Comparison of Outcomes between 7-French and 10-French Plastic Stents for Patients with Biliary Tract Cancer (담도암 환자에서 7-프렌치와 10-프렌치 플라스틱 스텐트의 결과 비교)

  • Background/Aim: There is little data on whether plastic stents with a larger diameter are patent for longer than small stents in patients with bile duct cancer. The aim of this study was to compare the stent survival between 7-French (Fr) and 10-Fr plastic stents and evaluate the factors affecting stent survival. Methods: Patients with biliary obstruction due to biliary tract cancer were enrolled at Yonsei University Wonju College of Medicine from January 2010 to October 2014. Results: A total of 215 patients (7-Fr:10-Fr = 89:126 patients) were retrospectively enrolled. The primary tumor sites were common bile duct (n = 111), hilar (n = 45), and ampulla of Vater (n = 59). Rates of stent migration and stent obstruction were not different between the two groups. The median duration of stent survival was 3.3 months in the 7-Fr group and 5.9 months in the 10-Fr group (p = 0.543). The diameter of the stent did not have an effect on stent survival (hazard ratio 1.11, 95% confidence interval 0.71-1.73, p = 0.649). Conclusions: 7-Fr and 10-Fr stents have similar rates of stent migration and stent obstruction. The stent survival of 7-Fr was not inferior to 10-Fr stents in the management of biliary tract cancer.

Accuracy of spring-style and friction-style mechanical implant torque wrench (스프링형과 마찰형 기계식 임플란트 토크 렌치의 정확도에 관한 연구)

  • Purpose: The aim of this study was to compare the accuracy of two different styles of mechanical implant torque wrenches before and after sterilization and repeated use. Materials and methods: Thirty new spring-style and friction-style mechanical implant torque wrenches from five different manufacturers were selected as subjects of study and assessed in terms of accuracy. Three manufacturers produced, spring-style (Dio, Neobiotech, Osstem) torque wrenches, and other three manufactures produced friction-style (Dentium, Osstem, Shinhung) torque wrenches. The procedure of target torque measurement was performed 10 times for each device and a digital torque gauge (MGT-12, Mark-10 Corp, USA) was used to record the measurements. All torque wrenches were used repeatedly for 20 times and then sterilized. This whole procedure was repeated 100 times and all torque wrenches were retested. The collected data was analyzed using a SPSS Statistics Analysis program to evaluate the accuracy of the torque wrenches and Mann-Whitney-U test was used for a comparative analysis. Results: After sterilization and repeated use procedure, results did not show any significant difference between error values in the spring-style torque wrench group. However, the results demonstrated an increase in error values within the friction-style torque wrench group (P<.05). Conclusion: There was no difference between the accuracy of new spring-style torque wrenches before and after 100 cycles of sterilization and repeated use (P>.05). There was significant difference between the accuracy of new friction-style torque wrenches before and after 100 cycles of sterilization and repeated use (P<.05). Within the limitation of this study, spring-style torque wrenches were more accurate than friction-style torque wrenches in delivering target torque values.

Interference-free French door design using four-bar linkage mechanism (4절 링크를 이용한 프렌치 도어의 간섭 방지 설계)

  • The French doors have the advantage that they can use inner space more efficiently due to without of partition between two rooms. However, when they are used for refrigerators, the door gaskets for sealing may cause interference of themselves during opening and closing, which causes fatal effect on sealing by worn out of the gaskets as well as increases door opening force. This research proposes a new mechanism for the French doors using the parallelogram motion of 4-bar linkage mechanism, which does not make any interference between gaskets. We manufactured the French doors of proposed mechanism to verify that they do not cause any interference during opening and closing, as well as opening force is decreased. The use of our developed mechanism is not limited to refrigerators, but can be extended to other industrial products with the French doors.

  • In today's era of media convergence, users can watch a drama by choosing a story like a game or enjoy a game directed like a movie. These two media material interactive storytelling, and there have been discussions about them for many years. The purposes of this study are to propose a new branch structure for interactive storytelling and find a new perspective for the method of proceeding with a branch structure. The main research targeted a game that materialized interactive storytelling best. Based on the findings, the study proposed a volume branch structure including a plot border and adjuster concept for interactive storytelling to keep the story narrative and help the users feel interactions freely. This structure can be materialized only with real-time edited videos in the game media and will be possible in the application cases of a physical engine that continues to develop and artificial intelligence that has not been introduced in games actively.
