A Case of Chronic Diarrhea after Neurolytic Celiac Plexus Block -A case report- (복강 신경총 차단 후에 발생한 만성설사)

  • A neurolytic celiac plexus block produces long-lasting pain relief in upper abdominal cancer patients. Unwanted side effects such as local pain, hypotension, and diarrhea are common but the durations of these side effects are usually transient. Chronic diarrhea induced by a neurolytic celiac plexus block is rarely reported and is considered to be an autonomic neuropathy due to sympathetic denervation. A 73-year-old Klatskin tumor patient developed chronic diarrhea after a neurolytic celiac plexus block and the diarrhea was sustained for 3 months despite the use of conventional antidiarrheal treatments. We report a case of chronic diarrhea that was induced by a neurolytic celiac plexus block.

A Case Study of a Taeeumin Patient with Chronic Diarrhea treated with Cheongsimyeonja-tanggamibang (삼차신경통 태음인 환자의 만성 설사 치험례)

  • Objectives This study was aimed to report a significant improvement of chronic diarrhea patient, diagnosed as 'Taeeum type liver heat-lung dryness pattern' based on Sasang medicine. Methods The patient had treatment with Sasang constitutional medicine. The treatment was executed three times everyday from August 27th to September 3rd and two times everyday from September 3rd to September 16th of 2021. Clinical improvement was evaluated with Bristol stool form scale, the degree of abdominal discomfort and nausea compared to the day of hospitalization and the number of defecations and nocturia per day. Results The symptoms of diarrhea, abdominal discomfort and nausea were improved. The number of night urination was decreased from 3-4 times to once a night. Conclusion After the treatment with Sasang constitutional medicine, diarrhea, abdominal discomfort, nausea and the general condition of the patient also improved.

  • Objectives: This study reported a clinical case of a chronic diarrhea patient who showed improvement with Korean medicine treatment. Methods: A patient with chronic diarrhea was treated with herbal medicine (Omae-hwan and Omae-hwan-ryo), acupuncture, cupping, and moxibustion. Changes in stool form were measured using the Bristol Stool Scale (BSS). The frequency of defecation was checked every day. The Short Form 36-Item (SF-36) was used to compare patients' quality of life before and after treatment. Results: Korean medicine treatment improved the stool form, frequency of defecation, and quality of life. Conclusion: Korean medicine treatment containing Omae-hwan and Omae-hwan-ryo improves clinical symptoms and quality of life.

Complementary and Alternative Medical Therapies in Children and Adolescent with Chronic Disease : Utilizations and Patterns (만성 질환을 가진 소아, 청소년에게 시행되는 보완대체요법의 실태 연구)

  • Purpose : This study describes how often complementary and alternative medical(CAM) therapies, what patterns of CAM therapies utilization, what types of CAM therapies and why CAM therapies are used in children and adolescent with chronic disease. Methods : We interviewed and filled out questionnaires with parents of patients suffering from asthma, atopic dermatitis, congenital myopathy, epilepsy and so on from Mar 2004 to Aug 2004. Results : Two hundred fifty two parents answered questionnaires; 160 cases(63.5 percent) had used CAM therapies. Utilization of CAM therapies according to each chronic disease was as follows : congenital myopathy(100 percent), atopic dermatitis(91.1 percent), asthma(72.5 percent), chronic diarrhea (50.0 percent), epilepsy(17.1 percent) etc. Types of CAM therapy were as follows : dietary supplement (31.1 percent), herb medicine(23.4 percent), massage(14.2 percent), acupuncture(4.6 percent) etc. Sixty four(40.0 percent) case underwent more than two types of CAM therapies simultaneously. One hundred thirty three(83.1 percent) case were asked through non-medical prescription. Ninety nine(61.9 percent) case were recognized positive effect of CAM therapies. Parents' and childrens' ages were influential factors in CAM therapies utilization : the older the parents and the younger the children, the higher the utilization. Conclusion : Although patients receive treatment with western medicine, many of them not only undergo CAM therapies, especially more than two types, but also recognize the positive effects of CAM therapies. As a result, we suggest that it is easy to use of CAM therapies, and the positive effects of CAM therapies are prominent, domestically. Therefore, medical approach and study about the effectiveness and side effects of CAM therapies is necessary to avoid the imprudent use of such therapies.

Hemolytic-uremic Syndyome Associated with Bloody Diarrhea (혈변을 동반한 용혈성 요독 증후군 5례)

  • Purpose : Hemolytic uremic syndrome(HUS), known as a most common cause of childhood renal failure in western countries, has been a relatively rare disease in Korea. Although the reported cases were not related to any specific cause in Korea, there was an outbreak of HUS with bloody diarrhea in Japan last year. We report here that we experienced the several typical HUS last year. Patients : From Jan.1996 to Dec. 1996, five patients were diagnosed as HUS at Samsung Seoul Medical Center, Dept. Pediatrics. Results : 1) The age of onset was below 3 years in 3 cases and above 9 years in 2 cases. 2) All the cases happened between summer and autumn. Three patients had domestic travel and 4 patients drank well or spring water before the symptoms. 3) The clinical manifestation was generalized edema, oliguria, anuria and hematuria. The bloody diarrhea were present in 5 cases and 1 patient had operation with the impression of appendicitis. 4) There was no bacteria which was isolated from the blood or stool samples. 5) Renal biopsies were performed in 2 cases, because of protracted clinical course. One showed microthrombotic angiopathy and the other cortical necrosis with necrotizing glomeruli. 6) Complete recovery was the outcome in 4 cases and one case progressed to chronic renal failure. Conclusion : In conclusion, typical HUS associated with bloody diarrhea, epidemic and good prognosis can be found in Korea and careful surveillance of the pre-clinical cases will be necessary.

Ettect of Diets Containing Dried Whey and Chitin on Growth Rate of Broiler Chicks (Dried Whey와 Chitin의 첨가(添加)가 Broiler Chick의 성장(成長)에 미치는 영향(影響)에 관한 연구(硏究))

  • Two experiments, utilizing 204 broiler chicks of the Maniker strain, were conducted to study the effects of dried whey and chitin on the growth of chicks. In the first experiment, diets containing 5%, 10%, or 15% dried whey, or a control diet with no whey, were fed to chicks from 1 day to 4 weeks of age. There were no significant differences among the dietary groups, with respect to the rate of growth or protein or feed efficiency. In the second experiment, chicks were fed with diets containing no whey, 2% chitin, 20% dried whey, or 20% dried whey plus 2% chitin, from 1 day to 4 weeks of age. Adverse effects (diarrhea, crooked toes and enlarged cecum) were observed in the group fed 20% dried whey. The protein efficiency ratio (PER) and feed efficiency ratio (FER) tended to improve in the dietary groups with dried whey, as compared to the control group. No significant differences were observed in the total carcass nitrogen and lipid levels of dietary groups. But the chitin-supplemented diets tended to improve on the growth rate compared to the groups without chitin. The results of the two experiments suggest that the growing broiler chick can tolerate up to 15% dried whey in the diet, without any harmful effect on growth. Also, chitin may improve the poorer growth rate of chicks fed dried whey, but not all of the adverse effects of whey.

  • Four cases of acute or acute-on-chronic kidney injury were referred for hemodialysis therapy. All cases had a history of uremic syndrome including refractory vomiting, anorexia, diarrhea and depression. They already had been treated with conventional therapies but the intractable clinical signs were remained without response of medications. Intermittent hemodialysis (IHD) sessions were executed in four cases. This study describes the clinical signs and biochemical features of four hemodialysis patients with dialysis adequacy indexed by Kt/V. This report introduced the estimation of hemodialysis adequacy method with clinical cases for establishment of standardization of hemodialysis therapy assess in dogs.

  • Microsporidia in humans emerged with the AIDS epidemic. Enterocytozoon bieneusi is the most significant species' and causes chronic diarrhea, wasting, papillary stenosis, acaculous cholecystitis, bile duct dilatation and sclerosing cholangitis and is responsible for $30-50\%$ of all cases of in people with AIDS. Microsporidiosis has been reported in immunosuppressed, and in immunologically normal individuals. Variety of study has revealed the mode of transmission and possible reservoir of E. bieneusi. Its sites of infection suggest that transmission occurs by ingestion. Transmission has been speculated to occur via infected animals to human, person to person. There is evidence that E. bieneusi occurs in pigs, monkeys, and possibly other animals such as cattle, dogs, cats, llamas and rabbits. E. bieneusi from infected humans has been transmitted experimentally to macaques and to pigs. These observations reflect indirectly the zoonotic nature of E. bieneusi and indicate that cross species transmission is a real possibility. Meanwhile, In recent report, thirty-two percent of the pigs were found to be positive with rates higher over the summer months in US.

  • Human norovirus (hNoV) infection accounts for the vast majority of virus-mediated gastroenteritis cases worldwide. It causes self-limiting acute illnesses in healthy individuals lasting for a few days, however, in immunocompromised patients, hNoV can establish chronic and potentially fatal infections. Since its discovery in 1968, much effort had been made to develop cell culture and animal infection models to no avail. Only recently, some promising breakthroughs in the development of in vitro infection models have been made. Here, we will contrast and compare those models and discuss what further needs to be done to develop a reliable and robust cell culture model.

  • Autoimmune enteropathy is a rare chronic diarrheal disease of infancy. Clinicopathologically, this entity is characterized by chronic secretory diarrhea, villous atrophy with crypt hypoplasia of a small intestine and/or associated autoimmune disorders, and absence of severe immunodeficiency. For the confirmation of diagnosis, antienterocyte autoantibody should be delineated. The treatment of choice of this disorder is immunosuppression. We has been experienced a case of autoimmune enteropathy without autoimmune disorders in a 10-month-old male infant. He developed protracted diarrhea from 5 months of his age and has been appeared to be failure to thrive. Antienterocyte autoantibody was demonstrated by immunohistochemistry and western blotting. He was successfully treated with corticosteroid and FK506. This is the first case report of autoimmune enteropathy without autoimmune disorders in Korea.
