Purpose: This descriptive study was conducted to project the number of nurse anesthetists needed in hospital settings, up to the year 2015. Method: Necessary data and information were collected from various funded reports, professional literature, web sites and personal visits to national and private institutions. The number of nurse anesthetists needed was projected after considering the total number of cases requiring anesthesia including deliveries, workload ratio for caesarean section to total number of deliveries, and percent of deliveries requiring and anesthesia. Result: The projected number of nurse anesthetists needed for hospital settings are as follows: 1. The number of registered anesthesia personnel in Korea in 2002 was 2,481 anesthesiologists and 543 nurse anesthetists but only 60% of anesthesiologists and 30% of nurse anesthetists (147) were actually practicing in the field of surgery. 2. By the year 2015, the total number of projected nurse anesthetists needed in hospital settings will be between 214 and 265. Conclusion: In order to match the supply to the need, the professional organizations should direct efforts towards enacting legislation. Educational systems should identify strategies to initiating an adequate number of nurse anesthetist programs at the master's level as well as standardizing curriculums across programs.
Purpose: This study aims to examine level of perception and performance of privacy protection behavior of anesthesia and operating room (OR) nurses for patients who underwent general anesthesia surgery. Methods: Data collection was conducted from August 2020 to January 2021 for a total of 101 participants, consisting of 49 patients and 52 nurses. Independent t-test and Pearson's correlation were conducted using SPSS 21. Results: Anesthesia and OR nurses showed the highest score in patient privacy, followed by patient information management, body privacy, and the lowest score in communication. There was a significant difference between the patient information and the communication. Conclusion: Anesthesia and OR nurses had the highest level of perception and performance of patient privacy protection behavior for body privacy, and the lowest for communication. In addition, there was a significant difference in patient information management and communication. In order to protect the privacy of patients undergoing general anesthesia surgery, efforts are needed to learn standardized nursing knowledge, attitudes, and practice.
오늘날 급격한 통신기술의 발전은 우리 삶의 다른 모든 분야와 마찬가지로 의료 환경에도 많은 변화와 새로운 패러다임을 요구하고 있다. 그러나 새로운 컴퓨터 기술이 의료산업을 급속도로 변화시켜 나가고 있음에도 불구하고 그 변화가 실제로 이루어지고 있는 현장을 보는 것은 그리 쉽지 않다. 본 연구에서는 근 미래의 수술실에서 마취과 간호사의 시각으로 마취 과정에 관한 새로운 인터랙션 컨셉을 탐색하고자 하였다. 연구는 5년의 기술적 사회적 시각이 반영되었으며, 근 미래의 수술실에서 새로운 기술이 어떻게 환자 만족을 높이고 간호사의 업무 효율을 향상시켜 궁극적으로 의료 서비스의 품질을 향상시키는지를 보여주고 있다. 연구 결과인 "KEY" 는 다양한 직관적인 인터랙션방식을 통하여 마취과 간호사의 업무에 새로운 비전을 가져왔으며, 이것으로 인해 간호사는 수술과 관련된 기기들로부터 완전히 자유로워 모든 시간동안 환자에게 주의를 기울일 수 있게 되었다. 본 연구의 결과는 안정성, 비용 등의 문제로 아직 초기 단계에 불과하다. 실제적 측면에 있어서 완성도를 높이기 위해 보다 높은 수준의 프로토타입 제작을 통한 논리적 사용자 테스트와 사용자 평가가 요구되며 이를 바탕으로 컨셉의 세밀한 보완 연구가 뒤따라야 한다.
본 연구는 회복마취간호사의 직무스트레스, 조직문화 및 전문직 정체성, 직무만족도 정도를 확인하고 직무만족도에 영향 미치는 관련요인을 규명하고자 실시하였다. 149명의 회복마취간호사를 대상으로 자료를 수집한 결과, 직무스트레스는 $3.86{\pm}0.47$, 조직문화는 $3.19{\pm}0.34$, 전문직 정체성 $3.46{\pm}0.40$, 직무만족도는 $3.19{\pm}0.39$였다. 직무만족도는 간호사의 소속에 따라 차이가 있었고(t=2.563, p=.011), 직무만족도는 직무스트레스 중 보수 및 대우(r=.373, p<.001), 조직문화 중 혁신지향 문화(r=.507, p<.001)와 관계지향 문화(r=.559, p<.001), 그리고 전문직 정체성(r=.544, p<.001)과 양의 상관관계로 나타났다. 회복마취간호사의 직무만족도에 영향을 미치는 요인은 전문직 정체성(${\beta}=.391$, p<.001), 관계지향 조직문화(${\beta}=.302$, p<.001), 혁신지향 조직문화(${\beta}=.209$, p=.004) 순이었고 세 변인이 설명하는 직무만족도의 총 설명변량은 48.7%였다. 본 연구의 결과를 토대로 회복마취간호사의 직무만족도를 높이기 위해서는 전문직 정체성 향상과 관계 및 혁신지향 조직문화를 활성화하기 위한 방안이 고려되어야 할 것이라 사료되며, 전문직 정체성 확립을 위한 지속적인 교육과 관계 및 혁신지향 조직문화 형성을 위한 전략 개발 및 운영을 제언한다.
Purpose: This study was done to identify job tasks and task elements of Korean nurse anesthetists according to type of medical institution. Methods: A job task scale which consisted of 9 job tasks and 40 task elements was developed. Data were collected from December, 2009 to February, 2010 from 182 nurse anesthetists who were working in medical institutions (response rate: 75.8%). Results: Forty-eight percent of nurse anesthetists were independent from anesthesiologists in anesthetic practice. Preanesthetic nursing assessment was much more frequent in small hospitals than in general hospitals (p<.05), and anesthetic nursing intervention, administering the anesthetics, monitoring the patient's status during anesthesia, and provision of safety and compliance with anesthetic ethics were much more frequent in general hospitals than medical centers (p<.001). There were no differences among the medical institutions for job tasks in post-anesthetic nursing interventions (p=.229), administering anesthetics (p=.354) and monitoring patients' status during anesthesia (p=.099), providing safe anesthetic environment (p=.896), and management of ancillary personnel/equipment (p=.617). Conclusion: Results indicate that nurse anesthetists contribute significantly to anesthetic practice in small hospitals and general hospitals. Therefore, it recommended that nursing leaders make efforts to enact legal nurse anesthetist-related policies for safe and high quality anesthetic nursing care.
Purpose: The purpose of this study was to develop a tool for measuring the intra-operative caring as perceived by regional anesthesia patients. Methods: A preliminary instrument with 54 questions, based on a literature review and semi-structured interviews with 11 regional anesthesia patients, was developed. A group of experts revised individual questions, and 34 questions were finally selected. This tool was tested with 137 regional anesthesia patients admitted to D university hospital in B city from August to October 2008. Results: The validity and reliability of the tool were tested using factor analysis. After item analysis, one question with a correlation coefficient under .30 was discarded After performing factor analysis on the final 33 questions, 7 factors were identified; holistic needs fulfillment, consideration, protective environment, cautiousness, concern, information, physical comfort. The total variance shown in the test was 73.5%. A Cronbach alpha of 0.96 showed the reliability of the instrument. Conclusion: Results of this study indicate that the tool is statistically reliable and valid to measure the intra-operative caring perceived by regional anesthesia patients. This tool can be utilized usefully in assessing the effects of nursing interventions for regional anesthesia patients.
목적 : 암환자에게 있어 통증은 삶의 질을 좌우하는 가장 중요한 증상이다. 현재의 통증 치료 방법 즉, 적절한 약물 치료 방사선, 마취, 신경차단 등의 방법으로 97%정도가 통증 조절이 가능함에도 불구하고, 여러 국내외 문헌에서는 암환자의 $60{\sim}70%$가 통증을 호소하고 있음을 보고하고 있다. 이에 이 연구에서는 의료진 측면에서 적절한 통증 치료의 장애가 되는 요인을 찾고, 진통제 및 암환자의 통증에 대한 잘못된 인식과 편견을 파악하여 보다 효과적인 교육프로그램을 개발하는 기초 조사를 마련하고자 한다. 방법 : 1998년 5월 1일부터 5월 31일까지 한달 동안 서울 시내에 있는 네개의 종합병원 내과에서 근무하고 있는 의사와 간호사를 대상으로 통증 관리에 대한 지식과 태도에 관한 도구를 수정한 32개 문항의 설문지를 통해 조사하였다. 설문의 내용은 통증 사정이 6문항, 진통제 투약이 5문항, 진통제 약물 작용이 8문항, 진통제 분류가 11문항, 그리고 진통제 투약 태도 2문항으로 이루어졌다. 결과 : 의사가 99명, 간호사가 152명이 참여하였으며, 전체 정답율은 의사가 71.6%, 간호사가 70.3%였다. 그러나, 그 중 진통제 분류에서는 80% 이상의 정답율을 보였으나 나머지에서는 대부분 $50{\sim}60%$정도의 정답율을 보이면서 통증 관리에 대한 낮은 지식 정도를 보여 주었다. 통증 사정에서는 간호사의 지식이 유의하게 높았으며, 진통제의 약리작용과 진통제 분류에서는 의사의 지식이 유의하게 높았다. 그러나, 전체 통증 관리에 대한 지식과 태도에 있어 의사 평균이 21.40이었으며, 간호사의 평균은 20.87로서 유의한 차이를 보이지 않았다. 결론 : 암환자 통증 관리에 대해 의료진의 지식 정도이 부족함이 나타났고, 이에 대한 적극적인 대안으로 의사와 간호사를 위한 교육프로그램 개발과 교육이 절실히 필요한데 일시적이고 일회적인 교육보다는 지속적이고 장기적이고 자동적으로 운영되는 체계가 필요하다.
본 논문은 보건과학분야에서 활용되는 마네킹시뮬레이션 교육을 알아보고자 한다. 마네킹 시뮬레이션은 의과대학 교육뿐 아니라, 응급구조학과 교육, 구조구급 분야에 종사하는 소방공무원, 간호사, 치과대학, 마취과학등 의료의 여러 영역에서 다양하게 이용되고 있다. 본 연구는 시뮬레이션의 역사, 시뮬레이션의 진화과정, 시뮬레이션을 이용한 문제중심학습(problem-based learning, PBL)의 활용에 이르기까지 마네킹시뮬레이션 교육의 현황을 설명하고자 한다.
본 연구는 일 감염병 전담병원 간호사의 COVID-19와 관련된 직무스트레스, 감정노동, 임파워먼트 및 재직의도 정도를 파악하고 COVID-19와 관련된 직무스트레스, 감정노동, 임파워먼트가 재직의도에 미치는 영향을 확인하고자 한다. 본 연구의 자료수집은 S시 소재 감염병 전담병원 간호사 162명을 대상으로 구조화된 설문지를 통해 2021년 8월 27일부터 2021년 9월 17일까지 실시하였고, SPSS/WIN 25.0을 이용하여 빈도와 백분율, 평균과 표준편차, 독립표본 t-검정, 분산분석, 피어슨상관계수, 다중선형회귀분석을 이용하여 분석하였다. 일반적 특성에 따른 차이 분석 결과 재직의도는 근무부서 및 근무만족도에 따라 유의한 차이가 나타났고, 재직의도와 COVID-19와 관련된 직무스트레스, 감정노동, 임파워먼트간의 상관관계를 분석한 결과 재직의도는COVID-19와 관련된 직무스트레스와 유의한 부적 상관관계(r=-0.215, p=0.006)를 나타냈고 임파워먼트와 유의한 정적 상관관계(r=0.343, p<0.001)를 보였다. 다중선형회귀분석 결과 재직의도에 영향을 미치는 유의한 변수로 근무만족도, COVID-19와 관련된 직무스트레스, 임파워먼트가 확인되었으며(F=23.751, p<0.001) 설명력은 30.0%이었다. 이상의 결과를 통해 효율적인 간호인력 관리를 위한 전략개발의 기초자료를 제공하고자 한다.
Purpose: The purpose of this study was to explore nurses' experience with caring for COVID-19 patients in a negative pressure room amid the spread of the pandemic. Methods: This study was a qualitative research, and focus group interviews were used to collect data. Three focus groups comprising 19 nurses were interviewed from February 17 to 25, 2021. All interviews were recorded and transcribed verbatim with the consent of the participants. The verbatim transcripts were scrutinized using thematic analysis. Results: Two main themes emerged from the analysis: 'Struggling in an isolated space' and 'Limitations of nursing infrastructure and system'. The nurses caring for COVID-19 patients experienced anxiety and fear about the infection, physical exhaustion, emotional burnout, and a sense of duty as a nurse. They also acknowledged the lack of guidelines, increased task and burden, limitations of nursing care, and the demand for improving the limitations of the nursing system. Conclusion: The results of this study demonstrate that nurses caring for COVID-19 patients encounter physical and emotional problems within the limited healthcare system. The study suggests that comprehensive interventions are needed for nurses. Furthermore, detailed guidelines, strengthening of nursing personnel, and improvements to the nursing system are vital to effectively cope with the pandemic. The government and medical institutions should be aware of the needs of nurses and what they are going through, and make efforts to improve the quality of life of healthcare workers and create a safe healthcare environment.
