북한이탈 어머니의 영유아 자녀 양육 경험 (The Early Childhood Caring Experience of North Korean Refugee Mothers)

  • 목적 본 연구의 목적은 북한이탈 어머니의 영유아 자녀 양육 경험이 무엇인지 그 경험 과정을 확인하고, 양육과정을 통해 심리사회적 문제를 어떻게 해결하는가에 대한 실체이론을 개발하는 것이다. 방법 연구참여자는 이론적 표본추출을 통하여 선정하였고 연구 자료는 심층면담과 참여관찰 및 참여자의 의무기록 등을 사용하여 수집한 후 Strauss와 Corbin (1998)의 근거이론방법을 적용하여 분석하였다. 결과 연구 결과 개방코딩 과정에서 62개의 개념과 23개의 하위범주, 11개의 범주가 도출되었으며 북한이탈 어머니의 영유아 자녀 양육 과정의 핵심범주는 '희망을 가지고 키워가기'로 밝혀졌다. 시간 흐름에 따른 양육 과정은 '양육 환경 변화 인식기', '양육 방법 조정기', '자신감 형성기'의 3단계로 나타났으며 양육 과정의 유형은 기대하기형, 지켜보기형, 걱정하기형의 세 가지 유형으로 구분되었다. 결론 본 연구의 결과는 영유아 자녀를 돌보는 북한이탈 어머니들의 양육경험을 이해하고 개별적인 간호를 제공하는데 기초가 될 이론적 기틀로 활용할 수 있을 것이며 북한이탈주민이 급증하고 이에 따라 동반 입국하는 아동의 수도 함께 증가하고 있는 현 시점에서 이 아동들의 건강 유지 및 증진을 위한 간호 실무에 매우 중요한 초석이 될 것이다.

  • Purpose: The purpose of this study was to investigate the effects of oral health education on oral health knowledge, oral health behavior, and oral hygiene status of children from North Korea. Methods: Participants were 32 North Korean children defectors (15 in the education group, 17 in the control group). The oral health education program, including theoretical training and toothbrush training, was done once a week for 4 weeks. Effects of the education program were assessed for oral health knowledge, oral health behavior, and oral hygiene status at pretest, 0, and 4 weeks after the intervention. Data were analyzed using repeated measures ANCOVA with the SAS program. Results: Children in the education group showed increased oral health knowledge and behavior over time compared to the control group and an improvement in oral hygiene status including significantly decreased S-PHP and Snyder test for oral micro-organism. Conclusion: Results indicate that oral health education is effective in improving oral health knowledge, oral health behavior and oral hygiene status. These improvements could lead to a better quality of life for North Korean children defectors.

북한이탈 어머니들의 양육효능감에 영향을 미치는 변인들에 관한 연구 : 결혼만족도와 사회적 지지 그리고 문화적응을 중심으로 (A Study on the Variables Affecting Parenting Efficacy of North Korean Refugee Mothers : With a Focus on Marital Satisfaction, Social Support, and Acculturation)

  • The purpose of this study was to identify the variables affecting parenting efficacy of North Korean refugee mothers. The selected variables were marital satisfaction, social support, and acculturation. The participants consisted of 105 mothers who had defected from North Korea with preschool children living in Seoul, Gwangmyeong-si, and Gyeongsangnam-do provinces. The data thus collected were analyzed by descriptive statistics, t-tests, correlation, and multiple regression analysis. The results indicated that the marital satisfaction and social support had meaningfully positive effects on the parenting efficacy of North Korean refugee mothers. However, marginalization was found to be a predictor of competence-anxiety in parenting. The selected variables derived from the environmental systems in maternal parenting were significant factors in the parenting efficacy of North Korean refugee mothers and furthermore, their sociocultural environments were also important in parenting. These findings clearly showed that marital satisfaction and social support were the most significant variables of parenting efficacy of North Korean refugee mothers. The results of this study appear to have a great deal of utility as preliminary data to improve the parenting efficacy of North Korean refugee mothers.

북한이탈 학령전기 아동을 위한 건강교육 프로그램 개발 및 중재효과 (Development and Effects of a Health Education Program for North Korean Preschool Defectors)

  • Purpose: This study was conducted to develop a health education program for preschoolers who have defected from North Korea with their mothers, and to evaluate the effects on health knowledge and behavior. Methods: A non-equivalent control group pre-post test design was used with 58 preschoolers who were assigned to either the experimental or control group (29 for each group). The program was composed of five sessions in health education and contracts. To test the effectiveness of the intervention, health knowledge and behaviors, and total bacterial colony counts on hands were measured at one pretest and two post tests (1 week and 4 weeks after the intervention ended). Data were analyzed using the SAS program. Results: Health knowledge and behavior in the experimental group increased significantly compared to the control group. The effects of the intervention were evident even at 4 weeks after the intervention ended. Total bacterial colony counts in the experimental group decreased significantly at the 1 and 4 week posttest intervention compared to the control group. Conclusion: Results indicate that this program is effective in improving health knowledge and behavior in these children and therefore can be utilized to ensure efficient management their health care.

북한 이탈 아동의 신체 및 심리적 건강상태 평가 (Physical and Psychological Health Status of North Korean Defector Children)

  • The purpose of the study was to evaluate the physical and psychological health status of North Korean defector children and identify associated factors. The participants in the study were 103 children ranging in age from 7 to 14 years old. Physical health status was evaluated through height and weight measurements as well as clinical laboratory results. Psychological health status was assessed using structured survey interviews. Data were analyzed utilizing Pearson correlation coefficients, t-test, ANOVA, and descriptive statistics. On arrival in South Korea, 19% of the children did not meet the child growth standards of South Koreans for height and 15% for weight. However, twelve weeks after, these percentages had decreased significantly. It found that 38.5% of children presented mild posttraumatic reaction and 40.4% presented moderate reaction. Scores for externalizing and internalizing behavior problems were $0.38{\pm}0.30$ and $0.59{\pm}0.36$ respectively. Posttraumatic reaction was higher in girls (t=0.41, p=.03), and boys showed higher externalizing problems (t=2.04, p=.04). Externalizing problems were correlated with gender and internalizing problems were associated with posttraumatic reactions and externalizing problems. The findings indicate that North Korean defector children need timely developmental assessments and tailored intervention programs to meet child growth standards need to be developed.

  • Purpose: The purpose of this study was to investigate the knowledge and attitude on tuberculosis by parents of North Korean refugees' children. Methods: A descriptive study was conducted. Data were collected from July to November, 2014 in Hanawon and 74 North Korean refugee parents participated in the study. Data were analyzed using the SPSS 21.0. Results: The score for knowledge was 17.72 out of 30 points, for awareness in attitude, 49.34 out of 60 points and for prevention behavior in attitude, 48.39 out of 60 points. There were positive significant correlations among knowledge, attitude and health interest. There was also a significant positive correlation between awareness and prevention behavior in attitude, and between prevention behavior and health interest. Conclusion: North Korean refugee parents showed a low level of knowledge and a negative attitude. The implication of these results is an awareness of the need for education on the causes, transmission and importance of treatment for tuberculosis. This kind of education can guide North Korean refugee parents to obtain correct information and positive attitudes and therefore, be able to effectively practice appropriate health behaviors in tuberculosis management for their children.

  • Purpose: The purpose of this study was to identify effects of parenting stress in North Korean refugee fathers on their parenting behavior. Methods: A descriptive study design was used. Data were collected during 2014 and 2015 in a settlement support center for North Korean refugees and 99 North Korean refugee fathers participated. Data were analyzed using SPSS 21.0. Results: Warmth encouragement parenting behavior received the highest score. The score for parenting stress was 81.98 out of 180, and 32.3% of participants were found to need professional help (${\geq}PSI\;90$). There were significant negative correlations between warmth encouragement parenting behavior and parenting stress. There were also significant positive correlations between rejection neglect parenting behavior and parenting stress. Predictors of warmth encouragement behavior were parent-child dysfunctional interaction which explained 10.1% of the variance. Predictors of rejection neglect behavior were parent-child dysfunctional interaction, number of children and wife's education level which explained 33.4% of the variance. Conclusion: Findings show that North Korean refugee fathers' parenting stress significantly influenced warmth encouragement and rejection neglect parenting behavior indicating a need to identify ways to decrease parenting stress. Parenting education can guide North Korean refugee fathers to strengthen parent-child positive interactions and therefore promote their children's growth and development.

북한이탈가정 어린이의 구강위생상태 평가 (Evaluation of oral hygiene status in children of North Korea refugees families)

  • Objectives: This study was conducted to identify the oral hygiene status of children and adolescents from North Korean (NK) refugee families residing in South Korea (SK) and to find ways to improve it. Methods: This study examined 88 children in NK refugee families. Their oral hygiene ability were estimated using the Patient Hygiene Performance (PHP)-index for children. The Independent Samples t-test and one-way ANOVA were conducted on the oral hygiene status according to the characteristics of children of NK refugees, and covariance analysis was conducted by controlling for significant variables in the simple analysis. Statistical significance levels were set at than p<0.05. Results: The overall PHP-index of normal, poor, and severe poor were 14.8%, 5.7%, and 79.5%, respectively. Adjusted for sex, age, country of birth, duration of settlement, covariance analysis was performed to determine the PHP-index according to oral examination. PHP-index was 3.55±1.56 when the oral examination was conducted and 4.26±0.98 when the oral examination was not conducted; this difference was significantly (p=0.022). Conclusions: Children and from NK refugee families have severely poor oral hygiene. Therefore, education to improve the oral hygiene status is necessary.

  • Purpose: The purpose of this study was to investigate the relationships between parenting behavior, parenting efficacy, adaptation stress and PTSD (Post Traumatic Stress Disorder) among mothers who have defected from North Korean. Methods: A descriptive study was conducted to illustrate the parenting behavior, parenting efficacy, adaptation stress and PTSD and report the relationships among them. Data were collected between August and November of 2009 in Hanawon and 105 mothers who met eligibility criteria participated in the study. Results: Ignorant/controlling parenting behavior of parenting showed the highest score in our study. The level of parenting efficacy was $2.72{\pm}0.36$ and adaptation stress level was $3.35{\pm}0.47$ showing perceived hostility the highest and homesickness the lowest. Participants with moderate to severe PTSD accounted for 79.5% of the mothers, and 75% of them reported symptoms for 3 months or more. Authoritative parenting behavior showed negative relationships with parenting efficacy, cultural crisis and fear, adaptation stress and PTSD. Conclusion: Mothers who have defected from showed negative parenting behaviors and high adaptation stress level and PTSD while parenting efficacy was high. These findings indicate that parenting intervention programs which can guide these mothers towards positive parenting behaviors need to be developed.

북한이탈가정 아동의 분리경험과 애착양상 (North Korean Refugee Children's Separation Experiences and Level of Attachment)

  • Objective: Taking note of the fact that North Korean refugee children experience frequent separation from their mothers and long-term maternal deprivation during their childhood, this study examined the relationship between children's separation experiences and attachment. Methods: A total of 37 children aged from 5 to 9 were assessed on their attachment using the Separation Anxiety Test, and their mothers reported on their child's separation experiences. Results and Conclusion: The major findings of this study were as follows: first, the result of evaluating their attachment pattern showed North Korean refugee children had a high level of insecure attachment with a high tendency for avoidant attachment. This avoidant attachment tendency is probably due to growing up in a repressed emotional environment by frequently experiencing maternal separation in North Korea, China, and even after settling down in South Korea. Second, children's secure attachment level was higher if they did not experience separation from their mother, if their mother had a high level of education in South Korea, or if they lived with a big family.
