Chronic Disease Management using Smart Mobile Device (스마트 모바일 기기를 이용한 만성질환 관리)

  • According to the recent trends in the growing elderly population, the chronically ill have increased. Thus the importance of the health care issues for them has emerged. In this paper, we want to implement a chronic disease management system using smart mobile devices. Proposed chronic disease management system is consisted of the biometric sensor, smart mobile devices, the patient management server, patient management DB, and patient symptoms analysis agent. The biometric sensor detects a biological information. Smart mobile devices receive the patient information from the sensor and transmit the information to the patient management server. The patient management server, patient management DB, and patient symptoms agent analysis agent analyze to process data delivered through a wireless communication network. Bio-signals includes modules of ECG, blood pressure, blood sugar and PPG. We are able to determine the current health status by monitoring measured biometric data through chronically ill health management system. We will focus on the individual service to be appropriate for a patient group in a mobile environment.

  • According to the recent increasing trend of the ages, numbers of patients with chronic diseases are increasing and issues for health care are importantly emerged. In this thesis the research implements U-health care system for health care of patients with chronic diseases. The suggested system for health care of patients with chronic diseases composes bio measurement system, mobile gateway and medical information management server, and bio-signals are composed with modules such as electrocardiogram, blood pressure, blood sugar, oxygen saturation if configured as client. Blood sugar check was considered and implemented to be chosen the ways to transmit through bio measurement system or through gateway. Suggested bio measurement system and mobile gateway are transmitted through Bluetooth. The transmitted biodata is searched by observing health check through mobile gateway, by transmitting through network server, and by using client. By implementing bio signal observation system of patients with chronic diseases, present health check is available by monitoring measured bio data, and various bio signals are transmitted in the mobile environment.

Self-rated Health and Health Service Utilization of Chronic disease Patients (만성질환자별 주관적 건강상태와 의료시설 이용행태)

  • This study, based on the assumption that self-rated health status affects the use of health care utilization, aims to comprehend self-rated health status, as well as the differences in medical facility use depending on self-rated health status, and analyze according to chronic diseases to use it as the basis of a practical guide development for chronic disease patients' use of medical facilities. Used representative Korean Longitudinal Study of Ageing data to analyze 7,486 participants. There is a difference in Chronic disease patients' use of health care utilization depending on self-rated health, and this difference was the most apparent in Oriental healthcare service and dental treatment use. This study was unable to control all confounding variables affecting medical service use, so we want this studied to be used to assist future studies.

A Study of Current Status and Activities of the Community Health Practitioners in Rural Areas in Korea (한국의 농촌지역에서의 보건진료원 현황 및 활동분석연구)

  • 한국의 농촌 보건의료 문제를 해결 하기위하여 일차보건의료가 도입되었으며 일차보건의료에서 핵심적인 역할을 수행하고 있는 보건진료원의 활동을 분석하였다. 분석 목적은 보건진료원의 일반현황과 보건진료원 활동에 대한 구체적인 분석을 통하여 활동개선 방안을 제시하는데있다. 분석방법은 사회조사 방법을 적용하였으며 조사는 1998년 7월과 11월기간에 구조화된 설문지를 보건진료원 전원에게 송부하여 스스로 작성하게 하였다. 1880명의 보건진료원을 대상으로 설문지를 배포하였으며, 1663명의 응답자가 작성한 설문지가 최종분석되었으므로 분석율은 88.5%였다. 분석결과 보건진료원의 일반현황으로 광역사의 보건진료원 평균 연령은 39.7세이며 시지역 보건진료원의 연령이 군지역보다 높았으며 통계적으로도 유의한 차이가 있었다. 총근무기간도 시지역의 보건진료원의 12.4년-13.6년으로 농촌지역보다 걸었으며 통계적으로도 유의한 차이가 있었다. 보건진료원의 평균 관할 인구수는 901명이었다. 보건진료소 관할지역의 노인인구 및 만성질환자의 분포가 매우 높았다. 보건진료소로부터 민간의료기관이 위치한 거리는 시지역 (7.1-11.3km) 보다 농촌지역이 12.1km로서 지리적 접근성이 시지역보다 낫은 것으로 나타났다. 활동건수분석결과 월평균 가정방문수는 평균 47.8건, 일평균 내소자수는 14.1건 및 일평균 전화상담은 5.2건으로 나타났다. 연령별 활동수준은 40세이상이 4세 이하보다 월평균 가정방문수와 일평균 내소자수가 많았으며 통계적요로도 유의한 차이가 있었다. 경력이 많을수록 일평균가정 방문수 및 전화상담 건수가 많았으며 통계적으로 유의한 차이가 있었다. 근무조건에 대한 만족도는 80%정도로 높았다. 연구결과 보건진료원의 활동지역내 노인인구 및 만성질환자의 비율이 높고 민간의료기관의 지리적 접근성도 낮고, 관할인구규모가 적어서 인구규모를 늘리고 노인 및 만성질환자 건강관리 프로그램의 개발이 더욱 요구되며 농촌지역주민의 건강관리자로서 보건진료원의 활동을 강화할 필요가 있다.

Effect of Thought s of Suicide of Elderly that Care for a Chronic Invalid (노인의 자살생각에 영향을 미치는 생태체계요인 -중증만성질환자 돌봄 노인을 중심으로-)

  • In this study, I tried to find out the ecosystem factors which influence suicide thoughts of elderly that care for a chronic invalid. Ecosystem factors are consist of personal system, familial system and social system, tried to analysis ecosystem factors with characteristics of social population that influence suicide thoughts. For the study, I collected 274 samples from people of 55 year-old care a chronic invalid adults who reside of in Gyeonggi-do. I used factor analysis, reliability analysis, multi-regression, t-test, ANOVA with SPSS 18.0. The results are as follows. First, suicide thoughts had affected by personal characteristics and the duty of supporting selected independent variable. Second, familial system as familial characteristics and familial support had a strong influence on suicide thoughts by long-term care and deficient familial support. Third, the result of study, social support and participation of social activities apply as independent variable then every factors of social support are negative influence but participation of social activities are not influence.

Analysis of research trends on mobile health intervention for Korean patients with chronic disease using text mining (텍스트마이닝을 이용한 국내 만성질환자 대상 모바일 헬스 중재연구 동향 분석)

  • As the widespread use of mobile health intervention among Korean patients with chronic disease, it is needed to identify research trends in mobile health intervention on chronic care using text mining technique. This secondary data analysis was conducted to investigate characteristics and main research topics in intervention studies from 2005 to 2018 with a total of 20 peer reviewed articles. Microsoft Excel and Text Analyzer were used for data analysis. Mobile health interventions were mainly applied to hypertension, diabetes, stroke, and coronary artery disease. The most common type of intervention was to develop mobile application. Lately, 'feasibility', 'mobile health', and 'outcome measure' were frequently presented. Future larger studies are needed to identify the relationships among key terms and the effectiveness of mobile health intervention using social network analysis.

The Effect of Transition to Living with Chronic Diseases on Depressive Symptoms (만성질환 진단이 노인의 우울수준에 미치는 영향: 주요 5대 만성질환의 초기 진단기를 중심으로)

  • This study explored whether the transition to living with a chronic disease changes the level of depression symptoms among middle-aged and older adults in South Korea. We hypothesize that the transition to living with chronic disease leads to a higher level of depressive symptoms. A nationally representative sample (N = 6,284) of adults 45 years and older from the Korean Longitudinal Study of Ageing (KLoSA) was analyzed. Multi-regression modeling was used to examine the association between the diagnosis of chronic disease and the level of depressive symptoms in patients. The findings highlight the need for policy makers, clinicians, as well as patients and their caregivers to become more aware of mental health risks in patients diagnosed with chronic diseases, particularly first-time patients. This study contributes to encouraging greater psychosocial support, including monitoring the level of depressive symptoms of patients who develop chronic conditions and providing appropriate treatments for those at the highest risk.

An Analysis of Impact on the Quality of Life for Chronic Patients based Big Data (빅데이터 기반 만성질환자의 삶의 질에 미치는 영향분석)

  • The purpose of this study is to investigate the effect of personal factors and community factors on the quality of life based on the presence of chronic patients based on the Big Data Platform. As a method of study, second data of 2017 community health survey and Statistics Korea by City·Gun·Gu public office were used and a multi-level analysis was conducted after separating EQ-5D index, individual factor and community factor. As a result, men, age, education level, monthly household income, having economic activity, the number of sports infrastructure were positively associated with the quality of life, and subjective health not good, extremely perceived stress were negatively associated with the quality of life. Research will continue to provide a platform independent of hardware that can utilize the cloud and open source for medical big data analysis in the future.

Reliability and Validity of the Korean version of autonomy preference index among patients with chronic disease (만성질환자 대상 한국어판 자율성 선호도 도구(K-Autonomy Preference Index)의 신뢰도 및 타당도 검증)

  • The purpose of this study was to validate the Korean version of autonomy preference index (K-API) for chronic patients. Participants were 569 chronic patients. Construct validity and reliability of K-API were examined using exploratory and confirmatory factor analysis, and Cronbach's α test. Original API was translated to Korean, and we conducted contents validity test, and pilot test. The K-API consisted of 10 items divided into two domains: (i) Decision making preference (DMP); (ii) Information seeking preference (ISP). K-API explained 53.4% of autonomy preference; the two-factor structure showed an acceptance fit. Cronbach's α was. 77 for DMP, and. 75 for ISP. Validity and reliability of the K-API were established, and this study provides additional evidence for the usage of the API in Asian region.

The Study on Health Care Consumer's Medical Information Serching Behavior according to Chronic Disease (의료소비자의 만성질환 유무에 따른 의료정보 탐색 행태에 관한 연구)

  • This study aims to understand health care consumer's information searching behavior according to their presence of chronic diseases. For this purpose, 240 patients' data of university hospital located in Seoul were collected by surveys from 9 February to 11 February 2016. For statistical analysis, we used SPSS Ver. 26.0. The main results of this study are as follows: First, Chi-square test showed that chronic patients used expert information sources and experiential information sources more than those without chronic diseases. Second, chronic patients were found to search nutrition management, exercise management, and patient case. Lastly, in the case of hospital-related searches, patients with chronic diseases searched more waiting time and medical expenses than those without chronic diseases. The significance of this study is to provide basic data for efficient information provision and establishment of marketing strategies reflecting information needs of health care consumers.
