Changing Tendency of the Psychiatric Hospitals' Acute Wards for Return to Society in Japan (사회복귀를 고려한 일본 정신과 급성기치료병동의 변화경향)

  • Purpose: This study shows a changing tendency of Japanese psychiatric hospitals, presenting "Visions in Reform of Mental Health and Medical Welfare", changed to "Hospitalized Medical Treatment to Living in the Community" and reducing the number of beds since 2004. Methods: This study uses a documentary survey, especially Jananese "Visions in Reform of Mental Health and Medical Welfare System" and a field survey on hospital built for early treatment and return to society after 2004. Results: Change tendencies of psychiatric hospitals are 1) diversification of isolation room and ward, 2) cluster of patient's room, 3) phased organization of space, 4)individualization. Implication: This study will be an important data for researching plan of Korea psychiatric hospital which needs to change by global tendency.

A Study on the efficient Composition of a Nurse Station and Attached Spaces in General Hospitals (종합병원 병동부의 간호사 대기실 및 관련공간의 효율적인 구성에 관한 연구)

  • The purpose of this study is to suggest reasonable composition and layout of a nurse station and attached spaces-which are central sector of patient caring in the inpatient department- in the view of funtional efficiency. The method of this study is examining other studies and case study of inpatient department in general hospitals. through this research we find out the plan type of inpatient department and layout of nurse station and attached spaces in the view of 'the efficiency of circulation' and 'the easiness of observation' . It is also found out that the nursing delivery system effects to the composition and layout of nurse station attached spaces. Therefore, the efficient design of inpatient department by this study can accomplish 'patient-focused' medical environment and aid to develop their health.

Effects of Wholistic Hospice Nursing Intervention Program on Pain and Anxiety for In-patient of Hospice Palliative Care Unit (전인적 호스피스간호중재 프로그램이 입원한 호스피스환자의 통증과 불안에 미치는 효과)

  • Purpose: This study was to test the effects of wholistic hospice nursing intervention program on pain and anxiety for in-patient of hospice palliative care unit. This study's design was one-group pre-post test quasi- experimental research. Methods: The subjects of study were 27 patients who were over 18 years old and admitted in hospice palliative care unit of S hospital in P city with agreement in hospice palliative care in their terminal disease. The experimental group subjects participated in holistic hospice nursing program took 120 minutes per session, a total of 1,200 minutes altogether for 10 sessions. The period of data collection was from April 6, 2004 to April 20, 2005. The collected data were analyzed by Paired t-test with SPSS/WIN 12.0 program. A Wholistic Hospice Nursing Intervention Program (named ‘Rainbow Program’) was used as a experimental tool in this study. This was developed by the authors. It was provided by interdisciplinary hospice team (nurses, medical doctors, social worker, pastors, art therapists, and volunteers). In addition, Korean Version of Brief Pain Inventory (BPI-K) by Young-Ho Yun(1998) was used to test degree of pain in physical aspect. And State-Anxiety Inventory was developed by Spielberger(1975) and translated by Kim, Jung-Tack & Shin, Dong-Gyun(1978) was used to test the degree of state-anxiety in emotional aspect. Results: (1) Hypothesis No. 1 "The experimental group which received Wholistic Hospice Nursing Program will have a lower degree of pain than before" was supported (t=-10.585, P= .000). (2) Hypothesis No. 2 "The experimental group which received Wholistic Hospice Nursing Program will have a lower degree of state-anxiety than before" was supported (t=-8.234, P= .000). Conclusion: Our results testified that this Wholistic Hospice Nursing Intervention Program was effective to decrease pain and state-anxiety of the in-patients of hospice palliative care unit. Therefore it can be used and applied actively in practice as a useful model of interdisciplinary team approach by hospice professionals in hospice palliative care unit.

  • This is a methodological study that verifies the validity and reliability by translating and modifying the Korean version of Behavior Health Care Competency (BHCC) tool. This study verified the content validity, construct validity, and reliability of 230 nurses working in general hospitals. The BHCC tool was analyzed using a total of 4 factors and 16 items. The tool had a high level of reliability and validity. The BHCC is a measurement tool considering the mental health care of non-psychiatric nurses, and it has meaning in that it establishes basic material so non-psychiatric nurses can manage the patient's physical and potential psychiatric problems effectively.

  • This study was carried out to know the main defense mechanisms used by adolescent victims of school violence. Subjects of this study are composed of 41 adolescent victims(clinical group) and 40 normal adolescents(control group). Clinical group is divided into four subgroups of inpatient, outpatient, day hospital, and school groups. Used scales are Ewha Defense Mechanisms Test(EDMT) and Staittrait anger scale. Several important results are found. Adoescent victims use neurotic defense mechanisms of neurotic and mature level less frequently than normal adolescents. In clinical group day hospital adolescents use more mature defense mechanisms than outpatient adolescents. Displacement and acting out are correlated with trait and sate anger. Somatization is correlated with trait anger, and regression is correlated with state anger significantly.

  • In order to observe the protein composltlOns of soluble extracts of P. westermani, and their changes during early developmental stages, the crude saline extracts of 4, 6, 8, 10 and 12 week-old worms which were harvested from experimentally infected dogs were analysed by disc-PAGE. The results were as follows: 1. A total of 15 bands were identified from electrophoregrams of respective developmental stages. Of them, 5 bands were recognized throughout the developmental stages. 2. The number and protein amount of identified bands changed according to the worm development from 4 weeks to 12 weeks. However, tLe bar::ding patterns of 4 and 6 week-old worms and 8 and 10 week-old worms were similar each other. 3. Of 15 identified bands, Band 1 was recognized only in 12 week-old worms whereas Bands 3, 4, 8, 9, 10, 11 and 15 gradually lowered their amount according to dcvelor:ment to disappear in 12 week-old. In addition, Band 5 became a major band in 12 week-old while Band 6 turned to a minor band at the same age. The possible relations of changing patterns of protein bands with worm development were discussed.

Sintering and Mechanical Properties of Silicon Nitride Prepared with a Low-cost Silicon Nitride Powder (저가의 $\beta$-상 분말을 사용한 질화규소의 소결 및 기계적 특성)

  • A refractory grade low-cost silicon nitride powder was chemically analyzed, purified, and gas pressure sintered with the sintering additives. As-received powder contained a significant amount of free-Si, 0.72 wt% of Fe, 0.5 wt% of al and 0.31 wt% of Ca. Oxygen and carbon contents of the powder were 3.3 wt% and 0.4 wt%, respectively, and it consisted of 96% of $\beta$-phase and 4% of $\alpha$-phase. After lowering the Fe content and nitriding treatment, the powder was sintered with 6 wt% yttria and 2 wt% alumina for 1 h between 1823 K and 2123 K in order to examine the sintering behavior. Fully dense samples were obtained by sintering at 2123k for 2h. For comparison, a commercially available high-grade powder was also sintered at the same time. The low-cost powder showed much slower densification rate than the high-grade powder. Fully dense sample prepared from the low-cost powder contained a number of coarse grains with a low aspect ratio, and its hardness, fracture toughness, flexural strength and thermal shock resistance were not as good as those of the sample prepared with the high-grade powder.

First Case of Urinary Tract Infection by Lactococcus garvieae in Korea (우리나라에서 Lactococcus garvieae에 의한 요로감염의 사례)

  • The genus Lactococcus is a gram-positive, catalase-negative, non-motile bacterium. It is a facultative anaerobe and can be cultured at 10~40℃. The genus Lactococcus consists of 16 species, of which Lactococcus garvieae and Lactococcus lactis are known to cause disease in humans. This study reports the first case in which L. garvieae was identified in the urine culture of a 74-year-old woman. The patient confirmed the findings of acute urinary tract infection through blood tests, microbial identification tests, antibiotic susceptibility tests, and computed tomography performed at a hospital. The patient was admitted to the nephrology ward and was treated with IV fluids and erythromycin antibiotics and discharged 5 days later. This is the first case in Korea in which L. garvieae was isolated from the urine of a patient with a urinary tract infection and is expected to be useful in treating patients with L. garvieae infection in the future.

Nursing Professor's inspection and Status of Patient's Records and Informed Consent for Clinical Practice of Nursing Student in Korea and Japan (한·일 간호대학생의 임상실습 시 환자의 설명동의 및 기록관리와 지도실태)

  • Objectives: In recently. the management and protection on individual information in patient's medical & nursing records have been very important, and that need a guideline. The purpose of this study was to investigate the status of using the patient's nursing records of nursing students in clinical practice, to find and discuss the patient's informed consent, and status of education and management concerned to patient's nursing records. Methods: This study used a mailing survey. data collected from September 24th to October 31th in 2002. The subject were 333 professors who are major in adult nursing, pediatric nursing, psychological nursing of 111 university of nursing department and nursing college. And then we received the survey mail from 103 professors that respondent rate was 30.9%. Results: The characteristics of study subjects showed 49.0% of university. 51.0% of college of nursing. 50.0% of the subjects practiced point the patient by oral approval in clinical practice. But when the decision of the patient was very difficult, 21.6% of the subjects take to informed consent from his or her families. During the clinical practice, 49.0% of the subjects were explain to patient about clinical practice and contents of the nursing student, only 7.8% of the subjects were explain to patient with nursing records. 52.0% of the subjects were took out records from the hospital, only 17.6% of the subjects had standard of the patient's informed consent and standard of handling practice records. 17.6%-92.2% of the subjects that educate and manage concern to patient's nursing records.

Development of a CD Program Applied Logotherapy for Psycho.Spiritual Care of Late Adolescents with Terminal Cancer (청소년 후기 말기 암 환자의 정서적.영적 돌봄을 위한 의미요법 CD 프로그램 개발)

  • Purpose: The purpose of this study was to develop a CD program of applied logotherapy for psycho spiritual care of late adolescents with terminal cancer. Methods: Keller & Song's ARCS theory and a model for developing learning materials was applied to develop this program composed four distinct phases: planning, designing, developing, and evaluation stages. Results: This program was entitled 'Finding meaning in my life' and consisted of 5 sessions and its educational contents were made up as follows: "First Secret" is 'learning three natures of the human mind', "Second Secret" is 'learning creative values first method to find meaning of life', "Third Secret" is 'learning experiential value as second method to find meaning of life', "Fourth Secret" is 'learning attitudinal value as third method to find meaning of life', and "Fifth Secret" is 'Becoming the master of my life'. The sub-menu was made up of 'Beginning', 'Opening mind', 'Learning'. 'Laughing Song', 'Experiencing'. Conclusion: This CD program applied logotherapy with flash animation technique as an emotional and spiritual nursing intervention program for easier and more scientific application in pediatric oncology and hospice care area.
