The Analysis of Balance and muscle Activity according to Stair Height Gait Training in Adult Hemiplegia (성인 편마비환자의 계단 높이에 따른 보행훈련시 균형 및 근활성도 분석)

  • 본 연구는 뇌졸중으로 인한 성인 편마비 환자의 계단 높이에 따른 보행 훈련을 통하여 균형과 근활성도 변화를 분석하는데 목적이 있다. 뇌졸중으로 진단받은 편마비 환자 30명을 대상으로 5cm, 10cm, 15cm 높이의 계단 보행 훈련군으로 나누어 계단 보행 훈련을 실시하였으며, 임상적 균형능력 측정도구인 Berg Balance Scale(BBS), Timed up and go(TUG), Functional Reach Test(FRT)와 근활성도를 측정하였다. 연구결과 균형변화에서는 세 군간 계단 보행 훈련 전과 후의 균형 변화량이 통계학적으로 유의한 차이가 나타나지 않았으나(p>.05), 10cm 높이 계단 보행 훈련군에서 가장 큰 변화량을 보였다. 근활성도변화에서는 계단 오르기시 10cm 높이 계단 보행 훈련군에서 넙다리곧은근, 넙다리두갈래근, 장딴지근, 15cm 높이 계단 보행 훈련군에서 모든 근육들이 통계학적으로 유의한 차이를 보였으며(p<.05), 계단 내리기시 근활성도 변화에서는 5cm 높이계단 보행 훈련군에서 앞정강근와 장딴지근, 10cm 높이 계단 보행 훈련군에서 모든 근육들이, 15cm 높이 계단 보행 훈련군에서 넙다리두갈래근, 앞정강근, 장딴지근이 통계학으로 유의한 차이를 나타내었다(p<.05). 따라서, 높이에 따른 계단 보행 훈련은 편마비 환자의 임상적 균형 능력과 근활성도를 변화시키는데 영향을 줄 수 있으며, 특히 10cm 높이 계단 보행 훈련군에서는 다른 높이의 계단 보행 훈련군에서 보다 약간 상대적인 증가를 보였다.

The Effects of Community Ambulation Training on the Gait Ability and Stroke Impact Scale in Stroke Patients (지역사회 보행 훈련이 뇌졸중 환자의 보행 능력과 뇌졸중 영향 척도에 미치는 효과)

  • The study was conducted to determine the effect of community ambulation training and treadmill training on the gait ability and stroke impact scale in patients with hemiplegia due to stroke. Twenty-two patients with hemiplegia due to stroke were assigned to the community ambulation training group(n=11) or treadmill training group(n=11). Both groups were executed conventional treatment for 5 times per week for 6 weeks 30 minutes per session. Each group performed additional exercise for 30 minutes. Post treatment, compared to the treadmill training group, community ambulation training group showed significantly increased velocity, cadence, stroke impact scale(p<.05). These results support the perceived benefits of community ambulation training to augment on the gait ability and stroke impact scale of stroke patients. Therefore, community ambulation training is feasible and suitable for stroke patients.

Effect of Fast-Tempo Auditory Stimulation during Treadmill Gait Training in Patients with Hemiplegia (편마비환자의 트레드밀 보행 훈련 중 빠른 템포 음향이 미치는 효과)

  • This study examined the differences in the improvement of walking ability between treadmill training with fast-tempo auditory stimulation (FTAS group) and the general treadmill training (control group) in hemiplegia. The subjects were allocated randomly into both groups. Eleven patients of the FTAS group and 11 patients of the control group underwent training for 30 minutes per day for 6 weeks. Before and after intervention, the walking ability was measured by the 10m gait-evaluation and the number of steps in 20m walking. The gait velocity was improved further in the FTAS group than the control group (p<.05). This study provides evidence of the efficacy of treadmill training with FTAS in improving the gait ability when compared to the general methods in patients with chronic hemiplegia.

  • In general, dizziness is caused by various situations, but among them, symptoms due to dysfunction of the motor system belonging to the nervous system are the most severe, accompanied by nausea and vomiting. Treatment of these dizziness includes drug therapy, surgical therapy, and rehabilitation. Drug therapy and surgery are generally performed in vest rehabilitation training, which is a rehabilitation therapy because of the risk of aftereffects. The vestibular rehabilitation training includes eye training, posture stabilization training, and walking training. Among them, walking training is performed in a certain space under the supervision of a doctor or a professional therapist, so that the time and space burden is increased. In order to solve this problem, we implemented gait training contents which can be used for rehabilitation training by using the augmented reality technology. It is expected that it can be utilized as dizziness rehabilitative contents which can be used in medical environment through clinical tests for patients with dizziness.

The Effects of the Obstacle Walking Training on Gait and Balance in Stroke Patients (장애물보행훈련이 뇌졸중환자의 보행 및 균형에 미치는 효과)

  • Objectives :This study aimed to determine whether obstacle walking training can improve gait and balance in stroke patients. Methods : Obstacle walking training and Flatland walking training was accordingly applied in each group for 30 minutes per session, 5 times per week for 4 weeks. Gait was assessed using a 10MWT and Balance was FRT, respectively, before and after training. Results : 10MWT and FRT were significantly increased in experimental groups after training (p<.05) but there were no significant difference in control group. There were a significant difference between the groups.

  • 연구목적 : 본 연구의 목적은 8주간의 점진적인 속도증진 훈련이 뇌졸중환자의 족저압, 보행주기, 보행대칭성에 어떠한 영향을 주는지 알아보는 것이다. 연구방법 : 연구에 참여한 대상자들은 뇌졸중으로 진단 받고 N병원에 입원한 편마비환자 20명(실험군 10명, 대조군 10명)을 대상으로 하였다. 이들은 모두 물리치료와 작업치료를 받았고, 실험군은 주3회, 8주간의 트레드밀 보행훈련를 받았고, 대조군은 받지 않았다. 실험전, 후에 F-scan을 이용하여 보행주기, 보행대칭성을 검사하였다. 연구결과 : 8주 후, 보행주기는 실험군의 양하지지지기 I II, 단하지지지기에서 모두 유의하게 증가하였으나, 대조군에서는 양하지지지기 II, 단하지지지기에서 유의하게 증가하였다. 유각기는 실험군과 대조군 모두 유의하지는 않았다. 보행대칭성은 입각기 대칭성에서 대조군과 비교하여 실험군에서 유의하게 증가하였으나, 유각기 대칭성은 실험군과 대조군 모두 유의하지 않았다. 결론 : 뇌졸중 환자에 있어 보행훈련은 보행주기과 보행대칭성과 를 개선시키며, 특히 점진적인 속도 증진훈련은 양하지지지기, 단하지지지기, 입각기 보행대칭성에 효과적인 방법으로 사용될 수 있을 것으로 보인다.

The Effect of Mental Practice on Gait and Quality of Life in Stroke Patients (상상훈련이 뇌졸중 환자의 보행 및 삶의 질에 미치는 영향)

  • The aim of this study was to evaluate the effects of mental practice to improve on gait and quality of life in stroke patients. Participants were randomly allocated to two groups: experimental(n=10) and control (n=10). Both groups received neural development treatment training for 6 sessions 1 hour per week during 6 weeks. Experimental group practiced additional mental practice for 3 session 10 minutes per week 6 weeks. Results were evaluated by spatial-temporal parameter, 10m walk test, and quality of life. There were significantly increased by mental practice in outcomes of the gait performance from the gait velocity, cadence, double support, single support, 10m walk test. There were significantly increased by mental practice in outcomes of stroke impact scale from the strength, social participants, and stroke recovery. In conclusion, the mental practice improves gait performance and quality of life in stroke patients. The results suggest that mental practice training is feasible and suitable to stroke patients.

The Effect of Ankle Joint Muscle Strengthening Training and Static Muscle Stretching Training on Stroke Patients' Plantar Pressure and Gait (발목관절의 근력 강화 훈련과 정적 근육 신장 훈련이 뇌졸중 환자의 족저압 및 보행에 미치는 영향)

  • This study was intended to implement ankle joint dorsi flexion training against ankle muscule strength weakening that erodes stroke patients' gait performance to examine the effect of the training on stroke patients' plantar pressure and gait ability. In this study, 36 stroke patients diagnosed with stroke due to cerebral infarction or cerebral hemorrhage were divided to measure 10MWS which are stroke patients' gait variables maximum plantar pressure by area of the sole by collecting data using an F-scan system during gait. Given these results of the study, compared to other training groups, the ankle muscule strength reinforcing training group showed statistically significant increases of maximum plantar pressure in the great toe, the toe and the first metatasal areas too and thus it can be said that this training increases forward thrust during stroke patients' foot end taking off and positively affects stroke patients' ability to perform gait.

Case Study of Gait Training Using Rhythmic Auditory Stimulation(RAS) for a Pediatric Patient with Cerebellar Astrocytomas (리듬청각자극(RAS)을 사용한 소뇌 별아교세포종(CA) 환아의 보행훈련 사례 연구)

  • This single case study is to examine the gait parameter changes of a 12-year old patient with Cerebellar Astrocytomas using RAS in gait training program. Kinematic and temporospatial changes were analyzed using VICON 370 Motion Analysis System. A total of nine RAS gait training sessions was provided and each training program took 30 minutes. Gait analysis revealed that the patient showed improvement in cadence, velocity, stride length, and step length and improved the range of joint movements by showing gait patterns similar to normal distribution from a pathological pattern. This study showed possibilities to apply the RAS technique to the various population including clients with cerebellum damaged; however more further research should be done in this area.

Effects of Visual Feedback Treadmill Gait Training Program Combined with Virtual Reality Technology and a Force Plate Measurement System on Gait Ability and Quality of Life in Stroke Patients (가상현실과 힘판을 통한 시각적 되먹임 트레드밀 보행훈련이 뇌졸중 환자의 보행능력과 삶의 질에 미치는 영향)

  • The purpose of this study was to improve the gait ability and quality of life of stroke patients by combining virtual reality technology and a visual feedback gait training program with entertainment elements. Ten stroke patients with circumduction gait were selected. The visual feedback treadmill gait training program using virtual reality technology and a force plate measurement system was conducted 30 minutes a day, 5 days a week, with 25 sessions in 5 weeks. To investigate the effects of this gait training program, evaluations using the joint range-of-motion test, muscle activity tests, Berg balance scale (BBS), gait analysis, and stroke-specific quality-of-life scale (SS-QOL) were performed before and after intervention. Statistically significant differences were found in the joint range of motion and muscle activity of the affected side from the initial swing phase to the mid-swing phase of the gait cycle, dynamic balance, gait function, and quality of life (p <0.05). The results of this study indicate that the gait training program improved the foot drop, muscle activity, dynamic balance, and gait ability of stroke patients with circumduction gait, thereby improving the quality of life of the patients. Therefore, we recommend the application of the visual feedback treadmill gait training program using virtual reality technology and a force plate measurement system to improve gait ability and quality of life of stroke patients with circumduction gait.
