A Study on the Exposure Dose of Frequent Workers and Radiation Workers in a University Hospital (일개 대학병원의 수시출입자와 방사선작업종사자의 피폭선량에 관한 고찰)

  • In this study, we tried to provide basic data for radiation safety management by comparing and analyzing the exposure doses of radiation workers and frequent workers at C University Hospital in Incheon. From January 2021 to December 2022, surface dose and deep dose were analyzed for 30 radiation workers and 8 frequent workers who worked at C university hospital in Incheon. Radiation workers were targeted at radiation technicians and nurses working in the radiation oncology department and nuclear medicine department, and frequent visitors were targeted at frequent workers who manage and clean facilities in the same radiation management area. In the radiation worker group, 3.1 per 10,000 radiation technologist, 1.2 per 100,000 nurses, and 4.5 per 1,000,000 frequent workers showed the possibility of developing side effects on the lungs. The probability of radiation oncology was 1.1 per 10,000 for radiation technologist and 5.2 per 1,000,000 for nurses, and the probability of radiation technologist in nuclear medicine was 2.9 per 10,000 and for nurses was 7.1 per 1,000,000. It is hoped that this study can be used as basic data in future revisions on frequent workers, and it is considered that it will be used as basic data in the field of obstacles in relation to the stochastic effect of radiation in the future.

의료용 선형가속기의 안전관리 기준 개발

  • 목적 : 방사선치료는 복잡하고 다양한 장비를 이용하여 고에너지 고선량의 방사선을 다루기 때문에 세심한 정도관리를 바탕으로 기술적 관점에서의 적절한 방사선치료장비 사용과 실제 환자치료시의 절차들에 대한 완벽한 지침서가 마련되어야 할 필요가 있다. 본 연구의 목적은 의료용 선형가속기에 연관된 방사선사고와 잠재적 사고유형들을 분석하여 환자와 종사자의 안전을 보장하고 사고를 예방키 위한 안전관리 기준을 개발하고자 하였다. 대상 및 방법 : 본 연구에서는 국내 13 개 방사선치료기관에 대한 안전관리 실태를 자체 개발한 100 여 문항의 설문으로 조사, 분석하였다. 또한, 국제적으로 사용하고 있는 안전관리기준들과 보고된 방사선 사고사례 들을 조사하였고, 방사선치료 전 과정을 통해 잠재적 사고유형을 추정함으로서 방사선치료의 안전관리를 위 해 필요한 요구사항들을 도출하고 이를 바탕으로 의료용 선형가속기에 관련된 안전관리 기준을 마련하였다. 결과 : 본 연구에서는 방사선치료 시 환자와 종사자의 안전을 보장하고 사고방지를 위하여 단계별 안전관리 기준을 개발하였으며, 방사선 치료과정에서 일어날 수 있는 다양한 실수나 사고들을 일목요연하게 관리 할 수 있는 흐름도를 구축하였다. 결론 : 본 연구 결과는 방사선치료에 사용되는 선형가속기에 대한 방사선의 안전과 품질보증을 개선하게 될 것으로 생각된다. 또한, 잠재적 방사선 사고의 예방뿐 아니라 국내의 방사선치료 안전관리 기준 마련에 기초자료로 활용될 수 있을 것이다.

Radiation Exposure on Radiation Workers of Nuclear Power Plants in Korea : 2009-2013 (국내 원전 종사자의 방사선량 : 2009-2013)

  • Although the perfomance indicators of the nuclear power plants in Korea show optimal, it requires detailed analysis and discussion centered on the radiation dose. As analysis methods, analysis on the radiation dose of nuclear power plants over the past five years was assessed by comparing the relevant radiation dose of radiation workers and per capita average annual radiation dose of the world's major nuclear power stations was also analyzed. The radiation workers over the annual radiation dose limit of 50 mSv were not. The contrast ratio of the radiation exposure according to the reactor type was the normal operation of PHWR was 6.2% higher than those of the PWR. This shows the radiation work of PHWR during normal driving operation is much more than those of PWR. According to the Performance Indicators of the World Association of Nuclear Operator, the annual radiation dose per unit in 2013 showed 527 man-mSv of Korea is the best country among the major nuclear power generating states, the world average was 725 man-mSv. The annual per capita radiation dose is about 80% less than 1 mSv of the public dose limit and also the average per capita dose showed a very low level as 0.82 mSv. Workers in related organizations showed 1.07 mSv, the non-destructive inspection agency workers showed 3.87 mSv. The remarkable results were due to radiation reduced program such as development of radiation shielding and radiation protection. In conclusion, the radiation exposured dose of nuclear power plants workers in Korea showed a trend which is ideally reduced. But more are expected to be difficul and the psychological insecurity against the operation of the nuclear power plants is existed to the residents near the nuclear power plants. So the radiation dose reduction policy and radiation dose follow up study of nuclear power plants will be continously excuted.

Medical Radiation Exposure Dose of Workers in the Private Study of the Job Function (의료기관 방사선 종사자의 직무별 개인피폭선량에 관한 연구)

  • Purpose: With increasing medical use of radiation and radioactive isotopes, there is a need to better manage the risk of radiation exposure. This study aims to grasp and analyze the individual radiation exposure situations of radiation-related workers in a medical facility by specific job, in order to instill awareness of radiation danger and to assist in safety and radiation exposure management for such workers. Materials and Methods: From January 1, 2010 December 31, 2010, medical practitioners working in the radiation is classified as a regular personal radiation dosimetry, and subsequently one year 540 people managed investigation department to target workers, dose sectional area, working period, identify the job function-related tasks for a deep dose, respectively, the annual average radiation dose were analyzed. Frequency analysis methods include ANOVA was performed. Results: Medical radiation workers in the department an annual radiation dose of Nuclear and 4.57 mSv a was highest, dose zone-specific distribution of nuclear medicine and in the 5.01~19.05 mSv in the high dose area distribution showed departmental radiation four of the annual radiation dose of Nuclear and 7.14 mSv showed the highest radiation dose. Conclusion: These results suggest that medical radiation workers working in Nuclear Medicine radiation safety management of the majority of the current were carried out in the effectiveness, depending on job characteristics has been found that many differences. However, this requires efforts to minimize radiation exposure, and systematic training for them and for reasonable radiation exposure management system is needed.

Radiation Exposure Dose on Persons Engaged in Radiation-related industries (방사선관계종사자의 피폭선량에 대한 연구)

  • From Jan 2002 to June 2011, we evaluated 4419 cases of radiation dose of 323 radiation related individuals consist of physician, nurses, technician and others in local C national university hospital. On annual analysis, year 2003 ranked the highest and 2007 the lowest dose. Dose was relatively higher in male than female. Dose was highest in 30s on age basis analysis. Dose was high in order of physician, nurse, and technician. Average radiation dose was high in order of cardiovascular center, radiologic intervention ceter, radiologist individuals, and fluoroscopic contrast study room. Those doses did not excess the standard dose recommended by ICRP (20mSv/year). However unlike average dose, there are wide variations of dose in individuals. Therefore radiation related workers should do one's best in personal radiation exposure dose management for achievement of minimum dose of radiation.

Occupational Radiation Exposure to Hospital Workers: On the Effect of Hematological Change (병원 내 방사선 작업종사자의 만성적 방사선 피폭에 의한 혈액학적 성분 변화에 관한 연구)

  • Purpose: This prospective study was conducted to reveal the haematological index change by low level radiation exposure in radiological environment our hospital workers. Materials and Method: We gathered the cumulative dose by Thermoluminenscent Dosimeters (TLD) over 9-yr period and examined hematological index counts change (RBC, Hb, Platelet, WBC, Monocyte, Lymphocyte, Neutrophilic, Basophilic, Eosinophilic) both occupational workers and controls. Of a total 370 occupational workers and 335 controls were compared. Results: This analysis has led to the following general observations 1) The average cumulative dose in male and female were $9.65{\pm}15.2\;mSv$, $4.82{\pm}5.55\;mSv$ respectively. 2) In both male and female, there were very low relationship between occupation period and cumulative dose (r< ${\pm}0.25$). 3) Occupation period was more increased, in male, WBC counts decreased and increased workers, RBC counts decreased workers were more than controls group (p<0.05). In female, WBC counts decreased and increased workers and W-eosino counts decreased workers were more than controls group (p<0.01). 4) Cumulative dose was more increased, in male, W-Lympho counts decreased workers and Platelet counts deceased workers were more than controls group (p<0.05). In female, W-lympho counts decreased workers and RBC counts decreased workers were more than controls group (p<0.05). Conclusions: We can find some kinds of blood index abnormal distribution in occupational radiation workers by comparing with controls. Occupational workers cannot avoid radiation exposure, in spite of the control it. Actually low level radiation adverse effect occurred not dose but probability. So workers must always try to reduce exposure by ourselves, furthermore as long as possible the government should provide rapidly that national system on radiation control for worker's health.

A Study on the Radiation Exposure Dose of Clinical Trainees in the Department of Radiology: A Case Study at C University Hospital (방사선(학)과 임상실습생의 수시출입자 피폭선량에 대한 고찰: C 대학병원 사례 연구)

  • In this study, radiation exposure doses were measured in the course of clinical practice of radiation workers, radiological technologists in the radiation-related worker group, and preliminary-radiological technologists who were classified as frequent visitors. Radiological technologists who worked in the radiation area of C University Hospital in Incheon for a year from January 2021 and 121 students who completed clinical practice at the same medical institution from July 1 to August 31 were the subjects of the study. The nominal risk factor based on ICRP 103 was used to evaluate the probability of side effects due to the exposure dose to the lungs, which are organs at risk of damage due to radiation exposure dose. During the clinical practice period, radiology students, who were classified as frequent visitors, had a surface dose of 0.98 ± 0.14 mSv and a deep dose of 0.93 ± 0.14 mSv. In other words, 6.7 per 1,000,000 for shallow dose and 6.4 per 1,000,000 for deep dose were found to have side effects due to exposure to the lungs. This is a value in terms of exposure dose in one year. Considering that the radiation (science) education course is 3 or 4 years, systematic management and attention to prospective radiation workers who are going to clinical practice are required, and the stochastic effect of radiation In relation to this, it is considered that it will be used as basic data for radiation safety management.

Analysis of the Awareness of Medical Radioactive Waste Management Plans (with Focus on Busan and Gyeongsangnam-do) (의료방사성폐기물 관리방안에 대한 인식 분석 - 부산, 경남을 중심으로 -)

  • This study was conducted to propose medical radioactive waste management methods by background factors of radioactive workers, their awareness of safety management and reduce the difficulty of self-disposal. A population of 102 radiotechnologist who work at hospital in Busan was the subject of this study and a survey was conducted to them. the analysis for the collected data used SPSS/PC+Win13 version and one-way, ANOVA was carried out of verify differences between the groups. The result showed that most of workers had correct awareness of radioactive waste management. Also, about the difficulty of self-disposal, legal procedures were mentioned most often, and as efficient improvement of management methods is concerned, changing the awareness of safety management and disposal was proposed. According to this study, the right way of managing medical radioactive waste is to change the awareness of radioactive workers by reinforcing regular training.

기획 - 원자력 안전규제

  • 우리나라가 급속한 성장을 이루는 과정에서 원자력은 이를 뒷받침하며 함께 성장해 왔다. 그러나 그 과정에서 안전보다 성과를 강조하다보니 안전을 경시하는 비정상적 환경이 조성되었다. 그 결과 숨겨져 있던 과거의 원전 비리가 수면 위에 드러났고, 같은 시기에 후쿠시마 사고로 인한 오염수 유출과 전력난까지 발생하면서 국민의 불안과 불신을 가중시켰다. 국민들은 정부가 불안감을 해소해 주고 원칙을 지키는 안전규제, 신뢰할 수 있는 안전규제를 마련할 것을 요구하고 있다. 원자력 안전규제의 독립성을 확보하기 위해 지난 2011년 설립된 원자력안전위원회는 출범 이후 새로운 안전규제 제도 시스템을 마련하는 데 총력을 기울여 왔다. 원전 비리 같은 비정상 관행을 근절하기 위해 공급자 검사 제도, 성능검증기관 관리 제도 등 새로운 규제 시스템이 마련되었고, 방사선 작업 종사자 보호 강화와 생활 주변 방사선 감시 효율성 제고를 위한 다양한 제도적 장치들도 구비되었다. 이외에도 원자력안전규제기금 설치, 원전 부지별로 설치된 원자력안전협의회 실효성 제고 등 소통과 협업을 위한 시스템을 정착시켰다. 그러나 단순히 제도와 시스템이 만들어졌다고 해서 원자력 안전이 저절로 확보되는 것은 아니다. 차고 다닐 옥의 종류가 바뀌면 걸음걸이도 바꾸어야 한다는 개옥개행(改玉改行)의 고사성어에서처럼 제도가 바뀌면 그에 따른 일하는 방식 문화가 달라져야만 변경된 제도 시스템이 완전히 정착될 수 있을 것이다. 이를 위해서는 규제기관의 노력도 중요하지만 무엇보다 필요한 것은 원전 및 방사선 관련 사업자, 이해 관계자 나아가 일반 국민들이 이러한 달라진 제도 시스템을 이해하고 이에 적극 협조해 나가는 것이다. 본 기획 기사는 그러한 취지에서 그간 원자력안전위원회가 안전규제를 공고히 하기 위해 추진해 왔던 주요 규제 정책, 제도들을 소개하는 데 주안점을 두었으며, 이를 통해 원전 및 방사선 관련 사업자와 일반 국민들의 이해를 바탕으로 새로운 제도 시스템이 조기에 성공적으로 정착될 수 있기를 희망한다.
