파일의 요인구조와 정신장애 간 차이검증)

  • Object : This study aimed to identify the factor structure of Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile (K-CBCL 1.5-5 DP) in infants and toddlers with mental disorders and verify differences in K-CBCL 1.5-5 DP among the diagnosis groups. Methods : The participants were 265 mothers of infants and toddlers with mental disorders who completed K-CBCL 1.5-5 DP. The data was analyzed using AMOS 25.0 and SPSS 25.0. Results : First, the bifactor model was the most suitable for the factor structure of the K-CBCL 1.5-5 DP. Second, there were significant differences among the diagnosis groups, such as communication disorders, pervasive developmental disorders, emotional disorders, and developmental delays. It was confirmed that the pervasive developmental disorder and emotional disorder groups showed significantly higher dysregulation compared with the communication disorder group. Conclusion : This study confirmed that infants and toddlers had dysregulation problems. Using the bifactor model, the multidimensional nature of the K-CBCL 1.5-5 DP was assessed. It was also meaningful that dysregulation could contribute to onset and deepening of symptoms of pervasive developmental disorders and emotional disorders in infancy.

Development and Validation of Indirect Trauma Scale of Social Disaster (사회적 재난으로 인한 간접외상 척도의 개발과 타당화 연구)

  • The purpose of this study was to develop scale for the measurement of indirect traumatization occurred by social disaster and test their validity. To achieve this purpose, this study conduct a research through the following procedure. First, to develop the inventory, various responses of indirect traumatization occurred by social disaster were gathered from Korean adults participated in open questionnaire. 41 items were made. Second, exploratory factor analysis were performed and 21 items were selected in this step. The Indirect Trauma Scale of Social Disaster(ITSSD) consisted of 4 factors, each with 4-to-8 items, respectively. Four factors include ① private coping responses ② symptom responses ③ distrust responses of world ④ moral emotion cause of social perpetrators. Appropriate levels of reliability were established for the ITSSD. Third, Indirect Trauma Scale of Social Disaster was validated by confirmatory factor analysis, and 21 items were fixed. To 300 participants differed from development step, confirmatory factor analysis was performed. 4 factors structure derived from the exploratory analysis was appropriate. And 4 factors indicated reasonable fit index such as TLI(.913), CFI (.924) and RMSEA(.077). In addition, ITSSD identified a significant positive correlation with Posttrauma Risk Checklist, Korean Depression Scale, State-Trait Anxiety Inventory-KYZ and negative with Acceptance-action Questionnaire-2. But that was unrelated to Forgiveness Trait Scale and Life Satisfaction Scale. Also the result showed that Women experienced high level of indirect trauma of social disaster than men.

Clinical Characteristics and Heart Rate Variability of Foreign Domestic Violence Victims in Korea (국내 거주 외국인 가정폭력 피해 여성의 임상적 특징 및 심박변이도)

  • Objectives: Domestic violence is related to many psychiatric diseases, such as depression, anxiety disorder, and PTSD. Heart rate variability (HRV) is an index of autonomic control of the heart and is related to cardiovascular and emotional disorders. Although there have been some studies on the effects of domestic violence on women's mental health, relatively little information is available on HRV in this population. The aim of this study is to investigate demographic data, psychological features, and HRV in female victims of domestic violence and difference between Korean and foreign female victims. Methods: A total of 210 female victims of domestic violence (166 Korean women and 44 foreign women) were recruited for this study. Psychological symptoms were measured using the Hamilton Rating Scale for Anxiety (HAM-A), Hamilton Rating Scale for Depression (HAM-D), and Impact of Event Scale-Revised (IES-R). HRV measures were assessed by time-domain and frequency-domain analyses. Results: The mean score of HAM-A was 13.81, that of HAM-D was 12.92, and that of IES-R was 33.61 ; there were no significant differences between Korean and foreign women in these measures. In HRV time domain analyses, approximate entropy (ApEn) was significantly increased in foreign women compared to the Korean women. The square root of the mean of the sum of the squares of differences between adjacent NN intervals (RMSSD) was significantly decreased in foreign women compared to Korean women. There were no significant differences in the other HRV variables between Korean and foreign women. Conclusion: Female victims of domestic violence in Korea are associated with depression, anxiety, and PTSD symptoms. The physiologic factors of a female victim's nationality could be related to higher ApEn and lower RMSSD in foreign female victims. These findings have important implications for future study to study the relationships among ethnic and environmental factors and HRV variables.

The Patient Recognition, Acceptability and Evaluation of Feasibility for Day Surgery (종합병원 일부 입원환자의 당일수술에 대한 태도와 당일수술 적용 가능성 평가)

  • Objectives . This study was conducted in order to investigate the degree of recognition, acceptability, and altitude towards day surgery of patients who were hospitalized with diseases that were candidates for day surgery; in order to analyze the average length of stay for treatment of the ailments; and to analyze the percentage of patients who could be discharged on the same day after the surgery ,using the post-anesthesia discharge scoring system. Methods : Data was collected between February 1 and March 31, 1999 from 353 patients who received surgery for cataract, adenoid hypertrophy, inguinal hernia, strabismus, ptosis, cholelithiasis, hemorrhoid, or anal fistula, at a general hospital in Daegu city. The patients were interviewed and surveyed by a post-anesthesia discharge scoring system(PADS) in order to collect data on patient condition such as vital signs, activity and mental status, pain, nausea and vomiting, surgical bleeding, intake and output after the surgery. Results : Among the 353 patients, 52.7% were after of the day surgery and 52.7% were interested in day surgery. Of the respondents, 43.1% said 'my ailment was not serious and the surgery was simple' and 30.4% said 'according to my condition rest at home was desirable' as the reasons for wanting day surgery Alternatively, 56.5% of those declining day surgery said the 'uncertainty of staying home' was the reason. The greatest concern in discharging within 24 hours after surgery was a post-op emergency situation. On the other hand, the shortened hospitalization was the largest advantage of day surgery with 39.1% responding this way, followed by the savings in hospitalization costs (25.8%) and emotional stability (13.7%). The majority of those surveyed (47.6%) believed that discharge should be determined within 1-2 days after the surgery. The average hospital stay was 3.1 days for dischargeable ailments. Pain (45.6%), nausea and vomiting (10.5%), and headache (7.9%) were the common symptoms following surgery. The percentage of patients who were able to be discharged within 24 hours after surgery revealed 95.2% were dischargeable after approximately 3 hours, 99.2% dischargeable after 12 hours, and 100% dischargeable after 24 hours. Conclusions : According to the PADS score, the cataract extract and strabismus correction patients were eligible for day surgery and the further evaluation concerning the reason for delayed recovery of the other diseases is needed.

From the Onset of Panic Symptoms to Getting to a Psychiatric Treatment : The Change by Improved Public Awareness of Panic Disorder in Korea (증상의 발현부터 치료의 시작까지 : 한국인의 공황장애 인식도 변화가 치료적 접근에 미친 영향)

  • Objective : This study aimed to investigate the general process from the symptom onset to the psychiatric treatment in Korean panic patients and the effect of improved public awareness on it. Methods : This study has a retrospective design. The subjects were the new patients with panic disorder who visited the psychiatric outpatient clinic in twelve university-affiliated hospitals all across Korea. The medical chart was reviewed retrospectively and the data were collected including chief complaints of symptoms, recent stressors, the time to visit the psychiatric outpatient clinic, and visit of other departments and diagnostic approaches for their symptoms. Results : A total of 814 participants were included in the study. The most common department other than psychiatry the panic patients visited were cardiology (28.3%), general internal medicine (16.0%) and neurology (11.4%). The most frequently used diagnostic tests were a echocardiography (17.9%), 24-hour Holter monitoring (11.2%), and brain MRI (8.2%). Only 37.3% of participants visited psychiatric clinic directly. About 80% of participants visited psychiatric department within 1 year after their first panic symptoms and it took $13.8{\pm}13.7weeks$ on average. Comparing before and after 2012, the number of participants increased who visit directly the psychiatric clinic without visiting other departments (p=0.002) and without visiting emergency room (p<0.001). Conclusions : Our results suggest that a substantial number of patients visit departments other than psychiatry when they experience first panic symptoms. However, most patients begin psychiatric treatment within 1 year after their first symptoms and the number of patient are increasing who visit psychiatric department directly without visiting other departments.

Result of Multiphasic Military Personality Inventory Among Grave's Disease in 19-Year-Old Men (19세 남성에서 그레이브스병의 다면적 군인성검사 결과)

  • Objectives : The purpose of this study was to quantitatively assess the degree of Graves' Disease affecting mental health through the results of multiphasic military personality inventory (MPI) in 19-year-old men. Methods : We collected and analyzed the results of the MPI for 592 healthy subjects and 148 Graves' disease patients among the 19-year-old men who visited the Military Manpower Administration from February 2008 to January 2010. Results : Among demographic characteristics, both Graves' disease and control group are 19-year-old men. Educational level, socioeconomic status, parental relationship, and BMI were not statistically different between the two groups (χ2=0.089, p=0.766; χ2=5.090, p=0.278; χ2=5.988, p=0.112 ; t=-0.635, p=0.526). In the MPI, among validity scales, the score of the faking-good subscale was significantly lower in the Graves' group (t=3.507, p<0.001), but in the faking-bad and infrequency subscales scores were not significantly different between two groups (t=-1.700, p=0.090; t=-0.519, p=0.604). Among the neurosis scale, anxiety, depression, somatization, and personality disorder subscale scores were all significantly higher in Graves' disease group (t=-3.323, p<0.001; t=-4.210, p<0.001; t=-6.202, p<0.001; t=-2.872, p<0.01). Among the psychopath scale, each score of schizophrenia and paranoia subscales were not statistically different between the two groups (t=-0.158, p=0.874; t=-0.846, p=0.398). Conclusions : According to MPI result, we can confirm that Graves' disease patients in 19-year-old-men may appear features such as anxiety, depression, somatization and personality tendency (introversion or impulsivity) than the control group.

The Development of the Korean Form of Childhood Attention Problem(CAP) Scale: A Study on the Reliability and Validity (한국형 소아기 집중력 문제척도: 신뢰도 및 타당도 연구)

  • The purpose of this study was to examine the reliability and validity of a Korean form of Childhood Attention Problem(CAP) scale. CAP were administered to 98 normal elementary school students as control group and 98 attention deficit hyperactivity disorder patients. Male students showed high scores than female students in both subscale and total scores, but not statistically significant. There were no significant difference in CAP scale between male students and female students in attention deficit hyperactivity disorder patients. In the reliability test, the test-retest reliability coefficient was highly satisfactory and that of inattention subscale was 0.83, impulsivity subscale was 0.70 and total score was 0.82. In the reliability test by internal consistency, the Cronbach $\alpha$ coefficient was highly satisfactory and that of inattention subscale was 0.91, overactivity subscale was 0.89(p<0.05). The concurrent validity between CAP scale and ADDES-BV scale was 0.85 in attention deficit hyperactivity disorder patient group and 0.73 in normal control group(p<0.05). In discriminant validity test between attention deficit hyperactivity disorder patient group and normal control group, the patient group showed higher score(p<0.05). The total discriminant capacity of the patient group in CAP was 93.4%. In this point of view, CAP scale showed high reliability and validity in applying to Korean subjects and was proved to be the good and simple screening test tool for attention deficit hyperactivity disorder research and can help many young patient to treat early.

Development of the Perceived Stress Response Inventory (스트레스반응 지각척도의 개발)

  • The perceived stress response inventory(PSRI) was developed to measure 4 types of current stress responses : emotional, somatic, cognitive, and behavioral responses. 242 patients with psychiatric disorders(71 patients with anxiety disorders, 73 patients with depressive disorders, 47 patients with somatoform disorders, 51 patients with psychosomatic disorders) and 215 healthy subjects completed the questionnaire including the PSRI. Global assessment of recent stress(GARS) scale, perceived stress questionnaire(PSQ) and symptom checklist-90-revised(SCL-90-R) were also administered at the same time. Factor analysis for each of 4 types of stress responses yielded 8 factors : negative emotional responses, general somatic symptoms, specific somatic symptoms, lowered cognitive function and general negative thinking, self-depreciative thinking, impulsive-aggressive thinking, passive-responsive and careless behavior, and impulsive-aggressive behavior. Both test-restest reliability(r= .83 -.93) and internal consistency(Cronbach's alpha : .79 -.96 for each of 8 subscales and .98 for total items of the scale) were all at statistically significant levels. Total scores of the PSRI significantly correlated with total scores of GARS scale, PSQ, and global indicies of SCL-90-R, respectively. The patient group had significantly higher scores than healthy subjects in each of all the subscales except impulsive-aggressive behavior subscale. These results suggest that the PSRI is a reliable and valid tool stable over time which may be effectively used for the research in stress-related field including psychosomatic medicine.

Effect of Repeated MSG Administration on Cognitive Ability and Anxiety of Juvenile Rats (MSG의 반복투여가 어린랫드의 인지능력과 기억력에 미치는 영향)

  • The objective of this study is to investigate the effect of MSG on cognitive function and anxiety by the T-maze and elevated-plus-maze test and repeated oral dose toxicity in SD rat of MSG. The rats were treated with MSG of control group, low group (3 g/kg) and high group (5 g/kg) intragastrically for 4 weeks, respectively. We examined the body weight, the clinical signs, T-maze, Elevated-plus-maze, hematological analysis and serum biochemical analysis, we also observed the histopathological changes of liver, kidney in rats. No significant differences in body weights, biochemical analysis and histopathological observations between control and MSG treatment group were found. In the elevated plus maze (EPM), MSG-treatment group has more open arm visited than controls. MSG-treatment group has been more activated in T-maze test. These data indicate the continuous high MSG intake could be increased the anxiety and could be decreased cognitive ability. In conclusion, MSG is physiologically safety, but high MSG intake could be increased the anxiety and could be decreased cognitive ability in juvenile rat.
