DENTAL TREATMENT IN A PATIENT WITH LENNOX-GASTAUT SYNDROME UNDER GENERAL ANESTHESIA (레녹스-가스토 증후군 (Lennox-Gastaut syndrome) 환자의 전신마취 하 치과치료)

  • Lennox-Gastaut syndrome (LGS) is a severe form of childhood epilepsy that is defined by generalized multiple type seizures, slowness of intellectual growth, and a specific EEG disturbance. Children affected might previously have infantile spasms or underlying brain disorder but etiology can be idiopathic. LGS seizures are often treatment resistant and the long term prognosis is poor. A 14-year-3-month old, 20.5 kg girl with LGS was scheduled for dental treatment under general anesthesia. The patient presented with multiple caries and heavy calculus. General anesthesia was induced and maintained with inhalation agent, Sevoflurane. And caries treatment, scaling, subgingival curretage were performed. LGS patients usually have poor oral hygiene due to mental retardation and have high risk of seizure during dental treatment. In dental treatment under general anesthesia for LGS, it must be considered of seizure control during peri-operative period. Also periodic recall check, instruction of oral hygiene must be performed.

  • The purpose of this study is to assess the usefulness of thermography in patients with musculoskeletal disease. Thermography is noninvasive, easy to reading, objective and physiologic instrument by measuring and imaging infrared energy emitted from skin surface. Thermography can show the skin temperature changes in various conditions of the body with musculoskeletal disease. This literature review was done for the usefulness of thermography in diagnosing musculoskeletal disease. In conclusion, thermography was adapted in radiculopathy, MPS, peripheral neuropathy, RSD, Raynaud's phenomen, TMJ dysfunction, etc. It was useful as a secondary diagnostic method in those diseases, also possible as a primary diagnostic method in RSD, Raynaud's phenomen. And, it might be reliable tool for estimating disease procedure and consequence after treatment. But, the objectivity of the reading and the development of the operating method are required for further adaptation in musculoskeletal disease.

A Study for possibility of Diagnostic Indicators by measuring the Fingertip Temperature after Cooling Load in Raynaud's Patients (레이노 환자의 한랭 부하 후 손가락 끝 온도 측정을 통한 진단 보조지표의 가능성 연구)

  • The purpose of this study is to suggest a quantitative supplementary index for patients with suspected Raynaud phenomenon. The subjects were 99 patients with suspected Raynaud phenomenon, 48 patients with low likelihood, 16 patients with middle range and 35 patients with high range. The test was instilled in ice water at $15.2{\pm}0.8^{\circ}C$ for 10 minutes of both hands and then venous injected of 99m-Tc 370 Mbq(10 mCi). After 5, 10 and 20 minutes, temperature changes of both fingers were measured. As a result, the temperature of the fingers was $32.6{\pm}4.9^{\circ}C$ in patients with a low diagnostic probability of Raynaud syndromes and $22.7{\pm}6.0^{\circ}C$ for those with a high diagnostic probability(p<0.05). In conclusion, we could confirm the difference of Raynaud phenomenon and finger temperature, and confirmed the possibility of secondary diagnosis as a quantitative index of Raynaud's diagnosis.

  • 최근 전자파 응용 기기의 보급이 급격히 늘어나고 있어 이 분야의 종사자들뿐만 아니고 일반인에게도 전자파에 노출되는 기회가 많아지고 있다. 모든 방사선은 생체 조직에 대하여 정도의 차이는 있으나 여러 가지 기능장애를 일으키는 것으로 알려져 있으며 수 GHz이하의 산업용 전자파 에도 과도히 노출되는 경우 소위 전자파 증후군이라 불리는 병변을 일으킨다. 그 중에는 전자레인 지의 누설 전자파가 백내장이나 유방암을 유발한다는 보고도 있으며 휴대전화기의 송신 전자파가 뇌종양, 백혈병등의 원인이 될 수 있다는 연구 결과도 나와 있으며 그 밖에도 전자파로 인한 고혈 압, 두통, 기억감퇴 등도 다수 보고되고 있다.

Implementation of Mobile Virtual Colored Overlay for People with Scotopic Sensitivity Syndrome (광과민성 증후군자를 위한 태블릿 PC와 스마트폰 용 가상 색 오버레이 구현)

  • A film colored overlay has been used as an assistive device for dyslexics, Recently, several virtual colored overlays which can be used in computer were developed. But mobile virtual overlay has not been developed yet. In this paper, I implemented a mobile overlay application which is based android operating system and displays a colored overlay of screen all the time while user can freely interact with rest of apps in normal manner by using root window and service. A method is presented to determine the source color of a virtual overlays by estimating alpha value of alpha blending algorithm through measurement of the chromaticity and transmissivity of film overlays and I implemented all colors which are presented by using Intuitive Overlays. Test results of the developed virtual overlay show that all colors of the overlays are almost identical to the colors of Intuitive Overlay by using the chroma meter CS-200A.

A Case Report of Carpal Tunnel Syndrome with Raynaud's Phenomenon Treated by Bee Venom and Carthami Flos Pharmacopuncture (봉약침과 홍화약침으로 치료한 레이노 현상을 동반한 수근관증후군 환자 1례)

  • Objectives : This study is to report the effect of Pharmacopuncture therapy on a patient suffering from the pain and cold intolerance of hand caused by Carpal tunnel syndrome with Raynaud's phenomenon. Methods : We had treated the patient diagnosed as Carpal tunnel syndrome with Raynaud's phenomenon by Sweet BV and CF pharmacopuncture. We injected Sweet BV and CF into acupuncture points on both hands - Sweet BV into Baxie (EX-UE9), CF into Naegwan ($PC_6$) and Daereung ($PC_7$). And then we evaluated her symptoms by VAS (Visual Analog Scale). Results : Clinical symptoms about Carpal tunnel syndrome with Raynaud's phenomenon were remarkably improved by Sweet BV and CF Pharmacopuncture. Conclusion : Therefore, we concluded that pharmacopuncture therapy - Sweet BV, CF etc. - may be useful to treat Carpal tunnel syndrome with Raynaud's phenomenon.

  • Objectives : The purpose of this study is to evaluate the conservative treatment for FBSS patient. Method : We used Acupuncture, Beevenom, CHUNA manual therapy, Herbal medication, for this patient. And we measured of VNRS and ODI score to evaluate the conservative treatment effects. Result : Patient's LBP & Rt.leg pain VNRS and ODI score were decreased.

  • Neuroleptic malignant syndrome is a serious complication of levodopa withdrawal in patients with Parkinson's disease. We report a patient with advanced parkinsonism who developed neuroleptic malignant syndrome in setting of withdrawal of levodopa intake.

An Analysis of Computerized Implementation of Film Colored Overlays Based on Optical Characteristics and User Preference (필름 색 오버레이의 광학적 특성과 사용자 선택을 반영한 컴퓨터 구현성 분석)

  • The feasibility of a virtual colored overlay is based on constraints of computer and user preference and it is evaluated for people with Scotopic Sensitivity Syndrome. We draw ${\alpha}$ and chromaticities of virtual colored overlay which will be implemented on IT devices by measuring optical characteristics of 2 representative film overlays and simulating them. We analyzed the influence of two illuminating conditions on the virtual colored overlay. We find that it is possible to get the multiple combinations of alpha and source RGB values to match a chromaticity which is presented in CIE-Luv color space under ideal white condition of display, a user can select one among them with respect to clarity and comfort. Under 100 lx difference of illumination conditions, the changes of chromaticities are negligible, but luminances are increased $37.8cd/m^2$ average(std 2.006) at high illumination condition.

  • Lennox-Gastaut syndrome (LGS) is a type of childhood epilepsy, and this syndrome can compromise dental treatment. LGS represents variable etiologies, multiple types of intractable seizures, and cognitive impairment in most patients. A 7-year-4-month old girl with LGS was referred to our hospital for dental treatment under general anesthesia. Clinically, the patient presented with multiple dental caries. The patient had been prescribed a ketogenic diet for the management of repetitive seizures. In addition, she experienced severe types of hypoxia during medical examination using chloral hydrate. Therefore, dental treatment was performed under general anesthesia. LGS patients have a high risk of seizure during dental treatment and can induce a lot of complications. Dental treatment under general anesthesia for LGS patients can be carried out careful preoperative evaluation and in cooperation with medical care.
