A Retrospective study of the Cumulative Survival Rate and change of peri-implant marginal bone around implants associated with maxillary sinus augmentation (상악동 골 이식술을 동반하여 식립된 임플란트의 변연골변화와 생존률에 관한 후향적 연구)

  • Statement of problem: Insertion of endosseous implants in the atrophic maxilla is often complicated because of lack of supporting bone. Augmentation of the floor of the maxillary sinus with autogenous bone & bone substitute graft has been proven to be a reliable treatment modality, at least in the short term. The aim of this study is to evaluate the factors of implant survival rate associated with maxillary sinus lift with grafts. Material and methods: The sinus floor was augmented with bone grafts derived from modified Caldwell-luctechnique(71 subject, 93 sinus, 180 implants), the autogenous bone or autogenous+Bio-oss. Before implant installation the width and height of the alveolar crest were increased in the first stage procedure in 10 patients while in the other 61 patients augmentation and implant installation could be performed simultaneously width and height of the alveolar crest > 4 mm) or delayed installation. Results: In all case bone volume was sufficients for implant insertion. 14 of 180 inserted implants were lost during follow up and the healing period Patient received implant supported overdenture(5 patients) or fixed bridge(62 patients). Conclusion: Within the limit of the result of this study, we conclude that bone grafting of the floor of the maxillary sinus floor with bone for the insertion of implants might be a reliable treatment modality and the autogenous bone graft and delayed installation method might be the factors for good results.

Complications of implants confronting the extruded posterior teeth (정출된 구치와 대합되는 임플란트의 합병증)

  • Purpose: This study investigated the complications and patterns of implants that confronted with the extruded natural teeth. Materials and Methods: Among patients who received implant treatment between 2006 and 2018 at Chosun University Dental Hospital, only implants that had a maintenance period of at least 5 years and that both implants and confronting natural teeth could be measured and compared on a radiograph were selected. They were divided into two groups. Group 1, experimental group consisted of Implants confronting the extruded natural posterior teeth (n = 167: maxilla (Group 1max) = 92, mandible (Group 1man) = 75) and Group 2, control group consisted of Implants confronting the normal natural posterior teeth (n = 656: maxilla (Group 2max) = 272, mandible (Group 2man) = 384). Results: The incidence of complications between Group 1 and Group 2 was statistically significant. In particular, there was a statistically significant difference between the frequency of marginal bone loss and the amount of marginal bone loss. Conclusion: The extruded natural tooth may cause more frequent complications including bone resorption in the opposing implant.

Effect of immobilization of the recombinant human bone morphogenetic protein 2 (rhBMP-2) on anodized implants coated with heparin for improving alveolar ridge augmentation in beagle dogs: Radiographic observations (양극산화 임플란트 표면에 적용된 헤파린과 골형성단백질(rhBMP-2)이 치조골 증대에 미치는 효과: 방사선학적 평가)

  • Purpose: The aim of this study was to evaluate the effect of immobilization of the recombinant human bone morphogenetic protein 2 (rhBMP-2) on anodized titaum implants coated with heparin to enhance the vertical alveolar ridge augmentation in the supraalveolar peri-implant defect region. Materials and methods: 18 pure titanium implants (7.0 mm in length, 3.5 mm in diameter) were manufactured for this study. All implants were anodized and designed insertion reference line marked with laser at the apical 2.5 mm from the fixture platform. Implantation of 6 noncoated anodized implants (Control group), 6 anodized implants physically adsorbed with rhBMP-2 by dip and dry method (BMP group) and 6 anodized implants chemically immobilized 3,4-dihydroxyphenylalanine (DOPA)-heparin/ rhBMP-2 (Hep-BMP group) was performed in the both mandibular of three male adult beagle dogs using split-mouth design. Radiologic examinations were performed immediately after implant placement and 4 and 8 weeks after implant placement. The amount of mesio-distal bone augmentation was evaluated by measuring the vertical distance from the platform to the marginal bone. Statistical analysis was performed using one-way analysis of variance (SPSS version 18.0) and multiple comparison analysis of The Kruskal-Wallis test and the Mann-Whitney U test. Statistical significance was established at the 5% significant level. Results: At the 4 weeks vertical alveolar ridge augmentation of Control group, BMP group and Hep-BMP group is $0.09{\pm}0.22mm$, $1.02{\pm}0.72mm$, and $1.29{\pm}0.51mm$, At the 8 weeks $0.11{\pm}1.26mm$, $1.11{\pm}0.58mm$, $1.59{\pm}0.79mm$ according to radiographic observations. The two experimental groups showed a significantly increasing in vertical bone height compared with the control group (P<.05). However, there is no significant difference between the BMP group and Hep-BMP group (P>.05). Conclusion: The rhBMP-2 coated implants were enhanced the vertical bone growth in the supraalveolar peri-implant defect area. However, there is no significant difference between chemically and physically coating method.

Ultrasonographic Utility for Arthroscopic Examination of Knee (슬관절 관절경 검사 시 초음파 검사의 유용성)

  • Purpose: To evaluate the relationship between the real pathology & abnormal finding found by ultrasonography. Without an MRI test being done beforehand, an arthroscopy is done after an ultrasonography to show abnormal lesions during a knee abnormality. Materials and Methods: The subjects were 42 patients out of 49 cases, excluding those with rheumatoid arthritis, septic arthritis and patients suspected with a ligament tear, which were examined by ultrasonography alone before receiving a knee arthroscopy in our hospital from July 2007 to July 2008. In every case, a physical examination, simple X-ray and knee ultrasonography was done. An arthroscopy was performed when there was ultrasonographic abnormal finding. Before the procedure, a MRI test was not performed and when abnormal findings were found by an arthroscopy, an appropriate surgery was done. Results: During the ultrasonographic examination, there were various sized effusions in the suprapatellar pouch. Also, in addition there were eleven cases of medial meniscus abnormalities, sixteen cases of lateral meniscus abnormalities, and two cases of cystic lesions. Throughout the arthroscopic examination, there were 14 cases of medial meniscus abnormalities, 20 cases of lateral meniscus abnormalities, 15 cases of cartilage damages, 9 cases of medial pathologic plica, 2 cases of intra-articular loose bodies, 5 cases of chondromalacia, 2 cases of cyst, and 2 cases of synovitis. When an effusion abnormality was found by the ultrasonography in a suprapatellar pouch, there was a 100% probability of knee pathology. When a medial meniscus abnormality was found with an ultrasonography, there was a 90.9% probability of a real pathology. When a lateral meniscus abnormality was found there was 81.2% probability of a real pathology. Ultrasonography was 100% accurate when it came to cystic lesions. Conclusion: Knee ultrasonography performed before an arthroscopy seems to be a very useful examination method when suspecting intra-articular lesions.

Initial Changes of Implant Stability from Installation during Early Bone Healing (임플란트 식립 후 초기 안정성의 변화)

  • The concept of implant stability was basically originated from the relative condition of bone-implant interface and has some meanings for evaluation of that interface. In addtion, it has been used for the investigation of initial bone healing process after fixture installation because a degree of micromotion around interface can affect unfavorable clinical results. The purpose of this study is to investigate the mode of initial bone healing from fixture installation through prospective trial. Thirty fixtures were consecutively installed in mandibles of 26 patients with single tooth loss area and then healing abutment were secured for one-stage surgery meothod. Resonance frequency analysis was performed with one week interval during 12 weeks and periapical radiographs were taken at each month. Although marginal bone level change was not shown through observation period (P>0.05), statistical difference of implant stability was shown through 4 and 6 week (P<0.05) and was not shown after 6 week (P>0.05) according to the bone quality. Initial bone healing process is a successive process of bone resorption and favorable bone healing result might be postulated at 4 week interval after installation through RFA.

RADIOGRAGHIC STUDY OF MARGINAL BONE LOSS AROUND OSSEOINTEGRATED IMPLANTS AFTER FUNCTIONAL LOADING (보철물 장착후 골 유착성 임플란트 변연골 소실에 관한 방사선학적 연구)

  • Regular radiographic examination has been considered as an essential diagnostic method for the osseointegrated dental implantation. This study investigated the marginal bone loss through the measurement on the periapical radiograph around 46 endosseous dental implants. Marginal bone loss have been observed for 5 years after delivery of prostheses. The results were as follows : 1. According to the experimental periods, average marginal bone loss in total implant was 1.821mm in the first year, 1.833mm in the second, 1.892mm at third, 1.910mm at forth and 1.957mm at fifth after delivery of prostheses(P<0.05). 2. The average bone loss was 1.832mm in the maxillae and 1.819mm in the mandible in three years but there was no significant difference between the upper and lower jaw(P>0.05). 3. The average bone loss was 1.824mm in males and 1.818mm in females in five years but there was no significant difference according to the sex(P>0.05). These results indicate that the most of the alveolar bone loss occur within the first year after delivery of dental prostheses resulting in the exposure of polished neck positions, and the bone level stabilizes at first thread portion of the implant fixtures.

COMPARATIVE STUDY ON THE MARGINAL BONE LOSS OF IMMEDIATE NONSUBMERGED AND SUBMERGED ENDOSSEOUS DENTAL IMPLANTS PLACED INTO EXTRACTION SOCKETS OF DOGS (발치후 즉시 매식한 Nonsubmerged와 Submerged 임플랜트의 변연골 흡수양상에 관한 비교연구)

  • The conventional osseointegration protocol calls for waiting up to 12 months for ossification of an extraction socket to heal before placing an endosseous implant. In this study, the possibility of placing a pure titanium implant directly into an extraction socket immediately after extraction was investigated. And the marginal bone loss of immediate nonsubmerged and submerged endosseous dental implants placed into extraction sockets was also compared. Pure titanium Nobelpharma Branemark implants and solid screw type ITI implants were placed into premolar extraction sockets of two adult dogs and allowed to heal for a period of 3 months, followed by functional loading of the implant. Radiographic examination was performed before implantation, immediately after implantation and 3, 6, 9, 12 months after implantation. The results obtained were as follows : 1. Immediately placed nonsubmerged ITI implants and submerged Branemark implants showed favorable radiographic osseointegration status and there were minimum marginal bone loss. 2. There were no significant differences in radiographic finding of osseointegration between conventional and immediate implantation. 3. Gingival tissue around implants showed more inflammatory signs than that of adjacent natural teeth. This study suggest that pure titanium Branemark implants and submerged ITI implants have the potential to integrate when placed immediatly after extraction of the teeth and warrants further investigation.

  • Osteochondroma is a hyperplasic bony lesion caused by subperiosteal displacement of physeal cartilages, and the most common benign bone tumor. Osteochondromas rarely cause vascular complications, but when encountered, acute ischemia, phlebitis, and arterial puncture are the most common. They are caused by the mass effect or by the sharp end of the tumor. Deep vein thrombosis may be caused by impaired venous blood flow or a hypercoagulable blood state, but sometimes no predisposing cause can be found. We experienced a patient with deep vein thrombosis caused by the mass effect of an asymptomatic osteochondroma, and treated him successfully by excising the osteochondroma and performing a saphenous vein graft after resecting obstructed vein.

The risk factors for implant survival and marginal bone loss: a retrospective long-term study (임플란트 장기간 유지와 변연골 소실에 영향을 주는 요인들에 대한 후향적 연구)

  • Purpose: This study aims to investigate the risk indicators contributing to implant failure, and analyze the relationship between risk indicators and marginal bone loss (MBL) through long-term follow-up over 3 years. Materials and Methods: From 2003 to 2017, patients' medical charts with a history of dental implant surgery at Chonnam National University Dental Hospital were reviewed retrospectively. The patient's demographic variables, and clinical variables were recorded. Periapical radiographs were used to evaluated the changes in MBL around implants. And we analyzed implant survival rates. Multiple regression analysis with backward elimination was conducted to correlate the patient's clinical variables and implant failure and Pearson correlation analysis was performed to the correlated between implant long-term survival rates and MBL and initial stability. Results: In multiple regression analysis, there was a statistically significant negative correlation between abutment connection type (β = -.189, P < .05), with or without SPT (β = -.163, P < .05), diabetes (β = -.164, P < .05), osteoporosis (β = -.211, P < .05) and MBL. Anticoagulant medication influenced the long-term success rate of implants. PTV values at the second implant surgery showed a statistically significant negative correlation with long-term implant survival (P < .05). Conclusion: For the long-term success of the implant, the appropriate abutment connection type must be selected and the periodic SPT is recommended. Systemic diseases such as diabetes and osteoporosis and anticoagulant medication should be considered. Furthermore, since high PTV at the second implant surgery correlated with the long-term survival rates of the implant, initial stability should be carefully considered before undergoing the prosthetic procedure.

The Effects of Professional Tooth Cleaning and Plaque Control Instruction on Reduction of Peri-implantitis (전문가치면세정술과 세균막관리교육의 임플란트 주위염 감소 효과)

  • The purpose of this study was determine the effects of professional tooth cleaning and plaque control instruction(PT & PCI) on reduction of peri-implantitis. A total 80 implant patients were investigated using O'Leary plague index(PI), L${\ddot{o}}$e & Silness gingival index(GI), implant bone loss(BL). While the PT & PCI was conducted by using 'Watanabe method' after scaling for the experimental group, the engine polishing and a plaque control instruction was operated by rolling method after scaling for the control group. The collected data were analyzed with t-test, ANOVA, paired t-test and stepwise multiple regression. PI and GI of the experimental group significantly decreased than the control group(p<0.05). BL, also tended to decreased in the experimental group than the control group(p=0.155). Multiple regression analysis, the factors that was highly correlated with PT & PCI on the PI and GI. According to the implant characteristics, implant location and duration were closely related to PI(p<0.05). Therefore, the PT & PCI is one of the effective methods to reduce the failure caused by the peri-implantitis. Based on this conclusion, that it would be meaningful if the proposed PT & PCI is applied to the oral health management programs of the implant patients.
