Quantitative Analyses of Cells using Photoshop after the H&E Staining of the Synovia of Osteoarthritis and Rheumatoid Arthritis Patients (H&E 염색 이미지의 포토샵 분석을 이용한 골관절염과 류마티스 관절염 활막 세포의 정량 분석)

  • Synovium is the soft tissue that lines the non-cartilaginous surfaces within joints. It has been reported that synovial cells are activated during the pathogenesis of rheumatoid arthritis. In this study, we quantitate and compare the cellular composition of synovia derived from individuals with non-inflammatory osteoarthritis (OA) and those with inflammatory rheumatoid arthritis (RA). Synovia from OA (n=8) and RA (n=5) patients were used for hematoxylin and eosin (H&E) staining. A light microscopic examination has shown that RA synovia were morphologically thickened and hypertrophied as compared to OA synovia. We also performed an immunohistochemistry (IHC) analysis to classify cell types in the synovia using CD68, CD90, or PGP9.5 markers. As a result, we obtained quantitative data regarding the cell populations, which are macrophages in the lining layer and FLSs in the subintimal layer of the synovium. Further Photoshop analyses of the H&E images could allow the counting of the number and layer of the cells in the synovium. The number and layers of the macrophage cells were increased in the lining layer of the RA synovia as compared to the OA synovia. FLS cells also were increased in the subintimal layer of RA synovia. Therefore, quantification of the H&E stained images via Photoshop is a possible analysis protocol for synovium study. This quantitation also supports the idea that the increases in cell number and cell activation are important processes for RA pathogenesis.

Effect of Program for Pain Reduction in Patients with Rheumatoid Arthritis: A Systematic Review (류마티스 관절염 환자의 통증 감소를 위한 프로그램의 효과: 체계적 문헌고찰)

  • Purpose: The purpose of this study was to establish the foundations for the application of appropriate programs for pain reduction in patients with rheumatoid arthritis. Methods: The literature on patients with rheumatoid arthritis were found on an electronic search site for South Korean academic papers. The search terms "rheumatism" and "rheumatoid arthritis" were used independently. The combined terms "rheumatism and pain" and "rheumatoid arthritis and pain" were also used. For the meta-analysis, the R version 3.5.1 program was utilized. Results: The meta-analysis of eight papers showed a large effect size of -4.11. The programs were most effective in the order of aquatic exercise, aromatherapy, self-help education, muscle strength exercises, and tai chi. Conclusion: This study could provide the basis for presenting appropriate programs for pain management in patients with rheumatoid arthritis.

Thymic Stromal Lymphopoietin (TSLP) Gene Polymorphisms are not Associated with Rheumatoid Arthritis in a Korean Population (TSLP 유전자의 다형성은 한국인 류마티스관절염 발생에 영향을 미치치 않는다)

  • Thymic stromal lymphopoietin(TSLP) is a novel IL-7-like hematopoietic cytokine. Human TSLP is produced by epithelial cells, stromal cells, and mast cells. The TSLP gene is highly expressed in synovial fluid specimens derived from rheumatoid arthritis (RA) patients. We previously identified four single nucleotide polymorphisms (SNPs) and one variation site in human TSLP gene. In this study, we analyzed the genotypic and allelic frequencies of the TSLP SNPs between RA patients and healthy controls. We also investigated the relationships between SNP genotypes and the RF levels and anti-synthetic cyclic citrullinated peptide (CCP) levels in RA patients. We then calculated the haplotype frequencies defined by these SNPs for both groups. The genotype and allele frequencies of the TSLP SNPs did not differ significantly between the RA patients and the healthy controls. We also found that TSLP SNPs in the RA patients had no significant association with the levels of RF or anti-CCP. Our results suggest that TSLP SNPs are not associated with susceptibility to RA.

Association of Thymic Stromal Lymphopoietin Receptor (TSLPR) Polymorphisms with the Susceptibility of Rheumatoid Arthritis in a Korean Population (한국인 류마티스 관절염의 감수성과 TSLPR 유전자 다형성의 연관성)

  • Human thymic stromal lymphopoietin receptor (TSLPR) might play an important role in the development of inflammatory and allergic responses. We previously identified eleven single nucleotide polymorphisms (SNPs) and two variation sites in the TSLPR gene and showed that all the SNPs of the TSLPR gene are associated with susceptibility to atopic asthma. The present study aimed to investigate whether the TSLPR gene SNPs are associated with susceptibility to rheumatoid arthritis (RA). We compared the genotype and the allele frequencies of the TSLPR SNPs in 457 RA patients and 570 healthy controls. The genotype and the allele frequencies of the TSLPR gene SNPs in the RA patients were not significantly different from the respective frequencies of the healthy controls. Additional analysis showed that the genotype and the allele frequencies of the TSLPR gene SNPs did not appear to be associated with RA in female RA patients. The TSLPR gene SNPs in the RA patients did not affect the production of rheumatoid factor (RF) and antisynthetic cyclic citrullinated peptide (CCP). Our results suggest that the TSLPR gene SNPs are not associated with susceptibility to RA in the Korean population.

Cancer of Unknown Primary Site Mimicking Retroperitoneal Fibrosis (후복막강섬유화증으로 오인된 원발 부위 불명암 1예)

  • Retroperitoneal fibrosis (RPF) is a rare disease characterized by marked fibro-inflammatory tissue in the retroperitoneum. Approximately 70% of cases of RPF are idiopathic, while the rest can be secondary to several other causes. The diagnosis is mainly obtained by imaging modalities such as computed tomography (CT). However, histological examination should be considered when the clinical manifestations and imaging studies suggest malignancy. In particular, in malignant diseases with retroperitoneal metastases, abnormal collagen plaques are formed from an exuberant desmoplastic response, which may not be distinguishable from RPF on CT scans. Therefore, even if CT suggests RPF, biopsy is essential to identify malignant disease because it typically results in a fatal prognosis. Here, we report a case of metastatic adenocarcinoma of unknown primary site that developed only in the retroperitoneum and was initially diagnosed as RPF based on CT findings.

Improvement Effect of the Eucommia ulmoides Extracts on CIA-induced Rheumatoid Arthritis Animal Models (두충 추출물의 류마티스관절염 동물모델에 대한 개선 효과)

  • The present study aimed to evaluate the effect of Eucommia ulmoides extracts on rheumatoid arthritis biomarker in a CIA-induced DBA/1 mice. For evaluation, Eucommia ulmoides extracts was administered orally at dose of 100 mg/kg/day for 4 weeks after production of an animal model of rheumatoid arthritis and we confirmed the treatments' effects based on serum biomarker, radiological, structural parameter analysis. Compared to the negative control group, the Eucommia ulmoides extracts treatments significantly reduced the serum level of inflammation and immunoglobulin markers (i.e., TNF-α, IgG, and hs-CRP), and significantly decreased the monocyte count of white blood cells. Furthermore, the Eucommia ulmoides extracts treatments effectively preserved the joint destruction, and little the joint deformation. Moreover, compared to the negative control group, the Eucommia ulmoides extracts treatments increased the bone volume, and significantly decreased bone inflammation. The results indicate that the Eucommia ulmoides extracts improved rhrumatoid arthritis symptoms. Thus, the Eucommia ulmoides extracts may be a novel therapeutic option for the management of rheumatoid arthritis.

The Clinical and Radiographic Features of Temporomandibular Joint Dysfunction in Patients with Rheumatoid Arthritis (류마티스성 관절염 환자에서 나타나는 측두하악관절장애의 임상적 및 방사선학적 특징)

  • The aims of this study was to investigate the clinical and radiographic features of temporomandibular dysfunction in the patients with RA to elucidate whether the RA patients would be a risk group for TMD. The 35 patients with temporomandibular joint dysfunction were included for this study, of which 15 had rheumatoid arthritis diagnosed by a rheumatologist, and other 20 was control group and they didn't have any history or clinical signs related to it. Clinical symptoms and signs of temporomandibular joint disorders, radiographic and MRI findings of temporomandibular joint were investigated. The results were compared between two groups. In RA group bilateral pain, morning stiffness, reduced opening range, and crepitations were more frequently reported than control group. Radiologic findings such as sclerosis and flattening of condylar head, marginal proliferation, and erosion of glenoid fossa were more frequently observed in RA group than control group. Disk destruction, cortical bone erosion, and intraarticular enhancement were more prominenet in RA group. From theses results, it can be concluded that many RA patients will develop TMD symptoms and the structural changes of TMJ is more extensive than the usual TMD cases.

Therapeutic Effects of Binsosan(檳蘇散) on Adjuvant-induced Rheumatoid Rats (Freund's complete adjuvant로 유발시킨 rat 류마티스성 관절염에 대한 빈소산의 치료 효과)

  • 실험목적 : 빈소산은 11가지 생약으로 구성된 복합 한약 처방으로 관절염을 포함한 다양한 염증성 질환의 치료제로 사용되어 왔으나, 관절염에 대한 직접적인 효력평가는 찾아 보기 힘들다. 따라서 본 실험에서는 빈소산 추출물이 Freund's complete adjuvant (FCA)로 유발된 rat 류마티스성 관절염에 미치는 치료 효과를 dexamethasone (15mg/kg, 복강 투여) 의 효과와 비교 평가하였다. 실험방법 : 류마티스성 관절염은 FCA (10mg in 1ml paraffin oil 0.1ml/rats)를 좌측 후지에 피내 투여하여 유발하였다. 실험동물은 Wistar 랫트를 사용하였고, FCA 투여 14일 후 유사한 무릎관절 둘레를 나타내는 류마티스성 관절염 유발 rat와 정상 rat 및 실험군을 그룹당 9마리씩 나누었다. 실험동물은 100 또는 200mg/kg의 빈소산 추출물을 FCA 투여 14일 후부터 14일간 경구 투여하였으며, dexamethasone은 15mg/kg 농도로 복강 투여한 다음, 희생하여, 체중, 연골내 collagen 함량 및 chondroitin sulphate, heparin sulphate 및 hyaluronic acid와 같은 뼈내 glycosaminoglycan 함량의 변화를 각각 관찰하였다. 실험결과는 항염 효과가 이미 입증되어 있는 dexamethasone 15mg/kg 복강 투여군과 비교하였다. 결과 : FCA 투여는 현저한 체중, 연골내 collagen 함량 및 chondroitin sulphate, heparin sulphate 및 hyaluronic acid와 같은 뼈내 glycosaminoglycan 함량의 감소와 함께 유발 관절 둘레 및 조직내 prostaglandin $E_2$의 증가와 같은 전형적인 류마티스성 염증을 초래하였으나, 이러한 류마티스성 관절염 소견은 dexamethasone 및 모든 용량의 빈소산 추출물 투여에 의해 현저히 억제되었으며, 특히 빈소산 투여군에서는 투여 용량 의존적인 감소가 인정되었다. 결론 : 이상에서 빈소산 추출물은 투여 용량 의존적인 prostaglandin $E_2$ 억제를 매개하여 FCA 유발 류마티스성 관절염에 대한 치료 효과를 나타내는 것으로 관찰되었다. 따라서 새로운 관절염에 대한 치료제로서 개발 가능성이 있을 것으로 판단된다. 한편 빈소산 추출물은 주로 prostaglandin $E_2$ 억제작용에 의해 항염 효과를 나타내는 것으로 관찰되었으나, 금후 다른 작용기전에 대한 연구와 빈소산의 구성성분 중 유효 성분 규명을 위한 실험이 수행되어야 할 것으로 판단된다.

Effect of Microcurrent Therapy on Interleukin-6 Expression in Adjuvant Induced Rheumatoid Arthritis Rat Model (미세전류치료가 아주반트 유도 류마티스관절염 유발 흰쥐의 인터루킨-6 발현에 미치는 영향)

  • 연구목적 : 미세전류 자극이 아주반트로 류마티스 관절염을 유발한 실험동물의 염증반응과 통증에 미치는 효과를 알아보기 위하여 실험동물의 발허리발가락관절내 염증반응 정도를 나타내는 인터루킨-6(interleukin-6)의 발현과 핫플레이트(hot plate)를 이용한 발도피지연시를 측정하여 미세전류의 효과에 대하여 알아보고자 하는데 목적이 있다. 연구방법 : 실험동물은 무작위로 대조군(n=18)과 미세전류를 적용한 실험군(n=18)으로 구분하였고, 각 군당 6마리씩 1일군, 7일군, 14일군으로 배정하였다. 류마티스 관절염 유발후 1일, 7일, 14일에 모든 실험동물의 열통각 역치를 나타내는 발도피지연시와 발허리발가락관절내 인터루킨-6의 발현정도를 측정하였다. 각 집단 내의 기간에 따른 발도피지연시와 인터루킨-6의 면역반응성은 일원배치 분산분석을 실시하였고, 사후분석으로는 Duncan의 다중범위검정을 실시하였다. 실험군과 대조군을 비교하기 위하여 독립표본 t-test를 실시하였다. 연구결과 : 실험결과는 다음과 같다. 1) 아주반트 주사 1일후, 실험군과 대조군에서 인터루킨-6 면역반응성과 발도피지연시는 비슷한 양상을 보였다. 2) 인터루킨-6 면역반응성은 아주반트 주사 7일, 14일 후 대조군이 실험군보다 유의하게 증가되었다(p<.05). 3) 발도피지연시는 아주반트 주사 7일, 14일 후 실험군이 대조군보다 유의하게 증가되었다(p<.05). 결론 : 이상의 결과로부터, 미세전류 자극이 아주반트로 유발된 류마티스관절염 모델에서 활액 조직내 염증반응을 감소시키고 열통각역치는 증가되는 것을 알 수 있었다.
