  • 연구목적 : 조선(朝鮮) 왕실자녀(王室子女)의 출산과정을 일지(日誌) 형식으로 기록한 "호산청일기(護産廳日記)"는 후기 조선(朝鮮)의 임산(臨産)과 분만, 산후 및 신생아 증상에 대한 의학적 처치 및 산실배설(産室排設)과 출산의례를 고찰할 수 있는 가치 있는 의학사료(醫學史料)이다. 연구방법 : 1. "호산청일기(護産廳日記)"에 기록된 세 번의 출산에 대해 출산 전후 산부(産婦)와 신생아의 실제 상황과 증상, 의관(醫官)의 판단과 치법(治法), 처방(處方)내용, 치료경과 등 의학적 처지의 내용을 살펴봄으로써 당시 산과의학(産科醫學)의 경향성, 발달정도 등을 가늠해보았다. 2. "호산청일기(護産廳日記)"에 기록된 세 번의 출산에 대해 조선왕실(朝鮮王室)의 출산형식 및 관련의례에 드러난 당시 의학의 출산에 대한 의철학(醫哲學)이 어떠하였는지 고찰하였다. 결과 및 결론 : 산전(産前) 의관(醫官)의 정기문진으로 산모와 의사 간 관계형성 및 산부(産婦)의 안정을 도모하였으며, 의녀(醫女)의 진찰소견을 의관(醫官)이 판단하여 처방하였다. 임산(臨産)에 불수산(佛手散)과 인삼차(人蔘茶), 산후어혈증(産後瘀血症)에 가미궁귀탕(加味芎歸湯)을 빈용(頻用)하였으며, 화반곽탕(和飯藿湯)의 식치(食治)가 주요 산후조리법이었다. 신생아의 구급(救急)에 특히 우황(牛黃)을 빈용(頻用)하고 증상에 따라 유모(乳母)가 복약(服藥)하기도 하였으나, 조산(早産)된 신생아의 청색증(靑色症)(cyanosis)으로 보이는 증후(症候)와 사망례(死亡例)가 보인다. 산실배설(産室排設) 및 현초(懸草), 권초제(捲草祭) 등 관련의례에는 출산을 '하늘과 직접 소통하는 하나의 의례(儀禮)'로 생각하여 외부 환경의 방해 없이 산부(産婦)와 신생아의 천계(天癸)가 잘 작동하도록 배려한 의철학(醫哲學)이 드러나 있다.

A comparative study of the acoustic characteristics of the vowel /a/ between children with spastic and dyskinetic cerebral palsy (경직형과 불수의운동형 뇌성마비아동의 /아/ 모음 음향학적 비교)

  • This study aims to compare the acoustic characteristics of vowel phonation in children with spastic and dyskinetic cerebral palsy (CP). Thirty-four children aged 4-12 years with CP participated in the study (spastic 26, dyskinetic 8). Voice samples for the acoustic analysis were extracted from a sustained vowel /a/. All acoustic measures were made using Praat. Group differences were compared by an independent t-test or Welch-Aspin test, if the equivalence assumption was not met. The results of this study are as follow. First, maximum phonation time(MPT) was significantly shorter for the dyskinetic CP than for the spastic CP. Second, shimmer percent was significantly increased in the dyskinetic CP than in the spastic CP. Lastly, there were no significant group differences in both the first formant and the second formant. These findings indicate that the dyskinetic CP has a poorer respiratory capacity and poorer laryngeal function than the spastic CP. On the other hand, both groups have a comparable ability to articulate the vowel /a/. The results of the present study help speech language pathologists identify the speech motor control ability of children with two types of CP (spastic and dyskinetic) and help to make an intervention plan associated with a specific type of CP.

A Literature Review about Labor theory and practice - Focused on Bulsusan(佛手散) - (임산(臨産) 의론(醫論)과 의방(醫方)에 대한 문헌고찰(文獻考察) - 불수산(佛手散)을 중심으로 -)

  • Object : From the ancient times, the importance of childbirth has been well recognized by Korean Traditional Medicine. Numeral methods have been developed to ease the labor process and keep the mother and child healthy from conception to labor. Bulsusan(佛手散) is one of the main remedies to healthy labor in KTM, both widely known and applied as well. Method : This paper examines the labor theory and practice of KTM focused on Bulsusan which is composed of Angelica gigas Nakai(當歸) and Cnidium officinale MAKINO(川芎). Result : 1. From the ancient times until the Q$\bar{i}$ng period, much attention was placed to the handling of the placenta, as it was conceived as bearing much relation to the health of the mother and her fate, and thus included in the labor process. 2. There was a recognition of the 'birth pulse[離經脈]', an intense change in the pulse that presents itself prior to somatic signs of labor. 3. There were numerous prescriptions that were administered beforehand to ease the process. They are mostly constituted with medicinals that nurture Gi(氣) and stimulate its flow, which in turn makes the fetus firm and reduces the volume, easing the labor process. 4. The medical practice of labor-induction was called 'Choesaeng(催生)'. The prescriptions which functioned as such were mostly constituted with blood medicinals such as Angelica gigas Nakai and Cnidium officinale MAKINO, those which nurture both Gi(氣) and blood, and medicinals that physically lubricate the labor pathway such as honey, oil and Talcum(滑石). Conclusion : Bulsusan can be used in most problems concerning pregnancy and labor, and cases of emergency blood loss due to injury. The term 'bulsu(佛手)' infers to the medical ability of the great doctor who takes care of major blood-loss situations resulting from discharge of dead fetus, cesarean delivery, etc. The prescription name takes after this meaning, as it deals with similar conditions in its effect.

Constitution of Prescription and Medicinal Effect & Adaptation Diseases of 'Bullsoosan(佛手散)' in Korean Medical Books (한국(韓國) 의서(醫書)에 보이는 불수산(佛手散)의 처방구성(處方構成)과 효능(效能)·주치(主治)에 대한 고찰)

  • Subjects : A literature research on the constitution and medicinal effect & adaptation diseases of "Bullsusan". "Bullsusan" is a herbal prescription composed of Angelicae Gigantis Radix(當歸) and Cnidii Rhizoma(川芎). Objectives : Through the researching on the records of "Bullsusan" in Korean Traditional Medical Books, gain the literature evidence for adaptation to these days child labor as a pre-labor keeping herbal medicine. And have detailed consideration on the constitution of prescription and medicinal effect & adaptation diseases of "Bullsusan". Methods : First, researched the records of "Bullsusan" in Korean Traditional Medical Books which were included at A Series of Korean Medicine(韓國醫學大系) and analysed component ratio, nickname, herbal manufacture and drug processing method, medicinal effect and adaptation diseases. Second, referred related Korean and Chinese researches that examined the medicinal effect and adaptation diseases of "Bullsusan" by scientific experimentation. Conclusions : We found total 46 records of "Bullsusan" from 20 kinds of Korean Traditional Medical Books included at A Series of Korean Medicine. Prescription component ratio of Angelicae Gigantis Radix and Cnidii Rhizoma were 3:2, 1:1, 2:1, 1:1. 3:2 had most 20 records and 1:1 had second 14 records. Especially 1:1 had a tendency of having nickname "Goonguitang", but not must had. First herbal manufacture was powder, it had 8 records. First drug processing method was decocting with water and alcohol, had 19 records. Medical Effects of "Bullsusan" can be induced to next 8, that were "remove get bad blood, give birth new blood", "easy labor by reducing fetal volume", "acceleration of labor", "test of fetal survival, elimination of dead embryo", "elimination of placenta", "revive", "allaying pain", "nourish the blood". From these medical effects, 9 adaptation diseases can be induced. That were "threatened abortion", "womb ache and vaginal bleeding by spontaneous abortion", "pre-labor keeping(prevention of hard labor)", "acceleration of labor", "hard labor", "missed abortion", "postnatal vaginal bleeding, dizziness, asthma, headache, womb ache", "postnatal mastoptosis and mastodynia", "first aid symptom like as dizziness, unconsciousness, stroke caused by excessive bleeding". The medical effect of "acceleration of labor" and "elimination of placenta" have been examined by modern clinical research. The effect of "remove get bad blood, give birth new blood", "allaying pain" and "nourish the blood" have been examined by modern experimental study. But overdosing on "Bullsusan" to pregnant mouse can cause natural abortion, so the proper dose of "Bullsusan" in pregnant period is very important.

A study on antioxidant and anti-inflammatory effects of domestic blended essential oils (국내산 블렌딩 정유의 항산화 및 항염 효과 연구)

  • Previous studies have been conducted on domestic materials as a single extract. Research on complex mixtures for maximizing plant characteristics by individual extraction and potential interference with effects is insufficient. Therefore, this study confirmed the GC-MSD according to the extraction of essential oils for Agastache rugosa O. Kuntze(AR), Pinus densiflora Sieb. et Zuccarini(PD), Curcuma longa, Curcuma domestica(CC), Zingiber officinale Roscoe(ZR), Foeniculum vulgare Miller(FV), and Citrus medica L. var. sarcodactylis Swingle(CS). The cytotoxicity, antioxidant, and anti-inflammatory properties of the blending oil were confirmed to confirm its potential as a cosmetic material. As a result of analyzing GC-MSD aroma components, the main components were estragole of AR, à-Pinene for PD, Zingiberene for CC and ZR, Anethole from FV, and D-Limonene for CS. At a concentration of 100 uL/mL with no confirmed cytotoxicity, NO production was inhibited by 70.62%, DPPH radical scavenging activity was 64.03%, and ABTS radical scavenging activity was 89.55%. Through this, blended essential oil suggests the possibility of useful application as a raw material with antioxidant and anti-inflammatory effects in the cosmetic and food industries.

  • This study selected 9 species of domestic native plants (Baechohyang, Red Pine, Turmeric, Ginger, San parsley seed, Bulsugam, Hwangchil, Tangja, Valerian) that have been proven effective for depression, and based on the analysis results of each fragrance component, 9 species were selected. Antioxidant (DPPH, ABTS), cytotoxicity (MTS), and anti-inflammatory (Nitric oxide) experiments were performed by combining essential oils. As a result of the analysis of fragrance components, DL-Limonene (38.44%), g-Terpinene (8.9%), Estragole (5.18%), and a-Pinene (1.73%), which were previously studied in combination oil, were identified. DPPH radical scavenging ability, which is an antioxidant activity, showed 75.4%, ABTS radical scavenging ability was 74.04%, and NO production inhibition was 33.14% at a concentration of 5uL/ml with no cytotoxicity confirmed. Through this, the effect of domestic blending essential oils on the improvement or prevention of depression is verified, and scientific efficacy and ingredient studies are conducted in a mutually cooperative manner to seek solutions to depression and provide basic data to confirm whether or not depressive symptoms are improved. want to do.

Comparison of using CBCT with CT Simulator for Radiation dose of Treatment Planning (CBCT와 Simulation CT를 이용한 치료계획의 선량비교)

  • The use of cone-beam computed tomography(CBCT) has been proposed for guiding the delivery of radiation therapy. A kilovoltage imaging system capable of radiography, fluoroscopy, and cone-beam computed tomography(CT) has been integrated with a medical linear accelerator. A standard clinical linear accelerator, operating in arc therapy mode, and an amorphous-silicon (a-Si) with an on-board electronic portal imager can be used to treat palliative patient and verify the patient's position prior to treatment. On-board CBCT images are used to generate patient geometric models to assist patient setup. The image data can also, potentially, be used for dose reconstruction in combination with the fluence maps from treatment plan. In this study, the accuracy of Hounsfield Units of CBCT images as well as the accuracy of dose calculations based on CBCT images of a phantom and compared the results with those of using CT simulator images. Phantom and patient studies were carried out to evaluate the achievable accuracy in using CBCT and CT stimulator for dose calculation. Relative electron density as a function of HU was obtained for both planning CT stimulator and CBCT using a Catphan-600 (The Phantom Laboratory, USA) calibration phantom. A clinical treatment planning system was employed for CT stimulator and CBCT based dose calculations and subsequent comparisons. The dosimetric consequence as the result of HU variation in CBCT was evaluated by comparing MU/cCy. The differences were about 2.7% (3-4MU/100cGy) in phantom and 2.5% (1-3MU/100cGy) in patients. The difference in HU values in Catphan was small. However, the magnitude of scatter and artifacts in CBCT images are affected by limitation of detector's FOV and patient's involuntary motions.
