이에 인공적인 apical barrier를 형성시켜 근관의 즉각적인 충전을 할 수 있는 MTA(Mineral Trioxide Aggregate, DENTSPLY Tulsa Dental, U.S.A)를 이용하였다.

  • Adhesion of composite resin to tooth structure has been of tremendous signgicance in clinical dentistry. Due to the lack of adhesion between composite restorative resins and enamel and dentin, microleakage occurs at the tooth/restoration interface. This may lead to discoloration, secondary caries, marginal breakdown, postoperative sensitivity, and even pulpal pathology. According to extensive use of composite resin, every effort on improving bonding strength and reducing microleakage between a tooth and composite resin has been continued. This study was conducted to determine the difference in microleakage in enamel and dentin treated with air-abrasion, acid etching and combination when restored with composite resin. Class V cavities were prepared on 30 premolars. The specimens were divided into following groups. group 1:air-abrasion+Scotchbond Multi-purpose group 4 :air-abrasion+All-Bond 2 group 2:acid etching+Scotchbond Multi-purpose group 5 :acid etching+All-Bond 2 group 3:combination+Scotchbond Multi-purpose group 6 :combination+All-Bond 2 #combination:air-abrasion + acid etching The specimens were filled with Z-100 after application of Scotchbond Multi-purpose and All-Bond 2. Thermocycling was conducted by alternately dipping the specimens in $5^{\circ}C$ and $55^{\circ}C$ water for 30 seconds 500 times. 1% methylene blue was applied and the specimens were left for 24 hours at $37^{\circ}C$. After washing out the dye, the tooth was sectioned buccolingually along the axis. The sectioned surface was observed with stereoscope for dye penetration. The author has measured the microleakage in teeth prepared with air-abrasion, acid ethching and combination to study the difference in microleakage following different methods of tooth surface treatment and has come to following results. 1. In comparing microleakage between groups, group 1 and 4 showed statistically significant difference from group 2, 3, 5 and 6(p<0.05). There was no significant difference among group 2, 3, 5, 6(p>0.05) nor between group 1 and 4(p>0.05). 2. In comparing microleakage among tooth surface treatment methods, Air-abrasion group showed significantly more microleakage than acid etching group and combination(airabrasion + acid etching) group(p<0.05). Combination(acid etching+air-abrasion)group tended to show lesser microleakage than acid etching group, but this was not statistically significant(p>0.05). 3. In comparing microleakage between bonding agents, there was no statistically significant difference between Scotch bond Multi-purpose and All-Bond 2(p>0.05).

  • The purpose of this study was to evaluate the marginal microleakage of condensable composite resin restorations according to flowable resin lining of internal cavity wall. The eighty extracted human molar teeth without caries and/or restorations are used The experimental teeth were randomly assigned into four groups of ten teeth each. Eighty caries-free extracted human molars were used in this study. The conventional class II cavities (box-shaped on mesial and distal surface, faciolingual width : 3mm, gingival wall depth : 1.5mm) were prepared 1mm below cementoenamel junction with a # 701 carbide bur. The teeth were divided into four groups, and then each group were subdivided into A & B group according to flowable resin & compomer lining ; Group 1-A : Tetric Ceram filling, Group 1-B : Tetric Flow lining and Tetric Ceram filling, Group 2-A Ariston pHc filling, Group 2-B : Tetric Flow lining and Ariston pHc filing, Group 3-A SureFil filling, Group 3-B : Dyract Flow lining and SureFil filling, Group 4-A : Pyramid filling, Group 4-B : Aeliteflo lining and Pyramid filling. To simulate as closely as possible the clinical situation during retoration placement, a "restoration template" was fabricated, and the condensable resin was filled using a three-sited light-curing incremental technique. All the materials used were applied according to the manufacturers' instructions. The specimens were stored in the 100% humidity for 7 days prior to thermocycling (100 thermal cycles of 5~55$^{\circ}C$ water with a 30-second dwell time) The specimens were immersed in 2% metyleneblue dye for 24 hours, and then embedded in transparent acrylic resin and sectioned mesiodistally with diamond wheel saw. The degree of marginal leakage was scored under stereomicroscope ($\times$20) and the data were analyzed by Kruskal-Wallis test and Wilcoxon signed ranks test. The results were as follows : 1. In the gingival margins of all the group, microleakage of subgroup B was less than subgroup A. 2. In the group 1, 2, 4, there was significant differences between subgroup A and B (p<0.05), but in the group 3, there was not significant different between group 3-A (SureFil) and group 3-B (Dyract flow/SureFil) (p>0.05). 3. In the subgroup A and B, there was significant different between all group except group 4 of subgroup A. From the results above, it was suggested that the cavity lining of flowable resin and flowable compomer in condensable resin restoration decrease microleakage at gingival margin, and does improve their ability to seal the gingival margin of class II preparation.

  • The purpose of this in vitro study was to compare the apical leakage in extracted teeth filled with gutta-percha subsequent to dressing with one of three different calcium hydroxide preparations. Thirty six extracted teeth with single canal were used in this study. After working length determination, canals were prepared with K flexo files to a #40 at the working length. Step-back flaring was produced by using #45, #50 K flexo files and #2, #3, #4 Gates Glidden burs. The teeth were randomly divided into 3 groups of 10 each : the remaining six teeth were used for negative and positive leakage control: Group 1, dressed with pure calcium hydroxide powder (Sigma, USA) mixed with distilled water; Group 2, dressed with Metapaste (Metadent, Korea) ; Group 3, dressed with Vitapex (Neo Dental, Japan). Teeth were sealed with Caviton (GC, Japan) and incubated in 100% humidity, at 37$^{\circ}C$ for 1 wk. All kinds of calcium hydroxide were removed from the canal with a MAF and 5% NaOCl. The canals were filled with AH-26$^{\circledR}$ sealer and gutta-percha using lateral condensation technique, incubated in 100% humidity, at 37$^{\circ}C$ for 2 days for the sealer to be set. The teeth were coated twice with nail varnish except for an area of approximately 2mm surrounding the apical foramen. All specimens were placed in 2% methylene blue solution for 2 days. The root were sectioned longitudinally, the amount of apical leakage was measured to the most coronal part of the root canal to which the dye had penetrated. The independent measurements were made for each root using a stereomicroscope ($\times$40 magnification) and the average was recorded for statistical analysis. The results were as follows ; 1. The mean of apical leakage in group of pure calcium hydroxide ranged 0.102$\pm$0.156mm, in Metapaste$^{\circledR}$ ranged 0.062$\pm$0.069mm, and in Vitapex$^{\circledR}$ ranged 0.067$\pm$0.072mm. 2. Group of pure calcium hydroxide exhibited more leakage than those of 2 manufactured calcium hydroxide preparations, but it was not statistically significant. 3. Group of water-based Metapaste$^{\circledR}$ showed lesser leakage than that of oil-based Vitapex$^{\circledR}$, but it was not statistically significant.

  • The purpose of this study was to assess the effect of catalase used following bleaching for the elimination of hydrogen peroxide residues from human teeth on the microleakage at the tooth-resin composite interface. In this study, class V cavities were prepared on the buccal or lingual surfaces of seventy extracted human molar teeth, and crown of sixty teeth were immersed in 30% hydrogen peroxide at $37^{\circ}C$ for 5 days except for negative control group. Then the teeth were rinsed with water and distributed randomly into seven groups of 10 each and were conditioned as following Negative control group: No bleaching Positive control group : bleaching and no application of catalase (C-40) Experimental group 1 : one cycle of catalase application for 3 min. and water rinse for 2 min. after bleaching Experimental group 2 : two cycles of catalase application for 3 min. and water rinse for 2 min. after bleaching Experimental group 3 : three cycles of catalase application for 3 min. and water rinse for 2 min. after bleaching Experimental group 4 : four cycles of catalase application for 3 min. and water rinse for 2 min. after bleaching Experimental group 5 : five cycles of catalase application for 3 min. and water rinse for 2 min. after bleaching The cavities of each groups were restored with composite resin. The teeth were thermocycled, stained with 2% methylene blue, and sectioned buccolingually. Degree of dye penetration at tooth-restoration interfaces were examined by stereomicroscope(${\times}30$) at occlusal and gingival margin The results were as follows : 1. On the occlusal margin, there was no significant difference in the microleakage between the negative coltrol group and experimental groups (p>0.05). But on the gingival margin, experimental groups showed higher microleakage than the negative coltrol group (p<0.05). 2. On the occlusal margin, positive coltrol group showed higher microleakage than experimental groups (p<0.05) and among the experimental groups, group 1 showed higher microleakage than group 3, 4, 5 (p<0.05). 3. On the gingival margin, there was no significant difference between the positive coltrol group and experimental groups, and between experimental groups (p>0.05). The result indicated that catalase used in bleached cavity for the elimination of hydrogen peroxide residues from human teeth maybe reduced microleakage at the tooth-resin composite interface.

  • This study evaluated the marginal microleakage of five single step adhesives. Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were prepared on both buccal and lingual surfaces of extracted human molar teeth. Prepared teeth were randomly divided into five groups and restored using one of the single step adhesives and composite resins:Prompt L-Pop/Filtek Z-250 (Group 1), AQ Bond/Metafil CX (Group 2), One-Up Bond F/Palfique Toughwell (Group 3). Futurabond/Admira (Group 4), Xeno III/Spectrum TPH (Group 5). The restored teeth were thermocycled. Microleakage was assessed by dye penetration using 2% methylene blue dye solution. The teeth were bisected buccolingually and evaluated for microleakage under steromicroscope. The data were statistically analysed by Kruskal-Wallis test and Mann-Whitney tests. The results of this study were as follows:1. Microleakage of enamel margins in group 3 was statistically higher than that in groups 1, 2, 4, 5 (p < 0.05). 2. Microleakage of dentin margins in group 1 was statistically higher than that in groups 2, 5, and that in group 3 was statistically higher than that in groups 2, 4, 5 (p < 0.05). 3. Dentin marginal microleakage was higher than enamel marginal microleakage in all experimental groups. In conclusion, Prompt L-Pop showed the least leakage at enamel margin, and AQ Bond showed at dentin margin in this study. Marginal miroleakage in dentin was higher than that in enamel.

  • The purpose of this in vitro study was to evaluate the microleakage of 5 curing methods in class V composite restorations which are composed of two-step light curing, pulse-delay cure, low curing-light intensity, moderate curing-light intensity and high curing-light intensity. In this study, class V cavities were prepared on buccal and lingual surfaces of 50 extracted human upper or lower molars on cementum margin. Single Bond adhesive and Z-100 shade A2 were applied for each group following the manufacture's instruction. The experimental teeth were randomly divided into 5 groups of 10 samples (20 surfaces) each. Group 1: two-step light curing; Group 2: pulse-delay cure; Group 3: low curing-light intensity; Group 4: moderate curing-light intensity; Group 5: high curing-light intensity. After 500 thermocycling between $5^{\circ}C$ and $55^{\circ}C$, the 60 teeth were placed in 2% methylene blue dye for 24 hours, then rinsed with tab water. The specimens were embedded in clear resin, then sectioned buccolingually through the center of restoration with a low speed diamond saw. The dye penetration on each of the specimen was then observed with a stereomicroscope at ${\times}20$. The composite resin/tooth interfaces were examined under Scanning Electron Microscopy. The results were statistically analyzed using the Kruskal-Wallis One Way ANOVA and Dunn's Method. The results of this study were as follows. 1. In all groups, the leakage values seen at the enamel margin were significantly lower than those seen at the dentin margin(P<0.05). 2. No group in this study showed significant differences in leakage values at both the enamel and the dentin margins(P<0.05). 3. In all groups, the gaps seen at the enamel margin were significantly lower than those seen at the dentin margin(P<0.05). 4. The gaps in this study showed significant differences and two-step light-curing and low curing-light intensity produced significant less gap than high curing-light intensity(P<0.05).
