A Basic Design and Characterization on Composite Bone Plate for Bone Fracture Healing (골절 치료를 위한 복합재료 고정판 기초 설계 및 특성 평가)

  • This paper aims to enhance the efficiency of bone fracture healing with bone plate made of fiber reinforced composite materials. The composite bone plate was designed as the same dimension and shape as the existing stainless steel bone plate. To find out the appropriate stacking sequence of the composite bone plate the variations of strain distributions were calculated using FE analysis when the bone plates were applied to the fracture site. From the analysis result it was found that the composite bone plate whose Young's modulus is lower than that of metal bone plate gave more uniform strain distribution and provided appropriate condition for callus formation and its development.

  • In this paper, finite element analyses for estimating the behavior of fractured femur just after the operation were carried out by using ABAQUS 6.71. A stainless steel bone plate and composite bone plates with various stacking angles were considered to find out the effect of bone plate properties on bone fracture healing. In order to simulate the actual state, contact conditions between the plate and bone and fractured bones were imposed on the finite element models and the whole analysis was divided by two steps; screw fastening step and load bearing step. The stress and strain distributions at the fracture site for the cases of the stainless steel and composite bone plates were analyzed and compared with. From the analyses it was found that the composite bone plate had potential advantages for effective bone fractures healing relieving stress shielding effect.

  • In this paper, finite element analyses were used to estimate the strain distribution at the fracture site of a tibia bone. A stainless steel bone plate and various composite bone plates were considered to find out the best conditions for callus generation while bone fracture was cured for 16 weeks. Through this research, the appropriate load condition which makes the strains between the appropriate range($2{\sim}10%$) was sought. From this analysis, it was found that lower level of external load is needed for the appropriate strain for the case of composite bone plate application and it was also found that the composite bone plate had potential advantages for effective bone fracture healing relieved stress shielding effect.

  • This clinical report describes the dental treatment of canine teeth fractures in three cats. These animals were diagnosed by oral examination and dental radiography as having fractured teeth with pulpal exposure. After endodontic treatment was completed, root canal filling material was removed from the coronal access and a trial post placement was performed. The post was permanently cemented after shortening. Teeth were etched and a bonded composite resin core formed and then shaped using rotary instruments. Fractured canine teeth were treated without any complications observed during a six-month follow-up period, proving that fractured cat canine teeth can be treated successfully with root canal therapy followed by restorative treatment with posts and composite resin.

  • In 1974, Casson et. al. reported midfacial degloving approach to repair the midfacial bone fracture. After then, this approach has been used frequently to treat the lesions on nasal cavity, nasopharynx, facial plastic surgery and midfacial trauma. Midfacial degloving approach consists of 1) bilateral sublabial incision 2) complete transfixion incision/ septocolumellar incision 3) bilateral intercartilaginous incision 4) bilateral pyriform aperature incision. This approach provides proper access for midfacial bone structure without facial scar but has post-operative complications such as transient epistaxis, infraorbital nerve paresthesia and nasal crust. We treated three patients using midfacial degloving approach to correct traumatic deformity in midface area. In two patients, rhinoplasty with autogenous rib graft was done simultaneously. So we report these cases with review of literatures.

  • This paper describes the bone healing process of fractured long bones such as a tibia applied by composite IM rods using finite element analysis. To simulated tissue differentiation process mechano-regulation theory with a deviatoric strain was implemented and a user's subroutine programmed by a Python code for an iterative calculation was used. To broadly find the appropriate rod modulus for healing bone fractures, composite IM rods were analyzed considering the stacking sequence. To compare mechanical stimulation at fracture gap, two kinds of initial loading conditions were applied. As a result, it was found that the initial loading condition was the most sensitive factor for the healing performance. In case a composite IM rod made of a plain weave carbon fiber/epoxy (WSN3k) had a stacking sequence of $[{\pm}45]_{nT}$, the healing efficiency was the most effective under a initial load of 10%BW.

  • The 234 patients who received treatment of midfacial fractures at the Department of Oral and Maxillofacial Surgery of Chonnam University Hospital from Jan. 1, 1992 to Dec. 31, 1996 were analyzed clinicostastically. Results obtained were as follows ; Male predominated over females by a ratio of 5.3 to 1. The frequently developing age groups were first 3rd (25%), 4th (21%) and the 2nd (18%) decade on succession. The peakest month was the August (16%), and May (11%), September (9%), October (9%). When it comes to the reasons for in-patients, traffic accident was predominant to 38%. In the 234 cases of midfacial fractures, zygomaticomaxillary complex fracture was the most by 37%. The most common with injury show that facial laceration marked by 49%, neurologic injury 24%, and mandibular fracture 20% each by each. About the time from injury onset to operation, 55% of cases were less than a week while the others (45%) more than a week. 3 plates were used for operation : 2 for zygoma or maxilla fracture and 3 for zygomatiomaxillary complex fracture. especially 4 for Le Fort I fracture, 5.5 for Le Fort I, II ; I, III ; II, III fracture, 7 for Le Fort I,II,III fracture were used. 20 patients (8%) appealed their complication and the most common was reported as infection. Above results suggest that early diagnosis and treatment of fracture site, systemic condition and associated injuries are necessary, and coorperative treatment with medical department should be performed.

Management of an Open Comminuted Fracture of the Metacarpus using Circular External Skeletal Fixation in a Korean Water Deer (Hydropotes inermis argyopus) (고라니에서 발생한 손허리뼈 개방 복합골절의 원형 외부골격고정법 적용)

  • A Korean water deer was rescued after being hit by a motor vehicle. Clinical and radiographic examination revealed a grade III open fracture of the right metacarpus with severe soft tissue injury. The water deer underwent closed reduction of the fracture, which was stabilized by the application of the Ilizarov method of 4-ring circular external skeletal fixation (CESF) as a minimally invasive technique. The water deer was successfully released into the wild. CESF was advantageous for the stabilization of open comminuted fracture and the management of a severely contaminated wound in a Korean water deer. CESF can be used successfully in wild deer with good tolerance and minimal complications.
