The study on waiting time has been conducted from the psychological perspective since Maister (1985). In using medical institutions, especially general hospitals, it is not avoidable to wait for a long time. As a result, the waiting time gives clients psychological stress, which causes medical consumers to be more dissatisfied and decreases their rate of revisit. Accordingly, it is very urgent to study on the waiting time for hospitals' marketing and better image-making. This study is intended to find out how hospitals image and clients revisit is influenced by waiting environment and consumers' attitude, variables of waiting time perceived in medical services. Based on this study, those medical institutions are required to improve their medical service and waiting environment. Accordingly, they should convert waiting time into more efficient and comfortable recess and provide better environment for physical service and effective human services. As those medical institutions have relatively worse image than other businesses, they should actively study on ways of better image-making in the future.

진료 및 간호서비스는 다른 제조업과는 달리 무형의 서비스에 대한 간접비의 비율이 두드러지는 특성을 지니며, 이러한 특성으로 인해 의료기관의 전략적 계획수립, 수익성 분석, 경제성 분석, 의료서비스 제공의 질 관리 등의 의사결정에 적극 활용되어야 하는 원가 분석에 어려움을 겪어왔다. 전문 간호 분야인 가정간호는 1회 방문 시에 다양한 활동들이 모여서 가정간호서비스가 수행되므로 가정간호의 원가분석에는 각 행위들이 모인 활동에 대한 분석에 근거한 정확한 원가 정보가 요구되고 있다. 활동기준원가의 기본 요소는 자원, 활동, 자원 동인, 활동 동인, 원가 대상이며 이에 따라 가정간호사업에서 활동기준원가의 각 요소를 정의하고 전통적 원가 계산이 아닌 활동을 중심으로 가정간호사업의 업무 알고리즘을 분석하여 활동기준원가관리시스템을 설계한다. 가정간호사업의 활동기준원가관리시스템은 가정간호의 효율적 운영과 의료기관의 경영 개선과 항께 실질적 원가계산을 통해 정부가 건강 보험 수가를 결정하고, 보험 수가의 적절성을 평가하는데 중요한 기초 자료를 제공하게 될 것으로 기대한다.

의료시스템은 모든 것이 병원 네트워크를 중심으로 의료 서비스가 제공되어 진다. 하지만 자연재해, 천재지변과 같은 갑작스런 환자 발생으로 인한 응급 이동 사항, 혹은 진료자의 외진 및 환자 방문시에는 일반 데스크탑 PC에서 이루어지는 상태의 의료 서비스가 아닌 휴대하기 간편하고 언제 어디서나 의료서비스를 받을 수 있는 Personal Digital Assistant (PDA)와 같은 휴대용 기기에서의 의료시스템이 필요하다. PDA는 휴대가 용이하며 유선 네트워크 시스템에 구애받지 않는 무선 모바일 장비로서 무선 통신이 가능한 곳이라면 언제 어디서라도 환자에 대한 의료 정보를 조회할 수 있다. 또한 PDA의 통신 기술로서 병원과의 실시간 통신이 가능하여 응급 환자에 대해 빠른 진료를 제공 할 수 있다. 이와 더불어 임베디드기기 프로세서의 발달로 인하여 연산처리 기술도 많은 향상을 가져왔고 이로 인하여 기존의 PDA에서의 문제였던 DICOM영상의 조회 및 처리속도에도 많은 향상을 가져왔다. 이에 본 연구에서는 의료영상 표준인 DICOM표준을 따르며 이동성이라는 장점을 통하여 언제 어디서나 손쉽게 진료자가 환자의 의료정보 및 의료 영상을 조회할 수 있도록 하는 PDA용 모바일 의료영상 조회 시스템을 PXA255 기반의 Embedded Linux 상에서 QT/C++을 사용하여 구현하였다.

This study was performed to examine the want for home-visit health care of health center and health sub-center in rural olders and to provide the basic data to develop strategies for efficient and effective home-visit health care delivery of public health facilities. The questionnaire survey by interview was conducted to 355 olders whose ages were all over 65 years, residing at a rural community, Myun, Gyeongsangbuk- do. Among study population, 64.5% replied that their self-rated health status were 'poor', 14.1% had low ADL and 14.9% had low IADL. Among study population, 73.5% replied that they had health problem which were in need of medical personnel's care. The existence of health problem were significantly different according to sex, age, marital status, health security status, occupation, economic status, circumstances for medical care, self-rated health status, ADL, and IADL(p<0.05). Among olders with health problem which were in need of medical personnel's care, 19.5% wanted to receive the home-visit health care. The degree of want for home-visit health care was higher significantly in olders whose ages were 75-year old or more(p<0.05), jobless olders(p<0.01), the aged persons who were not in harmony with other family members, olders whose self-rated health status were 'poor' and olders with low IADL. The major reasons why they wanted to receive the home-visit health care services were 'they had no helpers when they were sick' (64.7%), 'long distance to the medical facilities from their residence'(23.5%). The medication service was the most need service among home-visit health care services. The reasons why they didn't want to receive the home-visit health care services were 'we could walk and move' (60.0%), 'we wanted to have a direct contact with doctor' (25.7%) in the order of high rate. In multiple logistic regression analysis, the degree of want for home-visit health care were higher significantly in olders who were not in harmony with other family members and olders whose self-rated health status were 'poor'(p<0.05).

Objectives : The purpose of this study was to analyze the effect of outpatient cost sharing reduction on health care utilization of children under 6 years old. Methods : The data in this analysis was the health insurance claims data between August 2006 and July 2008. The study group was divided into two age groups, namely 0-5 years old and 6-10 years old. This study evaluated the impact of policy change on office visits per person and expenditure per visit. In order to do so, the double difference analysis is used. Results: The results showed that outpatient cost-sharing reduction has never really had a huge impact on office visits per person and expenditure per visit. Conclusions: This study showed that the outpatient cost sharing reduction for children under 6 years old policy is not working. Therefore, cost sharing of National Health Insurance by income class is needed.

The purpose of this study was to forecast which factors provided satisfaction for the foreign medical tourist. The study concluded that the following key factors most affected the satisfaction of medical tourism: total travel budget, number of visits for medical services, occupation, education, monthly household income, budget for medical expenses, and the visiting period. Overall satisfaction of medical tourism explained was 43.5%. The influence of satisfaction on medical tourism through a word-of-mouth and the revisit intention was significant. These conclusions suggest that overall satisfaction with medical tourism based on this study's key factors may be an important factor for increasing the word-of-mouth and the revisit intention by the medical tourist in the future.

Effects on Health Center and Hospital Visit Patient Characteristics and Overall Satisfaction As a result of comparing the characteristics of visiting patients in the public health centers and hospitals, the number of patients at the public health centers was relatively higher than the hospital patients, the number of medical benefits patients was higher, and there were more vaccinations or physiotherapy patients than the purpose of disease treatment.There was a small number of private insurances,The older adults were more. The most influential variable on the overall satisfaction level of the public health center is the facility satisfaction And doctor satisfaction had a weak effect.In the case of hospitals, the facility satisfaction Also, the degree of satisfaction with doctors, satisfaction with medical expenses, and satisfaction with treatment environment were in order.

Medical images are an effective means to identity medical abnormalities.. Patients typically have medical images taken at different clinics during lifetime, and they often wish to have second interpretation on medical images showing substantial diseases. At present, since personal medical images are distributed to multiple clinics, there is a bit discomfort that patients directly bring their images by hands to get the second interpretation from another physician. With these two motivations, we design a cloud service for archiving medical images and interpreting medical images by physicians. We present the design and implementation of the service, and show its practical value as low-cost personal healthcare service. By using the service, patients can retrieve and review their medical images anytime and have a convenience of acquiring second opinions on their medical images at low-cost without visiting a clinic.

Although the patient's problem with access to health information has been improved due to rapidly developing information technologies, such as the internet, some patients still do not have enough ability to understand, interpret, and analyze the health information. Given this view on the patient's asymmetric information problem, if a doctor provides sufficient effort to help patients understand and interpret medical information, the efficiency of patient's medical care use could be improved. This paper shows firstly that the patient's inefficient use of medical care originates from his information problems, such as the misperception of the effectiveness of medical care and secondly suggests that if the doctor makes sufficient effort to correct patient's information problems, the inefficiency can be ameliorated. This paper also suggests the manipulation of a doctor's payment method can lead a doctor to provide optimal level of efforts which can in turn lead patients to use the optimal level of medical care. With an optimal level of effort, a doctor can more easily achieve a patient's compliance with the newly recommended amount of medical care.
