Randomized, Double Blind, Placebo-Controlled Study of Bofu-tsusho-san on Obese Patients (비만환자에 대한 방풍통성산(防風通聖散)의 무작위배정, 이중맹검, 위약-대조군 임상시험)

  • Objectives The clinical trial was designed to investigate the safety and effects of Bofu-tsusho-san extracts on the change of the weight, body compositions, serum in obese patients. Methods This study was a 4-weeks, double blind, comparative clinical trial. Eligible subjects had a body mass index(BMI) greater than $25\;kg/m^2$ and waist circumference(WC) longer than 85 cm in woman or 90 cm in man. Among 38 subjects, 36 subjects were randomized either to Bofu-tsusho-san or placebo. After 4 weeks of treatment, we measured anthropometric factors(weight, height, WC, BMI etc.), abdominal fat area by CT scanning, serum lipid(total cholesterol(TC), triglyceride(TG), HDL cholesterol, LDL cholesterol), blood level of variety(glucose, adiponectine, leptin, C-reactive protein(CRP) etc.), blood pressure(BP). Adverse events also evaluated. Results BMI, BP, TG, CRP were reduced and weight, WC, score of KOQOL(Korean version of obesity-related QOL scale), SRI(Stress response inventory) were significant changed in Bofu-tsusho-san. But there were no considerable difference between Bofu-tsusho-san and placebo. there were no serious adverse events in either group. Conclusion There were limitations in this study that it conducted within a short period of 4 weeks. but its weight and WC loss effect was significant and it had few adverse events.

Comparative Study of Characterizing Components and Biological Activities of Bangpungtongseong-san Formulation (방풍통성산 제제의 성분 및 생리활성 비교 연구)

  • The purpose of this study was to investigate the quality of two different commercial Bangpungtongseong-san (BTS) extract granules (BTS-2 and BTS-3) by comparing with BTS decoction (BTS-1). The contents of characterizing components and biological activities of two different commercial BTS extract granules were compared with those of the BTS decoction. The contents of characterizing components were analyzed with HPLC. The antioxidative effects were determined by measuring 2,2-diphenyl-1-picrylhygrazyl (DPPH) radical scavenging and superoxide dismutase (SOD)-like activity. Also, we compared the effects on lipid accumulation and reactive oxygen species (ROS) production during differentiation of 3T3-L1 preadipocytes. The contents of five components except liquiritin and sennoside A were higher in BTS-1. The DPPH radical scavenging and SOD-like activity were higher in BTS-1. BTS-1 significantly inhibited lipid accumulation during differentiation of 3T3-L1 preadipocytes and showed stronger effects than BTS-2, BTS-3. In addition BTS-1 showed stronger inhibition effects on ROS production during differentiation of 3T3-L1 preadipocytes than BTS-2, BTS-3. These results indicate that BTS decoction has strong biological activities than commercial BTS extract granules. It is also consistent with the contents of characterizing components.

The Inhibitory Effects of Bangpuangtongseoung-san on the Obese gene and Obese Inhibitory about Obese-mouse induced by High Fat Diet (방풍통성산(防風通聖散)이 고지방사료(高脂肪飼料) 식이(食餌)로 유발(誘發)된 비만(肥滿)생쥐에서 비만유전자(肥滿遺傳子) 및 관련인자(關聯因子)에 미치는 영향(影響))

  • Objectives : In this study, the aim was to investigate the inhibitory effects of Bangpungtong-seoung-san on the obese gene and obese inhibitory about obese-mouse induced by high fat diet. Methods : In order to investigate the effects of Bangpuangtongseoung-san(BPTS) on the obese gene and obese inhibitory, C57BL/6 mice were induced by high fat diet. C57BL/6 mice were divided into three groups(normal, high fat diet with control, high fat diet with BPTS extract) and fed for 13weeks. Results : 1. The change of body weight and the final increase of body weight were decreased significantly. 2. The amount of the adipocyte in body weight was decreased significantly. 3. In primary adipocytes, ${\beta}3AR$ gene expression was increased significantly and leptin gene expression was decreased in $100{\mu}g/ml$ density significantly. 4. In primary adipocytes, the amount of TNF-${\alpha}$ was decreased in $100{\mu}g/ml$ density significantly and the amount of leptin was decreased but did not show significance. 5. In adipocytes tissue, the expression of leptin was decreased and the expression of ${\beta}3AR$ was increased. Conclusions : These results suggest that BPTS may inhibit the expression of the obese gene mad obese inhibitory about obese-mouse induced by high fat diet.

A Study on the Safety of Bangpungtongsung-san in Rats (랫드에서 방풍통성산의 안전성 연구)

  • Objectives : This study was conducted to investigate the safety of Bangpungtongsung-san in rats. Methods : The safety of this prescription on acute toxicity was evaluated by single dose toxicity study. Rats were orally administrated in a single dose of 0 and 2,000 mg/kg(limited dose) Bangpungtongsung-san. There were 7 rats in each groups. All animals were sacrificed after 14 days of treatment. After single administration, mortality, clinical signs, and body weight changes were observed for 14 days. Three parameters(autopsy finding, clinical chemistry, and hematology) were tested on the last day. Results : In this study with rats, Bangpungtongsung-san treatment did not show any acute toxicity. No mortality was noted for 14 days of treatment. There were no adverse effects on clinical signs, body weight changes, and autopsy finding at all treatment groups. The clinical chemistry parameters attesting to liver and kidney functions as well as the hematological parameters were within the normal ranges. Conclusions : It is considered that $LD_{50}$ of Bangpungtongsung-san is over 2,000 mg/kg in oral administration by rats. This finding of the safety of Bangpungtongsung-san is expected to strengthen the position of this prescription as nontoxic medicine.

Effect of BangPungTongSungSan(BPTSS, 防風通聖散) on acute methamphetamine-induced locomotor activity and c-Fos expression in mice (방풍통성산(防風通聖散)의 급성 메스암페타민에 의한 보행성 행동량과 c-Fos발현에 대한 효과)

  • Objectives : The BangPungTongSungSan(BPTSS) has been used as a therapeutic agent for cerebrovascular disease, cerebral hemorrhage, hypertension, diabetes and obesity in oriental medicine. The present study designed to investigate the effect of BPTSS on behavioral change and neuronal activation induced by acute methamphetamine(METH, 2 mg/kg, i.p.) in C57BL/6 mice. Methods : Mice received the oral administration of BPTTS(25, 50 and 100 mg/kg) 1 h prior to saline or METH administration. Locomotor activity was measured for 90 min using videotractking method and c-Fos expression, as marker of neuronal activation, was identified in a separate groups of mice by immunohistochemistry. Results and conclusions : Methamphetamine injection significantly increased locomotor activity and c-Fos expression in the nucleus accumbens and striatum. Interestingly, BPTTS(100 mg/kg) significantly suppressed locomotor activity and c-Fos expression in the nucleus accumbens and striatum by acute exposure to METH. These results suggest that BangPungTongSungSan may be effective in suppressing the reinforcing effect of methamphetamine by modulation neuronal activity.

Mechanism Study of Bangpungtongseongsan(BTS) on the Cerebral Hemodynamics in Mice (방풍통성산(防風通聖散)이 백서(白鼠)의 뇌혈류량역학(腦血流力學)에 미치는 기전(機轉) 연구(硏究))

  • Objective : Bangpungtongseongsan(BTS) has been used in oriental medicine for many centuries as a prescription for a heat syndrome of apoplexy. The effects of BTS on the vascular system are not well known. This study is designed to identify the effects of BTS on the regional cerebral blood flow(rCBF), arterial blood pressure and action mechanism in mice. Methods : We measured the change of rCBF and BP by BTS. Secondly, we investigated changes of rCBF and BP for 30 minutes interval after venous inject 0.01, 0.1, 1.0, 10.0mg/kg each density BTS infusion into mice, which were premanaged with methylene blue(10mg/kg, i.v), indomethacin(3mg/kg, i.v.), propranolo(3mg/kg, i.v.) for 30minutes. Results : rCBF increased significantly by BTS in a dose-dependent and BP was not affected by BTS in mice. Pretreatment with methylene blue significantly inhibited rCBF increased by BTS and aceelrated BP not affected by BTS. Pretreatment with indomethacin significantly inhibited rCBF increased by BTS and inhibited BP not affected by BTS. Conclusion : These results show that BTS causes the increasement of rCBF and non-change of BP. The action mechanism is related to prostaglandid activated by cyclooxygenase.

Documentational Study and Observation from the View of Hyungsang Medicine on Bangpungtongseong-san (방풍통성산(防風通聖散)의 문헌(文獻) 및 형상의학적(形象醫學的) 고찰(考察))

  • The following conclusions were obtained from the studies on Bangpuntongseongsan both from the documents and Hyungsang medicine. Bangpungtongseong-san was created by Yu Wan-so to relieve both interior and exterior of disease from the pathogenic fire, and it cures wind syndrome and dry syndrome. Bangpungtongseong-san is of light herbal combination and it works in the upper part of the body and is mainly applied to skin disease. Perspiration without harming the exterior and purgation without hurting the interior shows that it is not a severe prescription belonging to meditation therapy. It is mostly used for curing the disease of internal heat caused by over drinking and consuming heavy food, and it has special relationship with Yangmyung meridian. It is mentioned in the chapters of spirit, head, face, eye, ear, nose, throat, skin, hair, prescription, wind, dryness, fire, internal damage, epidemic infectious disease, carbuncle and cellulitis, ulceration, and pediatrics of ${\ulcorner}$Donguibogam${\lrcorner}$. It is usually applied to those who belong to Yangmyung type of the six meridian types or wind type, who has excessive heat, people with red complexion, reddened nose, pimples over the face and nose, coarse heel, loss of hair due to wind-heat, and to those who tend to have dandruff. Through examination over the cases treated with Hyungsan medicine, Bangpungtongseong-san was found efficacious in bloodshot eyes, brandy nose, loss of hair, various skin problems, tetanus, acute alcoholism, paralysis of hand and foot, deafness, and tinnitus.

Efficacy of BangPungTongSung-San Used in the Cases of Inflammatory Dermatitis (창양(瘡瘍)의 방풍통성산(防風通聖散) 치험예)

  • BangPungTongSung-San is usually perscribed in Hyungsang medicine for those of Yangmyung type, Pung type, those who have inner fever as well as redness of the tip of the nose, and for those with the external shape who tend to have dandruff. Following conclusions were obtained from the treatment of 8 patients suffering from inflammatory dermatitis. The causes of inflammatory dermatitis are mainly wind-fever, poisonous fever of the Samcho, dry-fever, and congenital conditions. The inflammatory dermatitis usually occurre to people who belong to Yangmyung types having vigorous Yangmyung meridian and body heat. Some patients who did not belong to the Yangmyung type were either those with much body heat or having congenital conditions inherited from their parents. There was no difference in the frequency of disease between men and women. The disease occurred at the times bodily conditions were weak or significant physiological changes took place. The term for the treatment required at least 3 months. Three more months are considered to be needed in treatment even after the completion of the remedy observing the return of the disease. The inflammatory dermatitis is easily occurred and recurred to those who belong to Yangmyung type and therefore the best prevention would be appropriate diet and bodily manner in daily life.

  • Objectives : Excessive nutrition intake and lack of physical activity induce obesity through accumulation of fat in human body. The obesity have been categorized as a disease and known to cause several chronic diseases and cancers. In Japan, Bangpungtongsung-san has been applied as one of the therapies of the obesity and it's pharmacological uses have been studied for a long time. The purpose of this study is reviewing researches of Bangpungtongsung-san related to the obesity. Methods : By searching with the keyword of "Bangpungtongsung-san" or related words such as "Bofutsushosan" and 'TJ-62" from PubMed and off-line libraries, many reports were gathered and reviewed from the perspective of the obesity. Results and Conclusions : It has become evident by reviewing the reports that Bangpungtongsung-san is useful as a therapy of the obesity though some case reports showed side effects. Also, the mechanisms of Bangpungtongsung-san on obesity were reviewed and suggested.

Functionality Analysis of Korean Medicine Fermented by Lactobacillus Strains (유산발효에 의한 발효한약의 기능분석)

  • Through the process of fermentation with Lactobacillus strains this study has evaluated the functionality of the traditional Korean medicine Bangpungtongsungsan after the addition of four other medicinal ingredients. In order to facilitate the growth of the Lactobacillus strains brown sugar was added to the herbal substances used. For both DPPH radical scavenging activities and SOD-like activities the medicinal mixture, when fermented through heterogeneous co-cultures, scored higher (at 77% and 42%, respectively) than when not fermented (at 31.7% and 36.3%, respectively). The co-cultured Korean medicine inhibited the growth of Bacillus subtilis PCI 219, Pseudomonas aeruginosa KCTC 2004, Staphylococcus aureus subsp. aureus KCTC 1916 and Propionibacterium acnes KCTC 3314. The inhibiting effects on ${\beta}$-hexosaminidase released from RBL-2H3 cells caused by the mixture, with and without fermentation, was seen to be similar (57% and 60%, respectively).
