Effect of Radiation Therapy on Voice Parameters in Early Layngeal Cancer and Normal Larynx (방사선요법이 초기 후두암 및 정상후두의 음성지표에 미치는 영향)

  • 초기후두암에 대한 방사선요법은 수술적 치료에 비하여 정상 후두기능 즉 발성 기능을 최대 한 보존할 수 있는 장점이 있다. 그러나 방사선 치료법은 병적 혹은 정상 후두조직에 섬유화, 점막부종 혹은 점액선 간소등의 변영을 초래할 수 있기 때문에 어느 정도의 음성장애를 유발하게 된다. 방사선요법이 후두의 발성기능에 미치는 영향을 알아보기 위하여 초기후두암 환자와 두경부암으로 인하여 정상후두에 방사선 조사를 받았던 환자 및 정상대조군 각 20명에 대하여 음향 및 공기역학적 음성검사를 시행하여 음성장애의 특성을 비교, 검토하였다. 초기 후두암 환자들에게 사는 방사선 치료 후 음성지표들이 관찰되었고, 정상후두에는 큰 영향을 미치지 않았다.   • Voice is a physical phenomenon, generated by vocal fold and expiratory airflow. Dysphonia should come from abnormal vocal fold and airflow. Occassionally larynx looks normal in show, but it is actually not. There should be undetected structural or functional abnormalities. So when ENT doctors face dysphonia patients who looks normal larynx, should make a diagnosis through close observation. In this review article we present some dysphonia diseases which looks normal larynx. For example vocal fatigue, vocal fold paresis, posterior glottic diastasis, muscle tension dysphonia and psychogenic dysphonia.

Intelligibility and Aerodynamic Study of Tracheopharyngeal and Tracheoesophageal Speechs (기관인두발성과 기관식도발성에 대한 이해도 및 공기역학적 검사)

  • Selected characteristics were compared in the speech of three tracheoesophageal, five tracheopharyngeal and ten normal laryngeal adult speakers. Tracheoesophageal speakers use Blom-Singer voice prosthesis after total laryngectomy and tracheopharyngeal speakers use tracheopharyngeal myomucosal shunt after near-total laryngectomy. Intelligibility judgement was based on standard Korean monosyllabic and bisyllabic word lists of 50 items. Aerodynamic study was composed of maximum phonation time, phonaton quotient. phonation pressure and mean air flow rate. Results indicate that intelligibility of tracheopharygeal speech is more similar to normal laryngeal speech than tracheoesophageal speech using Blom-Singer voice prosthesis.

The Vocalization for Korean Traditional Song "Pansori" (국악(판소리) 발성법)

  • All singers can often develop voice trouble secondary to vocal abuse and overuse, but it is well known that traditional Korean singer (pansori) develop voice disorders more frequently than western style sunger. While laryngological concern for voice disorders arising in professional singers has received some attention, empirically motivated investigations of the underlying acoustic features of the singing voice have been relatively limited. Since all singers have a good knowledge of the voice and voice training, they would hardly give consent for treatment to a doctor unless he understood their desire to maximize their voice quality. The components of this report are composed of breathing, basic ekement, and vocalization, essencial fact, for getting a perfect voice for pansori. The breathing is based on hypogastric breathing. The main functions of breathing are energy and power of utterence, tempo of rhythm and seperating paragraph and controlling feelings according to dramatic situation. Vocalization is based on general vocalization. Main uses of it are maintaining singer's tone and harmony of cosmetic dual force.

  • Post-thyroidectomy dysphonia occurs very frequently. Causes of dysphonia include recurrent laryngeal nerve (RLN) or external branch of superior laryngeal nerve (EBSL) injury, intubation, laryngotracheal fixation and other causes. Patients commonly characterized hoarseness or their voice impairment inability to short or sing and loss of loudness. Although complete recovery can be expected mostly in 3 months without obvious nerve injury, dysphonia lowers patients quality of life. The present review discuss the very kinds of causes of post-thyroidectomy dysphonia.

  • Spasmodic dysphonia is a focal dystonia of the larynx and breathy voice is a typical sign of abductor spasmodic dysphonia. A group of patients with abductor spasmodic dysphonia have a number of acoustic characteristics including abnormal fundamental frequency fluctuations and abnormally long word duration. We report a abductor spasmodic dysphonia case have enlongated voice onset time voiceless consonants and breathy voice in wide band spectrogram. The patient have the acoustic characteristics only in telephone speaking at work time. We treated the patient with anticholinergic and anticonvulsant drug and supplementary voice therapy. The breathy voice and enlongated VOT were disappeared after those treatment.

  • Yuri Vasilyev - actor, director and drama teacher. Russian meritorious artist, winner of the stage "Medal of Friendship" awarded by Russian President Vladimir Putin; academician of the Petrovsky Academy of Sciences and Arts in Russia, professor of the Russian National Academy of Performing Arts, and professor of the Bavarian Academy of Drama in Munich, Germany. The physiological sense stimulation method based on the improvement of voice, language and motor function of drama actors. On the basis of a systematic understanding of performing arts, Yuri Vasiliev created a unique training method of speech expression and skills. From the complicated art training, we find out the most critical skills for focused training, which we call basic skills training. Throughout the whole training process, Professor Yuri made a clear request for the actor's lines: "action! This is the basis of actors' creation. So action is the key! Action and voice are closely linked. Actor's voice is human voice, human life, human feeling, human experience and disaster. It is also the foundation of creation that actors acquire their own voice. What we are engaged in is pronunciation, breathing, tone and intonation, speed and rhythm, expressiveness, sincerity, stage voice and movement, gesture, all of which are used to train the voice of actors according to the standard of drama. In short, Professor Yuri's training course is not only the training of stage performance and skills, but also contains a rich view of drama and performance. I think, in addition to learning from the means and methods of training, it is more important for us to understand the starting point and training objectives of Professor Yuri's use of these exercises.

Effects of Laryngeal Massage on Muscle Tension Dysphonia: A Systematic Review and Meta-Analysis (근긴장성 발성장애의 후두마사지 효과: 체계적 고찰 및 메타분석)

  • Background and Objectives This study was to investigate the voice quality and articulation effects of laryngeal massage on muscle tension dysphonia (MTD). Materials and Method A systematic review of articles published between January 2000 and December 2020 in Cochrane, PubMed, ScienceDirect, SpingerLink, ERIC, and Naver Academic was conducted. From the total of 2094 articles identified, 10 peer-reviewed articles were included in a meta-analysis. Mean effect sizes of the variables related to voice quality (jitter, shimmer, harmonic to noise ratio or noise to harmonic ratio, high-F0, low-I, cepstral peak prominence) and articulation (F1, F2, F1 slope, F2 slope) were calculated by Hedges'g. Results Meta-analysis of the selected articles showed that laryngeal massage had medium to large effects on all variables of voice quality and articulation except F0-high and F1 slope in the MTD patients. Conclusion This study provided comprehensive clinical evidence that it is highly desirable to apply laryngeal massage to MTD patients.

Pulmonary Functionn and the Maximal Inspiratory and Expiratory Pressure, and Maximum Phonation Time Before and After the Specially Programmed Training (호흡훈련보조기구를 이용한 호흡훈련 전 후의 폐기능 호흡근력과 최대발성지속시간의 변화)

  • Whether respiratory muscle training is of benefit to the singing students is controversial. The purpose of the study is to investigate pulmonary function and the maximal inspiratory(MIP) and expiratory pressure(MET), and maximum phonation time in five female singing students before and after the specially programmed respiratory muscle training during 2 months. All singing students had average 4.8 years of formal classical voice training. Respiratory muscle training machine (Ultrabreath) was used to train respiratory muscle. Pulmonary function test data on simple pulmonary function, flow volume curve, static lung volumes are obtained from Vmax 6200. The MIP and MEP were measured using Spirovis, and the MPT were measured using hand-held stopwatch. Any pulmonary function test variables are not changed after respiratory muscle training. However, MIP and MEP were significantly increased between before and after respiratory muscle training. MPT increased significantly after training, compared to the pre-trained. MIP, MEP, and MPT after training in female singing students were 26%, 25% and 33% higher than those before training. The result indicated that the specially programmed respiratory muscle training is beneficial to improve respiratory muscle strength and vocal function without an increment in pulmonary function.
