Development of a Premature Infant Pain Scale (PIPS) (미숙아 통증지표(Premature Infant Pain Scale) 개발)

  • Purpose: To develop and validate a scale suitable and efficient scale for use in clinical practice as to assess pain in premature infants. Method: Pain indicators identified by observation of preform infants. A cohort of preform infants was studied prospectively to determine the construct validity, inter-rater reliability, and internal consistency of the scale. The PIPS uses four indicators of pain: corrected gestational age, heart rate, oxygen saturation, behavioral state. The validation study included 45 premature infants with gestational age of 37 weeks or less. Results: The inter-rater reliability of the PIPS was acceptable, with Pearson correlations ranging from.720 to.970. Internal consistency was high: Cronbach's alpha coefficients ranged from.551 to.653. There was a strong correlation between the PIPS and PIPP scores (each researcher's r=.743, each indicator's r=.914). Although gestational age showed no association between these factors and the sum, the other variables were positively associated with the sum. Time needed to calculate PIPS scores is was less than Premature Infant Pain Profile (PIPP) scores(p<.000). Conclusion: The validation data suggest that the PIPS is appropriate and efficient for assessing pain in premature infants. Further studies are required about to determine appropriate interventions for each pain score on the PIPS.

Pyloric Stenosis in Premature Infants -Report of two cases- (미숙아에서 발생한 유문협착증 2례)

  • The incidence of Hypertrophic pyloric stenosis (HPS) in premature infants is rare, the presentation is not typical, and the diagnosis delayed due to uncertain diagnostic criteria in abdominal ultrasonography (US). We report two premature infants with HPS diagnosed by US and upper gastrointestinal (UGI) contrast study. Patient 1. A premature female infant (birth weight 1950 gm at 34 week's gestation) with the onset of intermittent vomiting at 9 days of age was evaluated. US was normal at 13 days of life, however, abnormal at 41 days of life (pyloric muscle length 16.5 mm). Patient 2. A premature male infant (birth weight 1470 gm at 29 week's gestation) with the onset of intermittent vomiting at 10 days of age was evaluated. US showed pylorospasm at 11 days of life, however, findings compatible with HPS at 57 days of life (pyloric muscle thickness 11 mm). UGI contrast study at 48 days of life showed similar findings in both cases. Both patients had undergone pyloromyotomy. In conclusion, the diagnosis of HPS in premature infants requires careful follow-up by US and UGI contrast study.

Effect of Edible Coatings on Softening of the Stored Immature Plums (식용코팅이 미숙 자두의 연화에 미치는 효과)

  • Immature plums(Prunus salicina L.) were coated with edible coatings, calcium-added methyl cellulose(CaMC) and hydroxy propyl methyl cellulose-15(CaHPMC-15). Weight loss and respiration rate of immature plums coated with CaMC and CaHPMC-15 were lower only at the earlier storage period. The titratable acidity of plums was decreased and pH was increased during storage, Soluble pectin, pectinesterase, and polygalacturonase activity as the marker of softening were commonly increased with storage time, but those increases were lower in CaMC and CaHPMC-15-coated plums. Mechanical firmness became significantly soft during storage, while the coated plums were remained harder. As a result of sensory evaluation, softening of plum tissue was found during storage, but the coated plums had the harder texture. These data suggest that the application of CaMC or CaHPMC-15 coatings into immature plums can reduce the postharvest softening during storage.

  • Recently plant extracts have obtained increased attention because of their health-related beneficial aspects. In this study, we evaluated anti-oxidation, whitening and anti-wrinkle of four different extracts obtained from Rubus coreanus (ethanol extracts of unripe, ripe fruits, leaves/stems, and roots). The ethanol extract of unripe fruits of Rubus coreanus generally showed stronger whitening and the anti-wrinkle activities than other extracts of Rubus coreanus. As a results, we knew that the ethanol extract of unripe Rubus coreanus fruit had mote possibility as a whitening and anti-wrinkle ingredient for skin care products.

Technology and the Demand for Unskilled Labor After the Economic Crisis (경제위기 이후 기술 변화가 미숙련 근로자의 고용상황에 미친 영향)

  • This study focuses on the possibility that skill-biased technological change is one of the factors for relatively poor employment conditions of the unskilled after the economic crisis. Increasing employment share of skilled workers accompanying with rising wage premium for education since implies that labor demand has shifted toward the skilled. The decomposition of changes in wage share of skilled workers into between-industry and within-industry changes suggests that the increase in the demand for skilled labor has been largely due to within-industry changes, which can be seen as reflecting the effect of the skill-biased technological change. Also the regression results indicate that the employment share of skilled workers has more rapidly increased in the industries with higher ICT (Information Communication Technology) investment intensity since the mid 1990s, hinting to the possibility that skill-biased technological changes may have come from ICT.

  • As the pest of the Letinus edoeds, five insect species and six animal species are investigated injuring the fresh mushroom. Of these, insect and animals, Achorutes armatus, Scaphidium amurense and Philomycus confusa caused severe damage to mushroom fruitbody, and a greate deal of damage was done to mushroom lamella by mites (Rhizoglyphis sp., and Histiostoma sp.). As the pest of Lentinus edodes bed logs, two insect species from immature bed logs, eight insects and one isopod species from matured bed logs were investigated. Of those, Moechotypa diphysis was found to be the most injurious species from both immature and matured bed logs. As the pest of Pleurotus ostreatus, six insects and the two mite species were investigated from sporophore. Of those, Lycoriella sp. and Mycophilla sp. are considered to be very serious. while two insects and five mite species were investigated from the mushroom bed. Of those, one insect (Lycoriella sp.) and two mites (Histiostoma feroniarum and Throglyphus bankslogior) found to be abundant and caused serious damage.

A Case of Ileao-ileal Intrauterine Intussusception in a Preterm Neonate (미숙아에서 발견된 태생기 회장-회장형 장중첩증 1예)

  • Intussusception in a preterm neonate is a very rare disorder. We experienced a case of intrauterine intussusception presented with symptoms of the small bowel obstruction in a preterm infant whose gestational age was $28^{+2}$ weeks. Urgent ultrasonography of abdomen revealed no definite intussuscepted segment. At emergent surgery performed on the 11th days of life under the diagnosis of distal small bowel obstruction, an ileo-ileal intussusception with distal ileal atresia without perforation was found.
