IoT-based Smart Pillbox for Efficient Medication Management (효율적인 복약 관리를 위한 IoT 기반 스마트 약상자)

  • 본 연구에서는 약 관리를 효율적으로 할 수 있는 IoT 기반의 스마트 약상자를 개발하였다. 약을 정기적으로 먹어야 하는 사용자에 대해 약 복용 현황 및 남아 있는 약의 상황 등을 모니터링 하여 효과적으로 관리할 수 있다. 이를 위해 약을 보관할 수 있는 상자를 3D 프린터를 이용하여 제작하였으며, 사용자의 복약 현황을 나타내주는 LCD를 전면에 배치했다. 또한, 약상자 내부에 있는 서보모터를 활용하여 정해진 복용 시간에 정해진 양만큼의 약을 제공할 수 있도록 하였다. 제안하는 스마트 약상자를 이용하여 사용자의 복약을 효율적으로 관리할 수 있으며 복약 순응도를 높이고 약물의 오남용을 막는데 도움을 줄 수 있다.

Correlation between Outpatient's Medical Adherence and National Insurance Types in the Type 2 Diabetes Mellitus (제2형 당뇨병의 외래환자 복약순응도와 보험유형과의 관계)

  • The purpose of this study was to analyse the relationship between the characteristics of the patients who received oral antihyperglycemic drugs and their medical adherence in Korea. The study method was a cross-sectional study using the patient sample data of the Health Insurance Review and Assessment Service for 2016, and it was analyzed with 109 major components of diabetes drug. The medical adherence was slightly higher in male than female. The patriots & veterans(free) type had the highest medication adherence because they have low self burden to access medical institutions compared to other insurance types. It is expect that this study result will be used as a basic data to understand the burden of outpatients with health insurance and establish a policy to reduce of the self outpatients' burden with chronic diseases such as type 2 diabetes.

The Association between Types of Usual Source of Care and Medication Adherence among the Elderly (고령자의 상용치료원 유형이 복약순응에 미치는 영향)

  • This study investigates the effect of types of usual source of care on medication adherence among the elderly. Using the Korea Health Panel Survey in 2017, this study includes 3,623 elderly people 65 years of age or older taking prescription drugs for treatment of chronic diseases. The type of usual source of care was considered in three aspects: whether having a usual source of care or not, whether having a regular site only or having a regular site and doctor, size of the regular site. The types of usual source of care are heavily associated with medication adherence. Compared to those who have no usual source of care, those who have a regular site only are 1.4 times, and have both regular site and doctor are 1.8 times more likely to comply with the medication guidelines, respectively. The finding shows usual source of care has a positive effect in improving medication adherence of the elderly. The results can provide an evidence for policies for effective management of chronic diseases and efficient use of medical resources.

Development of a smart pillbox and improvement of the medication adherence for the efficient management of medicine administration (효율적인 복약 관리를 위한 스마트 약상자의 개발과 약물순응도의 향상)

  • As Korea is expected to enter an aging society in near future, the number of patients who need a long-term prescription is expected to increase as well. In particular, Korea shows the highest in both the incident rate and the death rate among OECD member nations. The current situation requires more strict monitoring and management for patients' taking medicine, there are a few practical problems such as personnel expenses. In order to realize the efficient management of medicine administration, this paper proposes a smart pillbox that is developed based on IT technologies. Through the experiments in which actual patients participate, it is proved that the smart pillbox is beneficial to the patients taking medicine.

Effects of Nurse-led Patient Education for Gout Patients (통풍환자에 대한 간호사 주도 환자교육의 효과)

  • Objectives: This study was performed to analyze the effect of nurse-led education on results for patients with gout. Methods: The face-to-face education was conducted by a specialist nurse. Patient satisfaction and drug adherence, serum uric acid was assessed. Results: Patient satisfaction was significantly higher in the nurse-led education group than in the usual care group. The serum uric acid level on the second visit was lower after patient education. In addition, the mean drug adherence in the nurse-led group was significantly higher than in the usual care group on the second and third visits. Conclusion: The nurse-led education about gout improves patient satisfaction, drug adherence, and serum uric acid level in patients with gout.

  • 본 논문은 스마트폰 환경에서 만성질환자의 약복용 순응률을 향상시키기 위한 앱을 모델링하고 구현하였다. 이 앱은 일상생활을 하면서 장기간 규칙적으로 약을 복용해야 하는 환자들을 대상으로 약복용을 지원할 수 있도록 모델링되었다. 또한, 스마트폰을 통해 모바일 환경에 있는 환자에게 약복용 알림, 복약지도, 순응률 조회 등의 모니터링 서비스를 제공한다. 앱 서버는 환자들의 약복용 스케줄 데이터를 관리하며, 이 데이터는 3GS 통신을 통해 스마트폰과 동기화 된다. 순응률은 약복용 알림에 대해 환자의 대응에 따라 자동으로 계산되며 조화될 수 있다. 이 앱 통신은 국제표준 IEEE P11073-10472으로 구현되었다.

  • Depressive disorder is a chronic disabling condition with recurrent episodes. Medication noncompliance is a significant problem for effective management of depressive disorder. However, compliance with antidepressant medication is poor. There are difficulties in assessing compliance accurately. Various methods for assessing compliance are in use. A patient's compliance with antidepressant may be affected by many factors including the clinical characteristics of depression, pharmacologic properties of antidepressants, patients' characteristics, accessibility treatment, and doctor-patient relationships. For enhancing the treatment compliance, psychotherapeutic considerations in patient-therapist relationship are needed.

Analysis of Factors Affecting Nontherapeutic INRs in Korean Outpatients with Mechanical Heart Valves (인공심장판막 치환환자의 치료범위를 벗어난 INR 원인분석)

  • Background: Following the implantation of heart valve prostheses, it is important to maintain therapeutic INR to reduce the risk of thromboembolism. The objectives of this study were to determine the causes of nontherapeutic INRs, and to identify the factors associated with nonadherences to warfarin therapy in Korean outpatients with prosthetic heart valves managed by a pharmacist-run anticoagulation service (ACS). Material and Method: A retrospective chart review was completed for all patients enrolled in the ACS at Seoul National University Hospital from March, 1997b to September, 2000. The data on 868 patients (5,304 visits) who were at least 6 months post-valve replacement were included. All possible causes of nontherapeutic INRs were documented for each patient visit. The association of covariates to noncompliance was investigated by univariate analysis. Result: The reasons for nontherapeutic INRs were identified as follows: inadequate dosage adjustment $(21\%)$, nonadherences to dosing regimen $(13\%)$, drug/herbal interactions $(12\%)$, changes in diet $(7\%)$, and indeterminate cause $(42\%)$. Younger age, shorter duration of ACS and longer duration of warfarin therapy were associated with nonadherence. Conclusion: In this study, nonadherence and interactions between diet and medications were found to be important factors influencing nontherapeutic INRs. Longer duration of enrollment in the ACS affected the adherence to warfarin therapy Positively whereas younger age and longer duration of warfarin therapy affected negatively.

Factors Influencing the Stages of Change in Medication Adherence in Patients with Hypertension (고혈압 환자의 복약순응행위 변화단계에 영향을 미치는 요인)

  • Purpose: The purpose of this study is to identify factors associated with the stages of change in medication adherence in patients with hypertension. Methods: Participants were 323 patients with hypertension. Sociodemographic/medication-related characteristics, stages of change, processes of change, self efficacy and decisional balance were self-administered. Results: Stages of change were significantly different according to gender, age, job and living arrangement. A multinominal logistic regression analysis has revealed that gender, age, living arrangement, self-liberation, and self-efficacy were significantly associated with the precontemplation stage. Age and self-liberation were significantly associated with the contemplation stage. Gender, age, living arrangement, and self-liberation were significantly associated with the preparation stage. Gender and helping relationship were significantly associated with the action stage. This model explained 52.0% of the stages of change in medication adherence. Conclusion: The tailored intervention strategies based on the stages of change may be needed for improving medication adherence in patients with hypertension.
