Patient Management Through Simulation Modeling in the Medical Center (시뮬레이션 기법을 이용한 검진센터의 환자관리방안)

  • This study aims to develop the methods for effective patient flow in the medical center through simulation modeling. To achieve this, we developed three simulation scenarios based on max/min processing time and addition of X-ray by 15 patient tracks from real hospital data. The simulation software used in this study is Flexsim HC 2.7. According to the scenario 1 on 15 patient tracks' LOS by max processing time, there is a great difference between average length of stay(LOS) and max LOS. And average LOS increases greatly depending on the number of patients by the hours. There is no need to add extra X-ray because the addition of X-ray has not much influence in average LOS. It is possible to make good decisions on patient flow management and medical equipment purchasing through simulation modeling. The concrete simulation scenario as a tool for decision support will contribute to efficiency in hospital management.

Discussion on the Value of Using Gait Analysis System Using Smart Shoes (스마트 신발을 활용한 보행분석 시스템 활용 가치에 대한 논의)

  • The purpose of this study is to verify whether the data measured by the researcher and the smart shoe sensor data are the same or similar by performing the 6 - minute walking test and time up and go test after putting smart shoes on a normal person. Ten normal adult males participated. After wearing smart shoes, they performed a 6-minute walk test and a time up and go test. The results of this experiment show that the accuracy of the current sensor is high. The difference in the distance of the 6-minute walking test is that the difference is because the turning point, which is not calculated in the actual 30-m track, measures the distance. From this point of view, it can be seen that smart shoes measure more accurate distance and it is expected that various tests will be possible through smart sensors.

Study on Correlation between Cognitive Impairment and Geriatric Depression or Geriatric Stress (인지기능과 노인성 우울, 노인성 스트레스의 연관성에 대한 연구)

  • This study examined the effects of geriatric depression and geriatric stress on cognitive impairment. In particular, the dementia groups were divided into Alzheimer's disease and vascular dementia. A normal group was used as the control group. For each group, the geriatric depression using the geriatric depression scale (GDS) and geriatric stress using the geriatric stress scale (GSS) was compared with the regression and correlation results of cognitive impairment. Statistical tests, such as descriptive statistics, Kruskall-Wallis, correlation analysis, and multiple regression analysis, were performed. For the Alzheimer's group, the GDS and GSS correlated with the cognitive impairment, but only the GDS showed a cause-and-effect relationship with cognitive impairment. In particular, the male group with Alzheimer's disease showed clear confirmation. In addition, geriatric stress was found to be associated with geriatric depression. In conclusion, geriatric depression affects the cognitive impairment directly and geriatric stress affects the cognitive ability indirectly through geriatric depression. In this study, the Alzheimer's disease, vascular dementia, and control groups had a small sample size. Therefore, the external validity in future studies can be increased using a larger sample size for each group.

Laboratory Management Status of Vestibular Function Test in Korea (국내 전정기능검사실의 운영 현황)

  • Objectives: This study was conducted to assess the current management status of the vestibular function test laboratories in Korea. Methods: Questionnaire about the management status of the vestibular function test laboratories was sent by email to the entire members of the Korean Balance Society. The contents of questionnaire included situation of employees who perform the tests, the types of vestibular function tests and equipment, frequency of the test and types of dizziness related questionnaires. Results: Forty-nine hospitals and clinics responded. All the 49 respondents answered that they have videonystagmography. Spontaneous nystagmus analysis by videonystagmogrphy was the most frequently tests for patients with dizziness. Questionnaires for dizziness were used by 27 respondents (55.1%) for initial evaluation of the dizziness patients. The Korean version of dizziness handicap inventory was the most frequently used dizziness related questionnaire. Conclusions: We analyzed the current management status of vestibular function test laboratories to comprehend the present condition of the vestibular function test. We think that these results will help to provide a standard for laboratory operations and prepare for the education, focusing on high-demand tests.

Animal Pathogenesis and Specificities of Leptospira (Korea) with Autopsied Human Tissue (Leptospira(Korea)의 병원성 및 leptospirosis 환자 조직과의 특이성)

  • Leptospira isolated from patients and natural paddy water were further studied to confirm their serologic specificity with the bacteria in infected animal tissue and autopsied tissue of patients died with leptospirosis. And pathologic patterns of the inoculated antimal and the virulence of the bacteria in the animals were also studied. The findings are summarized as follows; $LD_{50}$ dose of the bacteria in guinea-pig were $1-2{\times}10^9$ cells and mice were found to be susceptible to them even though the degree of susceptibilities were much inferior to guinea pig. The bacteria were recovered from various organs; demonstrated massive hemorrages due to diapedesis and monocyte infiltration were observed, in some cases, intramedullary hemorrages of the infected kidnies and hematuria were recognized. All of the hyperimmune sera were strongly reacted with the tissues from autopsied human case who died of EPHF(Epidemic Pulmonary Hemorrhagic Fever) in immunofluorescent antibody tests.

Effect of Evodiae fructus Methanol Extract on Virulence-Related Genes' Expression of Helicobacter pylori (오수유 메탄올 추출물이 Helicobacter pylori 병원성 관련 유전자 발현에 미치는 영향)

  • Helicobacter pylori, a gram-negative bacterium, is one of the risk factors that induces gastritis and gastric cancer. Therefore, much attention has been paid to the compounds that inhibit bacterial growth or eradicate bacteria. Evodiae fructus (EF), the fruit of Evodia rutaecarpa, has been used for treating diarrhea and abdominal pain. EF extract was already found to inhibit the growth of H. pylori. However, to the best of our knowledge, the effect of EF on the virulence factors of H. pylori has not been reported. In this study, when comparing the minimum inhibitory concentration (MIC) of the different methanol concentration extracts, the 95% methanol extract (EF95) showed the lowest MIC value. EF95 extract suppressed the expressions of cagA, vacA and ureB, but interestingly, it up-regulated the expression of ureA. A decrease in production of ammonia in the culture medium and the cell lysates indicated that EF95 inhibited the urease activity in H. pylori, which was the result of EF95 inhibiting the ureB expression. Although the mechanism by which EF95 extract regulates the virulence factors in H. pylori needs further study, EF95 could be used for treatment of gastric troubles induced by H. pylori.

The Survey for Awareness of Radiation Dose of CT and General X-ray Examination (전산화단층촬영검사와 일반촬영검사의 방사선 선량에 대한 인식도 조사)

  • The goal of this study is to awaken about risk occurred by CT examination. For radio-technologists working at 'S medical center' located in Seoul, we investigated a recognition about dose and risk CT and normal X-ray examination according by working experience in hospital, experience about CT examination and radiation source. For subjects of investigation, radio-technologists working at 'S medical center' located in Seoul helped us. We collected 131 questionnaires for a test of hypothesis. Cronbach @ coefficients of questionnaires were 0.825988 and 0.767161 and a rejection rate of p-value was below 0.05. SAS 9.1(SAS Institute Inc., Cary, NC, USA.) statistic package was used for hypothesis test. We used Mann-Whitney test, Kruskai-Wallis test, Two sample T-test, Two sample T-test with Bonferroni's Correction and One-way ANOVA methods. P-values of hypothesis about dose of CT and normal X-ray examination were 0.2291 ~ 0.9663. p-values of hypothesis about risk were 0.1924 ~ 1.0000. All of hypothesis is over rejection rate(<0.05). This study shows that radio-technologists of S medical center recognized that CT has higher dose and risk than general X-ray examination.

Status of Blood Products Release at a General Hospital in Gyeonggi-Do (경기도 일개 종합병원에서 혈액제제 출고 현황)

  • Blood products (BPs) can only be obtained through blood donation and hence represent a finite resource. BPs should therefore be used conservatively. However, BPs are being used indiscriminately without evidence. The purpose of this study was to evaluate the reasons for the use of BPs and their appropriateness. The investigation was carried out based on hemoglobin levels. Data were obtained from Nov 1, 2020, to Oct 31, 2021, from a hospital's OCS/EMR systems. The BPs were dispensed in 21,303 cases, and the number of hemoglobin levels >7.0 g/dL or higher among red blood cell drugs used by each treatment department was 1,173 (>7.0 g/dL). The misuse of blood transfusions is increasing social costs, with the adequacy of transfusion becoming increasingly important. Hence, each medical institution should review the transfusion guideline evaluation index, check the status of the release of BPs, and institute educational programs covering transfusion guidelines and continually evaluate their adequacy.

Diffuse Large B-Cell Lymphoma Manifesting as Miliary Nodules in the Lung: A Case Report (폐의 좁쌀 결절로 발현된 림프종 사례: 증례 보고)

  • Malignant lymphoma has various pulmonary manifestations on chest CT, including nodules, masses, areas of consolidation, and ground-glass opacity. These presentations can pose a diagnostic challenge, as they mimic other disease patterns. Herein, we report a case of diffuse large B-cell lymphoma (DLBCL) manifesting as miliary nodules in a 67-year-old male initially presenting with dyspnea and fever. Radiologic findings included diffuse, bilateral, multiple tiny nodules consistent with metastasis, miliary tuberculosis, and fungal infection. However, further investigations, including laboratory tests, imaging, and biopsies, led to the diagnosis of DLBCL involving the lungs. Herein we reported a rare case of lymphoma involvement of the lung presenting as miliary nodules. Accurate diagnosis relies on a comprehensive evaluation of the clinical history, physical features, laboratory test results, and imaging findings.
