  • Subacute Ankle Sprain patients, 3 tests have been acknowledge for diagnostic precision and thus most commonly used to test their soft tissues, muscles, and ligaments: ankle stress-valgus test, anterior-draw test, and MRI scan. Although not much different from the rest two in its diagnostic value, MRI costs patients some money and time. Also, we have reached the conclusion through an experiment with a group of patients in a certain hospital that the rest two methods is as efficient and useful as MRI in diagnosing ankle joint patients.

  • 60대 자영업자 A씨는 언제부턴가 식사를 하고 나면 씁쓰름한 신물이 식도를 역류해 올라왔다. 어떤 때는 음식물이 넘어와 되새김질을 하기도 했다. 상복부가 쓰리고 가슴 통증이 생겨 병원을 찾아 검사한 결과 '역류성 식도염' 진단을 받았다. 역류성 식도염은 잘못된 식습관에서 비롯되기도 하지만 A씨처럼 나이가 지긋한 경우엔 위 괄약근 조직이 약해져 나타난 것일 수 있다.

  • Thoracic foreign bodies (FBs) are serious and relatively frequent in emergency departments. Thoracic FBs may occur in association with aspiration, ingestion, trauma, or iatrogenic causes. Imaging plays an important role in the identification of FBs and their dimensions, structures, and locations, before the initiation of interventional treatment. To guide proper clinical management, radiologists should be aware of the radiologic presentations and the consequences of thoracic FBs. In this pictorial essay, we reviewed the optimal imaging settings to identify FBs in the thorax, classified thoracic FBs into four types according to their etiology, and reviewed the characteristic imaging features and the possible complications.

Multicentric Epithelioid Angiosarcoma of Bones Showing Angiotropic Spread: A Case Report (뼈에 발생한 혈관성 파종 양상을 보이는 다발성 상피상 혈관육종: 증례 보고)

  • Epithelioid angiosarcoma is a rare variant of angiosarcoma characterized by an epithelioid morphology that mimics carcinoma. Therefore, multicentric epithelioid angiosarcoma is easily misdiagnosed as bone metastasis from carcinoma and has an aggressive clinical course. Here, we present a rare case of a 61-year-old male with multicentric epithelioid angiosarcoma of the bone. Plain radiography, CT, and MRI revealed multiple osteolytic lesions in both femurs; some lesions showed soft tissue extension with cortical bone destruction. Interestingly, PET-CT revealed that the lesions were only distributed along the bones of the lower extremities, including the pelvic bones, femurs, and tibiae. Despite histological analysis initially suggesting metastatic carcinoma, after additional immunohistological staining, including that for vascular markers (CD31 and ERG), the final diagnosis was epithelioid angiosarcoma. A better understanding of the clinicoradiological features of this disease may help eliminate diagnostic confusion and provide better management.

Increasing Work Efficiency with Prevention of Reinspection for Bone Mineral Density (BMD) Exams (골밀도 검사의 재검사 방지에 따른 업무효율 향상)

  • Recently hospitals are implementing a One Stop Service as part of patient-care service. With the One Stop Service, medical treatment including consultation, inspection, and results are changing to be made available in one visit. Therefore most examinations are reserved for the same day; however, there are cases in which additional visits are necessary because of certain properties related to exams. This study compares and analyzes the number of reinspections before and after reforms. By designating the order of priority for BMD examinations and implementing education from information obtained in the OCS E-manual update, the number of reinspections for wards was reduced from 58 to 21, Outpatient departments were reduced from 51 to 12, and errors in reservations made by employees in the department of radiology were reduced from 98 to 11. Reinspections can be reduced with full understanding the factors related to reinspection and a background check prior to inspection in order to determine the order of priority for inspections. This will also reduce workers' stress and increase their efficiency and at the same time decease patient dissatisfaction and improve hospital reliability.

Status of Interchange of Medical Imaging in Korea: A Questionnaire Survey of Physicians (영상정보교류 실태 파악을 위한 의사 설문조사)

  • The purpose of this study was to summarize the results of a survey for physicians with specialties other than radiology about imaging studies of patients referred from other institutions. The survey was promoted through individual contacts or social network service and physicians who voluntarily responded to the survey were the subjects of the study. The questionnaire consisted of 11 questions about basic information and referrals about medical imaging. A total of 160 physicians from 30 specialties participated in the survey and 95.6% of the respondents worked in tertiary care center or general hospital. Patients were frequently referred with outside medical images. The most frequently referred imaging modalities were computed tomography and magnetic resonance imaging. However, radiological reports from outside institutions were rarely referred. Most physicians thought that reinterpretation for outside imaging is necessary to acquire a secondary opinion. In conclusion, considering that outside radiological reports are frequently missing and there are high demands on reinterpretation for outside imaging, guidelines for referral of radiological reports with medical imaging, basic elements of radiological reports, and reinterpretation need to be developed.

Evaluation of the Usefulness of Virtual Reality Equipment for Relieving Patients' Anxiety during Whole-Body Bone Scan (전신 뼈 검사 환자의 불안감 해소를 위한 가상현실 장비의 유용성 평가)

  • Purpose When performing a whole-body bone scan, many patients are experiencing psychological difficulties due to the close distance to the detector. Recently, in the medical field, there is a report that using virtual reality (VR) equipment can give pain relief to pediatric patients with weak concentration or patients receiving severe treatment through a distraction method. Therefore, in this paper, VR equipment was used to provide psychological stability to patients during nuclear medicine tests, and it is intended to evaluate whether it can be used in clinical practice. Materials and Methods As VR equipment, ALLIP Z6 VR (ALLIP, Korea) was used and the experiment was conducted after connecting to a mobile phone. The subjects were 30 patients who underwent whole-body bone examination from September 1, 2021 to September 30, 2021. After intravenous injection of 99mTc-HDP, 3 to 6 hours later, VR equipment was put on and whole body images were obtained. After the test, a survey was conducted, and a Likert scale of 5 points was used for psychological anxiety and satisfaction with VR equipment. Hypothesis verification and reliability of the survey were analyzed using SPSS Statistics 25 (IBM, Corp., Armonk, NY, USA). Results Anxiety about the existing whole-body bone test was 3.03±1.53, whereas that of anxiety after wearing VR equipment was 2.0±1.21, indicating that anxiety decreased to 34%. When regression analysis of the effect of the patient's concentration on VR equipment on anxiety about the test, the B value was 0.750 (P<0.01) and the t value was 6.181 (P<0.01). decreased and showed an influence of 75%. In addition, overall satisfaction with VR equipment was 3.76±1.28, and the intention to reuse was 66%. The Cronbach α value of the reliability coefficient of the questionnaire was 0.901. Conclusion When using VR equipment, patients' attention was dispersed, anxiety was reduced, and psychological stability was found. In the future, as VR equipment technology develops, it is thought that if the equipment can be miniaturized and the resolution of VR content images is increased, it can be used in various clinical settings if it provides more realistic stability to the patient.

  • 62-year-old patient who had past history of endoscopic submucosal dissection for early gastric cancer at September 2008, underwent endoscopic submucosal dissection of esophagus for early esophageal cancer at mid esophagus during health screening service. Because there was a high risk of lymph node metastasis at biopsy results, concurrent chemoradiotherapy was added to endoscopic submucosal dissection. There was a metachronous cancer at mid-esophagus at March 2013. He underwent endoscopic mucosal resection and photodynamic therapy. Concurrentchemoradiotherapy after endoscopic submucosal dissection is an effective treatment method.

  • Colorectal cancer produce focal mass or segmental thickening which can be detected with sonography. The purpose of this study was to describe sonographic findings of colorectal cancer. we reviewed sonograms of 51 patients with colorectal cancer in whom sonography was performed before colon study. In 51 patients who had more common coloretal cancer 27cases(53%) had 40 to 50 years of age(60%). Sonographic findings included segmental thickening 42cases, or irregular mass 9 cases. With careful examination, these findings can be detectable, and therefore bowel should be carefully examination in patients with sign and symptoms suggesting carcinoma of the colon and rectum.

  • Objective:This study was conducted to examine the Draw-A-Person test as a non-verbal indicator of concrete and abstract thinking. Method:Thirty child-psychiatric children and 27 normal children were individually assessed with Draw-A-person Test and KEDI-WISC, and the scores were compared between similarity test and DAP. Results:The results showed significant differences between the groups. In clinical group, there were significant correlations with DAP scores and similarity test scores, especially abstract scores on DAP were highly correlated with similarity test scores. Results indicate characteristics of abstract thinking may be more discernable on the DAP than concrete thinking in clinical group.
