Care Needs of Hospice Patients in Comparison with Those from the Family Caregivers' Perspective: Q methodology (호스피스 환자의 돌봄 요구와 가족이 인지하는 환자의 돌봄 요구 비교: Q 방법론)

  • Purpose: The purpose of tile study was to identify types of care needs of hospice patients and those from the family caregivers' perspective and to compare these two groups in reporting patients' care needs through Q-methodology. Methods: Twenty three Q-statements concerning care needs were selected through in-depth interviews of hospice patients. Data were collected from 20 hospice patients as well as 20 family caregivers respectively by sorting 23 Q-statements into 9 points standard. Data analysis was performed by using PC QUANL program. Results: Principal component analysis identified four types of care needs of the hospice patients. Overall, the accuracy of family caregiver reports was 48% in all types of care needs. Type 1 was named 'physical care needs type' for those whose greatest need was physical care to be free of pain and comfortable. The accuracy in Type 1 was 62.5%. Type 2 was named 'emotional care needs type' for those who would like to share love and intimacy with their family members. The accuracy in Type 2 was 20%. Type 3 was named 'spiritual care needs type' for those who would like to receive forgiveness from their God and prayers and visitation of clergy. The accuracy in Type 3 was 60%. Type 4 was named 'social care needs type' for those who would like to complete their ongoing work and to give service to others. The accuracy in Type 4 was 50%. Conclusion: There was a great difference between hospice patients and the family caregivers in reporting patients' care needs. Thus, hospice nurses need to educate family caregivers to more accurately assess patients' care needs.

A Study on the Christian Learning Environmental Arrangement in Church Educational Space (교회 교육공간에 관한 기독교교육적 환경구성)

  • As an element of education, the educational space cannot be separated from the purpose of education. The place of education is not only the passage to deliver actual curriculum, but also the purpose itself that can be accomplished through educational content. That is because the purpose of education cannot be achieved only with instructors, students, curriculum, and methods, but requires a change in the perception of the educational space that represents the goal and the place where it all can be implemented. Nevertheless, the problem that lies with educational space is easy to be overlooked and it has been rather considered as an issue related to the finances or scale of the church. The church educational space gives birth to faith and growth, where spiritual development and experience may occur. However, the reality follows the drawbacks of conventional school classroom arrangements and structures. In addition, even if the church educational space can be arranged according to the needs of its students, it cannot deviate much from the standard uniform format. In particular, the basic environment of church educational space is similar to that of standard school system in terms of arrangement of furniture such as chairs, desks, and its physical structure. As the school system was originally designed and tailored for the purpose of delivering knowledge and standardization, the space for church education must stay away from it. Humans are born and die in a space, where encounter with God also happens. Also, communication with God causes spacial conversion to humans, changing the place of their visitation. So the church educational space must be more meticulously designed and comprehensive than that of school which pursues physical, educational, psychological, social, and artistic purposes because the church educational space pursues the liturgical elements, as well. Therefore, the Christian learning environmental arrangements must seek liturgical elements, which is the major Christian value, by placing Christian artwork or symbols for church visitors. So in this research, I want to stress the role of Christian educational space for spiritual growth and pursue intrinsic and extrinsic changes in learning environment, leading to a greater awareness of the Christian educational space.

Evaluation of the Usefulness of a Wireless Signal Device for the Use of Contrast Agent in Computed Tomography (전산화단층촬영에서 조영제 주입에 따른 무선신호기 사용의 유용성평가)

  • The psychological anxiety of radiologists, as well as the patients, is growing with the increasing use of CT contrast agent side effects and the process of extravasation. In this study, a satisfaction survey was conducted regarding the wireless signal device after CT examination in patients and radiologists by employing a wireless signal device during a contrast-enhanced CT examination in order to determine its usefulness to the relieve psychological anxiety, such as anxiety and fear, of patients and radiologists when using contrast agents. The use of a wireless signal device was also intended to help radiologists in dealing with the side effects of contrast agents that may occur during a CT examination and preventing extravasation. Patients aged 20 years or older, who visited the C university hospital in Jeonnam province for 4 months from August to November in 2017, were surveyed. A total number of 90 patients (57 males and 33 females),who agreed to the study after CT examination, were included in the questionnaire survey. Meanwhile, 15 radiologists, who were working at a CT room and had an experience in using a wireless signal device, were surveyed. Patient satisfaction was $6.01{\pm}0.88$ before the use of a wireless signal device and $8.20{\pm}1.06$ after use, thereby showing an increased satisfaction after its use. Radiologist satisfaction was $8.46{\pm}1.06$ after use, thereby not showing a big difference from the mean patient satisfaction. The satisfaction was high at over 8 points in both groups. The contribution to psychological stability with the use of a wireless signal device was $8.98{\pm}0.65$ in patients with prior experience of side effects and $8.00{\pm}1.21$ in patients without prior experience of side effects. In conclusion, it is considered to improve satisfaction with the examination by helping the radiologists in taking immediate action with calling via the wireless signal device and providing the patients and radiologists with psychological stability by reducing their anxiety.

Comparison of Attitude Toward Antidepressants Between Medical and Surgical Group Outpatients in a Korean University Hospital (일 대학병원 내과계열 및 외과계열 외래 환자들의 항우울제에 대한 태도 및 인식에 대한 연구)

  • Objectives : Antidepressants are prescribed by not only psychiatrists but also general practitioners. However, patients' negative attitude and stigma against antidepressants are major obstacles to prescribe antidepressants in nonpsychiatric outpatient departments. We, therefore, examined patients' attitude toward antidepressant in medical and surgical group outpatient in Korea. Methods : Short item questionnaire was applied to medical group and surgical group outpatients who consented to this survey in a Korean university hospital. 100 medical and 100 surgical outpatients were surveyed. The questionnaire contained sociodemographic information, previous recognition of antidepressant, and general attitude toward attidepressants. Results : Of the 212 recipients, exclusive of erroneous and missing ones, we analyzed 200 recipients. 1) We found negative attitude toward antidepressants among both medical and surgical outpatients. 2) Medical group outpatients are answered more' antidepressants cause physically ill. 3) The persons who didn't take previously antidepressants showed more negative attitudes. 4) The most reason why they refuse to take antidepressants is concerns of side effects of antidepressants. Conclusion : There are highly negative attitude toward antidepressants between medical and surgical group outpatients. Study findings suggest the need for an active education on antidepressants to nonpsychiatric outpatients to maximize their therapeutic effect and compliance.

Clinical characteristics of Kikuchi disease in children (소아 Kikuchi 병의 임상 양상)

  • Purpose : Kikuchi disease is a subacute necrotizing lymphadenitis characterized by fever, cervical lymphadenopathy and benign self-limiting course in young women especially, but rare in children. Their etiopathology is still unknown, but involved viruses and autoimmune mechanism were proposed. We investigated the clinical and laboratory characteristics of Kikuchi disease in children. Methods : Five patients were diagnosed of Kikuchi disease from January 2001 to June 2006 in Kyunghee University Hospital. We reviewed their medical records retrospectively and analyzed clinical and laboratory findings. Results : The mean age of 5 patients (male to female ratio; 2:3) was 9 yr 9 mon(range: 8 yr 2 mon-12 yr 6 mon). The chief complaints were sustained fever and cervical lymph node that was unilateral, tender and swollen. All patients were treated with antibiotics before diagnosis was made, and rash developed in 2 patients. One patient showed necrotic change in ultrasound with which we suspected Kikuchi disease. Three patients were examined by CT scan additionally. The interval between admission and cervical lymph node excisional biopsy ranged from 6 days to 15 days, and mean period was 9.6 days. All pathological findings were compatible to necrotizing lymphandenitis. Conclusion : Unnecessary laboratory and imaging studies, and treatment with antibiotics tend to be done before making diagnosis, and that caused prolonged hospitalization. Therefore we studied the clinical and laboratory characteristics of Kikuchi disease in children.

A Survey of Doctors' Awareness of Simultaneous Vaccination (예방접종 동시접종에 대한 접종의사의 인식 조사)

  • Purpose: In this study, doctors were surveyed with a questionnaire to determine whether they performed simultaneous vaccination and whether there were any concerns about safety or anxiety. The purpose of this study was to determine any problems associated with doctors readily performing simultaneous vaccination. Methods: A trained surveyor visited 241 doctors from every institution registered with the National Immunization Program (NIP) located within six districts (gu) in the City of Busan (Dongnae-gu, Geumjeong-gu, Yeonje-gu, Suyeong-gu, Busanjin-gu, Haeundae-gu); a total of 155 (64%) valid responses were obtained. Results: Of the 155 respondents, 144 (93%) were already performing simultaneous immunizations and 141 (91%) had a positive view of the practice. However, among the 144 doctors performing simultaneous immunizations, 67 (47%) were not confident about its safety; side effects were seen after simultaneous immunization by 86 doctors, 35 (41%) of whom believed that the frequency or possibility of side effects in simultaneous immunizations was higher than that in sequential immunizations. Conclusions: The use of simultaneous immunization is expanding quickly. However, among the doctors performing simultaneous immunizations, a high percentage had concerns over its unproven safety and potential side effects, indicating the need for academic societies or government institutions to present evidence to address such concerns.

The Influence of Shop Crowdedness in Dining Franchise Liquor Shops upon the Perceived Values and Intent to Revisit (외식 프랜차이즈 주류 매장의 점포 혼잡성이 이용자의 지각된 가치와 재방문의도에 미치는 영향)

  • The purpose of this study is to analyze the influence of the crowdedness of dining franchise liquor shops on the perceived values and revisit intention. For this purpose, 361 customers who had visited dining franchise liquor shops located in Seoul and Gyeonggi-do areas were surveyed, and data was analyzed using AMOS 23.0 and SPSS 23.0 statistics programs. The analyses yielded the following findings. First, a higher level of crowdedness in the shop turned out to lower the perceived value of users. Second, a higher level of perceived value resulted in a higher level of revisit intent. Third, the analysis on the mediating effect of the perceived value showed that, in the relationship between the human crowdedness and revisit intention, both practical and hedonic values had mediating effects. However, in the relationship between the spatial crowdedness and the intent of revisit, the practical value had a mediating effect, while hedonic value did not exhibit such an effect. This study is meaningful in that it is presented as a theoretical establishment and an empirical study on dining franchise liquor shops.

Communal Ontology of Landmarks for Urban Regional Navigation (도시 지역 이동을 위한 랜드마크의 공유 온톨로지 연구)

  • Due to the growing popularity of mobile information technology, more people, especially in the general public, have access to computerized geospatial information systems for wayfinding tasks or urban navigation. One of the problems with the current services is that, whether the users are exploring or navigating, whether they are travelers who are totally new to a region or long-term residents who have a fair amount of regional knowledge, the same method is applied and the direction are given in the same way. However, spatial knowledge for a given urban region expands in proportion to residency. Urban navigation is highly dependent on cognitive mental images, which is developed through spatial experience and social communication. Thus, the wayfinding service for a regional community can be highly supported, using well-known regional places. This research is to develop the framework for urban navigation within a regional community. The concept of communal ontology is proposed to aid in urban regional navigation. The experimental work was implemented with case study to collect regional landmarks, develop the ontological model and represent it with formal structure. The final product of this study will provide the geographical information of a region to the other agent and be the fundamental information structure for cognitive urban regional navigation.

  • Green roofs are being created in various purposes to use spaces. Most of them have been built with a goal as a resting place. However, the use of the green roofs needs to be raised. Otherwise, maintenance and administration of green roofs can be loosened. In addition, budget or cost involved can be wasteful. Therefore, this study makes a proposition if the satisfaction of green roofs is higher, the use of green roofs could be increased more. We are willing to analyze how beneficial the awareness can affect for the satisfaction with regard to green roofs. The findings can be helpful to provide a foundation data for raising the use of green roofs. We believe that the awareness benefits and satisfaction could be different depending on the facility's status on green roofs. Therefore, we classified green roofs of 11 department stores, in Seoul, into four types. Among them, we paid attention to five department stores that were able to represent each type. We analyzed the awareness benefits and satisfaction depending on the type of green roofs. We analyzed casual relationships between them using Structural Equation Modeling. The Effect of Servicescape of an Eco-friendly Restaurant on Customer Perceived Value, Attitude and Behavior Intention (친환경 레스토랑의 서비스스케이프가 소비자의 지각된 가치, 태도 및 행동의도에 미치는 영향)

  • The purpose of this study is to secure the basic data for the physical environment of an eco-friendly restaurant by surveying and analyzing customer perceptions of the servicescape of an eco-friendly restaurant and to verify the organic causation of the servicescape of an eco-friendly restaurant and customer perceived value, attitude and behavior intention. The samples for empirical analysis were selected from the customers over 20 years who lives in Seoul and Kyung-gi suburbs have experienced visiting eco-friendly restaurant or green restaurant more than once a month. Total 300 copies of questionnaire were distributed for the survey from the second day to the fifteenth day of April for 14 days, and total 264 (88.0%) copies of survey questionnaire except for some questionnaires that had much strong lean tendency or the missing value was discovered. The research results are as follows; when a customer recognizes an eco-friendly restaurant favorably, he or she considers that servicescape plays an important role in deciding perceived value through tangible and intangible perceived values. Since, customer perceived value has a positive effect on attitude and behavior intention, the customer considers as affected on his/her behavior intention when satisfied with the eco-friendly restaurant, which is considered that positive customer attitude will have an effect on behavior intention. Thus, it is considered based on this in-depth analysis result that maintaining and providing servicescape of high standard in the manager's perspective will have a direct effect on ensuring tangible outcomes.
