Analysis of Influencing Factors on Commitment to a Plan for Physical Exercise among Korean Adults with Chronic Disease (만성질환자의 운동이행계획 수립에 미치는 영향 요인 분석)

  • Purpose: The purpose of this study was to analyze the factors affecting those commitment to a plan for exercise among Korean with chronic diseases by Pender's health promotion model. Methods: The participants in this study were 315 Korean adults over 18 years of age with chronic diseases who were recruited in hospitals or public health centers in Korea. The instruments used in this study were individual characteristics instrument, Rosenberg's self-esteem scale, Ware's perceived health status, Profile of Mood States and Pender's commitment to a plan for exercises scale. Results: The results of analysis showed that Korean adults with chronic diseases were perceived with relatively low commitment to a plan for exercise while they were perceived with moderate self-esteem, health status, and mood. There were significant differences in the commitment to a plan for exercise according to education level and economic level. The positive relationship between self esteem, perceived health status, and commitment to a plan for exercise were significantly correlated. The experience for exercise was the most influential variable on commitment to a plan for exercise accounting for 40.3% among Korean with chronic diseases. Conclusion: For planning a targeted strategy to increase regular exercise among Korean adults with chronic diseases, nurses have to assess the prior experience of exercise of them. These nursing activities lead to improve their quality of life and encourage to control their symptoms.

  • This study discusses the development of a u-Health care system that can detect quickly and cope actively with emergent situations of chronic disease patients who lead everyday life. If a patient's emergent situation is detected by personal health care host(PHCH), which is composed of acceleration and vibration sensors, GPS, and CDMA communication module, a text message on the patient's current location is transmitted to the hospital and the guardian's mobile terminal so that they can cope with the situation immediately. Especially, the system analyzes data from sensors by using neural network and determines emergent situations such as syncope and convulsion promptly. The exact location of patients can also be found in the electronic map by using GPS information. The experiments show that this system is very effective to find emergencies promptly for chronic disease patients who cannot take care of themselves and it is expected to save many lives.

  • The Multiple vital signs management system using Mobil phone is designed with Wireless sensor network and CDMA which are integrated to create a wide coverage to support various environments like inside and outside of hospital. Health signals from medical sensor node are analysed in cell phone first for real time signal analyses and then the abnormal vital signs are sent and save to hospital server for detail signal processing and doctor's diagnosis. We developed integrated vital access processor of sensor node to use selective medical interface(ECG, Blood pressure and sugar module) and control the self-organizing network of sensor nodes in a wireless sensor network. chronic disease such as heart disease and diabetes is able to check using graph view in mobile phone.

Concept Analysis of Volition for Health Behavior Practice in Patients with Chronic Disease (만성질환자의 건강행위 실천을 위한 의지(volition)에 대한 개념분석)

  • Purpose: The purpose of this study was to define and clarify the concept of volition for health behavior. Methods: For this study the process of Walker & Avant's concept analysis was used. Seventeen studies from electronic data basses met criteria for selection. Results: Volition can be defined by the following attributes: 1) planning, 2) maintenance of self-efficacy, and 3) self regulation. The antecedents of volition consisted of: 1) risk awareness, 2) outcome expectation, 3) perceived self-efficacy, and 4) social support. The consequences occurring as a result of volition were: 1) prevention of disability and complications, 2) improvement of functional ability, and 3) enhencement of quailty of life. Conclusion: Definition and attributes of volition identified by this results can be applied to develop measurements and intervention programs for chronic patients health behavior.

The usual source of healthcare and frequent visits to emergency departments (만성질환자의 상용치료원 보유 및 형태와 다빈도 응급실 이용)

  • Purpose: The purpose of the study was to investigate and compare the usual source of healthcare and frequent visits to emergency departments. Methods: The study subjects were 7,252 individuals with chronic diseases who filled out the questionnaire of the 2013 Korea Health Panel Survey. Data were analyzed using chi-square test and logistic regression. Results: Compared to having a public health center or clinic as a usual source of healthcare, it is 1.341 times more likely for a chronic disease patient to visit an emergency department if the hospital is her/his usual source of healthcare, while it is 1.656 times more likely for the patient to visit a general/tertiary hospital. Conclusion: It is important to investigate visits at the emergency department requiring primary care for diseases.

  • u-Healthcare는 유비쿼터스와 원격의료 기술을 활용한 건강관리 서비스를 말한다. 목표는 삶의 질 향상 추에 있다. 경제수준이 향상되면서 삶의 질에 대한 중요성이 대두되고 있으며, 만성질환자 및 인성 질환의 예방 및 효율적 관리는 개인 및 사회적으로도 중요한 의의를 가진다고 할 수 있다. u-Healthcare 서비스를 제공하기 위해서는 언제 어디서나 이용사의 건강상태를 진단할 수 있는 생체계측기술이 필요하기 때문에 u-Healthcare 측정 모듈의 개발은 매우 중요하다. 본 논문에서는 개발한 모바일 연동 다기능 u-Healthcare 측정 모듈을 이응하여 심전도, 체지발, 호흡수, 체온 및 운동량 측정 등의 신호를 측정하였다. 측정된 신호를 Bluetooth 기반의 transceiver를 통해 u-Healthcare Center로 전송하며, Diagnostic Monitor를 통해 신호를 분석하여 건강상태를 모니터링 할 수 있는 검증모니터를 구현하였다.

  • 본 논문은 스마트폰 환경에서 만성질환자의 약복용 순응률을 향상시키기 위한 앱을 모델링하고 구현하였다. 이 앱은 일상생활을 하면서 장기간 규칙적으로 약을 복용해야 하는 환자들을 대상으로 약복용을 지원할 수 있도록 모델링되었다. 또한, 스마트폰을 통해 모바일 환경에 있는 환자에게 약복용 알림, 복약지도, 순응률 조회 등의 모니터링 서비스를 제공한다. 앱 서버는 환자들의 약복용 스케줄 데이터를 관리하며, 이 데이터는 3GS 통신을 통해 스마트폰과 동기화 된다. 순응률은 약복용 알림에 대해 환자의 대응에 따라 자동으로 계산되며 조화될 수 있다. 이 앱 통신은 국제표준 IEEE P11073-10472으로 구현되었다.

  • 본 논문에서는 5개 이동통신 사업자 식별을 위한 사용자 에이전트와 개인휴대전화를 이용한 유 무선인터넷 통합서버에 의해 각 이동통신사업자를 경유한 유선 포털사이트 또는 특정 사이트(URL 접속)에 직접 접속하여 WAP 또는 ME를 동시에 지원할 수 있는 NT 또는 LINUX 통합서버를 구축한다. 이로서 휴대전화를 이용한 입학관리시스템, 휴대전화를 이용한 만성질환자의 원격 진료시스템, 휴대전화를 이용한 방송 순위집계 시스템과 사용자 참여방법, 시청률 및 여론조사, 무선성경 검색 등의 다양한 컨텐츠의 개발이 용이하고, LINUX/UNIX, NT 서버구축이 단순함으로서 이동전화의 장점인 휴대성으로 시간과 공간의 제약을 벗어나 다양한 무선 인터넷 검색이 가능해 질 것이다.

  • 본 논문에서는 만성질환을 갖고 있으면서도 일상생활을 하고 있는 만성질환 환자의 응급상황에 대비할 수 있는 u-Health 시스템 구축에 대하여 논한다. USN과 전자청진기, GPS, CDMA 그리고 RFID로 구성된 PHC(Personal Health Care) 장치에 의해 환자의 위급상황이 감지되면 위치정보를 포함한 문자메시지를 병원과 보호자의 모바일 단말기로 전송하여 즉각 대처할 수 있도록 한다. 특히 유비쿼터스 센서로 구성된 기울기/가속도 센서와 진동센서로 부터 수신되는 신호패턴을 분석하여 기절현상 또는 발작 증상과 같은 응급상황 여부를 판단한다. 이 시스템을 항상 주의가 필요한 만성질환자에게 적용시킴으로써 응급 상황에 신속히 대처하여 귀중한 생명을 구할 수 있을 것으로 기대된다.

Factors Influencing on Use of Complementary.Alternative Therapy on Chronic Patients (만성질환자의 보완대체요법 이용에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify influencing factors of using numbers and period of complementary and alternative therapy (CAT) among chronic patients. Methods: The study employed a descriptive design. The participants were 221 chronic patients. Data were collected in a pharmacy near a general hospital from April to July, 2008. Results: Influencing factors on using number of CAT were age, numbers of disease, period of used CAT and monthly expenses on CAT (the ability to explain, 79.9%). Influencing factors on using period of CAT were the period of illness and using number of CAT (the ability to explain, 44.9%). Conclusion: The results of this study suggest that chronic patients need counseling and guides about using CAT especially for those are older and have many kinds and longer period of diseases.
