Caplan Syndrome Mimicking Progressive Massive Fibrosis on CT: A Case Report (CT에서 가속화된 진행성거대섬유증으로 오인된 카플란 증후군: 증례 보고)

  • This report presents a unique case of Caplan syndrome that mimicked accelerated progressive massive fibrosis. The patient, a former coal miner, had been diagnosed with coal worker's pneumoconiosis 15 years prior and had been treated for rheumatoid arthritis for over 20 years. Accelerated progressive massive fibrosis and the development of multiple nodules with cavitation in the basal lungs were subsequently observed on serial CT scans. Here, the CT manifestations of Caplan syndrome are highlighted in a case in which Caplan syndrome mimicked accelerated progressive massive fibrosis.

Inhibitory Effect of Metal Surface on the Antimicrobial Resistance Microorganism (금속표면이 항생제 내성균주의 생육억제에 미치는 영향)

  • Background: The aim of this study was to comparatively evaluate the bactericidal effects of copper, brass (copper 78%, tin 22%), and stainless steel against methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant Enterococcus faecium (VREFM), and multidrug-resistant Pseudomonas aeruginosa (MRPA). Methods: The isolates (MRSA, VREFM, MRPA) used in this study were mixed wild type 3 strains isolated from patients treated at Uijeongbu St. Mary's Hospital in 2017. These strains showed patterns of multidrug resistance. The lyophilized strains were inoculated into and incubated for 24 hr in tryptic soy broth at $35^{\circ}C$. The initial bacterial inoculum concentration was adjusted to $10^5CFU/mL$. A 100-mL bacterial suspension was incubated in containers made of brass (copper 78%, tin 22%), copper (above 99% purity), and stainless steel at $35^{\circ}C$. Viable counts of bacteria strains were measured for 9 days. Results: In this study, the bactericidal effects of copper and brass on MRSA, VREFM, and MRPA were verified. The bactericidal effect of stainless steel was much weaker than those of copper and brass. The bactericidal effect was stronger on MRPA than on MRSA or VREFM. Conclusion: To prevent cross infection of multidrug resistant bacteria in hospitals, further studies of longer duration are needed for testing of copper materials on objects such as door knobs, faucets, and bed rails.

Disseminated Cryptococcosis in a dog due to Cryptococcus neoformans var. neoformans (한 마리의 개에 있어서 Cryptococcus neoformans var. neoformans에 의한 파종성 크림토콕스병)

  • 일반적인 치료에 반응을 나타내지 않는 호흡곤란증에 걸린 4년령의 Shepherd개에서 피부, 눈, 림프절의 병변을 동반한 치명적인 파종성 크립토콕스병이 진단되었다. 세포학적검사에 의해 난원형 내지 구형이고, 두꺼운 협막을 갖고 있는, 형태학적으로 Cryptococcus neoformans와 일치하는 효모균이 증명되었다. 이 병원체는 혈액, 오줌, 콧물, 견갑전림프절 흡인물, 피부생검 시료, 피부 면봉 시료 등을 25$\circ $C의 Pal씨 배지에 접종하여 용이하게 분리되었다 배양물을 PHOL염색액으로 염색하여 현미경으로 검사한 결과 얇은 막에 싸인 발아를 나타내거나. 나타내지 않는 구형 내지 난원형의 효모균이 증명되었다. 이 개는 ketaconazole 로 치료를 시작 한 후 6일만에 폐사하였다. 공기, 흙, 비둘기 배설물, 톱밥 등을 Pal씨 배지에 접종하여 C.neoformans가 배양됨으로써 역학적으로 환경이 병원소 역할을 한 것으로 판단되었다. 환축과 환경으로부터 분리된 균주는 세밀한 동정결과 neoformans (serotype AD)에 속하며, Filobasidiella neoformans "alpha" mating type을 나타내었다. 이 연구결과, Pal씨 배지가 크립토콕스병의 조기진단과 역학적 조사에 훌륭한 감별배지라는 것이 입증되었다. 개량된 Pal씨 배지는 C. neoformans의 genetic crossing을 판단하는데 성공적으로 사용할 수 있었다.

  • PCDH19-related epilepsy is an inherited disease occurring in female patients and characterized by early onset seizure, intellectual disability, and behavioral disturbances. It is caused by de novo or familial heterozygous variation of the PCDH19 gene located on Xq22.1. Our patient was hospitalized for multiple focal seizures. The magnetic resonance imaging was normal and electroencephalogram showed focal epileptiform discharges. The child's development did not progress; she began to manifest, cognitive, behavioral and language delays. Because of that, we performed an epilepsy gene panel test. We report a case of epilepsy with mental retardation limited to female patients with mutation of PCDH19.

  • Imaging studies play an important role in the detection, diagnosis, assessment of resectability, staging, and determination of patient-tailored treatment options for pancreatic adenocarcinoma. Recently, for patients diagnosed with borderline resectable or locally advanced pancreatic cancers, it is recommended to consider curative-intent surgery following neoadjuvant or palliative therapy, if possible. This review covers how to interpret imaging tests and what to consider when assessing resectability, diagnosing distant metastasis, and re-assessing the resectability of pancreatic cancer after neoadjuvant or palliative therapy.

  • The incidence of degenerative valve disease also increasing with the increasing life expectancy of the elderly population. Rheumatic valve disease is decreasing gradually and the incidence of calcified degenerative aortic stenosis (AS) is growing. Echocardiography is a very important tool for evaluating the prognosis and treatment method as well as the time of operation and diagnosis of heart valve disease. When evaluating valvular heart disease, 2-dimensional echocardiography, which observes all heart valves in detail, should take precedence. Understanding the clinical findings of degenerative valve disease and performing precise echocardiography are extremely important. In addition, an assessment of the severity of aortic stenosis is necessary to determine the surgical indications. An assessment of the severity by echocardiography was explained with three cases of degenerative aortic stenosis. To perform echocardiography accurately, it is necessary to understand degenerative valve disease and its clinical findings accurately.

대미 삼계탕 수출, 현재 이렇게 진행되고 있다

  • UR, WTO에 의해 지난 1997년 국내 닭고기 시장이 전면 개방된 이후 매년 닭고기 수입은 급속도로 증가해왔으며, 2003년말 고병원성 조류인플루엔자가 발병되기 전까지만 해도 수입 닭고기는 국내 닭고기 시장의 약 30%를 점유하는 등 국내 닭고기 시장을 크게 위협했다. 특히 미국의 경우 자국 내에서는 인기부위인 가슴육 위주로 소비하고 비 인기부위인 다리육은 우리나라에 덤핑가격으로 수출해 국내 닭고기 시장을 더욱 혼란스럽게 했다. 반면 국내 소비자는 미국, EU 등 선진국과는 달리 닭다리를 선호하여 다리육은 부족하고 가슴육은 남아도는 구조를 가지고 있어 우리 역시 남아도는 닭 가슴육을 이들 국가에 수출해야 할 필요성이 절실하게 요구돼 왔다. 게다가 현재 협상 중인 한 · 미 FTA가 체결되어 이로 인해 미국산 닭고기 수입에 대한 관세가 철폐될 경우 국내 닭고기 시장에 미치는 영향이 적지 않은 것으로 분석됨에 따라 국내닭고기 시장에 미치는 영향을 최소화 하고, 나아가 국내 육계산업의 경쟁력 확보를 위해서는 대미 닭고기 수출의 필요성은 더욱 중요해지고 있다. 이에 필자는 지난 9월 9일부터 16일까지 8일간 미국 워싱턴 D.C 미국 농무성 식품안전검사처(FSIS) 국제업무국(OIA) 등을 방문해 한국의 가축방역 및 축산물 위생검사제도에 대해 설명하고, 대미 삼계탕 수출을 위해 담당자와 기술적 내용을 협의했다. 본고에서는 그날 협의한 내용들을 간략하게 요약하여 게재코자 한다.

  • 폐쇄성 수면 무호흡증으로 인한 수면 중 잦은 각성은 수면의 질을 떨어뜨릴 뿐 아니라 졸음, 피로, 집중력 저하와 같은 주간 증상을 유발하게 되어 삶의 질을 떨어뜨리고 고혈압이나 부정맥 등과 같은 심각한 심폐질환 을 유발할 수 있다. 그러나 코골이와 달리 수면 무호흡증은 본인이나 타인에 의한 정확한 관측이나 진단이 어려워 전문병원에 입원하여 수면다원검사를 통하여 진단해야 하는 번거로움이 있다. 본 논문에서는 정밀하고 응답속도가 빠른 온습도 센서를 이용하여 호흡주기를 측정함으로서 폐쇄성 수면 무호흡증을 진단하고 경보를 발생하는 휴대형 수면 무호흡 측정기의 설계 및 구현 기법을 제안하였다.

  • In general, all physicians have some standardized diagnosis and treatment methods. However, there are differences in the precise order and examination depending on the hospital size, system, medical equipment, etc. To reduce this difference, the interest about standardized guidelines recently increased and a variety of research is being conducted. The uniform guideline cannot reflect the differences of each situation and environment to meet the hospitals. Therefore, standardized medical guidelines(=clinical pathway) should provide customized guidelines based on the relevant medical data to ensure the quality of the medical service and the doctor's autonomy. In this paper, we will analyze medical data made by two thyroid specialists in the same hospitals. Moreover, this paper mentions the implement of automatic generating clinical pathway system which consider its real hospital situation and result.

Characteristic MR Imaging Features and Serial Changes in Adult-Onset Alexander Disease: A Case Report (성인형 알렉산더병의 자기공명영상 소견 및 추적 관찰상의 변화: 증례 보고)

  • Adult-onset Alexander Disease (AOAD) is a rare genetically determined leukoencephalopathy that presents with ataxia, spastic paraparesis, or brain stem signs including speech abnormalities, swallowing difficulties, and frequent vomiting. The diagnosis of AOAD is frequently proposed based on the findings on MRI. We demonstrate two cases (37-year-old female and 61-year-old female) with characteristic imaging findings and changes in follow-up MRI in patients with AOAD, which were confirmed via glial fibrillary acidic protein (GFAP) mutation analysis. On MRI, the typical tadpole-like brainstem atrophy and periventricular white matter abnormalities were noted. The presumptive diagnoses were made based on the typical MRI appearances and, subsequently, confirmed via GFAP mutation analysis. Follow-up MRI demonstrated the progression of atrophy in the medulla and upper cervical spinal cord. Our report could help raise awareness of characteristic MRI findings of AOAD, thus helping clinicians use GFAP analysis for AOAD diagnosis confirmation.
