A Literature Review for Fall-Prevention Nursing Program Development based on the Fall Information of a Rehabilitation Hospital (일 지역 재활병원의 낙상 정보에 근거한 낙상예방 간호중재 프로그램 개발을 위한 문헌분석 연구)

  • This study is to design an effective fall-prevention nursing program. Researchers investigated the current condition and risk factors for falls in the small-medium sized rehabilitation hospital with descriptive research. We conducted integrative literature review to reflect the current approach of fall-prevention nursing program. We found that the risk factors for falls are related to the characteristics of clinical setting of the hospital. We suggested the nursing intervention with applying fall risk assessment scale that is sensitive to the elderly population and the intervention that shows maximum effect. In conclusion, this study proposed the way of selecting an effective fall-prevention nursing program based on a clinical setting and prospective of its application. This will contribute to the improvement in nursing practice with the critical view in hospital fall.

  • 최근 고령화 사회가 심각한 사회적 문제로 급부상하고 있으며, 이에 병원을 찾아 입원하는 비중이 이전에 비하여 높아지고 있다. 거동이 불편하거나 근력이 부족한 환자의 경우 스스로 거동할 능력이 다소 떨어지며, 낙상사고가 발생하면 부상 혹은 치명적일 경우 사망으로 이어질 수 있다. 하지만, 이들을 보살피는 간호 인력만으로 병원 내 모든 낙상사고를 파악하기에는 한계가 있다. 또한, 환자들의 낙상 탐지에 관한 연구는 지속해서 수행되어왔지만, 병원 내부환경에서의 낙상 탐지 연구는 부족하다. 이에 본 논문에서는 병원 내부환경에서 낙상을 탐지하기 위해 실제 병실에서 수집한 데이터로 YOLOv5 모델을 학습하여 환자 낙상 탐지모델을 구축 및 평가하였다.

The Effect of Knowledge, Attitude and Perceptions of Patient Safety Culture on Fall Prevention Activities in Mental Hospital Nurses (정신병원 간호사의 낙상에 대한 지식, 태도, 환자안전문화인식이 낙상예방활동에 미치는 영향)

  • The purpose of this study was to investigate the efforts of Knowledge, Attitude and Perceptions of Patient Safety Culture on Fall Prevention Activities in Mental Hospital Nurses. This study is a descriptive research study of 153 nurses working in Busan and Gyeongnam mental health hospitals, the data were collected from April 4 to December 31, 2018. Data were analyzed using IBM SPSS/win 24.0 program, which included t-test, ANOVA and multiple regression analysis. As a result, The attitude toward falls differed depending on subject's license or qualifications, the higher the education level. The higher the level of perceptions of patient safety culture, and the higher the work experience, the more prevention activities toward falls. The higher the perception of patient safety culture, the higher the attitude toward falls. The higher the prevention activities toward falls, the higher the attitude of falls and the perceptions of safety culture. As a result of multiple regression analysis of factors affecting Knowledge, Attitude and Perceptions of Patient Safety Culture on Fall Prevention Activities were 12.5%. Therefore, in order to promote fall prevention activities of mental hospital nurses, the knowledge needs to be expanded through continuous education. Education programs should be developed and provided to change attitudes toward falls. and At the hospital organization level, a wide range of support is required, including changes in the overall human and institutional environment for safety.

Effects of Long Term Care Hospital Care-givers' Fall Prevention Self Efficacy and Fall Prevention Health Belief on Fall Prevention Awareness (요양병원 간병사의 낙상예방효능감과 낙상예방건강신념이 낙상예방인지도에 미치는 영향)

  • The purpose of this study is to investigate the factors that affect the fall prevention awareness of care-givers working at long-term care hospitals. A convenience sampling method was used to select care-givers who worked at 7 different long term care hospitals which were located in D city and N city. Data were collected from July 2014 to August 2014. 200 data were used for analysis out of 215 data which were collected. The hierarchical regression analysis reveals the following results: Firstly, the effects of care-givers' education level, certificate status, period of work experience and fall-related learning hours on their fall prevention awareness level were statistically significant. Secondly, among fall prevention health beliefs, perceived benefit, perceived disability, perceived sensitivity and self-efficacy were positively related to the level of fall prevention awareness. Thirdly, while such factors as perceived benefit, perceived sensitivity and self-efficacy showed positive effects on the fall prevention awareness, the period of work experience had negative effects. The regression model shows the power of explanation of 31.7 percents. In conclusion, the study suggests a fine-tuned program to improve care-givers' fall prevention awareness in a way of promoting fall prevention self-efficacy, perceived benefits and perceived sensitivity while considering the care-givers' period of work experience.

Barriers to the practice of fall prevention by nurses and nurse assistants at geriatric hospitals (요양병원 간호사 및 간호조무사의 낙상예방활동 실천의 방해요인)

  • Older adults are at a high risk of falling, causing severe injuries and increased hospital stays and treatment costs. This can be a burden not only on the family but also on the national economy. Thus, fall prevention is very important in nurses' and nursing assistants' work. This study intended to grasp the real situation faced by nurses and nursing-assistants working at geriatric hospitals in J province, Korea. The researcher interviewed thirty-six nurses and nursing-assistants in four geriatric hospitals and performed four focus group interviews. Data were analyzed through an inductive content analysis based on Elo & Kyngäs's approach. Three categories and five subcategories were identified. Categories were patients, environmental, and personal factors. Patient factors included patients' impaired cognitive function. Environmental factors included lack of personnel support and assignment of inpatients without regard to fall risk. Personal factors included hopelessness regarding improvement and fear of being blamed. Promotion of fall prevention practice needs a gradual increase in nursing staff at geriatric hospitals, a system for assigning hospital room according to fall risk, and education and support system to reduce nursing burden.

Effects of Fall Prevention Educational Program for Nurses in Comprehensive Nursing Care Units (간호⋅간병통합서비스병동 간호사를 위한 낙상예방교육프로그램 효과)

  • This study was to examine the effects of fall prevention educational program on the related knowledge, attitude, and self-efficacy of nurses working in Comprehensive Nursing Care Unit, general hospital. A quasi-experiment with a nonequivalent control group non-synchronized design was used, Participants included a total of 40 nurses as the subjects (Intervention: n=20, Control: n=20) working in Comprehensive Nursing Care Unit, general hospital, Seoul. Data were collected form March through May, 2017. Data were analyzed by using the SPSS/WIN 21.0 program. Intervention group showed significant higher knowledge (F=189.03, p<.001), attitude (F=761.74, p=.003), and self-efficacy (F=188.50, p=.002) scores than them of control group. The fall prevention educational program was an effective intervention for improving the related knowledge, attitude, and self-efficacy of nurses working in Comprehensive Nursing Care Units, general hospital. Therefore, the results show the necessity of developing and applying the systemic nursing intervention to improve preventive behavior on falls.

Medication use as a Risk Factor for Falls in Hospitalized Elderly Patients in Korea (입원 노인환자의 의약품 사용과 낙상위험도 연구)

  • 낙상은 노인의 건강을 위협하는 가장 심각한 문제 중의 하나이며, 조기사망, 신체손상, 운동장애, 심리학적 기능장애를 유발하는 원인이기도 하다. 본 연구의 목적은 국내 노인전문 요양병원 입원 환자들의 의약품 사용이 낙상에 미치는 영향을 평가하는데 있다. 후향적으로 원내 의무기록 정보를 이용하여 환자-대조군 연구를 수행하였고, 2008년 1월부터 2010년 12월까지 3년 기간에 입원한 65세 이상을 대상으로 하였다. 입원기간 중 낙상을 경험한 34명의 노인환자들을 환자군으로 선택하였으며 낙상을 경험하지 않은 68명의 노인환자들을 무작위 추출하여 1:2의 환자군:대조군비율로 연구 대상 환자들을 선정하였다. 환자군이 복용한 의약품을 대조군이 복용한 의약품과 비교하였으며 각 계열별 의약품과 낙상위험도 관계를 평가하였다. 두 그룹간의 인구통계학적 특성은 유사하였고 연령, 성별, 복용 의약품수, 고혈압 유무, 혈중 크레아티닌 수치, 혈중 나트륨 수치, 혈압 또는 심박수에 유의한 차이는 없었다. 항히스타민제와 본 연구에서 유일하게 기타 수면보조제로 분류된 졸피뎀이 유의하게 낙상위험도를 증가시켰다.

Circumstances, Risk Factors, and the Predictors of Falls among Patients in the Small and Medium-sized Hospitals (중소병원 입원환자의 낙상발생 시 현황, 낙상위험요인 및 낙상발생 예측요인)

  • Purpose: This study examined the circumstances, risk factors, and the predictors of fall incidents among patients in the small and medium-sized hospitals. Methods: Fifty patients with any fall experiences were matched by gender, age, and medical departments with 100 patients without fall incident at the same hospital. Data were collected from 5 small and medium-sized hospitals. Data were analyzed using descriptive statistics, a Chi-square test, a Fisher's exact test, and a logistic regression with the SPSS/WIN 21.0 program. Results: In the patients with falls, the largest number of falls occurred during the day shift, in the patients' rooms, and while they were walking. Further 74.0% of the patients had physical injuries, and 34.0% had to take further medical diagnostic tests. Significant differences were found between the patients with falls and the others on 14 variables (cardiovascular disease, anemia, sedative-hypnotics, vasodilators, narcotic analgesics, dizziness, general weakness, unstable gait, walking aids, anger, anxiety, depression, orientation, and fear of fall). Narcotic analgesic use, dizziness, walking aids, and cardiovascular disease were identified as the predictors of fall incidents. Conclusion: These findings are hoped to be used in developing a fall risk assessment tool and fall prevention nursing programs for small and medium-sized hospitals.

Development of Fall Prevention Program for Improvement of Healthcare in Rehabilitation Patients Based on Image Processing : A Preliminary Investigation (영상처리 기반 재활 환자의 헬스케어 개선을 위한 낙상예방 프로그램 개발 : 예비연구)

  • The purpose of this preliminary study was to investigate the effect of fall prevention program on the occurrence of falls in inpatients at general hospital. Before the intervention, we identified the occurrence of the rate of falls in I hospital at Incheon and 190 patients received rehabilitation treatment. The causes of falls were carelessness of caregivers and therapists, increased burden of therapists, and height of beds. After recognizing the problems, the authors developed a fall prevention program through the PDCA model. The fall prevention program consisted of a video of fall prevention education and education for the patients and caregivers, environmental improvement and education of therapists for two months. After intervention, 230 patients were subjected to be included as analysis of the incidence of falls included As a result, the fall incidence was reduced by 34.1~66.7% in the pain clinic and 21.3~40.8% in the exercise clinic / department of occupational therapy compared to the before intervention. These findings show that the fall prevention program has a positive effect on the fall of inpatients, and it can be used as a model for fall prevention.

Differences in Health Status-related Characteristics Before and After Falls in Adult Hospitalized Patients (성인 입원 환자의 낙상전후 건강상태 관련 특성의 차이)

  • This study aims to investigate the changes in health status of inpatients before and after a fall accident, and it is a retrospective study using data from 328 inpatients who fell from January 1, 2016 to December 31, 2020, reported to the patient safety reporting system. The average age of the study subjects was 68.57(±14.13), and those in their 70s accounted for the most at 30.49%. Falls occurred on average 13.86(±25.03) days after hospitalization, and the time when the most falls occurred was between 22:30 and 06:59 with 42.99%. Before and after a fall during hospitalization, bowel problems (x2=314.0, p<.001), urination problems (x2=284.0, p<.001), intravenous fluid therapy (x2=85.16, p<.001), and walking (x2=69.77. p<.001), bedridden state (x2=51.60, p< .001), mental state and performance (x2=17.52, p<.001) patient's attitude (x2=220.17, p<.001), there was a statistically significant difference. It is necessary to develop an appropriate method and education program for fall prevention in hospital by considering the individual characteristics of inpatient.
