담췌관계의 자기공명영상의 최근경향

  • 최근까지 복부 자기공명영상은 간의 국소병변의 특성을 규명하는 데 가장 많이 사용되어 왔다고 할 수 있다. 그러나, 자기공명영상의 기법과 장비의 발달로 인해 그 적응증이 더욱 확대되어 가는 추세에 있고 , 특히 자기공명담췌관촬영술의 도입과 발전은 복부 자기공명영상의 적응증을 한층 넓히는 데 크게 기여하고 있다. 이와 함께, 2차원 또는 3차원 경사에코기법과 조영제를 병용하여 호흡정지 상태에서 자기공명혈관촬영상을 얻을 수 있게 되어 자기공명담췌관촬영술과 혈관촬영술을 복합하여 다양한 담췌관질환을 평가하는 데 이용하는 빈도가 증가하고 있다. 이 강의 에서는 최근에 널리 사용되는 자기공명담췌혈관촬영술 또는 역동적 역상과의 복합적 사용의 유용성을 돌아보고자 한다.

Optimization of Abdominal X-ray Images using Generative Adversarial Network to Realize Minimized Radiation Dose (방사선 조사선량의 최소화를 위한 생성적 적대 신경망을 활용한 복부 엑스선 영상 최적화 연구)

  • This study aimed to propose minimized radiation doses with an optimized abdomen x-ray image, which realizes a Deep Blind Image Super-Resolution Generative adversarial network (BSRGAN) technique. Entrance surface doses (ESD) measured were collected by changing exposure conditions. In the identical exposures, abdominal images were acquired and were processed with the BSRGAN. The images reconstructed by the BSRGAN were compared to a reference image with 80 kVp and 320 mA, which was evaluated by mean squared error (MSE), peak signal-to-noise ratio (PSNR), and structural similarity index measure (SSIM). In addition, signal profile analysis was employed to validate the effect of the images reconstructed by the BSRGAN. The exposure conditions with the lowest MSE (about 0.285) were shown in 90 kVp, 125 mA and 100 kVp, 100 mA, which decreased the ESD in about 52 to 53% reduction), exhibiting PSNR = 37.694 and SSIM = 0.999. The signal intensity variations in the optimized conditions rather decreased than that of the reference image. This means that the optimized exposure conditions would obtain reasonable image quality with a substantial decrease of the radiation dose, indicating it could sufficiently reflect the concept of As Low As Reasonably Achievable (ALARA) as the principle of radiation protection.

The Diagnostic Utility of PET-CT for the Preoperative Evaluation of Lymph Node Metastasis in Gastric Cancer Patients (위암 환자의 수술 전 림프절 전이 평가를 위한 PET-CT의 진단적 유용성)

  • Purpose: The purpose of this study was to assess the diagnostic value of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) for detecting the lymph node (LN) metastasis of gastric cancer. Materials and Methods: 119 patients (M : F=89 : 30; mean age: 64) with gastric cancer were referred for preoperative FDG-PET/CT scanning and spiral enhanced abdominal pelvic CT from June 2006 to July 2008, and these were the subjects of our study. All the patients underwent curative radical gastrectomy and lymph node dissection. A final diagnosis was made for all the patients by the histology of the surgical specimens. Results: Both PET/CT and enhanced CT showed similar sensitivity for detecting regional lymph node metastasis (32.6% vs 39.5%, respectively). PET/CT was more accurate than enhanced CT for detecting regional lymph node metastasis (67.2% vs 63.0%, respectively), and PET/CT showed better specificity (86.8% vs 76.3, respectively) and a better positive predictive value (PPV) (58.3% vs 48.6%, respectively). PET/CT showed better specificity (98.0% vs 88.2%, respectively) and accuracy (79.4% vs 73.9%, respectively) than enhanced CT for detecting early gastric cancer. PET/CT showed better specificity (64.0% vs 52.0%, respectively), a better PPV (60.9% vs 57.1%), a better negative predictive value (NPV) (48.5% vs 46.4%, respectively) and better accuracy (53.6% vs 51.8%, respectively) than enhanced CT for detecting advanced gastric cancer. Conclusion: FDG-PET/CT is more usefulness than enhanced CT for making the preoperative diagnosis of regional LN metastases from gastric cancers.

  • 본 논문에서는 복부의 전산화 단층촬영 영상으로부터 체지방의 양을 측정하기 위한 영상처리에서 사용되는 문턱치의 자동 설정 방법을 제안한다. 체지방의 정량적 측정은 비만과 관련된 진단 및 치료에 있어서 중요하다. 기존의 비만도 측정은 체중과 신장의 비, 허리와 둔부 둘레의 비, 손으로 잡히는 복부의 두께 등 단순한 측정방법을 사용하여 실제 지방의 양을 제대로 반영하지 못하는 단점이 있다. 이러한 단점을 보완하기 위하여 최근에 전산화 단층촬영 영상으로부터 영상처리를 통하여 직접 지방의 양을 측정하려는 시도가 있다. 전산화 단층촬영 영상을 이용하면 지방의 양을 정량적으로 측정할 수 있고 피하지방과 복강내지방 등 특정부위의 체지방의 양을 측정할 수 있다. 전산화 단층촬영은 밀도에 비례하는 하운스필드 단위 값으로 구성된 영상을 제공한다. 일반적으로 체지방은 하운스필드 단위 값이 -150에서 -50사이인 것으로 알려져 있다. 그러나, 체지방의 문턱치는 사람에 따라 다르고, 또한 같은 사람에 대해서도 촬영 부위에 따라 다르다. 본 논문에서는 이러한 차이를 히스토그램을 통하여 보이고 히스토그램의 가우시안 함수 근사로부터 체지방의 문턱치를 자동으로 설정하는 방법을 제안한다.

A Study on Obesity-related Factors Through the Convergence of Body Mass Index (BMI) and Fat-CT in Middle-aged Women Living in Incheon (체질량지수(BMI)와 복부지방단층촬영(Fat-CT)의 융합을 통한 인천지역 중년 여성의 비만 검사연구)

  • To figure out the causes of obesity against middle-aged women, this study aimed to provide basic data for setting an obesity-related policy through analysis on diverse related factors. Against the healthy middle-aged women who visited 'N' Hospital Health Center in Incheon from April to November 2014, anthropometric assessment, body composition test and abdominal fat distribution test were conducted using Fat-CT. They were carried out against 159 women who agreed with the purpose of the study, using BMD, Q-CT. According to Fat-CT, subcutaneous and visceral types accounted for 39% (76 women) and 61% (119 respondents) respectively. In terms of BMI, 'underweight ($18.5kg/m^2$)' and 'overweight ($25kg/m^2$ or above)' were 13 women (6.7%) and 60 women (30.8%) each with the average of $23.5{\pm}4.11kg/m^2$. Mean while, waist circumference, diastolic blood pressure, systolic blood pressure and neutral fat were statistically significant at the 5% significance level. As getting older, muscle weakness offsets increase in body weight with abdominal obesity. A group with low BMI should also be considered due to increase in waist circumference.

Relationship between the Distribution of Space doses in X-ray Rooms and the "Inverse Square Law of Distance" (X선 촬영실 내 공간선량의 분포와 거리 역자승 법칙과의 관련성)

  • In the present study, space doses generated during X-ray radiography of hand, head, and abdomen, etc. were examined and whether the intensity of space doses of scattering rays is attenuated by the "inverse square law of distance" was figured out. First, the space doses of X-ray with small amounts of generated scattering rays such as hand radiography were mostly attenuated by the "inverse square law of distance" and were not detected at all at a distance of 2m. Second, the space doses of X-ray with large amounts of generated scattering rays such as head or abdomen radiography attenuated in higher rates than the rates under the "inverse square law of distance" at distances ranging from 30cm to 1m from the center of the irradiation field and were attenuated by the "inverse square law of distance" at distances ranging from 1m to 2m. Therefore, in X-ray rooms, the subject should be at least 2m away from the center of the irradiation field in the case of hand radiography and X-ray exposure prevention actions using protective devices are required in the entire spaces of the X-ray rooms in the case of head or abdomen radiography.

Usability Evaluation of Applied Low-dose CT When Examining Urinary Calculus Using Computed Tomography (컴퓨터 단층촬영을 이용한 요로결석 검사에서 저선량 CT의 적용에 대한 유용성 평가)

  • The aim of this study was to evaluate the usability of applied Low dose Computed Tomography(LDCT) protocol in examining urinary calculus using computed tomography. The subjects of this study were urological patients who visited a medical institution located in Busan from June to December 2016 and the protocol used in this study was Adaptive Statistical Iterative Reconstruction: low-dose CT with 50% Adaptive Statistical Iterative Reconstruction (ASIR). As results of quantitative analysis, the mean pixel value and standard deviation within kidney region of image(ROI)of the axial image were $26.21{\pm}7.08$ in abdomen CT pre scan and $20.03{\pm}8.16$ in low-dose CT. Also the mean pixel value and standard deviation within kidney ROI of the coronal image were $22.07{\pm}7.35$ in abdomen CT pre scan and $21.67{\pm}6.11$ in low dose CT. The results of qualitative analysis showed that four raters' mean values of observed kidney artifacts were $19.14{\pm}0.36$ when using abdomen CT protocol and $19.17{\pm}0.43$ in low-dose CT, and the mean value of resolution and contrast was $19.35{\pm}0.70$ when using abdomen CT protocol and $19.29{\pm}0.58$ in low-dose CT. Also the results of a exposure dose analysis showed that the mean values of CTDIvol and DLP in abdomen CT pre scan were 18.02 mGy and $887.51mGy{\cdot}cm$ respectively and the mean values of CTDIvol and DLP when using low-dose CT protocol were 7.412 mGy and $361.22mGy{\cdot}cm$ respectively. The resulting dose reduction rate was 58.82% and 59.29%, respectively.

Abdominal Digital Radiography with a Novel Post-Processing Technique: Phantom and Patient Studies (새로운 후처리 기술을 이용한 복부 디지털 방사선 촬영: 팬텀과 환자 연구)

  • Purpose The aim of this study was to evaluate the diagnostic image quality of low dose abdominal digital radiography processed with a new post-processing technique. Materials and Methods Abdominal radiographs from phantom pilot studies were post-processed by the novel and conventional post-processing methods of our institution; the proper dose for the subsequent patient study of 49 subjects was determined by comparing image quality of the two preceding studies. Two radiographs of each patient were taken using the conventional and derived dose protocols with the proposed post-processing method. The image details and quality were evaluated by two radiologists. Results The radiation dose for the patient study was derived to be half of the conventional method. Overall half-dose image quality with the proposed method was significantly higher than that of the conventional method (p < 0.05) with moderate inter-rater agreement (κ = 0.60, 0.47). Conclusion By applying the new post-processing technique, half-dose abdominal digital radiography can demonstrate feasible image quality compared to the full-dose images.
